Assessment of Anthropometric Obesity Indexes by Correlation with Body Composition (비만치료에 유용한 단순비만지표의 선별)

  • Object : Waist circumference(WC), waist-hip ratio(WHR), waist-stature ratio(WSR), and body mass index(BMI) are commonly used for evaluating obesity. This Research were done to determine what is more sensitive obesity indexes(WC, WHR, WSR, BMI) Correlated with body composition such as body fat mass, body fat(%), visceral fat area, and fat free mass. And what is more sensitively correlated obesity indexes with % changes of body composition during weight reduction treatment. Methods : This clinical retrospective research were carried out 127 cases of female obese outpatients with weight reduction treatment during 1 month. Bioelectrical impedence analysis(for body composition) and body size(for anthropometric obesity indexes) were estimated in pre-treatment and post-treatment to evaluate the obesity indexes. Pearson correlation coefficients were used to select useful obesity index. Result & Conclusion : BMI is useful index for diagnosis and evaluation of obesity. WSR is sensitively correlated with visceral fat area and body fat(%). So, WSR is useful index for evaluating abdominal obesity and risk factors of metabolic syndrome. WC is correlated with both body fat mass and fat free mass. WHR is not optimal for diagnosis and evaluation of obesity.

The Identification, Diagnosis, Prospective, and Action (IDPA) Method for Facilitating Dialogue between Stakeholders: Application to the Radiological Protection Domain

  • This article reviews the experience of applying the Identification, Diagnosis, Prospective, and Action (IDPA) facilitating method as a means of promoting practices of dialogue between stakeholders in the radiological protection field. After presenting the characteristics of the IDPA method and its ability to promote active listening, participation, and dialogue among stakeholders facing complex situations, as well as the procedural aspects associated with its practical implementation, the article describes three examples of the application of the method in the field of radiological protection. The first one presents how the IDPA method supported a debate among decision-makers, authorities, experts, professionals, and representatives of non-governmental organizations about how to engage stakeholders in radiological protection. The second example presents how the IDPA method was used in a series of dialogue meetings to explore the challenges of the post-nuclear accident situation resulting from the Fukushima Daiichi Nuclear Power Plant accident. The third one presents the application of the method in the context of a training course organized by Nagasaki University in the affected area close to the damaged plant. Experience has shown that the IDPA method makes it possible to develop responses to problems posed in very different contexts and, in many cases, to find compromises regarding their solutions. The IDPA method has the merit of allowing each of the participants to better understand the situation they are faced with, even if such a positive result is not always achieved.

Bilateral Checkrein Deformities of the Hallux Following Intramuscular Sarcoidosis: A Case Report (근육 내 사르코이드증 후 발생한 양측 무지의 체크레인 변형: 증례 보고)

  • This report presents a unique case of checkrein deformities in both halluces following isolated intramuscular sarcoidosis, a rare occurrence given the infrequent musculoskeletal involvement in sarcoidosis. Typically resulting from flexor hallucis longus tendon entrapment by scar tissue post-trauma, the checkrein deformity reported in this paper presented with unusual metatarsophalangeal joint flexion and interphalangeal joint extension during ankle dorsiflexion. A 49-year-old woman with a history of intramuscular sarcoidosis presented with a great toe deformity and discomfort while wearing shoes, leading to a diagnosis of dynamic deformity, possibly attributed to tendon tethering by sarcoidosis. Surgical treatments, including abductor hallucis muscle intratendinous tenotomy, flexor hallucis longus Z-plasty lengthening, Weil osteotomy, and Kirschner wire fixation, significantly improved the functional scores and patient discomfort. This report underscores the importance of recognizing dynamic deformities and the potential for rare diseases, such as sarcoidosis, to cause such conditions, highlighting the need for careful diagnosis and tailored surgical intervention for atypical checkrein deformities.

Field Application of Rapid Neutralization Assessment Method Using Core Drilling in Concrete Structures (코어드릴링에 의한 중성화 신속평가 방법을 이용한 콘크리트 구조물의 중성화 현장 적용성 평가)

  • In this paper, we aimed to develop a new method for diagnosing the depth of neutralization in architectural and civil engineering structures using the core drilling method, which combines the speed of drilling with the accuracy of core ringing. When compared to the drilling method, the core drilling method showed a lower measurement deviation of 1-2mm (7.6%) in confirming the depth of neutralization. This is believed to be a result of potential interference during the sample collection process in the drilling method, where the drill may pass through aggregates, leading to overestimation, as indicated in previous studies. The rapid evaluation of neutralization using the core drilling method serves as an alternative to address the issues associated with both drilling and core ringing methods in diagnosing the depth of neutralization. It offers a solution to the inaccuracy caused by coarse aggregates and the cumbersome post-processing steps required for neutralization diagnosis. Our proposed technique aims to provide an accurate and expedited diagnosis of neutralization depth without the need for additional processes.

