Current Status of Noninvasive Ventilation Use in Korean Intensive Care Units: A Prospective Multicenter Observational Study

  • Background: Data on noninvasive ventilation (NIV) use in intensive care units (ICUs) are very limited in South Korea. Methods: A prospective observational study was performed in 20 ICUs of university-affiliated hospitals from June 2017 to February 2018. Adult patients (age>18 years) who were admitted to the ICU and received NIV treatment for acute respiratory failure were included. Results: A total of 156 patients treated with NIV were enrolled (mean age, $71.9{\pm}11.6years$). The most common indications for NIV were acute hypercapnic respiratory failure (AHRF, n=89) and post-extubation respiratory failure (n=44). The main device for NIV was an invasive mechanical ventilator with an NIV module (61.5%), and the majority of patients (87.2%) used an oronasal mask. After the exclusion of 32 do-not-resuscitate patients, NIV success rate was 68.5% (85/124); ICU and hospital mortality rates were 8.9% and 15.3%, respectively. However, the success rate was lower in patients with de novo respiratory failure (27.3%) compared to that of patients with AHRF (72.8%) or post-extubation respiratory failure (75.0%). In multivariate analysis, immunocompromised state, de novo respiratory failure, post-NIV (2 hours) respiratory rate, NIV mode (i.e., non-pressure support ventilation mode), and the change of NIV device were significantly associated with a lower success rate of NIV. Conclusion: AHRF and post-extubation respiratory failure were the most common indications for NIV in Korean ICUs. Overall NIV success was achieved in 68.5% of patients, with the lowest rate in patients with de novo respiratory failure.

Fracture resistances of zirconia, cast Ni-Cr, and fiber-glass composite posts under all-ceramic crowns in endodontically treated premolars

  • PURPOSE. The aim of the present study was to evaluate the fracture resistances of zirconia, cast nickel-chromium alloy (Ni-Cr), and fiber-composite post systems under all-ceramic crowns in endodontically treated mandibular first premolars. MATERIALS AND METHODS. A total of 36 extracted human mandibular premolars were selected, subjected to standard endodontic treatment, and divided into three groups (n=12) as follows: cast Ni-Cr post-and-core, one-piece custom-milled zirconia post-and-core, and prefabricated fiber-glass post with composite resin core. Each specimen had an all-ceramic crown with zirconia coping and was then loaded to failure using a universal testing machine at a cross-head speed of 0.5 mm/min, at an angle of 45 degrees to the long axis of the roots. Fracture resistance and modes of failure were analyzed. The significance of the results was assessed using analysis of variance (ANOVA) and Tukey honest significance difference (HSD) tests (${\alpha}=.05$). RESULTS. Fiber-glass posts with composite cores showed the highest fracture resistance values ($915.70{\pm}323N$), and the zirconia post system showed the lowest resistance ($435.34{\pm}220N$). The corresponding mean value for the Ni-Cr casting post and cores was reported as $780.59{\pm}270N$. The differences among the groups were statistically significant (P<.05) for the zirconia group, as tested by ANOVA and Tukey HSD tests. CONCLUSION. The fracture resistance of zirconia post-and-core systems was found to be significantly lower than those of fiber-glass and cast Ni-Cr post systems. Moreover, catastrophic and non-restorable fractures were more prevalent in teeth restored by zirconia posts.

  • Nondestructive Damage Evaluation (NDE) techniques yield the damage location and its size from the modal characteristics of pre-damaged and post-damaged structures. To predict the system reliability of the aging structure, results from the NDE are integrated into the element/component failure probabilities. The element/component failure probabilities can be calculated from failure functions for each element/component with the aid of techniques from a structural reliability analysis. In this paper, a method to estimate the system reliability of a structure that is based on the reliabilities of elements/components in a given structure is presented. The efficacy of the combination of the nondestructive damage detection and the structural reliability evaluation is demonstrated using pre-damaged and post-damaged modal data obtained from numerical simulations of a rigid frame.

  • The data are often missed and many errors of the data are generated in the input process for the post evaluation system on the construction projects, and the reliability of the data falls down much. Accordingly, the detailed analysis about missing and error of data was conducted to ensure reliability of the analysis results about post evaluation on the construction projects. As results in this study, a lot of input data were missed at the initial construction phase, and the data errors were found in the inaccuracy of reference reports, the lack of understanding about input data, and the failure of KRW unit.

