Effect of mat pilates and kinesio taping on shoulder posture and muscle characteristics of college students with round shoulder posture (매트 필라테스와 키네지오 테이핑이 둥근 어깨 자세를 가진 대학생의 어깨 자세와 근육 특성에 미치는 영향)

  • This study was conducted to investigate changes in posture and muscle characteristics by applying mat pilates and kinesio taping to college students with round shoulder posture and to investigate effective interventions. Subjects were 28 men and women with a round shoulder posture, and they were randomly assigned to a mat pilates group (MPG, n=14) and a Kinesio taping group (KTG, n=14). Each group performed the intervention for 4 weeks, and the distance from the ground to the acromion(DGA) and muscle characteristics (F: Frequency; S: Stiffness; D: Decrement) were measured before and after the intervention. Independent T-test was performed for the difference in the amount of change between groups, and the paired-sample T-test was performed for the difference between pre-post values within the group. Statistical significance level was set as 𝛼=.05. As a result of measuring the change in DGA in both groups, it was significantly decreased in MPG (p<.05). As a result of measuring the change in muscle characteristics of the two groups, the change in F in TL was significantly increased in MPG (p<.05), there was no change in TG, and the amount of change in MPG was larger in the amount of change between the two groups. appeared (p<.05). The change in S was significantly increased in MPG (p<.05). Therefore, the mat pilates conducted in this study is considered to be an effective intervention for improving the round shoulder posture and changing the muscle characteristics than kinesio taping.

The Effects of Therapeutic exercise with Electrical Stimulation on Pain, Range of motion, Muscle strength in patients after Anterior Cruciate Ligament Reconstruction (전기자극치료를 동반한 운동치료가 전방십자인대 재건술 환자의 통증, 관절가동범위, 근력에 미치는 영향)

  • This study was to investigate the effect of therapeutic exercise with electrical stimulation on knee joint pain, range of motion and muscle strength in patients who underwent anterior cruciate ligament reconstruction. The subjects of this study were recruited who diagnosed with anterior cruciate ligament rupture and undergoing anterior cruciate ligament reconstruction, total of 20 patients were randomly divided to the 10 control groups and 10 experimental groups. The therapeutic exercise was carried out for 3 weeks with three exercises made by referring to the previous study. The measurement tools used were knee joint pain measurements were made using the visual analogue scale, range of motion was measured using a goniometer, and the muscle strength was measured using a handheld dynamometer. In the statistical analysis, to compare about pre and post test the difference in each same groups was accomplished by using the paired t-test, and compare the difference between the different each groups was accomplished by using the independent t-test. The results of the study showed that the experimental group showed significantly enhanced results than the control group(p<.05). Based on these results, it is concluded that it is effective for the recovery of the patient if the therapeutic exercise with electrical stimulation in parallel with knee joint therapy are performed for the patients with anterior cruciate ligament reconstruction.

The Effect of Abdominal Massage on Sleep Colonoscopy Subjects (수면 대장내시경 검진자에게 시행한 복부마사지의 효과)

  • The purpose of this study is to investigate the effect of abdominal massage, on abdominal bloating and the recovery of bowel movement after sleep colonoscopy. The study design is the time difference before and after the non-equality control. The subjects were 22 patients each in experimental group and control group who recieved sleep colonoscopy at the G hospital in C city, The data collection period was from June 1, 2018 to August 25, 2018, and the control group's data were first collected with time difference from the collection of experimental group's data to prevent the spread of the experiment. Data were analyzed using IBM SPSS 24.0 and tested by independent sample t-test, $x^2-test$ and Fisher's exact probability test to verify the homogeneity of general characteristics of the subjects. Shapiro-Wilk was used to verify the normality of the abdominal circumference changes in the experimental and control groups. Mann-Whitney U test was used to verify the pre-homogeneity of the abdominal circumference of the subjects. The experimental results were verified by Fisher's exact probability test, Friedman test and Mann-Whitney U test. These results suggest that the abdominal massage can be used as a nursing intervention after sleep colonoscopy by reducing the gas discharge time($x^2=19.75$, p<.001) and abdominal bloating($x^2=29.93$, p<.001).

Occupational Therapy for Community Mobility in Stroke Patients : Systematic review (뇌졸중 환자의 지역사회이동을 위한 작업치료 중재: 체계적 고찰)

  • Objective : The purpose of this study was to analyze occupational therapy intervention on the community mobility for stroke patients, and to provide evidence of intervention in the clinical fields. Methods : A systematic review was executed according to the PRISMA checklist. The accessed database was PubMed, EMBASE, Cochrane Library (CENTRAL), ProQuest Dissertations & thesis (PQDT), RISS, and KoreaMed. We included the articles published from 2005 to September 2018. RoBANS checklist was used to evaluate the quality of the articles. Included articles, totally eight, were categorized according to the type of intervention. Results : The study design of the literature was varied from two-group randomized trial, quasi-experimental study, case-control trial, one group pre-post comparison study, and cross-sectional study. In the evidence level, 6 articles were included in level II (75%). The percentage of low risk of bias in each article ranged from 52.5%~87.5%. Four studies (50%) provided intervention based on virtual reality or virtual environment. The three (37.5%) provided intervention based on the protocol, and the other (12.5%) did wheelchair training. All studies reported significant effects of the intervention. Conclusion : This systematic review provided evidences to use proper intervention in the clinical fields. Various type of studies should be conducted to prove the effect of occupational therapy intervention for community mobility.

