Efficacy of Decompression and Fixation for Metastatic Spinal Cord Compression : Analysis of Factors Prognostic for Survival and Postoperative Ambulation

  • Objective : The goals of surgical intervention for metastatic spinal cord compression (MSCC) are prolonging survival and improving quality of life. Non-ambulatory paraplegic patients, either at presentation or after treatment, have a much shorter life expectancy than ambulatory patients. We therefore analyzed prognostic factors for survival and postoperative ambulation in patients surgically treated for MSCC. Methods : We assessed 103 patients with surgically treated MSCC who presented with lower extremity weakness between January 2001 and December 2008. Factors prognostic for overall survival (OS) and postoperative ambulation, including surgical method, age, sex, primary tumor site, metastatic spinal site, surgical levels, Tokuhashi score, and treatment with chemo- or radiation therapy, were analyzed retrospectively. Results : Median OS was significantly longer in the postoperatively ambulatory group [11.0 months; 95% confidence interval (CI), 9.29-12.71 months] than in the non-ambulatory group (5.0 months; 95% CI, 1.80-8.20 months) ($p$=0.035). When we compared median OS in patients with high (9-11) and low (0-8) Tokuhashi scores, they were significantly longer in the former (15.0 months; 95% CI, 9.29-20.71 months vs. 9.0 months; 95% CI, 7.48-10.52 months; $p$=0.003). Multivariate logistic regression analysis showed that preoperative ambulation with or without aid [odds ratio (OR) 5.35; 95% CI 1.57-18.17; $p$=0.007] and hip flexion power greater than grade III (OR 6.23; 95% CI, 1.29-7.35; $p$=0.038) were prognostic of postoperative ambulation. Conclusion : We found that postoperative ambulation and preoperative high Tokuhashi score were significantly associated with longer patient survival. In addition, preoperative hip flexion power greater than grade III was critical for postoperative ambulation.

천막상부 악성 신경교종에서 수술 후 방사선 치료의 역할 - 생존율과 예후인자 분석 - (Role of Postoperative Conventional Radiation Therapy in the Management of Supratentorial Malignant Glioma - with respect to survival outcome and prognostic factors -)

  • 목적 : 천막상부 약성 신경교종의 치료로서 수술 후 방사선 치료의 역할을 생존율을 중심으로 평가하며 그 예후 인자를 분석한다. 대상 및 방법 : 1985년 9월부터 1997년 3월까지 수술 후 방사선 치료를 받은 69례의 환자를 대상으로 분석하였고 나이의 범위는 7세에서 66세이며 중앙값은 47세였다. 42례(61$\%$)는 다형성 교모세포종 이었고 27례(39$\%$)는 악성 성상세포종이었다. 20례(29$\%$)는 Karnofsky 역할수행능력 점수가 80점 이상이었다. 43례(62$\%$)는 수술 전 3개월 이내에 증상을 호소하였다. 24례 (35$\%$)는 육안 상 완전 절제를, 40례(58$\%$)는 부분 절제를, 나머지 5례(7$\%$)에서는 뇌정위 조직검사만 시행하였다. 총 방사선 조사량은 50.4-61.2Gy(중앙값, 55.8; 최빈값, 59.4)로 하루 1.8-2.0 Gy 씩 33-83일 동안(중앙값, 48) 치료하였다. 단 3례는 수술 후 전신 상태의 불량으로 하루 3.0Gy 씩 각각 33, 36, 39Gy 까지 치료하였다. 추적 관찰율은 93$\%$였고 중앙간은 14개월이었다. 결과 : 전체 환자의 2년 및 3년 생존율과 중앙 생존기간은 각각 38$\%$, 20$\%$, 16개월이었다. 악성 성상세포종과 다형성 교모세포종의 중앙생존기간은 각각 34, 14개월이었다(p=0.0001). 완전절제, 부분절제, 뇌정위 조직검사군에서의 3년 생존율은 각각 38$\%$, 11$\%$, 0$\%$였다(p=0.02). 연령이 40세 미만, 40-59, 60세 이상 군의 3년 생존율은 각각 52$\%$, 8$\%$, 0$\%$였다(p=0.0007). 역할수행 능력점수 80점 이상과 80점 미만군의 3년 생존율은 각각 53$\%$, 9$\%$였파(p=0.008). 상기와 같이 절제 정도에 따른 3개의 수술 군과 3개의 연령 군을 포함한 다요인 분석에서 병리학적등급, 수술 정도, 연령이 유의한 인자였다. 그러나 2개의 수술 군(완전절제와 비완전 절제)과 2개의 연령 군(50세 미만과 50세 이상)을 공변량으로한 다요인 분석에서 연령대신 병리학적 등급, 수술 정도, 역할수행 능력이 유의한 인자였고, 이들 세 가지 요인을 갖춘 5례는 치료와 관련한 심각한 부작용 없이 3년 누적생존율이 100$\%$로 생존하고 있다. 결론 : 악성 신경교종의 치료에서 수술 후 방사선치료를 추가함으로써 기존의 수술 단독의 생존율을 향상시키는 역할을 확인할 수 있었다. 악성 성상세포종 환자 중, 특히 양호한 역할수행 능력과 완전 절제를 받은 환자군의 생존율이 가장 양호하였다. 다형성 교모세포종 환자에서는 특히 젊은 나이인 경우 적절한 신경학적 기능보존과 함께 최대한의 종양 절제가 선행되어야 할 것으로 생각된다. 그러나 양호한 예후 인자를 갖지 못한 대부분의 다형성 교모세포종 환자군의 생존율은 여전히 좋지 않아 이 환자 군에서는 수술 후 통상적인 방사선 치료보다는 다른 적극적인 치료가 추가 또는 시도되어야 할 것으로 생각된다.

