Study on Awareness of Preconception Care and Reproductive Health Behaviors in Pre-honeymooners (예비 신혼부부의 수태 전 건강관리 인식수준과 생식 건강증진 행위에 관한 연구)

  • Purpose: The purpose of this study was 1) to examine levels of awareness on preconception care and pregnancy, and reproductive health promoting behavior among pre-honeymooners; and 2) to explore the relationship between awareness levels of preconception care and pregnancy, and reproductive health promoting behavior. Methods: This study was a correlation design with a total of 134 participants (67 couples of pre-honeymooners). Data were collected with questionnaire Results: Mean score of self-perception of awareness of preconception care and pregnancy was 2.82 out of 4. Content awareness level was 6.83 out of 13, and reproductive health promoting behavior score was 3.02 out of 4, indicating low to mid-level. Women reported higher scores in all variables than those in men. Self-perception awareness and content awareness of care and pregnancy had a weak positive correlation (r=.18, p=.038). Reproductive health promoting behavior was positively related to self-perception awareness of care and pregnancy (r=.33, p<.001) and content awareness of care and pregnancy (r=.23, p=.008). Conclusion: Based on this results, a variety of nursing intervention strategies may need to promote reproductive health behavior such as raising awareness of the preconception care and pregnancy.

Effects of the Administration of GnRH and HCG on the Fetus in Pregnant Rats (임신 랫드에 투여한 GnRH와 HCG가 태아에 미치는 영향)

  • The effect of GnRH and/or hCG on the implantation, pregnancy, and the concentration of plasma estradiol and progesterone were studied in pregnant rats. GnRH 50, or 100ug and/or hCG 50 or 100 IU were administered once on day 2 or 9 of gestation, respectively. Rats were autopsied on days 8 or 16. Administration of GnRH on day 2 did not induce the prevention of implantation and termination of pregnancy but was able to induce termination of pregnancy administering on day 9. Administration of hCG induced delayed implantation on day 2 and termination of pregnancy on day 2 and 9. Administration of GnRH concomitant with hCG had no effect on prevention of implantation on day 2 but induced termination of pregnancy with a very increased fetal resorption on day 2 and with a moderate increased fetal resorption on day 9. Administration of GnRH concomitant with hCG on day 2 induced more increased termination of pregnancy compared to injection of GnRH or HCG and opposite result was observed on day 9. Plasma estradiol and progesterone concentrations by administering GnRH and/or hCG had no effect on the termination of pregnancy the pregnant rats.

Critical Discussion on Smoking During Pregnancy as a Form of Fetal Abuse: An Approach to Advocate for Fetal Right to Life (임신 중 흡연에 의한 태아학대: 태아생존권 옹호를 위한 접근)

  • Purpose: Smoking during pregnancy contributes to the risk of negative health outcomes in mothers and babies. The purposes of this study were to review the harmful effects of maternal smoking during pregnancy on fetal and child development, to discuss if maternal smoking should be criminalized as a form of child abuse, and to explore advocating for fetal rights. Methods: A variety of published literature and legal documents including the Korean constitution, criminal laws, and children's welfare laws were reviewed and critically analyzed. Results: Women who smoke during pregnancy are more likely to experience abortion related to placental dysfunction. Their unborn risk premature birth, fetal growth restriction, low birth weight, neurobehavioral disturbances, and/or other complications and newborn babies are also at risk for complications. The advocates for fetal rights can assert that maternal smoking should be regarded as a crime. Conclusion: Findings show that maternal smoking during pregnancy is a major risk factor for many adverse pregnancy outcomes. Effective strategies and health policies for smoking cessation during pregnancy are required to protect pregnant women and their babies.

Development of Pregnancy Risk Symptom Perception Scale (임신 위험 증상 지각 측정도구 개발)

  • Purpose: To develop Pregnancy Risk symptom Perception Scale (PRPS) and evaluate its validity and reliability. Methods: A preliminary 30-item version of PRPS was developed through literature review, in-depth interview, and Content Validity. Each item was scored on a four-point Likert scale. The preliminary scale was developed based on 301 pregnant women who visited a hospital. Date were analyzed using item analysis, factor analysis, confirmatory factor analysis, Pearson's correlation coefficients, and Cronbach's ${\alpha}$ (0.90 for total item, 0.80 to 0.88 for factors). Results: The PRPS consisted of 27 items. Three factors (physical, environmental, and emotional factors) explained 55% of the total variance. Cronbach's Criterion validity was supported by comparison with the Perception of Pregnancy Risk Questionnaire (r=0.34). In reliability test, the reliability coefficient of pregnancy risk symptom perception was high at 0.90. Conclusion: These results suggest that the pregnancy risk symptom perception scale developed in this study comprises items that can assess the level of pregnant women's pregnancy risk symptom perception in Korea. Its validity and reliability were proven. PRPS can be utilized to measure pregnant women's risk symptom perception during pregnancy. PRPS will contribute to the development of systematic prenatal care and effective risk management.

The Rare and Challenging Presentation of Gastric Cancer during Pregnancy: A Report of Three Cases

  • Pregnancy-associated gastric cancer is extremely rare. In many cases, it is diagnosed at an advanced stage because the symptoms during pregnancy are generally overlooked. We report three cases of gastric cancer during pregnancy with various outcomes. The first case included a patient with stage IV gastric cancer who received palliative chemotherapy. This patient had a preterm birth and died 7 months after diagnosis. The second case received neoadjuvant chemotherapy during pregnancy and a total gastrectomy was performed after delivery. She then received adjuvant chemoradiotherapy. This patient developed pulmonary metastasis and died of recurrence 41 months after surgery. In the third case, a distal subtotal gastrectomy was performed at week 14 of pregnancy, with no complications. The patient received adjuvant chemoradiotherapy. She is currently without recurrence 14 months after surgery. In patients with pregnancy-associated gastric cancer, treatment decisions are predominantly influenced by clinical stage and gestational age at diagnosis.

