암환자의 투병의지와 무력감 예측요인 (Predictors of Fighting Spirit or Helplessness/Hopelessness in People with Cancer)

  • Purpose: This study was done to identify predictors of the fighting spirit or helplessness/hopelessness in the patients' mental adjustment to cancer. Cancer patients' characteristics like performance status, metastasis and duration of diagnosis with demographic factors, spiritual support and social support were used as predictors of a fighting spirit or helplessness/hopelessness. Methods: A total of 124 ambulatory cancer patients completed the Mental Adjustment to Cancer (MAC) scale and responded in a structured instrument about their characteristics, spiritual and social support. Results: The results of multiple regression analysis revealed that confidence in the supporter ($R^2=.114$, p=.000), duration of cancer diagnosis ($R^2=.041$, p=.000) and faith ($R^2=.030$, p=.000) were predictive of a fighting spirit ($R^2=.185$, p=.000); whereas, education ($R^2= .074$, p=.001), performance status ($R^2=.055$, p=.000), satisfaction with social support ($R^2=.046$, p=.000), and metastasis ($R^2=.037$, p=.000) were predictive of helplessness/hopelessness ($R^2=.202$, p=.000). Conclusion: Social support, spiritual support and disease related factors like metastasis, performance status, and duration of cancer diagnosis need to be considered in a psychosocial nursing intervention for a fighting spirit or helplessness/hopelessness.

결혼 이주 여성의 미충족 의료에 미치는 영향 요인 분석 (Analysis of Factors Affecting Unmet Healthcare Needs of Married Immigrant Women)

  • Purpose: The purpose of this study was to identify the factors affecting the unmet healthcare needs of married immigrant women. Methods: This study was a secondary data analysis using data from the 2009 National Survey of Multicultural Families. Data collected from 58,735 married immigrant women who had spouses were analyzed using descriptive statistics, Chi-square test, and logistic regression. Results: Overall, 9.9% of married immigrant women have unmet healthcare needs. The significant predictors related to unmet healthcare needs were young age, high level of education, employed, country of origin, long period of residence, low income, uninsured, urban area, low level of subjective health status, and illness experience over past two weeks. In particular, four variables (long period of residence, low income, subjective health status, and illness experience over past two weeks) significantly predicted unmet healthcare needs for women from all countries of origin. Conclusion: The results of the study indicate that common predictors related to unmet healthcare needs of married immigrant women are a long period of residence, low income, subjective health status, and illness experience over past two weeks. Therefore intervention strategies to decrease unmet healthcare needs should focus on these significant predictors.

Predictors of Mammography Participation Among Rural Korean Women Age 40 and Over

  • Purpose. The study was conducted to identify predictors of mammography screening for rural Korean women according to 'Stage of Change' from the Transtheoretical Model which, along with the Health Belief Model, formed the theoretical basis for this study. Methods. A cross-sectional descriptive design was utilized. Through convenience sampling 432 women were selected from 2 rural areas. Data were collected by survey. Health beliefs constructs were measured with Champion's HBM Scale-Korea version. Mammography participation was measured using the Stage of Mammography Adoption Scale developed. by Rakowski, et al.(1992). Results. The most frequent stage of mammography adoption was 'contemplation' ($40.5\%$). Predictors of stage of mammography adoption included 'mammogram recommended by health professional' (beta==0.59, t=16.12, p=.000), 'perceived benefits' (beta=0.09, t=2.21, p=.050), 'perceived susceptibility' (beta=0.09, t=1.98, p=.050), and 'perceived barriers' (beta=-0.07, t=-2.05, p=.041). 'Mammogram recommended by health professional' demonstrated the greatest association with having a mammogram. Conclusion. Health professionals play key roles in improving mammography participation and should recognize the importance of their role in cancer prevention and be more actively involved in education and counseling on prevention of breast cancer.

고등학교 청소년의 삶의 만족도와 영향 요인 (Satisfaction with Life and It's Predictors of Korean Adolescents)

  • Purpose: This study was done to investigate the satisfaction with life in adolescents, and to identify factors affecting satisfaction with life for them. Method: The participants were 1,057 adolescents, Data was collected through self-report questionnaires which were constructed to include satisfaction with life, self-esteem, hostility and hopelessness. The data was analyzed using the SPSS program. Result: Satisfaction with life for adolescents was significantly different according to school type, grade, scholastic achievement, religion, monthly income of family, conversation with parents and conversation with friends. Satisfaction with life in adolescents correlated with self-esteem, hostility and hopelessness. Significant predictors influencing satisfaction with life in adolescents were self-esteem, hopelessness, school type, conversation with parents, monthly income of family and religion, and these predictors accounted for $37.8\%$ of the variance in satisfaction with life. Conclusion: The above findings indicate that satisfaction with life in adolescents is influenced by self-esteem, hostility and hopelessness. Therefore when nursing interventions are developed to improve satisfaction with life in adolescents, these factors need to be considered.

Knowledge Discovery in Nursing Minimum Data Set Using Data Mining

  • Purpose. The purposes of this study were to apply data mining tool to nursing specific knowledge discovery process and to identify the utilization of data mining skill for clinical decision making. Methods. Data mining based on rough set model was conducted on a large clinical data set containing NMDS elements. Randomized 1000 patient data were selected from year 1998 database which had at least one of the five most frequently used nursing diagnoses. Patient characteristics and care service characteristics including nursing diagnoses, interventions and outcomes were analyzed to derive the meaningful decision rules. Results. Number of comorbidity, marital status, nursing diagnosis related to risk for infection and nursing intervention related to infection protection, and discharge status were the predictors that could determine the length of stay. Four variables (age, impaired skin integrity, pain, and discharge status) were identified as valuable predictors for nursing outcome, relived pain. Five variables (age, pain, potential for infection, marital status, and primary disease) were identified as important predictors for mortality. Conclusions. This study demonstrated the utilization of data mining method through a large data set with stan dardized language format to identify the contribution of nursing care to patient's health.