  • The management of infection occurring in the space which remains after removal of lung presents considerable difficulties both for the patient and the surgeon. We have during the past one year, developed a procedure, which has been successfully applied in 4 cases of postpneumonectomy empyema by modification of Clagett's procedure. All their underlying lung diseases were pulmonary tuberculosis. The diagnosis of postpneumonectomy empyema was confirmed by thoracentesis, and bacteriologic cultures were obtained in all patients. Dead space thoracis was irrigated with various kind of antiseptic Solutions and debrided, filled with antibiotics saline solutions. The patients had a smooth post operative course except one case and was discharged from the hospital on the 3 weeks post-operative day in average. The failure case was wound leaking & redeveloped empyema, & waiting for reoperation.

Paradigm Shift in the Study of Treatment Resistant Depression (치료저항성 우울증의 연구에서 패러다임의 전환)

  • Treatment-resistant depression (TRD) is a major public health problem. It is estimated that about 30% of patients with major depressive disorder do not show substantial clinical improvement to somatic or psychosocial treatment. Most of studies for TRD have focused on the subjects already known as TRD. Patients with unipolar depressive episodes that do not respond satisfactorily to numerous sequential treatment regimens were included in the TRD studies. Such post hoc experimental design can be regarded only as consequences of having TRD, rather than as causal risk factors for it. Although informative, data derived from such studies often do not allow a distinction to be made between cause and effect. So, we should shift paradigm toward examining the risk for developing TRD in untreated depressed patients. To deal with this problem, untreated depressed patients should be enrolled in the study to identify biological markers for treatment resistance. The peripheral or central biological markers should be explored before starting treatment. Subsequent systematic administration of treatments with appropriate monitoring in the subjects can determine the risk for developing treatment resistance in untreated individuals. Such information could give a cue to improve the initial diagnosis and provide more effective treatment for TRD.

  • A dog (Chihuahua, 2-year-old, intact female) was referred to us because of cluster seizure. She had history of falling from height few days before presentation. Brain computed tomography (CT) results demonstrated fracture line on right temporal bone and hypodense, edematous changes of the adjacent brain parenchyma on right cerebral parenchyma. Based on history, clinical signs, and diagnostic imaging findings, this patient was diagnosed to traumatic brain injury. After diagnosis, the patient was well controlled with anti-inflammatory drug and anti-epileptic drugs. When 30, 480, and 1260 days after initial brain CT examination, we performed serial brain CT rechecks. This case report describes serial clinical and brain CT findings after traumatic brain injury.

  • Objectives : We will discuss the differences on EEG results of Post-traumatic embitterment disorder (PTED) and Major depressive disorder (MDD). Methods : We measured EEG on 21 sites (Fp1, Fpz, Fp2, F7, F3, Fz, F4, F8, T3, C3, Cz, C4, T4, T5, P3, Pz, P4, T6, O1, Oz, O2) of PTED and MDD. Then, we compared the results. Results : There is a significant result of a beta band between PTED and MDD. Conclusions : We became aware of the differences on EEG results of PTED and MDD. This can be used as the basis of diagnosis.

Application of PCR for diagnosis of porcine circovirus type 2 (돼지 써코바이러스 2형의 진단을 위한 PCR법 적용)

  • Porcine circovirus (PCV) is a small, nonenveloped virus that contains a single-stranded circular DNA genome of about 1.76 kb and belongs to the family circoviridae. The PCV-2 has been incriminated as the cause of post-weaning multisystemic wasting syndrome (PMWS) , an emerging disease in pigs. In the present study, a PCR assay was applied to detect PCV-2 in tissue samples. The presence of PCV-2 antigen in the porcine tissues was confirmed by indirect immunofluorescence (IIF) with PCV-2 specific monoclonal antibodies. And then DNA extracted from PCV-2 positive tissues was used as a template. One oligonucleotide primer suitable for PCR was selected from a published PCV-2 sequence (Genbank). Amplified PCR product was detected the same fragment lengths of 416 bp as a control. Based on these results, it was suggested that the PCR is a simple and sensitive method for support diagnostic purposes.

Cor triatriatum(A case report) (삼중방심 치험 1예)

  • Cor triatriatum is a rare congenital malformation of the heart, in which a septum stretches in a transverse or oblique plane through the left atrium, thus separating it into two compartments. The upper one connects with the pulmonary veins, and the lower one connects with the left ventricle. Due to the rarity of, and great difficulty in-diagnosing, cor trlatrlatum, data On the surgery of this disease are of necessity very limited and so accurate pre-operative diagnosis was very difficult to make. `We experienced a case of the cyanotic congenital heart disease which was diagnosed as a large atrlal septal defect with streaming venous blood from inferior vena cava to left atrium through atrial septal defect in August, 1982. We found that there were transverse septum in the left atrium through atrlal septal defect, the pulmonary venous drainage were located in the upper chamber of the left atrium, and the lower chamber was connected with the left atrial appendage, mitral valve and `left ventricle. But our case had not any opening in this transverse septum and the right atrium was connected with the upper chamber of the left atrium through the upper part of the atrlal septal defect, and was communicated with the lower chamber of the left atrium through the lower part of the atrlal septal defect. We excised the transverse septum and repair this atrial septal defect with Woven Dacron patch accompanying with the drainage of coronary sinus to right atrium. The post-operative course was not eventful and he was discharged with good result on the post-operative 8th day, and has been in good .condition up to now for longer than 10 months.
