  • Papillary thyroid carcinoma frequently invades the lymph node, trachea, esophagus and perithyroid tissue. However, direct extension to posterior pharyngeal area is known to be rare. A 64-year-old male was referred to our clinic presenting as posterior pharyngeal mass during gastrofiberscopy. The neck CT scan showed soft tissue mass in retropharynx and lymph node in right level III with calcifications. We performed the total thyroidectomy with selective(level II, III, IV) and anterior compartment neck dissection. In operative findings, the right thyroid mass were connected to the retropharynx through the posterior portion of inferior constrictor muscle. Histopathologic findings revealed the papillary thyroid carcinoma extended to retropharynx. We report a unique case with a literature review.

  • Liposarcoma is a malignant tumor originated from adipose tissue, accounting for 20% of all soft tissue sarcomas. A 69-year-old man with anterior and posterior neck mass visited to our department and we performed an excisional biopsy in level I. Histopathological examination revealed liposarcoma, so we performed wide excision in anterior and posterior neck, and adjuvant radiotherapy. We report a unique case of well-differentiated liposarcoma on anterior and posterior neck with literature review. To our best knowledge, such case has not been reported earlier in Korea previously.

  • Intrathoracic goiters can be classified anterior and posterior mediastinal goiter with its locations. Most intrathoracic goiters are retrosternally situated in the anterior mediastinal compartment. Posterior mediastinal goiters are rare, but might present a difficult diagnostic and surgical problem. Although thyroid goiters are nearly always amenable to a cervical approach, posterior mediastinal goiters may require a combined cervicothoracic approach with sternotomy or thoracothomy. We herein describe a case of posterior mediastinal goiter which was excised only by cervical approach. The relevant literature is briefly reviewed.

  • Objectives : This study was performed to evaluate the effects of the acupuncture therapy in combination with Soyeom pharmacopuncture therapy on acute whiplash injury by traffic accident. And thermal change of posterior neck and shoulder was observed. Methods : The clinical study was conducted to 20 cases of acute whiplash injury patients who had been admitted in Dept. of Acupuncture and Moxibusion, Woosuk University Hospital from December 1, 2010 to May 31, 2011. The patients were randomly divided into two groups. In one group(test group), patients were treated with the acupuncture therapy in combination with Soyeom pharmacopuncture therapy. And in the other group(control group), patients were treated with acupuncture therapy only. To estimate the efficacy of treatments, visual analog scale(VAS), neck disability index(NDI) of posterior neck pain were checked. And also thermal change of posterior neck and shoulder was observed by digital infrared thermographic imaging(DITI). Results : Both group represented effective improvement in VAS and NDI of posterior neck pain. Especially, test group is more effective than control group in VAS comparison, statistically. In test group, temperature of posterior neck and shoulder was decreased after treatments. But there was no significant difference between two groups on thermographic change. Conclusions : Through this study, Soyeom pharmacopuncture therapy is considered to be effective for reducing pain in acute whiplash injury patients. And after Soyeom pharmacopuncture therapy, temperature of posterior neck and shoulder show a tendency to decrease.

  • Background and Objective : Papillary thyroid microcarcinoma(PTMC) is known as slow growing cancer with good prognosis. However, extrathyroidal extension may increase the risk of cervical lymph node metastasis and local invasion to surrounding structures. The aim of this study was to assess the characteristic features of the tumor invading the posterior thyroid capsule. Material and Methods : We made a retrospective review of 123 PTMC patients with thyroid capsule invasion, pathologically staged as T3 or T4. 74 patients (60.2%) had invasion to posterior thyroid capsule (group A) while 49 patients (39.8%) had invasion to-anterior thyroid capsule or anterior wall of trachea (group B). We assessed the clinicopathologic factors of the patients according to the location of capsular invasion of PTMC. Results : There was no difference regarding age, gender, T and N classification and incidence of lymph node metastasis between two groups. Local invasion rate to recurrent laryngeal nerve was 6.8% in patients with posterior thyroid capsule invasion, while the incidence was zero in those with capsular invasion to other locations Conclusion : Increased risk of local invasion to the recurrent laryngeal nerve should be considered in patients with PTMC presenting invasion of the posterior capsule.

