Varied Flavonoid Contents of Citri Unshii Pericarpium Extracted from Several Traditional Herbal Prescriptions (진피의 복합처방에 따른 유효성분 추출율의 변화)

  • Objecitves: Peel of Citrus unshiu (Rutaceae) has been used in traditional herbal prescriptions. Hesperidin and naringin contents of Citri Unshii Pericarpium were determined and compared to herbal prescriptions. Then, the different extractions from prescriptions are explained with traditional herbal theories. Methods: The herb and herbal prescriptions were extracted in boiled water. A quantitative analysis was made of hesperidin and naringin using HPLC with-reversed-phase CIS column and a UV detector at 204nm. Elution was carried out at 1.0ml/min with $38\%$ methanol. Results and Conclusions: The extract of Citri Unshii Pericarpium for 180 min was the better method for extracting naringin and hesperidin. The flavonoid contents in remedies are different among several traditional herbal prescriptions. The reason for the difference can be explained with traditional drug interaction theories.

  • This study was investigated to make sure the range of the Colla Corii Asini treatment. the nature of disease and the pathology of it in D ongeuyboga. The following conclusion were reached through invetigations on the prescriptions that use the Colla Corii Asini as a ingredient. 1. The Colla Corii Asini was used a internal disease more than a surgical disease in the dongeuybogam. 2. The Colla Corii Asini was used lung and large intestine disease in abundance and especially anhidrosis. cough and dysentery. 3. The Colla Corii Asini was applied much more disease about blood in the dongeuybogam, but some was used almost bleeding disease. one was used blood deficiency. 4. The processed Colla Corii Asini was used more than the Colla Corii Asini in the dongeuybogam, because of convenience of preparation and ease of intestinla drug absorption. 5. The Colla Corii Asini was used more useful dysentery than tranquilizeing mind in cooperation with Rhizoma Coptidis in the dongeuybogam. 6. Instead of the Colla Corii Asini, we can apply the Colla Comus Cervi or they are mutual reinforcement.

Effects of Gamisinsunbulo-dan on Learning and Memory Function in the Dementia Rat by Ibotenic acid Damage (가미신선부노단이 ibotenic acid손상에 의해 유도된 치매 백서의 학습 및 기억장애에 미치는 영향)

  • This research was done to make the effective prescription and cope with various senile dementia. Sprague-Dawley rats were injured by ibotenic acid to make a damage on learning and memory functions of model rats. At first acquisition test and retention rest were done in the Morris water maze. And to evaluate the effects of the sample drug(GSD) on choline acetyltranferase and acetylcholine esterase, immunoreactive measurement and enzymatic activity measuring were carried out. The ibotenic acid were injected to hippocampus CA1 and CA3 area. Conclusion : GSD improved the learning ability in the acquisition test and memory function in the retention test significantly. And GSD increased the level of ChAT which is synthesizing acetylcholine in CA1 area, and at the same time it increased the level of AChE which is resolving acetylcholine. These results show that GSD improved the cholinergic catabolism and anabolism, and the increment of metabolic activity of cholinergic system. In other words, it contributes to the recovery of damaged learning and memory function by ibotenic acid. So it can be concluded that GSD will be helpful to cholinergic brain damage induced by primary or senile reduction of acetylcholine secretive activity.

Improving Effects of Chimae-eum on Learning and Memory Function in the Hippocampal Damaged Rat (치매음이 해마손상 백서의 기억기능회복에 미치는 영향)

  • In order to make an efficient prescription and cope with dementia, learning and memory functions of Sprague-Dawley model rats were tested with Morris water maze. And to evaluate the effect of the sample drug(CHM) on choline acetyltranferase and acetylcholine esterase, immunoreactive measurement and enzymatic activity measuring were carried out. Rats were injected with ibotenic acid through hippocampus CA1 and CA3 area. The results are as following.   • Purpose: This study investigated medication status by examining the effects of a medication management education program on the knowledge of medications and medication misuse behaviors in the elderly in a local community. Methods: This study used a non-equivalent control group quasi-experimental design. For the study, 116 subjects were assigned to the control group and another 116 subjects were assigned to the experimental group. The medication management education program consisted of 1:1 education, practice in medication management, consultation, and discussion. Data were analyzed using the SPSS 21.0 program. Results: Statistically significant differences were found between the experimental and control groups in terms of their knowledge of medications and medication misuse behaviors. Conclusion: The results indicate that the medical management education program is effective in improving the knowledge of medications and decreasing medication misuse behaviors. Therefore, this education program can be used as an intervention to improve the medication behaviors of the elderly in local communities.