성문상부암 환자에서 예방적 경부곽청술의 임상적 연구 (A Clinical Study of prophylactic Neck Dissection in Supraglottic Cancer Patients)

  • Background and objectives : There may be frequent lymphatic neck metastasis among supraglottic cancer patients after their primary surgery. The aim of this study was to evaluate the effectiveness of prophylactic neck dissections in supraglottic cancer patients to lower the neck metastasis. Material and method : The authors retrospectively reviewed medical records of 52 patients (7 females. 45 males) who had received the operations for the supraglottic cancer from 1995 to 2000. They were preoperatively examined with Computer Tomographic images, and also examined by the cervical neck palpations. Postoperative pathologic specimens were reviewed and the Pathologic stagings were confirmed by the pathologist. Results : Among 52 patients who received surgery for supraglottic carcinoma, 5 patients received ipsilateral neck dissection, 12 patients received ipsilateral neck dissection with postoperative radiation therapy, 10 Patients received bilateral neck dissection and 25 patients received bilateral neck dissection with postoperative radiation treatment. In comparison of the data of N0 to N+ conversion rate of neck, there were 11 ipsilateral T3 N0 patients and 2 patients(18%) were converted to N+ status, and there were 8 Patients with T4 N0 stages, and 3 patients(38%) were converted to N+ status. In comparing the data of contralateral neck dissection on N0 patient, there were 8 patients of T3 N0 patients, and there was 1 patient (13%) who showed recurrence. There were 5 patients of T4 N0 stages and there was 1(20%) patient with recurrences. When the ipsilateral N stages were N2a, and N2b, there 1 case of contraleral N+ conversions, respectively. In recurrences, there were 2 patients of N2b staged patients and 1 patient of N2c patient that showed recurrence during the follow up period. Conclusion : This data shows that at least an ipsilateral neck dissection is necessary in order to prevent the recurrences, and also suggests that bilateral neck dissection is necessary for the higher T stage. Since the higher N staged patients showed higher rate of recurrence, so that neck dissection is mandatory for the higher N staged patients.