DENTAL TREATMENT OF A PATIENT WITH BARTTER SYNDROME: CASE REPORT (Bartter 증후군 환아의 치아우식 치료: 증례보고)

  • Bartter syndrome is an inherited renal tubular disorder characterized by hypokalemia, hypochloremic metabolic alkalosis, hyperreninemia, hyperprostaglandinism, and normal blood pressure. Bartter syndrome is classified by neonatal and classic type. Clinical manifestation of Bartter syndrome considered in dental treatment include tendency to dehydration, vomiting and erosion, attrition and abrasion of the teeth. A 2-year-4 month old boy with Bartter syndrome type III was referred to the Seoul National University Dental Hospital for dental evaluation and treatment. He showed hypokalemic hypochloremic metabolic alkalosis and had been treated with indomethacin and potassium chloride. He had hypocalcified teeth with or without multisurface caries lesions in all dentition and the erosion of maxillary teeth was detected. Dental procedure under general anesthesia was scheduled due to multiple caries and his medical condition. The dental procedure was successfully performed. This case suggests that electrolyte imbalances need to be treated prior to dental treatment and complete coverage restoration is necessary to protect the eroded teeth. An appropriate management plan for the patients with Bartter syndrome should include considerations of the need for close interaction with the pediatrician for pre- and post-operative care. General anesthesia may be recommendable to manage the patients having multiple caries with Bartter syndrome.

Changes in the quadriceps-to-hamstring muscle ratio during wall squatting according to the straight leg raise test angle

  • Objective: The purpose of this study was to investigate the muscle activity ratio of the lower limb according to changes in straight leg raise (SLR) test angles on hamstring muscle shortening during squat exercises. Design: Randomized controlled trial. Methods: The subjects were 14 healthy adults who were informed of and agreed to the method and purpose of the study. The participants were classified into SLR groups according to two angles (over $80^{\circ}$ or under $80^{\circ}$) assessed using the SLR tests. After training and practicing the wall squat posture to be applied to the experiment, electromyography (EMG) was used to measure changes in muscle activity during the performance of a wall squat. After stretching, a sequence of pre-stretch tests were performed again, and the active and passive SLR tests were also reconducted; thereafter, a wall squat was performed again by attaching EMG electrodes. The EMG results before and after stretching were compared. Results: The muscle activity of the vastus lateralis oblique muscle increased in both groups. The muscle activity of the vastus medialis oblique muscle decreased in over both group. Rectus femorus activity increased in the under 80-degree groups but decreased in the over 80-degree group. The muscle activity of the biceps femoris muscle decreased after stretching in the over 80-degree group and increased in the under 80-degree group, and the semitendinosus muscle activity after stretching was decreased. The quadriceps-to-hamstring muscle (Q:H) ratio before and after stretching between groups showed that the hamstring muscle ratio decreased after stretching in both groups. Conclusions: The results of this study showed that the Q:H ratio before and after stretching between groups was not significantly different.

Comparison of ultrasound-guided stellate ganglion block at 6th and 7th cervical vertebrae using the lateral paracarotid out-of-plane approach for sympathetic blockade in the upper extremity

  • Background: The authors have performed ultrasound-guided stellate ganglion block (SGB) in our clinic using a lateral paracarotid approach at the level of the 6th cervical vertebra (C6). Although SGB at C6 is a convenient and safe method, there are ongoing concerns about the weak effect of sympathetic blockade in the ipsilateral upper extremity. Therefore, ultrasound-guided SGB was attempted using a lateral paracarotid approach at the level of the 7th cervical vertebra (C7). This prospective study aimed to compare changes in skin temperature after SGB was performed at C6 and C7, and to introduce a lateral paracarotid approach for SGB. Methods: Thirty patients underwent SGB twice: once at C6 and once at C7. For every SGB, the skin temperature of the patient's hypothenar area was measured for 15 min at 1-min intervals. Skin temperatures before and after SGB and side effects were compared between C6 and C7 groups. Results: The temperature of the upper extremity increased after SGB was performed at C6 and C7. There were significant differences between mean pre-SGB and the largest increases in post-SGB temperatures ($0.50{\pm}0.38^{\circ}C$ and $1.41{\pm}0.68^{\circ}C$ at C6 and C7, respectively; p<0.05). Significantly increased post-SGB temperatures (difference > $1^{\circ}C$) were found in 5/30 (16.7%) and 24/30 (80%) cases for C6 and C7, respectively (p<0.05). There were no significant differences in side effects between SGB performed at C6 or C7 (p>0.05). Conclusion: The lateral paracarotid approach using out-of-plane needle insertion for ultrasound-guided SGB performed at C7 was feasible and more effective at elevating skin temperature in the upper extremity than SGB at C6.

