급성 중이염과 삼출성 중이염의 진단과 치료 (Diagnosis and Management of Acute Otitis Media and Otitis Media With Effusion)

  • The American Academy of Pediatrics and American Academy of Family Physicians developed the clinical practice guideline on the management of acute otitis media. The American Academy of Pediatrics, American Academy of Family Physicians, and American Academy of Otolaryngology-Head and Neck Surgery developed the clinical practice guideline on the diagnosis and management of otitis media with effusion. These two guidelines provide evidence-based recommendations.

간호관리학실습 교과내용에 대한 중요도와 실습 경험에 대한 간호대학생과 임상현장지도자의 인식 비교 연구 (A Comparative Study on the Perceptions of Nursing Students and Clinical Instructors on the Importance of Learning Contents of Nursing Management and Practical Experience)

  • 본 연구의 목적은 간호관리학실습 교과내용의 중요도와 실습 참여 경험에 대한 학생과 임상현장지도자의 인식을 비교·분석해 간호관리학실습 개선의 기초자료를 제공하고자 하였다. 3개 대학 4학년 학생과 임상현장지도자를 대상으로 Mann-Whitney test와 교차분석 등으로 최종 343부 분석하였다. 연구 결과 간호대학생과 임상현장지도자 모두 간호관리학실습의 평가항목 중 '의사소통', '대인관계', '책임', '신뢰', '도덕성'을 중요 요소로 인식하였고 간호관리학실습 교과내용 중 가장 중요하게 인식한 상위 10개 항목 중 6개 항목('감염관리', '간호기록관리', '환자간호관리', '약품관리', '환자안전', '간호과오')은 두 그룹이 일치했다. 또한 임상현장지도자는 대인관계 및 관계윤리를 중요하다고 인식한 반면 간호대학생은 간호사의 '법적책임'을 중요하다고 인식했다. 두 그룹간의 실습경험 분석 결과, 간호대학생은 모두 간접 참여 경험이 높았으나 현장지도자는 직접 지도 경험이 높았다. 이와 같은 인식 차이가 확인된 교과내용은 학교와 실습기관의 유기적 파트너쉽 관계를 구축해 차이를 개선하고 간호대학생의 직접 실습 경험을 증대시키는 방안 마련이 필요하다.

지역사회 민간사례관리자의 실천 성공 경험의 내용과 의미 (Content and Meaning of case Management Practice Success Experience of Regional Society Civil Case manager)

  • 이 연구의 목표는 사례관리자들의 사례관리 성공경험을 탐색하여 현장에서 활용 가능한 목표를 도출하는 것이다. 이 연구의 방법은 자료 수집을 위해 전문 사례관리 사회복지사 20명에 대한 각 2회의 초점집단 인터뷰(Focus group interview)와 자료 분석을 위해 Glaser의 근거이론 방법(Grounded theory method)중 사건코딩을 사용하였다. 분석결과 4개 범주, 10가지 하위범주, 25가지 요소가 도출되었다. 성공경험의 내용은 다음과 같다. [1범주] 클라이언트의 만족과 능력, 하위범주는 (1)역량강화의 시작, (2) 변화의 목표 찾기,(3) 변화의 달성, (4) 클라이언트 주도의 성공, [2범주] 사례관리자의 능력, 하위범주는 (5) 균형있는 인간관 형성, (6) 기술과 능력 향상, [3범주] 조직과 환경, 하위범주 (7) 실천 지원체계확립, [4범주] 신뢰관계 (8) 사례관리자와 클라이언트의 신뢰향상, (9) 지역사회와 클라이언트의 신뢰향상, (10) 사례관리자와 지역사회의 신뢰향상. 이 연구의 결과는 현장의 사례관리 실천 목표를 분명히 하도록 하였고, 향후 기관평가, 사례관리영역에 반영하는 대안으로 제시했다.

병원 의사결정지원시스템의 서비스 품질이 경영성과에 미치는 영향 : K대병원 사례 중심으로 (A Study on the Effects of the Service Quality of Hospital's Decision Support System on Management Performance : the Case of K-University Hospital)

  • Recently, due to external environment like the changes in health policy and various healthcare accreditations, along with hospital's internal efforts to improve the quality of medical services, demands for the development of medical information systems are increasing. Some examples are clinical information like DUR (Drug Utilization Review), CVR (Critical Value Report), and automatic benefit processing by treatment purposes, or hospital DSS (Decision Support System) on overall medical practice. Such systems act as a guide in making clinic judgments during practice or in other medical practice, and their effects on the medical treatment improvements are being proven by previous studies. In the reality of increasing attention in the effects of medical treatment improvement, studies related to hospital DDS were mostly focused on clinical, technical, and engineering points of view, and studies focusing on the user viewpoint are very limited. In order to verify the effects of DSS on practice improvements and hospital's management performance, this study used a research model constructed to verify how SERVQUAL of hospital DSS affects hospital management performance in BSC (Balanced Score Card) point of view. To empirically verify the research model, a questionnaire was conducted on the basis of "K-University Hospital's DSS" on clinicians and hospital employees related to system development, and the relationships between the factors were analyzed through path analysis. As a result of path analysis, excluding reactivity, tangibility, confidence, reliability, empathy among service qualities, had partially significant effects on management performance factors (learning and growth, internal process, financial affairs). This study is to prepare the theoretical ground on the management performance analysis of hospital DSS, and suggest the service quality of the system that should be considered in the planning and development stages for improved system.

