Predictors of Maternal Identity of Korean Primiparas

  • Purpose: The purpose of this study was to identify factors influencing maternal identity of Korean primiparas. Methods: The data were collected by a self-report questionnaire in 2006. The participants were 210 healthy primiparous women who delivered at one of three medical centers and revisited the outpatient department for follow up between 4 to 6 weeks after childbirth. Data were analyzed using the SPSS WIN 17.0 program with descriptive statistics, t-test, one way ANOVA, Pearson correlation coefficient, and stepwise multiple regression. Results: Maternal identity was significantly correlated with Taekyo accomplishment, the culturally based prenatal preparation (p<.001). Transitional gratification to motherhood (p<.001), postpartum depression (p<.001), childcare stress (p<.001), infant temperament (p<.001), and social support (p<.001) were also significantly correlated with maternal identity. The stepwise multiple regression analysis showed that maternal identity was significantly predicted by transitional gratification to motherhood, infant temperament, childcare stress, and Taekyo accomplishment. These variables explained 31.0% of the variance of maternal identity. Conclusion: The results of this study suggest that postpartum nursing interventions to promote maternal identity should focus on reinforcing education and support for reducing childcare stress and infant difficulty, and increasing transitional gratification to motherhood. Also, prenatal encouragement and education for improving Taekyo accomplishment may be helpful to promote maternal identity after birth.

산후조리원의 모자건강관리 현황과 제도화방안 - 지역사회중심의 모자건강관리센터(TMIC) 개발을 위한 전략 - (A Model for Community Based Mother Infant Care Center - TMIC(transitional mother infant care center) using a Sanhujoriwon -)

  • The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

향요법마사지의 심리신경면역학적 효과 (Psychoneuroimmunologic Effect of Aromatherapy massage)

  • The purpose of this study was to explore the psychoneuroimmunologic effect of aromatherapy massage and to compare the effect with that of massage. This study is designed as a nonequivalent control group pretest-posttest quasi-experimental study and each twenty postpartum mothers volunteered the experimental group and control group. For experimental group, researcher used massage with 2% Jojova oil mixed with Lavender and Rosemary oil (Bush Boake Allen Inc., USA) for 20 minutes, whereas for control group Jojova oil was used as luburicant for massage. Skin temperature was measured with YSI Tele-thermometer(Simpson electric Co., USA) and the concentration of IgA in salivary was analyzed by immunoturbididimetric assay(Cobas INTEGRA, Roche, Swiss) at pre and post test. Also at this time fatigue were measured by Rhoten Fatigue Scale through self-report. The data were analyzed using SAS and hypothesis was tested with unpaired t-test. The results were as follows : 1) Score of fatigue decreased significantly after use of aromatherapy massage. 2) Skin temperature increased significantly after use of aromatherapy massage. 3) Concentration of salivary IgA increased significantly after use of aromatherapy massage. In conclusion, from the standpoint of psychoneuroimmunologic view, the results suggest that aromatherapy massage have greater effect compared with massage and can be effective nursing intervention to enhance relaxation of mind and body in the postpartum mothers.

모유수유 교육이 초산모의 모유수유 방법, 태도 및 지속정도에 미치는 효과 (The Effect of Breast-feeding Education on Method, Attitude, and Duration of Breast-feeding by Primipara)

  • The purpose of this study is to investigate the effect of breast-feeding education on skills, attitude, and duration of breast-feeding among primipara mothers. The educational session was offered during the period of June through September at the T medical center in T city. One hundred thirteen primiparas were selected for this study, 61 who received breast-feeding education(lecture, demonstration, and additional telephone counselling) served as the experimental group and 52 who did not as the control. Data collection was done by self-reporting method. the reliability of the instrument was from .77 to .87. Duration of breast-feeding was classified into I through V according to the WHO classification at the time point of 4 weeks of postpartum. The results were as follows : 1. The score for the experimental group on the skills of breast-feeding techniques was significantly higher than those of the control(t=9.12, p=.000). 2. The score for the experimental group in the attitude toward breast-feeding was significantly higher than those of the control(t=6.12, p=.000). 3. Duration of breast-feeding for the experimental group was significantly longer than that of the control at the time of 4 weeks of postpartum($x^2$=20.79, p=.001). In conclusion, the breast-feeding practice can be promoted effectively through the breast-feeding education.

