폐경전,후 여성에서 골다공증의 위험요인과 요추와 대퇴골의 골밀도 비교분석 (A Statistical compare analysis of bone mineral density of lumbar spine and femur, Risk Factors for Osteoporosis in the women of pre, postmenopausal)

  • 골다공증은 대표적인 성인병으로 유병율이 높고, 쉽게 골절을 일으켜 여러 가지 합병증을 유발하여 노후의 삶의 질을 좌우하는 중요한 질환이다. 폐경전,후 여성을 대상으로 골다공증 발생에 관련된 위험요인과 골다공증에 의한 골절시 환자에게 치명적인 결과를 초래하고, 유병률 및 사망률에 가장 큰 영향을 미치는 요추와 대퇴골의 골밀도를 측정하여 비교 분석하고자 본 연구를 실시하였다. 본 연구는 2004년 1월 1일부터 2004년 11월 30일 까지 11개월동안 D 보건소에 내원한 고객중 요추와 대퇴골 골밀도 검사를 시행한 $40{\sim}60$대 폐경전,후 여성을 대상으로 하였다. 설문지를 통하여 수검자의 일반적인 특징과 골다공증의 위험요인에 대해 조사하고, 설문에 충실히 응답한 사람 100명의 요추(L2-L4)와 대퇴골부위의 검사 기록지를 분석 하였다. 자료분석은 SPSS 10.1을 사용하여, 상관분석과 단계적 다중 회귀 분석을 하였다. 조사대상군 100명중 최대 골밀도치의 표준편차 -1.0이상을 정상, -1.0이하를 골다공증으로 정하여 요추에서 정상군이 61명(61%), 골다공증군이 39명(39%), 대퇴골에서 정상군이 72명(72%), 골다공증군이 28명(28%)이었다. 골다공발생에 영향을 미치는 위험 인자에 대한 다중 회귀 분석결과 가장 많은 변량을 설명하는 위험요인이 요추에서는 연령이 대퇴골에서는 체중이 통계적으로 유의 하게 나타났다(p<0.05). 연령이 증가하면서 골밀도의 감소가 나타났고, 폐경전에는 요추와 대퇴골의 골다공증의 발생이 통계적으로 유의한 차이가 없었지만, 폐경후에는 유의하게 요추부위의 골다공증 발생이 많은 것은 의미 있는 결과이다. 또한 골다공증 발생에 요추부위에서는 연령이 대퇴골에서는 체중이 가장 많이 관련 있는 것으로 나타났다.

유향흑호단(乳香黑虎丹)이 난소적출로 유발된 흰쥐의 골다공증 모델에 미치는 영향 (Effects of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) on Osteoporosis Induced by Ovariectomy in Rats)

  • Objectives Osteoporosis is an impending problem in the rapidly aging society. The purpose of this study is to identify the effects of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) on osteoporosis induced by ovariectomy in rats. Methods 24 rats were randomly assigned to a SHAM group, a control group, and a Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group (n=8). Ovaries were extracted and for 8 weeks, the rats were given dry feeds and Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group were given a mixture of dry feeds and Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$). At 8 weeks, their body weight, femur mass, tibia ash per body ratio, overall surface area and thickness of the trabeculae, overall surface area of the osteoblasts, and the number of osteoclasts were measured and levels of albumin, aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), total-cholesterol, calcium and estradiol were recorded. Results There was no significant difference in the weight of the femur and the tibia ash per body ratio. The Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group had significantly thicker trabeculae than the control group and showed a minimal increase of overall surface area of the trabeculae. The overall surface area of the osteoblasts and the number of osteoclasts decreased in the Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group. There were no statistically significant differences in AST, ALT, ALP, total cholesterol, phosphorus, and estradiol levels. On the contrary the Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) group had significantly higher levels of Albumin, triglyceride, and calcium. Conclusions It can be concluded that oral intake of Yoohyangheukho-dan ($R{\check{u}}xi{\bar{a}}ngh{\bar{e}}ih{\check{u}}-d{\bar{a}}n$) prevents the thinning process of the trabeculae. Thus, it may have positive effects on the treatment and prevention of osteoporosis in postmenopausal women.