중소병원 간호사의 근거기반 통증 관리 지식과 수행 및 근거기반 실무에 대한 태도 (Nurses' Evidence-Based Pain Management Knowledge, Performance and EBP Attitude in Small-medium Sized Hospitals)

  • Purpose. The purpose of this study was to investigate the evidence-based pain management knowledge, performance and Evidence-Based Practice(EBP) attitude of nurses at small-medium sized hospitals. Methods. The sample of this study were 214 nurses. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation and stepwise multiple regression using SPSS/Win 21.0 Results. Evidence-based pain management knowledge score averaged $3.95{\pm}0.43$, with significant differences depending on clinical experiences. Performance score averaged $3.77{\pm}0.51$. EBP attitude score averaged $3.21{\pm}0.57$, and significant differences were evident depending on hospital division and clinical setting. Evidence-based pain management knowledge and EBP attitude were significant predictors of evidence-based pain management performance. Conclusions. This result indicates that education and training programs to facilitate evidence-based pain management knowledge, performance, and EBP attitude are needed among nurses.

디지털 병원의 CDSS구현을 위한 CPG 개발 (Developing CPG for Implementation of CDSS in Digital Hospitals)

  • Purpose: The purpose of this study is to propose Clinical Practice Guideline(CPG) model and Clinical Index(CI) for implementing CDSS in digital hospitals. Methods: This study uses EMR data at department of family practice in A hospital; 636 patients, 570 diseases (based on ICD 10-CM criteria), and 37,000 data related with labs and treatments. This study focuses on disease J342 which is the most high rate of incidence. Results: Using the suggested model, this study calculates frequency matrix and probability matrix to find out the correlation of diseases and labs. This study indicates the lab sets of Disease (J342) as CI for CPG. Conclusion: This study suggests CPG model including Lab-based, Disease-Based and Case-based modules. Through 6 level cased-based CPG model, especially, this study develops Clinical Index(CI) such as the Incidence Rate, Lab Rate, Disease Lab Rate, Disease confirmed by Lab.

영농승계자 역량강화를 위한 영농실습 요구도 분석 (Needs Analysis of Farming Practice to Strengthen the Competence of Farm Successors)

  • This study aims to examine the needs analysis for agricultural management capabilities and farming practices that were emphasized by focusing to the inflow of farmers and the importance of the smooth succession. Data were collected from 134 farm successors. The Borich demand and the Locus for Focus model were hired for the study. Result were as follows. The competency group that the farm successors showed a high educational needs for was financial management. As for the detailed competencies that farm successors showed high educational needs for, based on Borich demand and the Locus for Focus model, the six detailed competencies of financial measures, financial analysis, financial management, sales planning, machine operation, and pest management were common and were derived as priorities. As for the direction of farming practice education for farm successors, based on these analysis results, education on machinery operation in the production sector should be expanded, and the management sector, more systematic and diverse education should be prepared to strengthen financial management capabilities.

효과적인 조리 실습을 위한 교수매체 개발 및 적용에 관한 연구 (Instructional Media Exploitation and Application for Efficient Culinary Practice)

  • This paper is focused on culinary practice. The objective of this research was to develop Instructional Media for efficient culinary practice at high school. For evaluation, Instructional Media were developed and used for culinary practice at a high school located in Chunan. Understanding, interest and scholastic achievement in culinary practice were evaluated and compared for the study group using the developed Instructional Media and the control group not using the developed Instructional Media. Understanding (p<.001), interest (p<.01) and scholastic achievement(p<.01) were higher in the study group than in the control group. The important contributions of this research include the reduction of practice time and improvement of scholastic achievement. The Instructional Media developed in this research will increase the students' interest, reduce the difficulties of culinary practice, and improve the students' culinary skill. The study and development of culinary practice is limited to CDs and illustrated books, and research on the development of various Instructional Media will continue.

충북지역 학교급식 조리종사자의 위생지식과 위생관리 수행수준 (Employees' Sanitation Practice Level and Sanitation Knowledge at School Foodservice Operations in Chungbuk Province)

  • The purpose of this study was to analyze employees' practice levels and knowledge of sanitation at school foodservice operations, to examine the sanitation conditions of current school foodservice, and to suggest an effective sanitary training program. A questionnaire survey was conducted on a total of 578 subjects and 501 reponses were made available for this study. The collected data was analyzed using SPSS of windows. The main results can be summarized as follows: Training through handouts was the most effective (53.2%) and lack of time caused by overwork (57.1%) made the practice of sanitation training difficult. The degree of employees' perceptions of the necessity of sanitary training programs was marked at 4.18 points. According to the foodservice employees' evaluation about the knowledge of sanitation concerning the sanitary training program contents, the area of environmental sanitation (96.3%) was the highest while food poisoning control (72.9%) was lowest. Foodservice employees' practice levels was ranked above 4 points (out of 5 points) in 9 areas. Practice levels of cleaning and disinfection management were highest while that of safety management was lowest. About the food service employees' practice levels of sanitation, the degree of practice and the application of knowledge was 4.39 points. There was a significantly positive correlation between the practice level of sanitation and sanitation knowledge(p<.01). Thus, educational material needs to be standardized in order to improve employees' sanitation practice level.