Effect of ankle strengthening exercise using BOSU ball and taping on plantar pressure in patients in their 20s with hallux valgus (보수볼을 이용한 발목 강화 운동과 테이핑이 엄지발가락가쪽휨증을 가진 20대 환자의 발바닥 압력에 미치는 영향)

  • This study was conducted to investigate the effect of ankle muscle strengthening exercise using BOSU ball and taping on static and dynamic plantar pressure in patients with hallux valgus. Subjects were 26 men and women in their twenties with hallux valgus, and they were randomly assigned to a taping group (TG; n=13) and an ankle strengthening group (ASG; n=13). Each group received the appropriate intervention over a total of 4 weeks, and static and dynamic plantar pressure were measured before and after the intervention. Independent T-test was performed to examine the difference in the amount of change between groups, and paired-sample T-test was performed to determine whether the difference between the pre-post values within the group was significant. The statistical significance level was set as α=.05. As for the ratio of the total body weight of the foot (Foot), the amount of change in the ASG of the left foot was significantly decreased compared to the TG within the group (p<.05). Significantly increased (p<.05). As for the degree of pronation of the foot (MP/change ratio), the amount of change in the TG of the right foot was significantly decreased compared to the ASG (p<.05), and in the force of the foot, the TG of the left foot was the amount of change within the group compared to the ASG. was significantly increased (p<.05), and in the group-to-group change, the amount of change in TG was significantly different than that of ASG (p<.05). Therefore, it can be considered that ankle strengthening exercise using bosu ball and taping are insufficient to give effective change to patients with hallux valgus.

Efficacy and Safety of COVID-19 Vaccines in Children Aged 5 to 11 Years: A Systematic Review (5-11세 소아에서 코로나19 백신의 효능 및 안전성에 대한 체계적 문헌고찰)

  • Purpose: To evaluate the efficacy and safety of coronavirus disease 2019 (COVID-19) vaccines in children aged 5-11 years, a rapid systematic review was conducted on published clinical trials of COVID-19 vaccines and studies that analyzed real-world data on adverse events after COVID-19 vaccination. Methods: A systematic search was conducted on medical literature in international (Ovid-MEDLINE) and pre-published literature databases (medRxiv), followed by handsearching up to January 4, 2022. We used terms including COVID-19, severe acute respiratory syndrome coronavirus 2, and vaccines, and the certainty of evidence was graded using the GRADE approach. Results: A total of 1,675 studies were identified, of which five were finally selected. Among the five studies, four consisted of data from clinical trials of each of the four types of COVID-19 vaccines (BNT162b2, mRNA-1273, CoronaVac, and BBIBP-CorV). The remaining study consisted of real-world data on the safety of the BNT162b2 vaccine in children aged 5-11 years. This systematic review identified that COVID-19 vaccines in recipients aged 5-11 years produced a favorable immune response, and were vaccines were effective against COVID-19. The safety findings for the BNT162b2 vaccine in children and early adolescents aged 5-11 years were similar to those data noted in the clinical trial. Conclusions: There is limited data on COVID-19 vaccines in children aged 5-11 years. Consequently continuous and comprehensive monitoring is necessary for the evaluation of the safety and effectiveness of the COVID-19 vaccines.

Effect of Group Cognitive Orientation to Daily Occupational Performance (CO-OP) Intervention on the Occupational Performance Skills of Children With Cerebral Palsy (인지 기반 작업수행(Cognitive Orientation to daily Occupational Performance: CO-OP) 그룹 중재가 뇌성마비 아동의 작업수행기술에 미치는 영향: 사례연구)

  • Objective : The purpose of this study was to investigate the effects of group Cognitive Orientation to daily Occupational Performance (CO-OP) on the occupational performance and social interaction skills of children with Cerebral Palsy (CP). Methods : A single-case experimental study with an ABA design was conducted from April to June 2021, targeting three children with cerebral palsy who met the study selection criteria. The baseline period consisted of 3 intervention sessions until the data were stabilized, and the intervention period consisted of 10 sessions (once a week, 60 minutes/session). For comparison before and after the intervention, motor development, visual perception, and daily life behavior were evaluated. To evaluate the quality of each session, a performance quality evaluation scale was used. Results : In all 3 subjects, the quality of work performance in the three common goal activities improved. The evaluation scores for visual perception, large and small movements, and daily activities were also improved. Conclusion : This study provided evidence that group CO-OP can improve the pre-academic skills and occupational performance of children with CP.