The Usefulness of Pre-operative Infertility Work-up in Assessing the Reversal Feasibility (난관복원술전에 실시한 불임검사의 임신율 예측지표로서의 유용성에 관한 연구)

  • Since the successful advent of IVF-ET, reproductive surgeons have been forced to compare pregnancy outcomes of surgical procedures for tubal infertility with those of IVF-ET. The current study was designed in an effort to determine the usefulness of pre-operative infertility work-up, especially diagnostic laparoscopy, in predicting the pregnancy rate and in assessing the reversal feasibility. 109 patients who underwent a microsurgical tubal reversal were reviewed. The patients were followed over one year. The total intrauterine pregnancy, spontaneous abortion, and ectopic pregnancy rates were 66.4%, 3.7%, and 7.3%, respectively. The result showed that the use of loupe might increase the chance of the ectopic pregnancy. I found that the final tubal length, especially the longest tubal length, the site of reanastomosis, and the methods of sterilization affected the pregnancy rates. A 85% intrauterine pregnancy rate was achieved in the patients with good preoperative assessments(Group II-1). There was only 10% incidence of intrauterine pregnancy in the patients with bad assessments(Group II-2). It is concluded that the pre-operative infertility work-up, especially diagnostic laparoscopy, is a good diagnostic tool in assessing the reversal feasibility.

Individual and Familial Risk Factors Associated with Female Adolescents Pregnancy in South Korea (중.고등학교 여학생의 임신경험에 영향을 주는 개인, 가족요인 탐색)

  • Objectives: In recent years, pregnancy rate among female adolescents has increased and caused a variety of physical and social problems. Therefore, this study aimed to investigate sexual behavior, delinquent behavior, and pregnancy rate among Korean female adolescents. Methods: In light of the growing interest in adolescent pregnancy, this study conducted a web-based survey, titled "The 2006 Youth Health Risk Behavior Survey." The Korean Center for Disease Control (KCDC) collected questionnaire responses from 34,200 young women. The collected data were analyzed through chi-square test and logistic regression using SPSS Win 14.0 version. Results: This study analyzed main factors, which can predict pregnant experience. The statistic results showed two types of the predicting factors: (1) personal factors: sexual relations after drinking (odds 25.1), Narcotic drug taking (odds 13.0), sexual violence act (odds 7.0), part-time job(odds 2.5), drinking, smoking ; and (2) environmental factors: stepfather(odds 4.2) and natural mother. Conclusion: The influential factor identification for predicting pregnancy rate is important to develop an effective education program for preventing the adolescent pregnancy. The education programs with referring to the identified factors can contribute to reducing the unwilling pregnancy rate of young woman.

Experience of Pregnant Women with Problem Drinking during First Trimester of Pregnancy (문제음주 여성의 임신초기 경험)

  • Purpose: The purpose of this study was to explore the meaning of pregnant women's experiences with drinking alcohol during first trimester of pregnancy Methods: The data were collected through in-depth interviews of 7 pregnant women who drank alcohol in the first trimester. Giorgi's phenomenological method was used for data analysis. Results: Findings included 6 main themes and 14 themes. The main themes concerning pregnancy and drinking were: 'Open attitude in drinking, History of drinking in family or spouse, Seeking information in how drinking affects pregnancy, Regret not doing planned pregnancy and not quitting drinking before pregnancy, Willing to stop drinking until the child birth, Awareness about importance of preconception care. Conclusion: The results of this study provide a deeper understanding of pregnant women's experiences of drinking alcohol during the first trimester of pregnancy. These results can be used in the development of strategies to prevent drinking alcohol during first trimester and to support preconception care and prenatal care.

A Study on Characteristic Factors Related to Low Back Pain and Mental Health of Pregnant Women (임부의 요통 관련 요인과 정신건강과의 관계)

  • Purpose: The study was to explore the level of low back pain and characteristic factors influencing low back pain (LBP) and mental health during pregnancy. Methods: The subjects were a total of 383 healthy pregnant women in S City and K-Do. Data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation. Results: 82.5% of the pregnant women answered the existence of LBP and 19.7% of them had high LBP. The preferred method of controlling LBP was 'Just endure'(42.3%). There were significant differences in pregnancy level (p<.05) and discomfort condition related to pregnancy (p<.01) according to low back pain. There were significant differences in pain intensity according to mental health. The correlation between pain level and pregnancy weeks (p<.001) and BMI in previous pregnancy (p<.001) was significant. The correlation between mental health and age was significant (p<.001). Conclusion: The majority of the pregnant women experienced LBP during pregnancy. However, they were not offered the best method of controlling the pain. Thus, for preventing LBP during pregnancy, we recommend regular exercises and BMI control.

  • This study is a bibliographic search about completion and maintenance of pregnancy. In oriental medicine, there is a proper temper and appearance that encourages optimal pregnancy. Invigoration of sperm is important for males to have descendants, and regulation of menstrual disturbances is necessary for female to have descendants. It is not good for both male and female to engage in sexual intercourse too often. Male and female shall have sexual intercourse. When they feel the connection, mind and body are sound, weather is fine, and they take position at ease. Previous studies on pregnancy describe minutely process of fetus's growth until the fifth month, and suggest the rules of fetus's health according to fetal development. Therefore, it is important to prevent acquiring diseases during pregnancy. Female needs to be careful and restful in the early stage of pregnancy. In the late period of pregnancy, she needs to be active.
