채식을 하는 수유부의 수유기간별 모유분비량과 수유양식에 관한 연구 (A Longitudinal Study on Human Milk Volume and Lactational Performance of Korean Lacto-ovo-vegetarians)

  • The longitudinal changes on human milk volume per day and lactational performance of 23 Korean lacto-ovo-vegetarians(primiparae=11, multiparae=12) from 0.5 to 5 months after parturition have been studied by test weighing method. The human milk volume per day tended to increase during lactation. The mean volume (SD, ml/day) at 0.5, 1, 2, 3, 4, 5 months were 502(102), 692(127), 697(100), 684( 125), 757(52), 703(70), respectively. The overall mean volume was 661$\pm$132ml/day. The peak milk volume during the lactation was observed at most lactating women at the 1st month. The distributions of the individual mean volume during the first 5 months of lactation were found over 750m1(8.7% ), 650-750ml(47.9% ), 550-650m1(2l.7% ), and 450-550ml(2l.7% ) The number of feeding per day was 8.1 ($\pm$1.0) at 0.5 months postpartum, which was consistently decreased. However, the mean volume per feeding was increased from 62($\pm$9.5)ml at 0.5 month to 112($\pm$13.3)ml at 5 months postpartum. The milk volume was correlated with the peak volume. and maternal age. but not with weight before delivery. maternal height and birth weight. No differences of milk volume and lactational performance between facto-ovo-vegetarian and nonvegetarian women were observed. This study suggests that the human milk volume cited in the recommended dietary allowances for Koreitns is over estimated.

수유부와 비수유부의 섭식과 체지방 및 인체계측의 비교 연구 (Postpartum Changes in Maternal Diet, Body Fat and Antropometric Measurements in Lactating vs Nonlactating Women)

  • Maternal body weight, skinfold thickness and circumference measurements were examined in 23 lactating women and 10 nonlactating women longitudinally from 1 week to 12 weeks postpartum. The dietary intakes of Korean lactating and nonlactating women were measured by 24-hour recall method and effects of maternal energy intake on body weight change were analyzed by Pearson correlation. The mean daily energy intake of breast-feeding(BF, n=12), combination of breast and formula-feeding (CF, n=11), and formula-feeding (FF, n=10) mothers were 1941kcal, 187kcal and 1727kcal, respectively. A significant decrease in weight was observed at all group. Weight losses at 12 weeks of postpartum in BF, CF and FF motheres were 3.18kg, 3.91kg and 5.15kg respectively. Weight losses increased as calorie intake decreased (${\gamma}$2=0.3803, p<0.05). The skinfold thickness and circumferences on all regions were decreased significantly except triceps and upper arm circumferences. However, there were no significant differences between lactating and nonlactating women. Anthropometric measurements decreased at the trunk but not at the limbs. Weight change was significantly correlated with changes of the scores for proximal circumference (${\gamma}$2=0.4999, p<0.05). There results suggest that lactation does not promete weight loss in well-nourished women and that the Korean RDA for energy in lactation may be too high.

수유 초기 모유 중 타우린 함량과 영아의 섭취량 추정 (Taurine Level in Human Milk and Estimated Intake of Taurine by Breast -Fed Infants during the Early Period of Lactation)

  • Taurine is only supplied to the infants from the breast-fed or formula milks because the enzyme activities of taurine biosynthesis are limited in early stages of infants . The objectives of present study were to quantitate the contents of tarurine in human milk and to estimate the intake of taurine by breast-fed infants during early period of lactation. Thirty -three lactating women, volunteered and delivered in R hospital in Serou. were recruited. Milk samples were collected every day at B1-B5 day(from 1 to 5 day dafter start of milk secretion), 15th and 30th day postpartum. Taurine contents were determined by HPLC equipped with RF-detector. The intake of taurine by infants was estimated by multiplization with the infant milk intake reported in our laboratory. The content of taurine was 406$\pm$174nmol/ml at B1-B5 day, and then gradually decreased to 359$\pm$125nmol/ml at 15th day and to 304$\pm$94nmol/ml at 30th day postpartum. The estimated intake of taurine was almost same as 24-25mg/day at B1-B5 day, and 15th , 30th days postpartum . This results was due to the increase of the intake of milk by infants.

