Clinical performance of FractionLab in patient-specific quality assurance for intensity-modulated radiotherapy : a retrospective study

  • Background: This study was aimed at comparing and analyzing the results of FractionLab (Varian/Mobius Medical System) with those of portal dosimetry that uses an electronic portal imaging device. Portal dosimetry is extensively used for patient-specific quality assurance (QA) in intensity-modulated radiotherapy (IMRT). Methods: The study includes 29 patients who underwent IMRT on a Novalis-Tx linear accelerator (Varian Medical System and Brain-LAB) between June 2019 and March 2021. We analyzed the multileaf collimator DynaLog files generated after portal dosimetry to evaluate the same condition using FractionLab. The results of the recently launched FractionLab at various gamma indices (0.1%/0.1 mm-1%/1 mm) are analyzed and compared with those of portal dosimetry (3%/3 mm). Results: The average gamma passing rates of portal dosimetry (3%/3 mm) and FractionLab are 98.1% (95.5%-100%) and 97.5% (92.3%-99.7%) at 0.6%/0.6 mm, respectively. The results of portal dosimetry (3%/3 mm) are statistically comparable with the QA results of FractionLab (0.6%/0.6 mm-0.9%/0.9 mm). Conclusion: This paper presents the clinical performance of FractionLab by the comparison of the QA results of FractionLab using portal dosimetry with various gamma indexes when performing patient-specific QA in IMRT treatment. Further, the appropriate gamma index when performing patient-specific QA with FractionLab is provided.

A Comparison between Portal Dosimetry and Mobius3D Results for Patient-Specific Quality Assurance in Radiotherapy

  • Purpose: The purpose of this study was to compare the clinical quality assurance results of portal dosimetry using an electronic portal imaging device, a method that is extensively used for patient-specific quality assurance, and the newly released Mobius3D for intensity-modulated radiotherapy (IMRT) and volumetric modulated arc therapy (VMAT). Methods: This retrospective study includes data from 122 patients who underwent IMRT and VMAT on the Novalis Tx and VitalBeam linear accelerators between April and June 2020. We used a paired t-test to compare portal dosimetry using an electronic portal imaging device and the average gamma passing rates of MobiusFX using log files regenerated after patient treatment. Results: The average gamma passing rates of portal dosimetry (3%/3 mm) and MobiusFX (5%/3 mm) were 99.43%±1.02% and 99.32%±1.87% in VitalBeam and 97.53%±3.34% and 96.45%±13.94% in Novalis Tx, respectively. Comparison of the gamma passing rate results of portal dosimetry (3%/3 mm) and MobiusFX (5%/3 mm as per the manufacturer's manual) does not show any statistically significant difference. Conclusions: Log file-based patient-specific quality assurance, including independent dose calculation, can be appropriately used in clinical practice as a second-check dosimetry, and it is considered comparable with primary quality assurance such as portal dosimetry.

용적변조회전 방사선치료에서 Portal Dosimetry를 이용한 선량평가의 재현성 분석 (Evaluate the implementation of Volumetric Modulated Arc Therapy QA in the radiation therapy treatment according to Various factors by using the Portal Dosimetry)

