Post traumatic malocclusion and its prosthetic treatment

  • Mandible fractures belong to the most common fractures encountered in maxillofacial trauma. Because mandible is such a unique structure with hinge joint and masticatory muscles attached to the body of mandible, attention must be paid to avoid displacement during treatment. Displacement during fracture reduction leads to malocclusion. Many TMJs function with complete comfort and apparent normalcy in adapted centric posture, even though they have undergone deformation caused by trauma. This clinical report describes the patient with post traumatic malocclusion and its prosthetic treatment. His fractured mandible was openly reduced in changed position, as a result his occlusion has been changed. He was treated by prosthetic method in so-called adapted centric posture.

  • Post-traumatic scar deformations are common in many skin injury cases. These are usually aesthetic problems, but if these deformations impair mobility and the patient feels uncomfortable, we must resolve them. In a patient with a lateral post-traumatic scar fold in the lateral canthus that shortened the horizontal orbital fissure and created traction on eye opening, we performed modified classic Z-plasty, resulting in aesthetic and functional improvements without recurrence.

  • This report describes the miniscalpel-acupuncture treatment of a 68-yr-old woman who had abnormal posture and involuntary dystonic movements of the head and neck. She suffered muscle contracture associated symptoms after she fell injured in 2014. Her neck had abnormal posture findings due to muscle tension. The spasms gradually worsened and did not respond to botulinum toxin injections. Involuntary dystonic movements may be due to peripheral post-traumatic cervical dystonia. Outcomes support cervical dystonia may be managed effectively with miniscalpel-acupuncture. Further research is recommended.

  • Objectives: The purpose of the case study is to report the effects of post traumatic stress disorder from sexual violence treated by an oriental medical treatment. Methods: The patient was treated by acupuncture at Baekhoi, Naegwan, Hapkok, Taeyang, Changmun and moxa at Baekhoi, Sin-Guel, Chungwan, Kwanwon. Results: The oriental medical treatment resulted considerable improvements for the patient. Conclusions: This study shows that the oriental medical treatment can be an effective treatment for sexual violence victims. However, the symptoms may recur, so we need to observe and manage the patients continuously.

  • Post-traumatic hematoma formation is a common complication of contusion. If the hematoma is large enough to aspirate or drain, it can be treated quickly and appropriately. However, if the hematoma is small or concealed by local swelling, it may be overlooked and left untreated. In most cases, a hematoma will resolve following conservative treatment; however, associated infection or muscle fibrosis can occur. Herein, we present the case of a patient with a chin deformity caused by a post-traumatic hematoma. The deformity was treated using botulinum toxin and triamcinolone acetonide injections as minimally invasive treatments. The course of treatment was good.

  • Post-concussion syndrome (PCS) following mild traumatic brain injury (mTBI) is a major factor that contributes to the increased socioeconomic burden caused by TBI. Myelin loss has been implicated in the development of PCS following mTBI. Diffusion tensor imaging (DTI), a traditional imaging modality for the evaluation of axonal and myelin integrity in mTBI, has intrinsic limitations, including its lack of specificity and its time-consuming and labor-intensive post-processing analysis. More recently, various fast MR techniques based on multicomponent relaxometry (MCR), including QRAPMASTER, mcDESPOT, and MDME sequences, have been developed. These MCR-based sequences can provide myelin water fraction/myelin volume fraction, a quantitative parameter more specific to myelin, which might serve as a surrogate marker of myelin volume, in a clinically feasible time. In this review, we summarize the clinical application of the MCR-based fast MR techniques in mTBI patients.

