  • Therapeuticre rehabilitation of young children has often included the use of walker as mobility aids. Traditional walkers have four vertical legs, a horizontal bar in front of child, and horizontal bars on either side which also serve as hand grip. Child pushes it. It is called 'anterior walker'. 'Posterior walker' have been introduced which the child pull along behind himself during ambulation. The purpose of this investigation was to compare specific gait characteristics of children with developmental disability, while they ambulated with anterior and posterior walker. Using the posterior walker, specific gait characteristics improve than using anterior walker.

전방 보행기와 후방 보행기가 뇌성마비아동의 보행 특성과 신체 정렬상태에 미치는 영향 (The Effects of Anterior Walker and Posterior Walker on Gait Parameters and Body Alignment of Children With Cerebral Palsy)

  • The purpose of this study was to compare the effects of anterior walker and posterior walker on gait parameters and body alignment of children with cerebral palsy. The intraclass correlation coefficient was .99 for intertester reliability. Intratester reliability was between .96 and .99. The use of posterior walker increased gait velocity and facilitated more upright posture. The measurement of joint angle program was found to be reliable to measure range of motion. This study has a limitation of generalizing the results to all children with cerebral palsy. Research is required to investigate the effect of posterior walker on energy efficiency.

경직성 양쪽다리 뇌성마비의 무릎관절 형태와 보행기 종류에 따른 근활성도 비교 (Comparison of Muscle Activations on Knee Joint Forms and Walker Types in Cerebral Palsy of Spastic Diplegia)

  • Purpose : The purpose of this study is to compare muscle activations of neck, trunk and leg in cerebral palsy of spastic diplegia with genu recurvatum and knee flexion contracture, when using anterior and posterior walkers. Methods : We selected 21 cerebral palsy and received the written consent to participate in this study. The inclusion criteria for participation required patients to have spastic diplegic CP; to be between 3~6 years of age, to have a GMFCS III grade, to have no botulinum toxin injection and orthopedics surgery within before six months starting the study. Measurements of muscle activities (sternocleidomastoid, splenius capitis, rectus abdominis, erector spinea, gluteus maximus, rectus femoris, medial hamstring and calf muscles) were evaluated anterior and posterior walker ambulations. Statistical evaluation of these data were accomplished by utilizing the paired t-test and independent t-test by SPSS 20.0 program. Significance level was set at p<.05. Results : The following results were obtained. There was significant difference on muscle activation of neck, trunk and legs(soleus except) in anterior and posterior walkers. There was no significant difference in muscle activation of neck but significant difference in muscle activation of trunk, legs between genu recurvatum and knee flexion contracture(rectus abdominis, medial hamstring when using anterior walker, rectus abdominis, erector spinea, gluteus maximus, medial hamstring when using posterior walker). Conclusion : The conclusion of this study is the different knee joint forms would have different effect on muscle activation of trunk and legs while cerebral palsy of spastic diplegic ambulated with anterior walker and posterior walker.

댄디워커증후군에 동반된 점막 밑 입천장갈림증 치험례 (Dandy-Walker Syndrome with Submucous Cleft Palate: A Case Report)

  • Purpose: Dandy-Walker syndrome is a rare congenital brain malformation that occurs one in every 25,000-35,000 live births, mostly in females. It is characterized by cystic enlargement of the fourth ventricle, agenesis or hypogenesis of the cerebellar vermis and enlargement of the posterior fossa. In this report, the authors aimed to address a rare case of a 14-months-old female Dandy-Walker syndrome patient that is presented with submucous cleft palate. Methods: A 14-months-old female patient admitted to our outpatient clinic, via the department of pediatrics, with the complaints of nasal regurgitation, choking and breathing difficulties. She was diagnosed as Dandy-Walker syndrome by magnetic resonance imaging evaluation, at another hospital and underwent a shunt operation for the hydrocephalus continuing treatments. On physical examination, she had structural abnormality of bifid uvula, and palpable notch in the posterior surface of the hard palate. Her submucous cleft palate was corrected, which used a double opposing Z-plasty under general anesthesia. Results: In a follow-up period of 2 months, no complications, such as wound dehiscence, necrosis and infection occurred, which shows satisfactory results. She consulted with pediatric neurologists and physical therapists for further evaluation and management of the abnormalities in the central nervous system. Conclusion: Dandy-Walker syndrome patient with a cleft palate is a very rare case to find, which only a few cases are reported around the world. Authors would like to share this case of Dandy-Walker syndrome patient, with submucous cleft palate, who underwent a double opposing Z-plasty that shows satisfactory results.