두개 내 다형성아교모세포종 환자의 방사선치료 결과 (The Outcome of Glioblastoma Patients Treated with Surgery and Radiation Therapy)

  • 목적 : 두개 내 다형성아교모세포종 환자의 방사선치료 후 생존율을 조사하고 그 예후 인자의 중요성을 알아보고자 하였다. 대상 및 방법: 1994년부터 2003년까지 삼성서울병원에서 병리학적으로 다형성아교모세포종으로 확진된 환자 중 근치적 목적으로 방사선치료를 시행한 120명을 대상으로 분석하였다. 남자는 64명(53$\%$), 여자는 56명(47$\%$)이었으며, 20례(17$\%$)의 종양은 다발성이었다. 수술은 육안적 완전 절제, 부분 절제 및 조직검사를 각각 22례(18$\%$), 59례(58$\%$), 29례(24$\%$)에서 시행하였고, 방사선치료는 4$\~$10 WV 선형가속기를 이용하여 54$\~$72Gy (중앙값 60 Gy)를 조사하였다. 대상 환자의 추적관찰기간은 2개월에서 62개월(중앙값 12개월)이었다. 결과 : 전체 환자군의 1년 및 2년 생존율은 52$\%$와 14$\%$이었으며, 중앙생존기간은 13개월이었다. 예후 인자에 따른 1년 생존율은 50세 미만은 64$\%$, 50세 이상은 41$\%$ (p<0.01), 수술 전 활동도가 ECOG 0 혹은 1인 경우 는 57$\%$, 2 이상은 44$\%$ (p=0.03)였다. 두개 내 병변 수에 따른 1년 생존율은 단일병변은 57$\%$, 다발성 병변은 23$\%$ (p=0.02)이었으며, 절제정도에 따른 생존율은 육안적 완전 절제, 부분 절제, 조직검사만 시행한 경우에 각각 67$\%$, 48$\%$, 47$\%$ (p=0.04)였다. 다변량 분석에 따른 예후인자로는 여자(p<0.01), 50세 미만(p<0.01), 활동도가 ECOG 0 혹은 1 (p=0.05), 육안적 완전절제(p=0.05)가 좋은 예후 인자이었다 결론 : 근치적 목적으로 방사선치료를 시행한 두개 내 다형성아교모세포종 환자의 중앙생존기간은 13개월로 기존의 문헌보고와 비슷한 수준이었다. 치료성적의 향상을 위하여 다양한 방사선치료 기술의 적용을 통한 방사선량의 증가와 효과적인 항암화학요법 약제의 개발 및 사용 등의 연구가 필요하고, 환자의 예후인자에 따라 개별화된 치료방법이 적용되어야 할 것으로 판단된다

Thyroid Function after Postoperative Radiation Therapy in Patients with Breast Cancer

  • Objective: The aim of this study was to assess thyroid function in breast cancer patients exposed to therapeutic external beam radiation. The focus was on possible progressive changes and any relationships between the incidence of primary hypothyroidism, the time required to become hypothyroid, and factors such as chemotherapy, hormonotherapy and immunotherapy. Materials and Methods: Seventy females undergoing 3D conformal and IMRT radiation therapy for breast cancers were enrolled in a non-randomized prospective study. The patients was divided into two groups: those after mastectomy or breast conserving surgery (BCS) were irradiated to a scar of the chest wall/breast and the ipsilateral supraclavicular and the axillary areas (supraclavicular radiotherapy group - SC-RT group - 32 patients) and the control group receiving adjuvant chest wall/breast RT only (BCT group - 38 patients).The total doses were 50.0 to 70 Gy in 5 to 7 weeks. The median follow-up term was 24 months (range, 1-40 months). Thyroid function was evaluated by measuring thyroid stimulating hormone (TSH), free thyroxine (fT4), and free triiodothyronine (fT3) levels. The minimum, maximum and mean thyroid gland doses for 20 Gy (V20) were calculated for all patients. Results: Statistically significant results were obtained for the SC-RT group. Two yearsa fter the end of RT the chance of an event was increased in 6% of the population (p=0.009) in the SC-RT group. In the BCT group no significance was noted. No statistically significant differences were found for V20, chemio-, immunotherapy and hormonotherapy or Ki67 values (p=0.12). No significant results were obtained for development of hypothyroidism and clinical factors (age, thyroid volume, treatment modalities). Conclusion: Radiotherapy is associated with a higher incidence of thyroid toxicity in breast cancer patients. Routine thyroid function monitoring should be recommended in such cases.