  • It is an interest problem to predict substance distributions in three-dimensional space. Recently, a research field as Geostatistics is advanced. It is a kind of inter- or extrapolation mathematically. Some useful means for the inter- and extrapolation are known, in which slide window method with neural networks is hopeful one. We propose multi-dimensional extrapolation using multi-layer neural networks and the slide-window method. The multi-dimensional extrapolation is not similar to one-dimension. It has plural algorithms. We researched line predictors and local-plain predictors I two-dimensional space. The both predictors are equivalent; however, in multi-dimensional extrapolation, it is very important to find the direction of predictions. Especially, since the slide window method requires information to predict the future in sampling data, if they are not ordered appropriately in the direction, the predictor cannot operate. We tested the extrapolation for typical two-dimensional functions, and found an excellent character of slide-window method based on local-plain. By using the method, we can extrapolate the function until twice-outer regions of the definitions.

백혈병 청소년의 회복력 영향 요인 (Predictors of Resilience in Adolescents with Leukemia)

  • Purpose: The purpose of this study was to identify the factors relating to resilience for adolescents with leukemia and examine the relationship between these factors. Methods: From June to September in 2014, 199 adolescents aged 11 to 21 participated in the study as they visited the out-patient clinic at C university hospital for follow-up care. To verify the predictors and the effects of resilience, uncertainty, symptom distress, perceived social support, spiritual perspective, defensive coping, courageous coping, hope, and self-transcendence were measured. Collected data were analyzed using hierarchical regression analysis with the SAS statistics program. Results: The final regression model showed that courageous coping, hope, and self-transcendence were significant predictors related to resilience in adolescents with leukemia and explained for 63% of the variance in resilience. Conclusion: The findings indicate that adolescent-oriented intervention programs enhancing courageous coping, hope, and self-transcendence should be provide for adolescents with leukemia in order to overcome illness-related stress and support physical, psychological and social adjustment.

노인의 인지기능 저하 예측요인: 노인실태조사 패널자료를 이용하여 (Predictors of Cognitive Function Decline of Elderly : Using Living Conditions and Welfare Needs of Older Korean Persons Panel Data)

  • Objectives : The aim of this study was to investigate predictors of cognitive function decline among community dwelling elderly. Methods : Data were analyzed from the Survey of Living Condition of Elderly panel study. Cognitive function was measured with the MMSE-KC at baseline and year 3. The study subjects were 5,464 community dwelling people aged 65 years or older who had no disability at baseline. Logistic regression analysis was used to predict cognitive function decline. Results : From 2008-2011, 4,417(80.8%) elderly people had no cognitive decline, and 1,074(19.2%) showed cognitive function decline measured by the MMSE-KC. After adjusting for demographics and baseline MMSE-KC score, the best predictors for cognitive function decline at 36 months were diabetes mellitus, smoking, low intensity physical activity, relationship with relatives and friends. Conclusions : Health promotion programs that are focused on the elderly are essential in preventing cognitive function decline. Promoting regular physical activity, and social relationships should be included in health promotion for elderly. When treating patients with diabetes, preventing cognitive impairment should be considered through education and counseling.

  • Purpose: This study was done to identify psychosocial factors that might be predictive of sexual desire, arousal, lubrication, orgasm, satisfaction, and pain in women with gynecologic cancer. Methods: Two hundred and twelve women with cervical, ovarian, or endometrial cancer completed questionnaires on the Female Sexual Function Index including sexual desire, arousal, lubrication, orgasm, satisfaction, and pain, and data on their psychosocial factors including body image, sexual attitude, sexual information, depression, and marital intimacy. Stepwise multivariable regression analysis was performed to explore psychosocial predictors of women’s sexual function domains. Results: Predictors were identified as sexual attitude, depression, sexual information, and body image for sexual desire; sexual information, depression, and sexual attitude for sexual arousal; sexual information, marital intimacy, and depression for lubrication; sexual information, marital intimacy, depression, and body image for orgasm; marital intimacy, sexual information, sexual attitude, and depression for satisfaction; sexual information, depression, and marital intimacy for pain. Conclusion: The results indicate that women’s sexual function needs to be approached to domains of female sexual function psychosocially as well as to general sexual function. These factors should be considered in future interventions to positively promote sexual function in women with gynecologic cancer.

관상동맥질환자의 건강행위이행에 영향을 미치는 요인 : 건강신념 변수를 중심으로 (The Influencing Factors on Health Behavior of Patients with Coronary Artery Disease)

  • The purpose of this study was to investigate the influencing factors on health behavior among patients with coronary artery disease. Method: The subjects were 95 patients who visited the out-patient department of a university hospital for follow-up. The four health belief concepts (motivation, benefit, barrier, seriousness), general self-efficacy, health behaviors on medication, diet, exercise, stress management, smoking, and drinking were measured. Result: There were significant differences in the health behavior scores of subjects according to family support and the experience of surgical procedure. Subjects were found to have a high degree of compliance in taking medication. However subjects reported the lowest degree of compliance in regular exercise. In the multiple regression analysis, surgical procedure and motivation were significant predictors to explain diet. Motivation and barrier were significant predictors to explain exercise. Self-efficacy, motivation and family support were significant predictors to explain stress management. Family support and seriousness explained 16% of variance in drinking. Also, family support explained 30% of variance in smoking. Conclusion: Since predicting factors on each health behavior indicator were different, then nurses should consider these differences to construct strategy enhancing patient's recovery.