  • Isoflavones are phytoestrogens and natural plant compounds which are similar to 17-${\beta}$-estradiol in chemical structure. It is known that they can act as estrogen agonists or antagonists, depending on endocrine estrogenic levels, but actions of isoflavones are rather complex due to large number of variables such as chemical structures and mechanisms. Some hypotheses on biological mechanisms have not satisfactorily been confirmed to date and human epidemiological and experimental studies have been relatively limited. Nevertheless, isoflavones and isoflavone rich foods have become a focus onf interest due to positive health benefits on many diseases, especially prevention of hormone-related cancers, cardiovascular disease, osteoporosis, and adverse postmenopausal symptoms, and improvement of physiological condition such as maintaining cognitive function. This review provides an overview of chemistry, analytical techniques (focused on human biospecimens), functions including biological mechanisms, and effects of isoflavones, on the basis of the available meta-analysis and review articles and some original articles, on health and cancer.

Quality Characteristics of Jinmal Dasik Containing Calcium-rich Shrimp Powder

  • Looking at the dietary patterns of women in their 20s and 30s as well as postmenopausal women, the intake of most nutrients was lower than the recommended levels. Shrimp is a low-calorie, high-protein food, and contains abundant minerals, and it is expected to help prevent osteoporosis. In this study, jinmal dasik was prepared by varying the shrimp content at 0, 1, 5, and 7%, and the quality characteristics were measured. Hardness, adhesion, chewiness, and cohesiveness of physical properties increased significantly as the amount added increased (p<0.001). The springiness decreased significantly as the amount of addition increased in the addition group (p<0.001). In the calcium, there was a significant increase as the amount added increased (p<0.001). In the potassium, there was a significant increase as the amount added increased (p=0.049). This study intends to highlight the benefits and possible uses of shrimp.

폐경 후 여성의 골격상태에 영향을 미치는 요인분석 (Some Factors Affecting Bone Mineral Status of Postmenopausal Women)

  • Osteoporosis, the typical metabolic bone disease of the elderly, is characterized by a reduction in bone mineral density (BMD) and increased fracture risk. Genetic and environmental factors are known to play a key role in bone metabolism, and diet is also considered to be one of the important factors. The purpose of the present study was to investigate the relationship among the factors affecting BMD, including stature, body weight, age, time period since onset of menopause, and biochemical markers of bone turnover in postmenopausal women. Seventy-eight postmenopausal women who visited health promotion center for health examinations volunteered to participate in this study and they were divided into two groups according to the time period since onset of menopause : women with a time period since onset of menopause of less than 5 years (Group 1) and women with a time period since onset of menopause of 5 years or more (Group 2). The demographic characteristics and dietary intake were surveyed using a questionnaire. BMDs of the lumbar spine and femoral neck of subjects were measured by dual energy X-ray absorptiometry. Serum levels of 25-hydroxy-vitamin D and parathyroid hormone (PTH), known to be indicators of bone related hormone status, were anlyzed. Serum samples were measured for calcium, phosphorus, alkaline phosphatase, and osteocalcin as bone formation indicators, and urine was analysed for deoxypyridinoline, creatinine, calcium, and sodium as bone resorption indicators. The results are as follow : The mean BMDs of the lumbar spin and femoral neck were $1.02 \pm 0.02 g/cm^2 and 0.81 \pm 0.02 g/cm^2 respectively, and the BMD level of Group 2 was significantly lower than tat of Group 1 (p<0.01, p<0.05, respectively). The mean daily intake of energy was 1838 $\pm$ 55 kcal. When nutrient intake was compared with the recommended dietary allowances (RDA) of the subjects, only calcium, vitamin A and riboflavin intake showed means lower than the RDA. The nutrient intake did not show any significant differences between Group 1 and 2 Serum and urine levels of biochemical markers of bone turnover did not show any significant differences between Group 1 and 2, and all were within the normal range. However, the PTH and deoxypyridinoline levels showed a tendency to be higher, and the osteocalcin level to be lower in Group 2 than in Group 1. Although age and years after menopause (YAM) showed negative correlations with lumbar spine bone mineral density (LBMD) (r= -0.38, p<0.001, and r= -0.26, p< 0.05, respectively), no correlation was found with femoral neck bone mineral density (NBMD). While height, body weight and body mass index (BMI) showed a positive correlation with LBMD (r= 0.32, p<0.001, r= 0.38, p<0.001, r= 0.22, p= 0.05, respectively), only body weight and BMI showed a positive correlation with NBMD (r= 0.30, p<0.01, and r= 0.27, p<0.05, respectivley). There was no significant corealtion between BMDs and the nutrient intake of subjects, except in the case of carbohydrates (r= 0.22, p<0.05). Also, serum and urine levels of bone turnover markers showed no significant correlation with nutrient intake. On the other hand, serum osteocalcin had a positive correlation with vitamin C intake (r= 0.22, p= 0.05), and urine deoxypyridinolin showed a negative correlation with niacin intake (r= -0.22, p= 0.05). Urinary na was negatively correlated with protein intake(r= -0.23, p= 0.05). The results suggested that it is difficult to prevent the decrease in bone mass among postmenopausal women eating the usual Korean diet. However, the BMDs of the lumbar spine and femoral neck were positively related to body weight ad BMI in postmenopausal women. Therefore, this study confirmed that one of the most effective ways to minimize bone loss in postmenopausal women would be to maintain an adequate body weight with balanced nutrient intake and activity in the pre-and postmenopausal periods.