Effect of A Combined Prescription on Liver Injury of Rats by $CCl_4$ (사역산합평위산가인진택사방(四逆散合平胃散加茵蔯澤瀉方)이 $CCl_4$에 의한 흰쥐의 간(肝) 손상에 미치는 영향)

  • Objectives : This study was undertaken to determine if a combined(SPe) has a protective effect against functional failure induced by $CCl_4$ in rat liver. Methods : Acute liver injury which initiated from free radical induced by $CCl_4$, were applied to rats and data were obtained. Liver injury was estimated by measuring aspartate aminotransferase(AST) and alanine aminotransferase(ALT) activity in serum. Lipid peroxidation was examined by measuring malondialdehyde, a product of lipid peroxidation. GSH activities in liver tissues were also measured. Results : When rats were treated intraperitoneally with $CCl_4$, serum AST and ALT were increased compared with the control, which was significantly inhibited by pretreatment of SPe. SPe also prevented reduction in GSH induced by $CCl_4$. Conclusion : Above results suggest that SPe exerts protective effect against $CCl_4$ by its antioxidant action in liver tissues. Thus, SPe may be used in prevention and treatment of drug-induced liver cell injury. However, the precise mechanisms of SPe protection remain to be determined.

Single Oral Dose Toxicity Test of Choweseuncheng-tang, a Polyherbal Formula in ICR Mice (조위승청탕의 마우스 경구 단회 투여독성 평가)

  • The object of this study was to evaluate the single dose toxicity of Choweseuncheng-tang (CWS), a polyherbal formula have been traditionally used as prevention or treatment agent for various diseases as Tae-eumin prescription on Korean medicinal theory, Sasang-euihak, in male and female mice. Aqueous extracts of CWS (yield = 11.00%) was administered to female and male mice as an oral dose of 2,000, 1,000 and 500 mg/kg (body weight) according to the recommendation of Korea Food and Drug Administration (KFDA) Guidelines. Animals were monitored for the mortality and changes in body weight, clinical signs and gross observation during 14 days after dosing, upon necropsy; organ weight and histopathology of 14 principle organs were also examined. As results, we could not find any CWS treatment related mortality and clinical signs, changes in the body and organ weights, gross findings and changes in histopathology of principle organs, except for some dose-independent accidental findings. The results obtained in this study suggest that the 50% lethal dose and approximate lethal dose of CWS aqueous extracts in both female and male mice were considered as over 2,000 mg/kg, the limited highest dosage recommended by KFDA Guidelines, and can be safety used in clinics.

Oral management of Stevens-Johonson syndrome, toxic epidermal necrolysis patients (스티븐 존슨 증후군과 중독성 표피 괴사 융해증 환자의 구강위생관리)

  • Steven-Johnson syndrome (SJS) and toxic epidermal necrolysis(TEN) are severe mucocutaneous reaction which are most frequently caused by drugs. Although the incidence of SJS and TEN is known to be relatively low, outcomes may be fatal. A systematic approach is required because morbidity rate is currently increasing and oral lesion is frequent. We investigated the clinical features and outcomes of 6 patients diagnosed as SJS and TEN and referred from the department of dermatology, Chonnam National University Hospital for oral care. Ketoconazol, Ofloxacin, Chlorphenesin, Amoxicillin, Pontal, Harnal, and Ciprofloxacin were suspected as the causative drugs. Average treatment period was 3.2 weeks, and two patients were referred to 'burn-patients' hospital. Most of oral lesion were cured be normal tissue, but scares with discoloration were observed. For intraoral management, antibiotic disinfection and steroid application were performed according to systemic treatment principles. Additionally, ingestion of zinc, antioxidants, and vitamin was recommended. The establishment of oral treatment principles is demanded because it has not been yet. Also, through investigation of drug side effect and careful prescription are required.

Assessment of Inappropriate Medication Use and Dosage in Elderly Patients (입원 노인환자의 부적절 약물사용현황 및 용량적절성 평가)

  • This study aimed to evaluate the patterns of inappropriate medication use and inappropriate dosage in elderly patients in Korea. A retrospective study was performed for the elderly 65 years or older admitted from January 2007 to December 2007 in a medical center, Seoul, Korea. Potentially inappropriate medication (PIM) use in the elderly was evaluated using Beers criteria. Eighteen drugs out of Beers criteria were included in the formulary of the institute. Inappropriate dosage was set using Beers criteria, CMS (the Centers for Medicare& Medicaid Services) guideline, Geriatric Dosage Handbook. As results, the patients with PIM were 2,172 during the study period. The commonly used inappropriate medications were drugs for the nervous system (n=1237, 44.78%), the alimentary System (n=663, 24.54%) and the cardiovascular system (n=494, 18.28%). The elderly patients with prescription of inappropriate dosage were 10% out of patients with PIMs. The commonly inappropriate dosage drugs were digoxin (n=75, 27.27%), diazepam (n=70, 22.55%) and ferrous sulfate (n=66, 24.00%). Logistic regression analysis showed the number of PIM, days of hospital stay as predictors related to inappropriate dosage use. In conclusion, CNS drugs were frequently prescribed as PIM and inappropriate dosages were identified. It is needed to develop a means of decreasing adverse drug events in elderly.