  • In clinical orthopaedics, bone resoption in the cortex is often seen post operatively on X-rays or bone densitometry after total hip replacement (THR) in the form of cortical osteoporosis or atropy. Stress shielding of bone occurs, when a load, normally carried by the bone alone, is shared with an implant as a result, the bone stresses are abnormal and with remodelling analysis this may cause extensive proximal bone resoption, possibly weakening the bone bed to the point of failure. The author made finite element models of the cemented and non-cemented type implanted femoral stem with bone resorption of the proximal medial femur and studied the feed back effect of the various degree of bone resoption to THR system by parametric analysis on the stress of the femoral stem and interface. The results of the present finite element analysis implied that the extent of proximal bone resorption has the effect of more increasing stress on the distal stem tip, cement mantle and interface in both type of femoral stem and this high distal stress possibly can cause the mechanical failure of loosening or failure after THR.

  • Weapon systems require reliability in the development phase for efficient combat readiness. Improved reliability in various manufacturing processes have been achieved using data analysis. However, data analysis in the development phase is difficult due to problems such as the lack of data, high cost, and the importance of security. Therefore, Post Logistics Support (PLS) data collected following integration is analyzed for long-term quality improvement of weapon systems. In this study, we propose a methodology for examining the correlation between the failure rate and PLS data as follows: First, key variables affecting reliability were identified the correlation between variables on the failure rate examined. Second, corresponding analysis was conducted for determining the correlation between patterns of categorical data. Third, extract categories with the higher contribution and quality of representation, and find the highest variable correlated with failure period through visualization. Then, after selecting patterns which have shorter failure period, the cause of decreased reliability was confirmed through frequency analysis. This study will contribute to improving reliability when developing new weapon systems and will help to strengthen the combat readiness of military.

Predicting the Compressive Strength of Thin-walled Composite Structure (복합재 박막 구조물의 압축강도 예측)

  • The initial buckling of thin walled structures does not result in immediate failure. This post buckling capability is used to achieve light weight design, and final failure of thin walled structure is called crippling. To predict the failure load, empirical methods are often used for thin walled structures in design stage. But empirical method accuracy depend on geometry. In this study, experimental, empirical and numerical study of the crippling behavior of I-section beam made of carbon-epoxy are performed. The progressive failure analysis model to simulate the crippling failure is evaluated using the test results. In this study, commercial software LS-DYNA is utilized to compute the collapse load of composite specimen. Six kinds of specimens were tested in axial compression where correlation between analytical and experimental results has performed. From the results, we have partially conclude that the flange width-to-thickness ratio is found to influence the accuracy of empirical and numerical method.

Analysis of cladding failure in a BWR fuel rod using a SLICE-DO model of the FALCON code

  • Cladding failure in a fuel rod during operation in a BWR is analyzed using a FALCON code-based model. Comparative calculation with a neighbouring, intact rod is presented, as well. A considerable 'hot spot' effect in cladding temperature is predicted with the SLICE-DO model due to a thermal barrier caused by the localized crud deposition. Particularly significant overheating is expected to occur on the affected side of the cladding of the failed rod, in agreement with signs of significant localized crud deposition as revealed by Post Irradiation Examination. Different possibilities (criteria) are checked, and Pellet-Cladding Mechanical Interaction (PCMI) is shown to be one of the plausible potential threats. It is shown that PCMI could lead to discernible concentrated inelastic deformation in the overheated part of the cladding. None of the specific mechanisms considered can be experimentally or analytically identified as an only cause of the rod failure. However, according to the current calculation, a possibility of cladding failure by PCMI cannot be excluded if the crud thickness exceeded 300 ㎛.

Static and fatigue performance of short group studs connector in novel post-combination steel-UHPC composite deck

  • Casting Ultra High-Performance Concrete (UHPC) on an orthotropic steel deck and forming a composite action by connectors could improve the steel deck fatigue performance. This study presents the mechanical performance of a proposed post-combination connection between UHPC and steel, which had a low constraint effect on UHPC shrinkage. A total of 10 push-out tests were conducted for static and fatigue performance investigations. And the test results were compared with evaluation methods in codes to verify the latter's applicability. Meanwhile, nonlinear simulation and parametric works with material damage plasticity models were also conducted for the static and fatigue failure mechanism understanding. The static and fatigue test results both showed that fractures at stud roots and surrounding local UHPC crushes were the main failure appearances. Compared with normally arranged studs, group arrangement could result in reductions of static stud shear stiffness, strength, and fatigue lives, which were about 18%, 12%, and 27%, respectively. Compared with the test results, stud shear capacity and fatigue lives evaluations based on the codes of AASHTO, Eurocode 4, JSCE and JTG D64 could be applicable in general while the safety redundancies tended to be smaller or even insufficient for group studs. The analysis results showed that arranging studs in groups caused obviously uneven strain distributions. The severer stress concentration and larger strain ranges caused the static and fatigue performance degradations of group studs. The research outcome provides a very important basis for establishing a design method of connections in the novel post-combination steel-UHPC composite deck.