Neurocutaneous Melanosis in Association with Dandy-Walker Complex with Extensive Intracerebral and Spinal Cord Involvement

  • Neurocutaneous melanosis (NCM) is a rare congenital syndrome consisting of benign or malignant melanotic tumors of the central nervous system with large or numerous cutaneous melanocytic nevi. The Dandy-Walker complex (DWC) is characterized by an enlarged posterior fossa with high insertion of the tentorium, hypoplasia or aplasia of the cerebellar vermis, and cystic dilatation of the fourth ventricle. These each two conditions are rare, but NCM associated with DWC is even more rare. Most patients of NCM with DWC present neurological symptoms early in life such as intracranial hemorrhage, hydrocephalus, and malignant transformation of the melanocytes. We report a 14-year-old male patient who was finally diagnosed as NCM in association with DWC with extensive intracerebral and spinal cord involvement.

고령자 보행보조로봇 제어기법 연구 (Research of Elderly Gait-assistant-robot Control System)

  • In this study, the control method of assistive robot was developed for the elderly. The control method of gait-assistant-robot was proposed considering the change of COP (Center of Pelves), BOS (Base of Support) and comparative analysis of the moving velocity for the elderly. We analyzed the movement of COP of the body and its velocity of the elderly equipped with manual walker and gait-assistant-robot. As a result, change in COP was greater from left to right than from anterior to posterior; also, the average velocity of the movement of COP and manual walker for manual walker gait was 0.7(m/s). Therefore, it is necessary to concern more on the left-right balance and synchronization of the velocity of COP.

Mega cisterna magna in bipolar mood disorder: a case report

  • Mega cisterna magna (MCM), one of the members of the Dandy-Walker complex, is a developmental malformation of the posterior fossa that is larger than 10 mm but morphologically does not affect the vermis and cerebellar hemispheres. Reports of psychiatric disorders associated with this anomaly are rare. We present the case of a patient with MCM who presented with a psychotic manic attack and was diagnosed with bipolar disorder. A 28-year-old female, single housewife, university graduate, presented with irritability, decreased sleep and appetite, distraction, and agitation. The patient also had a delusion of reference. In the clinical follow-up, an increase in energy and an increase in the amount of speech were observed. Her neurological examination was normal, and cranial magnetic resonance imaging revealed an MCM. The relationship and clinical significance of MCM with psychosis and mood disorders have not yet been fully elucidated. It is not known whether this association is accidental or based on etiological commonality. The purpose of this case report is to review the relationship between the cerebellum and psychiatric symptoms and to contribute to the literature.

PHACE association with intracranial, oropharyngeal hemangiomas, and an atypical patent ductus arteriosus arising from the tortuous left subclavian artery in a premature infant

  • PHACE association is a rare neurocutaneous condition in which facial hemangiomas associate with a spectrum of posterior fossa malformations, arterial cerebrovascular anomalies, cardiovascular anomalies, and eye anomalies. We reported a case of PHACE association in a premature infant showing facial, intracranial, and oropharyngeal hemangiomas with evidence of the Dandy-Walker variant and complicated cardiovascular anomalies, including a right-sided aortic arch and an atypical patent ductus arteriosus arising from a tortuous left subclavian artery. To our knowledge, intracranial hemangiomas are rare in PHACE association, and a concomitant oropharyngeal hemangioma has not been previously reported in the PHACE association literature. In infants presenting with large, plaque-like facial hemangiomas, it is important to conduct active cardiovascular and neurological evaluations. Special attention should be given to the laryngoscopic examination to search for additional hemangiomas in the airway.