  • 목 적 : 복잡하고 정교하게 계획된 용적변조회전 방사선치료(Volumetric Arc Therapy, VMAT)의 Portal Dosimetry를 이용한 치료 전 선량평가가 다양한 인자에 따라 재현성을 유지하는지 분석하고자 하였다. 대상 및 방법 : 실험에는 TrueBeam STx$^{TM}$(Ver.1.5, Varian, USA), Portal dosimetry application(Ver.10, Varian)과 Portal Vision aS1000 Imager(Varian, USA)을 사용하였다. 두경부암 환자 2명, 전립샘암 환자 3명, 폐암 환자 1명, 자궁경부암 환자 1명, 총 7명의 환자에 대하여 Portal Dosimetry용 VMAT 전산화치료계획(Eclipse, Ver.10.0, Varian, USA)을 수립하였다. 오전 치료 전과 후 오후 치료 종료 후 각 4시간 간격을 두고 3회씩 5일에 걸쳐 선량평가를 시행하였다. Gamma pass rate(GPR 3%, 3mm 95%신뢰구간)와 Beam 출력의 상관관계를 확인하기 위하여 선량평가 시행 전 Beam 출력 을 물등가모형과 이온전리함(IBA dosimetry, Germany)을 이용하여 측정하였다. Electronic Portal Imaging Device(EPID) 상태에 따른 GPR 의 변화를 확인하기 위하여 EPID의 영상 교정 (Dark field correction, Flood field correction) 전과 후로 나누어 선량평가를 시행하였다. 또한 다엽콜리메이터(Multi Leaf Collimator) 상태에 따른 GPR의 변화를 확인하기 위해 다엽콜리메이터 Initialize 전과 후로 나누어 선량평가를 시행하였다. 결 과 : Portal Dosimetry를 시행하여 얻은 모든 환자들의 각 시간대 별 GPR의 평균값은 97.11%, 96.09%, 95.37% 였고 최대 차이를 보인 환자의 경우 각 시간대 별 GPR의 평균값은 95.73%, 94.20% 93.23% 였다. 선량평가 시점의 Beam 출력을 측정한 결과 각 시간대별 평균값은 100.45%, 100.46%, 100.59% 였다. EPID의 영상 교정(Dark field correction, Flood field correction)을 시행하기 전과 후의 대상 환자들의 GPR의 평균값은 95.94 %, 96.01% 였다. 또한 다엽콜리메이터 의 Initialize 를 시행하기 전과 후의 대상 환자들의 GPR 의 평균값은 95.83%, 96.40%였다. 결 론 : 치료기 사용시간이 경과됨에 따라 대상 환자들의 GPR 평균값이 0.8% 감소함을 확인할 수 있었다. Beam 출력은 각 측정시점에 0.1% 오차범위 안에서 일정한 상태를 유지하였다. EPID의 영상 교정 전, 후 의 선량평가 결과 GPR은 평균 0.1% 차이를 나타내었다. 다엽콜리메이터의 Initialize 전, 후의 선량평과 결과 Initialize 시행 후 GPR이 평균 0.6% 상승하였고 다엽콜리메이터 상태에 따라 GPR이 변화할 수 있다는 것을 확인할 수 있었다. 복잡하고 정교하게 계획된 VMAT의 치료 전 선량평가 도구로서 재현성을 유지하며 Portal Dosimetry를 이용하기 위해서는 주기적인 장비의 점검뿐만 아니라, 선량평가에 영향을 미칠 수 있는 다양한 인자들에 대한 관리가 이뤄져야 할 것으로 판단된다.

Gamma Evaluation with Portal Dosimetry for Volumetric Modulated Arc Therapy and Intensity-Modulated Radiation Therapy

  • The aim of this study is to investigate the characteristics of portal dosimetry in comparison with the MapCHECK2 measurments. In this study, a total of 65 treatment plans including both volumetric modulated arc therapy (VMAT) and intensity-modulated radiation therapy (IMRT) were retrospectively selected and analyzed (45 VMAT plans and 20 IMRT plans). A total of 4 types of linac models (VitalBeam, Trilogy, Clinac 21EXS, and Clianc iX) were used for the comparison between portal dosimetry and the MapCHECK2 measurements. The VMAT plans were delivered with two VitalBeam linacs (VitalBeam1 and VitalBeam2) and one Trilogy while the IMRT plans were delivered with one Clinac 21EXS and one Clinacl iX. The global gamma passing rates of portal dosimetry and the MapCHECK2 measurements were analyzed with a gamma criterion of 3%/3 mm for IMRT while those were analyzed with a gamma criterion of 2%/2 mm for VMAT. Spearman's correlation coefficients (r) were calculated between the gamma passing rates of portal dosimetry and those of the MapCHECK2 measurements. For VMAT, the gamma passing rates of portal dosimetry with the VitalBeam1, VitalBeam2, and Trilogy were $97.3%{\pm}3.5%$, $97.1%{\pm}3.4%$, and $97.5%{\pm}1.9%$, respectively. Those of the MapCHECK2 measurements were $96.8%{\pm}2.5%$, $96.3%{\pm}2.7%$, and $97.4%{\pm}1.3%$, respectively. For IMRT, the gamma passing rates of portal dosimetry with Clinac 21EXS and Clinac iX were $99.7%{\pm}0.3%$ and $99.8%{\pm}0.2%$, respectively. Those of the MapCHECK2 measurements were $96.5%{\pm}3.3%$ and $97.7%{\pm}3.2%$, respectively. Except for the result with the Trilogy, no correlations were observed between the gamma passing rates of portal dosimetry and those of the MapCHECK2 measurements. Therefore, both the MapCHECK2 measurements and portal dosimetry can be used as an alternative to each other for patient-specific QA for both IMRT and VMAT.