  • Differential diagnosis of posterior neck pain is very challenging based on symptoms and physical examination only. Retropharyngeal calcific tendinitis is a rare and frequently misdiagnosed entity in various causes of neck pain. It results from calcium hydroxyapatite deposition in the longus colli muscle which is characterized by severe neck pain, painful restriction of neck movement, dysphagia, and odynophagia. We herein report a case of a patient with acute retropharyngeal calcific tendinitis, who complained of posterior neck pain, initially diagnosed and treated as a myofascial neck pain syndrome.

  • Purpose : Posterior Neck Pain and Back Pain are common diseases in human daily life. For reducting symptom and treatment of these diseases, many methods have been used and studied until now. The mechanisms of Ceragem Master Heat Bed(R)(Ceragem Co. Seoul, Korea) are infrared-heat therapy from natural nephrite and pressure therapy by up-down movement of natural nephrite ball. Through this study, authors would evaluate the clinical effectiveness of Ceragem Master Heat Bed(CMHB) on relieving posterior neck pain and back pain. Materials and methods : From 12-01-2000 to 01-13-2001, posterior neck pain and back pain patients were selected for experiment group(23patients) and control group(14patients). In both group, Visual Analog Scale(VAS) of pre-treatment stage is above 5 points. Experiment group were cared with acupuncture therapy, buhang therapy and CMHB. Control group were managed with only acupuncture therapy and buhang therapy. They were treated more than three times at least. At pre-treatment and post-treatment stage, we measured VAS for individual patients, and then compared effect of treatment in experiment group and in control group. Statistical analysis was preformed using SAS program. Results : In this study, a mean difference of VAS between pre-treatment and post-treatment stage was large in experiment(CMHB) group more than in control group(p〈0.01). Conclusion : Experiment(CMHB) group was more effective pain relief than control group in the treatment of posterior neck pain and back pain. CMHB was available for stimultaneous application of heat therapy and pressure massage together that gave another effects of mental stability and fatigue recovery. As the results of this study, CMHB could be used as a helpful treatment modality for posterior neck pain and back pain.

  • A congenital cholesteatoma is a benign mass formed from the keratinizing stratified squamous epithelium. It usually occurs in young children's anterosuperior part of the middle ear. A congenital cholesteatoma which originates from mastoid temporal bone or expands to posterior cranial fossa is rare. Standard treatment of an intracranial cholesteatoma is surgical removal with craniotomy. A 69-year-old woman was diagnosed with a congenital cholesteatoma of mastoid temporal bone that expanded to the posterior cranial fossa, which was successfully treated with transmastoid marsupialization without craniotomy. This is a first documented case of a congenital cholesteatoma of mastoid temporal bone that expanded to posterior cranial fossa, which was successfully treated with transmastoid marsupialization without craniotomy.

  • Purpose: Spindle cell lipoma(SCL) is an uncommon subcutaneous soft tissue neoplasm that arises in the shoulder and posterior neck of older male patients. The imaging appearance of SCL is not pathognomonic and can display some features overlapping with liposarcoma. We report a case of SCL on the posterior neck. Method: The patient is a 50 - year - old man with a slowly enlarging subcutaneous mass on the right side of posterior neck. Computed tomographic imaging revealed a 7.0 cm sized, well - circumscribed, heterogenous and fatty mass with enhanced solid components. Whole body Fluorine - 18 Fluorodeoxyglucose Positron emission tomogram(FDG PET-CT) showed little increase of FDG uptake on the right posterior neck and there was no distant metastasis. Results: The mass was surgically removed. The resection margin was free of tumor on frozen biopsy. Histopathologic examination indicated spindle cell lipoma consisting of a mixture of mature adipocytes and uniform spindle cells within a matrix of mucinous material. Conclusion: Although CT image of solidtary mass in posterior neck is similar with the one of liposarcoma, we should consider that it may be a spindle cell lipoma if PET-CT and other systemic studies reveal no distant metastasis. And we should perform fine needle aspiration to differentiate SCL from malignant lesions.

  • Identification and protection of the facial nerve is very important in the proper operation of the parotid tumor. Posterior approach which finds main trunk of the facial nerve by surgical landmark such as tragal pointer, tympanomastoid suture, and posterior belly of digastric muscle is most commonly used. In case of posterior located tumor, inferior approach may be used, in which the retromandibular vein is followed from the neck and inferior branch of the facial nerve is located. In general, the facial nerve lies superficial to the retromandibular vein. But we experienced the anomalous relationship of the facial nerve and the retromandibular vien. We report this case with a literature review.