10세에서 14세 사이 III급 부정교합자의 치아안면두개의 형태학적 특성에 대하여 (Dental and skeletal characteristics and growth in Class III malocclusion between skeletal ages of 10 and 14)

  • 이 연구는 두부X선 규격사진을 이용하여 III급 부정교합자의 안모를 연령 성별로 분류하고,각 분류에 대한 두개 안면 형태의 특성을 구명하기 위하여 시행되었다. 정상교합자 60명과 III급 부정교합자 60명을 표본으로 하였으며 각 군은 10세, 12세, 14세 군 남녀로 구분되었다. 50개의 선 및 각계측치를 Krogman-Walker의 기준평면을 기준하여 측정하였다. 1. III급 부정교합군은 전방두개저의 길이와 안상각이 정상교합자군에 비하여 작고 하악골체와 하악수직고경이 큰 것으로 나타났으며, 이러한 차이는 남녀 모두 10세부터 유의하게 나타났다. III급 부정교합군에서의 하악하연각의 유의한 증가는 남녀 모두 12세때 나타났다. 2. III급 부정교합자군에서의 상악골의 후방 위치와 상악골체 크기의 유의한 감소는 남녀 모두 10세부터 나타난다. 3. III급 부정교합자군에서 유의한 하안면부 수직고경의 증가는 주로 하악골체 크기의 변화에 기인하는 것으로 관찰되었다. 하악골체의 경우 III급 부정교합자군에서 더 크고 전하방에 위치하는 것이 관찰되었다. III급 부정교합자군에서의 전체적인 수직고경의 유의한 증가는 12세군에서 유의하게 나타나기 시작한다. 4. 본 연구에서는 III급 부정교합자군에서의 두개저부각의 감소와 (Ba-S-N, 3A) 하악골체의 전방 위치는 남녀 모든 연령층에서 일관되게 관찰되었다. 이 결과는 Rothstein 등에 의해 주장된 II급 부정교합자군에서의 두개저부각의 변화가 하악골체의 전후방 위치에 영향을 주지 못한다는 주장을 부정하며, 디벳 등에 의해 주장된 앵글씨의 부정교합분류와 두개안면형태의 상관성에 대한 연구를 지지한다.

IR센서 기반 보행보조기를 이용한 보행 시 경사상태에 따른 보행의지 파악에 관한 연구 (A Study on Walking Intention Detection of Gait Slope and Velocity of the Rollator Based on IR Sensor)

  • 본 논문은 보행의지를 파악하기 위해 IR 센서 기반 보행보조기를 이용하여 보행 시 슬관절 전방변위 측정과 하지의 근육활성도를 고찰하였다. 슬관절 전방변위 측정을 위해 보행보조기에 IR센서를 장착하였고, 근전도 (Bagnoli-8ch, Delsys Inc, USA)장비를 이용하여 대퇴직근, 대퇴이두근, 전경골근, 비복근을 측정 하였다. 실험에 참가한 피험자들은 성인 8명(나이 $23.7{\pm}0.5years$, 신장 $175.4{\pm}2.3cm$, 체중 $70.6{\pm}5.6kg$)이며 매주 30분씩 총 3주간 진행되었다. 보행 시 경사조건과 보행 속도의 증가에 따라서 슬관절 전방변위가 변화되는 결과를 확인할 수 있었다. 또한 근육활성도 결과, 전방경사 시 대퇴부 근육의 활성도가 증가하였고 후방경사 시 하퇴부 근육이 많이 활성화 되어 IR센서 결과와 유사한 경향을 확인할 수 있었다.