한방산후조리가 산모의 혈색소 변화에 미치는 영향에 대한 연구 (The Effect of the Oriental Postpartum Care in Hemoglobin Changes of Maternity)

  • Objectives : Anemia is the center of interest in every maternity, both during pregnancy and lactation. The oriental postpartum care has one principle : to invigorate Qi and blood greatly. In this study, we seek to confirm that most sufferers of anemia recover under oriental postpartum care. To assure that this study had significance in non-iron supplement, we made a comparative study between oral iron supplement and non-iron supplement groups in oriental postpartum care. Methods : A comparative study was conducted to discern the difference in hemoglobin change between admission and after 2 weeks under the circumstances of 30 oral iron supplemented and 38 non-iron supplemented women who were admitted to the hospital attached to Woosuk University Oriental Medical College from April to August 2001 for postpartum care. Results : The mean hemoglobin level of after 2 weeks was 11.95 mg% (p<0.001). The mean hemoglobin level after 2 weeks was 11.65 mg% and 12.18 mg% in the iron supplemented and non-iron supplemented women respectively. The difference between the two groups was not significant. Conclusions : Oriental postpartum care overcomes anemia significantly in postpartum maternity. This is confirmed significantly under the non-iron supplement.

수유기간별 모유의 엽산 함량과 수유부의 엽산영양상태 (Folate Content of Human Milk During Extended Lactation and Folate Nutritional Status of Lactating Women in Korea)

  • Lactating women have an increased need of folate in the breastfeeding period and, as a consequence, may be in risk of folate deficiency. Folate content of breast milk, furthermore, is important for infants to support exponential growth. However, little is known about the folate content of breast milk from Korean lactating women and their folate nutritional status. In this study, therefore, we investigated the folate status of Korean lactating women and the folate content of their breast milk during extended lactation. A total of 10 subjects who delivered full-term infants participated this study voluntarily. Dietary folate intakes were measured and blood and breast milk were collected at 1, 2, 3, and 6 months postpartum. The women who did not take folic acid supplements failed to meet the recommended intake(RI) of folate for lactating women during all the study periods but those who did met the RI. The unsupplemented women showed lower plasma folate concentrations compared to the supplemented women and all the women were in suboptimal folate status determined by plasma folate concentration throughout the study periods. But the supplemented women showed lower prevalence of suboptimal folate status only at 3 or 6 months postpartum. Plasma folate concentrations of both groups decreased with the progression of lactation. Erythrocyte folate concentrations were not different between the two groups, however, that of the unsupplemented reduced further as time progressed. Plasma homocysteine levels were not different between the two groups. Concentrations of erythrocyte folate and plasma homocysteine were not changed throughout the study periods. Folate contents of their breast milk through the study periods were not different between the two groups and it decreased as lactation progressed in both groups. The results of this study suggest that the folate nutritional status of Korean lactating women might be deteriorated with the progression of lactation without folic acid supplements.

Effects of Supplemental Recombinant Bovine Somatotropin (rbST) and Cooling with Misters and Fans on Renal Function in Relation to Regulation of Body Fluids in Different Stages of Lactation in Crossbred Holstein Cattle

  • The aim of this study was to investigate the effect of supplemental recombinant bovine somatotropin (rbST) and cooling with misters and fans on renal function in relation to regulation of body fluids in different stages of lactation in crossbred Holstein cattle. Ten, 87.5% crossbred Holstein cattle were divided into two groups of 5 animals each, housing in a normal shaded barn (NS) and in a shaded barn with a mister-fans cooling system (MF). The experiment in each group was divided into 3 phases, early- (Day 75 postpartum), mid- (Day 135 postpartum), and late stage of lactation (Day 195 postpartum). The pre-treatment study was conducted on the starting day of each stage of lactation and the treatment study was performed after the end of the pre-treatment, during which the animal was injected with 500 mg of rbST (POSILAC) every 14 days for three times. During the study, ambient temperature at the hottest period daily in the MF barn was significantly lower, while relative humidity was higher than that of the NS barn. The temperature humidity index (THI) in both barns ranged from 79-85 throughout the periods of study. Cows in the MF barn showed a lower rectal temperature and respiration rate as compared with cows in the NS barn. The effect of rbST administration increased both rectal temperature and respiration rates of cows housed in either the NS or MF barn. Milk yield significantly increased in cows treated with rbST in all stages of lactation. Increases in mammary blood flow, accompanied by increases of total body water (TBW), extracellular fluid (ECF), blood volume (BV) and plasma volume (PV), were observed in both groups of cows receiving rbST in all stages of lactation. No alterations of renal blood flow and glomerular filtration rate were observed in cows receiving rbST, but decreases in urinary excretion and fractional excretion of sodium, potassium and chloride ions appeared to correlate with reduction in the rate of urine flow and osmolar clearance during rbST administration. These results suggest that the effect of rbST supplementation to cows housed either in NS or MF barns on body fluid volume expansion is attributable to changes in the rate of electrolyte excretion by the kidney. The increased availability of renal tubular reabsorption of sodium, potassium and chloride ions during rbST treatment was a major factor in retaining body water through its colligative properties in exerting formation of an osmotic force mechanism.