Post-traumatic Stress Disorder and Post-traumatic Growth in Breast Cancer Patients - a Systematic Review

  • Breast cancer (BC) is potentially a traumatic stressor which may be associated with negative outcomes, such as post-traumatic stress disorder (PTSD) or positive changes, such as post-traumatic growth (PTG). This study aims to identify the core issues of BC related PTSD, PTG and psychological distress by interrogating the literature in BC survivors. We have also highlighted issues related to the assessment, diagnosis and clinical management of PTSD and PTG. The authors systematically reviewed studies published from 1985 to 2014 pertaining to PTSD, psychological distress and PTG in BC survivors with particular attention paid to incidence rates and causative factors. Multiple studies intimated that women with BC have evidence of PTSD at the initial stages of diagnosis, whereas PTG develops once patients undergo treatment. Early diagnosis and treatment of PTSD/PTG is paramount from literature review but the previously mentioned relationship between PTSD and PTG in BC patients could not be verified. It is evident from the literature that a small percentage of BC patients experience PTSD, while the majority experience PTG after BC diagnosis and treatment. Future research should include prospective studies focusing on high-risk patients, causative factors and the development of psychological interventions.

소방공무원의 지각한 현장출동 충격경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향 (The Influence of Field-Shock Experience and Post Traumatic Stress Perceived by Fire Officials upon Somatic Symptoms and Coping Methods)

  • 본 연구는 소방공무원들의 지각한 현장출동 충격 경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향을 규명하기 위하여 강원도 소재 소방안전센터에 근무하고 있는 소방공무원 남녀 362명을 대상으로 조사하였다. 수집된 자료는 Windows용 SPSS 19.0과 AMOS 19.0을 사용하여 빈도분석, 타당도와 신뢰도 분석을 위한 요인분석과 Cronbach's ${\alpha}$ 분석, 기술통계와 상관관계를 실시하였고, 가설 검정을 위해 구조방정식 모형을 이용한 분석을 실시하였다. 모든 통계분석은 유의수준은 p<.05로 설정하였다. 결과는 다음과 같다. 첫째, 지각한 현장출동 충격 경험은 외상 후 스트레스에 영향을 미치는 것으로 나타났다. 둘째, 지각한 현장출동 충격 경험이 신체증상에 영향을 미치는 것으로 나타났다. 셋째, 지각한 현장출동 충격 경험은 대처방식에 영향을 미치는 것으로 나타났다. 넷째, 외상 후 스트레스는 신체증상에 영향을 미치는 것으로 나타났다. 다섯째, 외상 후 스트레스는 대처방식에 영향을 미치는 것으로 나타났다. 여섯째, 신체증상은 대처방식에 영향을 미치는 것으로 나타났다. 이러한 결론으로 볼 때 충격적 스트레스를 해소하기 위한 제도적 방안으로 전 세계적으로 검증이 되어 실제 운영되고 있는 위기상항 스트레스 해소법(Critical Incident Stress Debriefing : CISD)을 소방서의 실정에 맞게 도입하여 운영하는 것이 필요할 것이다.

한국전, 베트남 참전 국가유공자들의 외상 후 스트레스가 우울에 미치는 영향에 대한 융복합적 연구 : 자아-적응유연성의 매개효과 분석 (The Effect of Post-Traumatic Stress on Depression of Korean and Vietnam War Veterans: Mediating Effect Analysis of Ego-Resilience)

  • 본 연구는 참전 국가유공자의 외상 후 스트레스와 우울의 관계를 파악하고, 외상 후 스트레스와 우울의 관계에서 자아-적응유연성의 매개효과를 확인하고자 하였다. 한국보훈협회의 협조를 얻어 2018년 3월부터 6월까지 한국전 또는 베트남 전쟁 참전군인 383명을 대상으로 자료 수집하였다. 수집된 자료는 SPSS 24.0 Statistics Program을 이용하여 외상 후 스트레스와 우울의 관계에서 자아-적응유연성의 매개효과를 계층적 다중회귀분석으로 분석하였다. 대상자의 참전 기간이 긴 경우(F=7.97, p<.001), 무직인 경우(t=2.09, p=.040), 경제 상태가 좋지 않은 경우(F=64.49, p<.001) 외상 후 스트레스가 높았으며, 외상 후 스트레스와 우울의 관계에서 매개변수인 자아-적응유연성이 부분 매개하는 것으로 나타났다. 본 연구결과를 바탕으로 참전군인들의 우울 중재를 위해 자아-적응유연성 향상에 초점을 맞춘 다양한 프로그램 개발 및 국가 차원에서의 체계적인 관리가 요구된다.