산후 첫주간의 모유수유 문제에 관한 연구 (Breast Feeding Practice according to the Breast-Feeding Problem during the First Week of Postpartum)

  • In Korea, breast-feeding practice has been decreased since 1980's. The purpose of this study was to determine the factors that influence breast-feeding practice. The total numbers of study subjects were 139 mothers in their postpartum one week. The study was carried out for the period of three months from June 17 to August 26, 1998. The tool for breast-feeding problem was developed by Kearney et. al(1990). The data were analyzed by SAS and the results were as follows ; 1. The reported breast-feeding problems were nipple pain, demand of frequent feeding, prolonged feeding, milk leakage, experience of difficulty in breast feeding. 2. Breast-feeding problem defined were prolonged feeding, baby's whimpering after breast-feeding, nipple pain, experience of difficulty in breast feeding, difficulty of breast feeding due to mother's general discomfort.

도시와 농촌 지역 성인여성의 생활양식, 건강실태 및 삶의 질 비교 (A Comparison of Lifestyle, Health Status and Quality of Life of Adult Women living in Urban and Rural Areas)

  • Purpose: This study was done to investigate the need to develop health promotion programs for adult women and to compare lifestyle, health status and quality of life in adult women in urban and rural areas. Method: The participants were women over 20 years old, 451 living in 3 cities and 436 living in 1 rural areas. Data collection was conducted from April 6 to August 30, 2004. Results: For lifestyle, the percentage of women having regular medical examinations, cholesterol tests, regular exercise, and high alcohol intake were significantly higher for urban women compared to the rural women. For health status, the percentage of women with health problems such as arthritic pain, urinary incontinence, pregnancy and postpartum complications, and the experience of violence were significantly higher for rural women compared to urban women. Rural women had significantly lower scores for health perception compared to urban women. For quality of life, rural women had significantly higher scores for quality of life, especially for the psychological wellbeing and stability subscales. Conclusion: The above findings indicate that it is necessary to develope a health promotion program which reinforces healthy lifestyle and health status for rural women, and quality of life, for urban women.

한방병원 산후조리센터 산모를 대상으로 한 모유수유 실태 및 인식조사 (A Survey on Understanding and Actual Condition of Breast-feeding of Woman at Oriental Medicine Hospital Postpartum Care Center)

  • at Jeonju Wonkwang University Oriental Medicine hospital postpartum care center. Results The majority of women(79.2%) had planned breast-feeding. However, only 32.1% of women currently have fed breast-feeding. 58% of mothers who did partial breast-feeding and breast-feeding currently have planned to breast-feed more than 7 months. The main reason of bottle-fed is concern of insufficient amount of breast milk(72.2%). Also the main reason of breast-feeding is good for baby's health(90%). The average score on the test of the understanding about the methods and advantages of breast-feeding was $13.98{\pm}1.92$ out of total 18.49.1% of women thought that taking herbal medicine would affect baby during breast-feeding. Also 71.7% of women thought that herbal treatments (acupuncture, herbal medicine) would be helpful when the amount of breast milk is insufficient. Conclusions This survey has showed that in spite of full breast-feeding plan, the percentage of actual breast-feeding rate is low. Timely education is needed to increase breast-feeding rate. It is worth to consider herbal treatment to increase breast milk amount. In addition to that it is necessary to create a hospital environment in which mothers can easily begin breast-feeding as soon as possible after delivery. Moreover, beyond the medical field, political and social supports for breast-feeding are needed.