Effect of isoflavone-enriched whole soy milk powder supplementation on bone metabolism in ovariectomized mice

  • BACKGROUND/OBJECTIVE: There is intense interest in soy isoflavone as a hormone replacement therapy for the prevention of postmenopausal osteoporosis. A new kind of isoflavone-enriched whole soy milk powder (I-WSM) containing more isoflavones than conventional whole soy milk powder was recently developed. The aim of this study was to investigate the effects of I-WSM on bone metabolism in ovariectomized mice. MATERIALS/METHODS: Sixty female ICR mice individually underwent ovariectomy (OVX) or a sham operation, and were randomized into six groups of 10 animals each as follows: Sham, OVX, OVX with 2% I-WSM diet, OVX with 10% I-WSM diet, OVX with 20% I-WSM diet, and OVX with 20% WSM diet. After an 8-week treatment period, bone mineral density (BMD), calcium, alkaline phosphatase (ALP), tartrate-resistant acid phosphatase (TRAP) 5b, osteocalcin (OC), procollagen 1 N-terminal propeptide (P1NP), and osteoprotegenin (OPG) were analyzed. RESULTS: BMD was significantly lower in the OVX group compared to the Sham group but was significantly higher in OVX + 10% I-WSM and OVX + 20% I-WSM groups compared to the OVX group (P < 0.05). Serum calcium concentration significantly increased in the OVX + 10% and 20% I-WSM groups. Serum ALP levels were significantly lower in the OVX + 10% and 20% I-WSM groups compared to the other experimental groups (P < 0.05). OC was significantly reduced in the OVX group compared to the Sham group (P < 0.05), but a dose-dependent increase was observed in the OVX groups supplemented with I-WSM. P1NP and OPG levels were significantly reduced, while TRAP 5b level was significantly elevated in the OVX group compared with the Sham group, which was not affected by I-WSM (P < 0.05). CONCLUSIONS: This study suggests that I-WSM supplementation in OVX mice has the effect of preventing BMD reduction and promoting bone formation. Therefore, I-WSM can be used as an effective alternative to postmenopausal osteoporosis prevention.

  • 폐경기 여성에서 호르몬 대체요법은 폐경기 증상의 경감, 골다공증에 의한 골절, 대장암의 위험도 감소 등의 효과가 인정되어 왔다. 현재도 많은 폐경기 여성에게 사용되어지고 있는 치료법이다. 그러나 그에 따른 부작용 또한 점차 밝혀지며 연구 되고 있다. 이러한 부작용 중 생명을 위협 할 수 있는 폐색전증은 호르몬요법을 사용할시 항상 각별한 주의를 요한다. 폐경기 호르몬대체요법이 필요할 경우 심부정맥혈전증과 폐색전증의 다른 위험인자나 환경적 요인 등을 가진 환자에게 그 사용에 있어 더욱 주위를 기울여야 한다. 또한 폐색전증의 가장 많은 증상인 호흡기 증상이 있을 경우 즉시 적절한 평가와 치료가 필요하다.

성숙한 쥐의 대퇴골과 요추의 조성과 골격관련 인자에 대한 난소절제와 칼슘섭취 효과 (Effect of Ovariectomy and Calcium Intake on Femur and Lumbar Compositions and Bone Related Factors in Adult Rats)