Treatment outcomes after adjuvant radiotherapy following surgery for patients with stage I endometrial cancer

  • Purpose: The purpose of this study is to evaluate the treatment outcomes of adjuvant radiotherapy using vaginal brachytherapy (VB) with a lower dose per fraction and/or external beam radiotherapy (EBRT) following surgery for patients with stage I endometrial carcinoma. Materials and Methods: The subjects were 43 patients with the International Federation of Gynecology and Obstetrics (FIGO) stage I endometrial cancer who underwent adjuvant radiotherapy following surgery between March 2000 and April 2014. Of these, 25 received postoperative VB alone, while 18 received postoperative EBRT to the whole pelvis; 3 of these were treated with EBRT plus VB. The median EBRT dose was 50.0 Gy (45.0-50.4 Gy) and the VB dose was 24 Gy in 6 fractions. Tumor dose was prescribed at a depth of 5 mm from the cylinder surface and delivered twice per week. Results: The median follow-up period for all patients was 57 months (range, 9 to 188 months). Five-year disease-free survival (DFS) and overall survival (OS) for all patients were 92.5% and 95.3%, respectively. Adjuvant radiotherapy was performed according to risk factors and stage IB, grade 3 and lymphovascular invasion were observed more frequently in the EBRT group. Five-year DFS for EBRT and VB alone were 88.1% and 96.0%, respectively (p = 0.42), and 5-year OS for EBRT and VB alone were 94.4% and 96%, respectively (p = 0.38). There was no locoregional recurrence in any patient. Two patients who received EBRT and 1 patient who received VB alone developed distant metastatic disease. Two patients who received EBRT had severe complications, one each of grade 3 gastrointestinal complication and pelvic bone insufficiency fracture. Conclusion: Adjuvant radiotherapy achieved high DFS and OS with acceptable toxicity in stage I endometrial cancer. VB (with a lower dose per fraction) may be a viable option for selected patients with early-stage endometrial cancer following surgery.

Treatment outcomes of neoadjuvant concurrent chemoradiotherapy followed by esophagectomy for patients with esophageal cancer

  • Purpose: To evaluate treatment outcomes and determine prognostic factors in patients with esophageal cancer treated with esophagectomy after neoadjuvant chemoradiotherapy (NCRT) Materials and Methods: We retrospectively evaluated 39 patients with esophageal cancer who underwent concurrent chemoradiotherapy followed by esophagectomy between 2002 and 2012. Initial clinical stages of patients were stage IB in 1 patient (2.6%), stage II in 5 patients (12.9%), and stage III in 33 patients (84.6%). Results: The median age of all the patients was 62 years, and the median follow-up period was 17 months. The 3-year overall survival (OS) rate was 33.6% in all the patients. The 3-year locoregional recurrence-free survival (LRFS) rate was 33.7%. In multivariate analysis with covariates of age, the Eastern Cooperative Oncology Group performance status, hypertension, diabetes mellitus, tumor length, clinical response, clinical stage, pathological response, pathological stage, lymphovascular invasion, surgical type, and radiotherapy to surgery interval, only pathological stage was an independent significant prognostic factor affecting both OS and LRFS. The complications in postoperative day 90 were pneumonia in 9 patients, anastomotic site leakage in 3 patients, and anastomotic site stricture in 2 patients. Postoperative 30-day mortality rate was 10.3% (4/39); the cause of death among these 4 patients was respiratory failure in 3 patients and myocardial infarction in one patient. Conclusion: Only pathological stage was an independent prognostic factor for both OS and LRFS in patients with esophageal cancer treated with esophagectomy after NCRT. We could confirm the significant role of NCRT in downstaging the initial tumor bulk and thus resulting in better survival of patients who gained earlier pathological stage after NCRT.

수막 혈관주위세포종의 임상상, 치료 및 장기 추적 결과 (Meningeal Hemangiopericytomas : Clinical Features, Treatment and Long-term Outcome)