임신 및 출산 여성의 요실금 및 대변실금 예방을 위한 케겔운동의 효과: 체계적 문헌 고찰 (Effect of Kegel Exercise to Prevent Urinary and Fecal Incontinence in Antenatal and Postnatal Women: Systematic Review)

  • Purpose: The aim of this study was to review the literature to determine whether intensive pelvic floor muscle training during pregnancy and after delivery could prevent urinary and fecal incontinence. Methods: Randomized controlled trials (RCT) of low-risk obstetric populations who had done Kegel exercise during pregnancy and after delivery met the inclusion criteria. Articles published between 1966 and 2012 from periodicals indexed in Ovid Medline, Embase, Scopus, KoreaMed, NDSL and other databases were selected, using the following keywords: 'Kegel, pelvic floor exercise'. The Cochrane's Risk of Bias was applied to assess the internal validity of the RCT. Fourteen selected studies were analyzed by meta-analysis using RevMan 5.1. Results: Fourteen RCTs with high methodological quality, involving 6,454 women were included. They indicated that Kegel exercise significantly reduced the development of urinary and fecal incontinence from pregnancy to postpartum. Also, there was low clinical heterogeneity. Conclusion: There is some evidence that for antenatal and postnatal women, Kegel exercise can prevent urinary and fecal incontinence. Therefore, a priority task is to develop standardized Kegel exercise programs for Korean pregnant and postpartum women and make efficient use of these programs.

Does bilateral uterine artery ligation have negative effects on ovarian reserve markers and ovarian artery blood flow in women with postpartum hemorrhage?

  • Objective: Bilateral uterine artery ligation (UAL) is a fertility-preserving procedure used in women experiencing postpartum hemorrhage (PPH). However, the long-term effects of this procedure on ovarian function remain unclear. The aim of this study was to investigate whether bilateral UAL compromised ovarian reserve and ovarian blood supply. Methods: This prospective study included 49 women aged between 21 and 36 years who had undergone a cesarean section for obstetric indications. Of these, 25 underwent uterine bilateral UAL to control intractable atonic PPH. The control group consisted of 24 women who had not undergone bilateral UAL. Standard clinical parameters, the results of color Doppler screening, and ovarian reserve markers were assessed in all participants at 6 months after surgery. The clinical parameters included age, parity, cycle history, body mass index, and previous medication and/or surgery. Color Doppler screening findings included the pulsatility index (PI) and resistance index (RI) for both the uterine and ovarian arteries. The ovarian reserve markers included day 3 follicle-stimulating hormone (FSH) levels, antral follicle count, and $anti-M\ddot{u}llerian$ hormone (AMH) levels. Results: There were no significant differences in the ovarian reserve markers of day 3 FSH levels, antral follicle count, and AMH levels between the study and control groups (p> 0.05 for all). In addition, no significant differences were observed in the PI and RI indices of the uterine and ovarian arteries (p> 0.05 for all). Conclusion: In this study, we showed that bilateral UAL had no negative effects on ovarian reserve or ovarian blood supply, so this treatment should be used as a fertility preservation technique to avoid hysterectomy in patients experiencing PPH.

허리 안정화 운동과 병행한 엉덩근육 강화 운동이 제왕절개 분만 후 만성허리통증을 가진 여성의 통증, 기능장애지수, 산후우울증에 미치는 영향 (Effects of the Gluteus Muscle Exercise combined with the Lumbar Stabilization Exercise on Pain, ODI, and Postpartum Depression in Women with Chronic Back Pain After C-section Delivery)

  • Background: This study aimed to investigate the effects of combining the lumbar stabilization exercise with the gluteal muscle strength exercise on pain, the dysfunction index, and postnatal depression in women experiencing chronic low back pain after a cesarean section. Methods: The study used a randomized design with two groups: one group (n=15) performed the lumbar stabilization exercise along with the gluteal muscle strength exercise, while the other group (n=15) only did the lumbar stabilization exercise. The intervention consisted of 30 minute exercise sessions, twice a week, for 6 weeks. The pain was assessed using the visual analog scale (VAS), the dysfunction index with the Oswestry disability index (ODI), and postnatal depression using the edinburgh postnatal depression scale-Korean version (EPDS-K). Results: Both groups showed significant improvements in their VAS, ODI, and EPDS-K scores within their respective groups (p<.05). Additionally, the between-group analysis revealed significant differences in post-test results (p<.05). Conclusion: The combination of the lumbar stabilization exercise and the gluteal muscle strength exercise proved to be more effective in reducing pain, improving the dysfunction index, and alleviating postnatal depression in women with chronic low back pain after a cesarean section. Therefore, incorporating these exercises could be beneficial as an intervention program for women experiencing chronic low back pain after a cesarean section.