Effect of Dietary Cation-Anion Difference during Prepartum and Postpartum Periods on Performance, Blood and Urine Minerals Status of Holstein Dairy Cow

  • Twenty four periparturient cows were used to determine the effects of DCAD on acid-base balance, plasma and urine mineral concentrations, health status, and subsequent lactation performance. Each group of 12 cows received either a diet containing -100 DCAD or +100 DCAD for 21 d prepartum. Both anionic and cationic groups were divided into two groups, one received a +200 DCAD and the other +400 DCAD diet for 60 d postpartum. Prepartum reduction of DCAD decreased DMI, urinary and blood pH, urinary concentrations of Na or K and increased plasma and urinary Ca, Mg, Cl and S. Also cows fed -100 DCAD diet consumed the most dry matter in the first 60 d after calving. Postpartum +400 DCAD increased milk fat and total solid percentages, urinary and blood pH and urinary Na and K concentrations, but urinary Ca, P, Cl and S contents decreased. Greater DMI, FCM yields were observed in cows fed a diet of +400 DCAD than +200 DCAD. No case of milk fever occurred for any diets but feeding with a negative DCAD diet reduced placenta expulsion time. In conclusion, feeding negative DCAD in late gestation period and high DCAD in early lactation improves performance and productivity of dairy cows.

모유 영양아의 단백질 섭취량과 성장 (Changes on Protein Intake and Body Weight of Breast-fed Infants during Lacation)

  • In order to investigate the protein intake of breast-fed infants, we examined sixty infants during the first 5 months of lactation. Human milk intake infants increased during lactation with the respective values of 525, 671, 734, 744 , 765 and 768g/day t 0.5, 1, 2, 3, , 4 and 5 months postpartum. The average protein contents of human milk showed 1.58, 1.38, 1.23, 1.11, 1.08and 1.07g/100g respectively. The protein intake of boys during the first 5 months of lactation averaged 9.11g/day which was higher than the 7.71g/day average for girls. Body weight of infants at birth was 3337g, which increased significantly during lactation. The protein intake per body weight of breast-fed infants in boys was significantly higher than that in girls(p<0.01). Protein intake of breast-fed infants had survey, a revaluation of the protein intake and recommended dietary allowance of protein during early infancy should be considered.

수유기간의 경과에 따른 한국인 인유의 칼슘 및 인 함량의 변화 (Longitudinal Changes in Calcium and Phosphorus Concentrations of Korean Human Milk)

  • 수유기간의 경과에 따른 한국인 인유의 수분, 총고형분, 회분, 칼슘, 인, 칼슘/인의 비 등의 변화를 파악하기 위하여 초유(분만후 0∼5일), 이행유(분만 후 6∼10일), 성숙유(분만 후 25∼35일째 및 분만 후 55∼70일째) 단계로 나누어 longitudinal로 추적·조사하였으며, 시유 및 조제분유와의 비교도 행하였다. 수분과 총고형분은 분만 후 22∼35일에서만 초유에 비하석 유의한 차이를 보여주었으며, 회분은 수유기간의 경과에 따라 점차 감소하는 경향이었다. 칼슘 및 인은 초유단계에 비하여 이행유, 성숙유단계 모두 유의하게 높았는데 칼슘은 분만 후 55∼7O일까지, 인은 분만후 25∼35일까지 계속 증가의 경향을 보여준 것이 특이하였다. 칼슘/인의 비는 초유단계에서 2.28, 이행유단계에서 1.76, 분만 후 25∼35일에서 1.68 그리고 분만 후 55∼70일에서 1.95였는데 이행유 이후 단계에서 모두 유의하게 낮았다. 회분, 칼슘, 인, 칼슘/인의 비에서 초산부와 경산부간에는 차이가 없었다. 성숙유의 총고형분 함량을 시판 국산 조제분유의 조유농도로해서 계산하여 본 바 회분과 칼슘/인의 비는 인유의 그것에 비슷하였으나 칼슘과 인은 높은 수준이었다.

분만 첫 15일간 이행유 분비량의 변화 (Changes in the Transitional Milk Yields during the First 15 Days Postpartum)

  • The longitudinal changes in the transitional milk yields of Korean lactating women(14 primiparae, 11 multiparae) have been studied by test-weighing method in the part of Kangwon Province form 7 day to 15 days postpartum. The transitional milk yields at 7, 10, 15 days postpartum were 531$\pm$148,598$\pm$156 and 639$\pm$169g, respectively. The mean milk yield was 589$\pm$162g from 7 to 15 days postpartum. The transitional milk yields between primiparae and multiparae appeared not significantly different but significantly different between mothers of boys and girl(p<0.05). The distribution of individual transitional milk yields were found 550-649g(28.0%), 450-549g(24.0%), 650-749g(13.3%) and 750-849g(13.3%). The transitional milk yields were not affected by mother's age, weight gain during pregnancy, gestational period and infant's weight at birth but affected by maternal height(p<0.05). Maternal weight reduction during the lactation had no correlation with the transitional milk yeilds.