  • This study explored the effect of calcium levels and/or ovariectomy on bone composition and its related factors using the female Sprague-Dawley rats which achieved peak bone mass in normal(0.5%) calcium intake during growth period. The rats were randomly divided into six groups and fed 0.1%, 0.5% and 1.5% calcium diets for 8 weeks after ovariectomized and sham operation. The results indicated that body weight gain was higher in ovariectomized groups than sham groups, regardless of dietary calcium levels and food intakes. Serum Ca and P concentrations were normal level regardless of dietary calcium levels and operation. Estrogen concentration was decreased in ovariectomized rat groups. Serum alkaline phosphatase activity and urinary hydroxyproline were increased in ovariectomized groups. When rats were fed normal Ca diet during growing period, weight, length and breaking force of femur were not significantly different in all groups but increased the same level. Generally, lipid contents in lumbar and femur were lower in low calcium groups and this effect was more pronounced in femur. In case of femur, the significant decrease in Ca contents of bone was observed in a relatively short period of feeding low Ca diet, even though it appeared th desirable peak bone mass had achieved through the growing period by supplementing the adequate amount of calcium. The marked decrease of estrogen levels after ovariectomy did not seem to influence greatly bone parameters measured except bone wet weight. Therefore, this study indicated that lower intake of Ca might be more important determinating factor against osteoporosis than postmenopausal state stimulated by ovariectomy in female rats. With normal or high intake of Ca it appears possible to prevent bone loss in postmenopausal period, and this might apply only in case of achieving peak bone mass in the growing with the adequate intake of calcium. 폐경후 골밀도 저하 여성의 골밀도와 식생활 관련 요인에 관한 연구 (Dietary Risk Factors Related to Bone Mineral Density in the Postmenopausal Women with Low Bone Mineral Density)

  • This study was conducted to detennine the dietary and lifestyle factors related to bone the mineral density (BMD) of postmenopausal women with decreased BMD. The subjects (N =163) were recruited from women who visited a orthopedic clinic for BMD check up. A trained dietition interviewed subjects individually to obtain data about dietary behavior, consumption frequency of foods known as main dietary source of calcium and potassium, and clinical symptoms. The risk factors were identified by correlation and multiple regression analysis of variables. The mean age of the subjects was 66.8 yews. Most of them showed low levels of education and monthly income. The mean age of menarche and menopause were 17.2 and 48 year, respectively. The mean menopause duration was 18.7 year. Most of the consumption of calcium was centered to vegetable foods. Education level were positively correlated (r =0.272, P < 0.05) with BMD whereas age, menarch age, menopause duration, number of children were negatively correlated (r=-0.355, r=-0.240, r=-0.283, r=-0.193, respectively, p < 0.05) with BMD. The consumption of soybean, radish were positively correlated (r=0.187, r=0.158, respectively, p < 0.05) with BMD. Potassium intake with rice showed significantly negative correlation with BMD (r =-0.189, P < 0.05), but calcium intake with brown seaweeds, bean sprouts were positively correlated (r =0.247, r =0.254 respectively, p < 0.05) with BMD. Protein intake with roasted pork was also positively correlated (r =0.216, P < 0.05) with BMD. Multiple regression analysis showed that the most prominent negative predictor influencing the BMD was age. Minor negative factors influencing the BMI were age of menarche, potassium intake from rice. But the significantly positively factors influencing the BMD were consumption of radish and soybean intake, education, and protein intake with roasted pork. In conclusion brown seaweeds, radish or soybeans can be promoted as cheap foods replacing milk and milk products for menopausal women with low income.

파골세포 분화에 복령 추출물이 미치는 영향 (Effect of Hoelen in RANKL-induced Osteoclast Differentiation)

  • Osteoporosis is an important public health issue in postmenopausal women. It is a major public health concern and is widely believed that osteoporosis results from imbalance between bone resorption and bone formation. Recently natural products from plants have been extensively studied as therapeutic drugs to treat and prevent various diseases. Hoelen (scientific name, Poria cocos) is a mushroom that is used in traditional Chinese medicine. Hoelen exhibits anti-inflammatory activity and has a protective effect on tumor progression. However, the effect of hoelen in osteoclast differentiation remains unknown. Thus, we examined the effect of hoelen in receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Hoelen significantly inhibited RANKL-induced osteoclast differentiation in bone marrow-derived macrophages (BMMs) in dose dependent manner without toxicity. Also, we showed that hoelen significantly inhibited the mRNA expression of tartrate-resistant acid phophatase (TRAP) and nuclear factor of activated T cells 1 (NFATc1) in BMMs treated with RANKL. In Particular, hoelen greatly inhibited the protein expression of NFATc1. Ectopic expression of NFATc1 partially reverses hoelen-mediated inhibition of osteoclast differentiation. Taken together, our results demonstrated that hoelen may be useful treatment option of bone-related disease such as osteoporosis, reumatoid arthritis, and periodontitis.