  • Objectives : Meningeal hemangiopericytoma is a rare tumor. Clinical and radiological features are similar to those of an ordinary meningioma. But its biological behavior is quite different from those of a meningioma as it brings profuse bleeding in the operation field, frequent recurrence and metastasis to other systemic organs even in the case of gross total resection. In order to find out the prognostic factors and to compare longterm outcome after various treatment modalities, the authors reviewed consecutive 20 operated cases of meningeal hemangiopericytoma to characterize their clinical features, surgical outcomes and effectiveness of the radiation therapy. Methods : Twenty patients with a hemangiopericytoma were treated between 1982 and 1999 in our department. They are composed of 13 patients of hemangiopericytoma and 7 patients who were initially diagnosed as angioblastic type meningioma and then confirmed as a hemangiopericytoma after review of their previous histopathology slides. The mean follow-up period was 99 months(1-256 months). The long-term outcomes after various treatment modalities were evaluated according to recurrence-free survival and image follow-up. Recurrence-free survival curves are compared between the patient groups according to the extent of removal and radiation therapy. Results : There were 10 cases of recurrence and 4 cases of distant metastases ; lung, liver and femur(2 cases) respectively. The 5-year recurrence rate was 37%, while the 10-year recurrence rose to 77%. There was also statistically significant difference of median recurrence-free survival between the completely-resected group(Simpson grade 1 or 2) and partially-resected group(Simpson grade 3 or 4 or 5) ; 137 months compared to 47 months, respectively(p=0.009). The median recurrence-free period after subtotal resection of tumor and postoperative radiotherapy was 47 months compared to 117 months of the patients who underwent gross total resection of tumor and did not receive radiotherapy. But radiation therapy alone did not show significant difference in recurrence-free survival. Marked tumor volume reduction and easy removal of tumors without bleeding were found in 2 recurred cases. Conclusion : The extent of tumor resection and presence of metastasis are the most important factors related to long-term outcome of the patients with hemangiopericytoma. Radiation therapy after the first operation did not have a role in extending the recurrence-free survival, but it affected favorably to decrease the size of residual mass and intraoperative bleeding during the second operation.

Value of Postoperative Radiation Therapy for Regional Control after Dissection in Head and Neck Squamous Cell Carcinoma Cases

  • Objective: We aimed to define clinicopathologic risk factors associated with regional recurrence (RR) and thus the effectiveness of postoperative radiotherapy (PORT) for neck control for head and neck squamous cell carcinomas (HNSCCs) with differing cervical lymph node status. Methods: A retrospective study was performed in 196 HNSCC patients with pathologically positive neck node (N+) to evaluate the high-risk factors for RR and to define the role of PORT in control after neck dissection and postoperative radiotherapy (PORT). Results: Overall, the RR rate after neck dissection and PORT was 29%. Extracapsular spread (ECS) was confirmed to be the only independent risk factor for RR. There were no significant risk factors associated with RR in the ECS- group. The 5-year disease-specific survival rate was 45%, which descended to 10% with the emergence of RR. Conclusions: ECS remains a determined risk factor for RR after neck dissection and PORT in patients with N+. PORT alone is not adequate for preventing RR in the neck with ECS after neck dissection. More intensive postoperative adjuvant therapies, especially combined chemotherapy and radiotherapy, are needed to prevent regional failure in HNSCC patients with ECS.

Radiation Therapy Following Total Keloidectomy: A Retrospective Study over 11 Years

  • Background Radiotherapy treatment after keloidectomy is known to be an effective method for reducing the rate of recurrence. However, to date, the appropriate total radiation dose and fractionation have not yet been confirmed. The authors performed a retrospective analysis to identify the appropriate radiation dose and fractionation in post-keloidectomy radiotherapy. Methods From May 2000 to February 2011, postoperative radiotherapy was performed on 39 lesions in 28 patients after total keloidectomy. The keloid lesions were confined to the ear lobes. Between May 2000 and May 2004, 14 keloids were treated with surgical excision, followed by a total radiation dose of 1,200 cGy in three fractions over four to five days (group 1). Between June 2004 to February 2011, 25 keloids were treated with surgical excision, followed by a total radiation dose of 1,500 cGy in three fractions over four to five days (group 2). Patients were given a survey asking them to report their experiences regarding reoperation, recurrence of symptoms, recurrence of the lesion, and satisfaction with the operation. Results Of the 28 patients who were treated, 20 underwent follow-up. Group 2 had more cases showing elevation with erythematous changes, whereas group 1 had more cases showing progressive stages of elevation than group 2. These differences were statistically significant. Moreover, a correlation was observed between the level of keloid elevation and the extent of symptoms. Conclusions We suggest 1,500 cGy of radiation in three fractions following keloidectomy for ear lobe keloids. A further randomized study is needed to assess the recurrence of keloids after radiotherapy.