  • 목 적: 전자포탈영상장치 기반의 환자특이적 정도관리를 위한 portal dose image prediction (PDIP)와 anisotropic analytical algorithm (AAA)을 비교하여 성능을 분석하고, AAA를 사용한 portal dosimetry의 임상적 사용 가능성을 평가하고자 한다. 대상 및 방법: 폐암 환자 15명과 간암 환자 17명, 총 32명의 환자를 후향적으로 선정하였다. PDIP와 AAA를 사용하여 검증용 치료계획을 생성하였다. 계산된 분포와 측정된 분포를 비교한 감마통과율(Gamma passing rate, GPR)과 다엽콜리메이터(Multileaf collimator, MLC) 위치 차이를 얻었다. 결 과: 폐암 환자군의 GPR 평균값은 PDIP 사용시 3%/3 mm에 대해 99.5% ± 1.1%, 1%/1 mm에 대해 90.6% ± 5.8%였다. AAA 사용시 3%/3 mm에 대해 98.9% ± 1.7%, 1%/1 mm에 대해 87.8% ± 5.2%였다. 간암 환자군의 GPR 평균값은 PDIP 사용시 3%/3 mm에 대해 99.9% ± 0.3%, 1%/1 mm에 대해 96.6% ± 4.6%였다. AAA 사용시 3%/3 mm에 대해 99.6% ± 0.5%, 1%/1 mm에 대해 89.5% ± 6.4%였다. MLC 위치 차이는 0.013 mm ± 0.002 mm로 적었으며, 감마통과율과 상관관계를 보이지 않았다. 결 론: 전자포탈영상장치 기반 환자특이적 정도관리를 수행할 때 AAA를 임상적으로 portal dosimetry 계산 알고리즘으로써 사용할 수 있다.

EPID (Electronic Portal Imaging Device)의 유용성에 관한 고찰 (Review on Usefulness of EPID (Electronic Portal Imaging Device))

  • 목 적: 방사선 치료 중 정확한 환자의 셋업 확인과 선량 측정용으로 사용되었던 film을 대신 하여 현재는 전자포탈영상장치(EPID)가 장착된 장비가 증가하고 있다. 이에 본 논문은 전자포탈영상장치 사용 시 자세확인의 정확성과 선량측정의 유용성을 평가해 보고자 한다. 대상 및 방법: 대한방사선치료학회, 대한방사선종양학회, Pubmed에서 "EPID", "Portal dosimetry", "Portal image", "Dose verification", "Quality control", "Cine mode", "Quality - assurance", "In vivo dosimetry"와 같은 용어로 검색하여 획득한 50개의 자료(1997~2012)를 대상으로 EPID의 역사와 선량측정(dosimetry), 자세확인(set-up verification), EPID 특성으로 구분하여 EPID의 유용성을 분석 하였다. 결 과: EPID는 1세대 Liquid-filled ionization chamber, 2세대 Camera based fluroscopic, 3세대 Amorphous-silicon 순으로 발전하였으며, EPID 촬영 모드에는 크게 EPID mode, Cine mode, Integrated mode로 나뉜다. 필름과 EPID의 절대선량정확성 평가를 한 결과 EPID는 1%, EDR2 필름은 3% 이내로 나타나 오차 측정 정확도가 필름에 비해 EPID가 우수하다는 것을 알 수 있었고, 치료계획 시스템으로부터 계산된 기준 조사면과 EDR2 필름, EPID로 측정한 기준 조사면의 선량 분포를 중첩하여 감마 분석한 결과 필름과 EPID 모두 허용기준 3%/3 mm와 2%/2 mm에서 감마값이 1을 초과하는 화소(r%>1)가 전체 화소의 2% 이내였다. 또한 업무 부하 비교에 있어 세기조절방사선 치료에서 전 과정 QA를 수행하는데 소요되는 시간은 EDR2 필름이 약 110분, EPID가 약 55분으로 측정되었다. 결 론: 전자포탈영상장치의 이용은 선량측정과 자세확인에 있어 기존의 복잡하고 번거로웠던 film과 전리조(Ionization chamber)를 대체하기에 충분하였으며, 특히 세기조절방사선치료의 정도관리에 있어 매우 유용하고 효율적이며 정확한 선량 측정 장치임을 알 수 있었다. 또한, 전자포탈영상장치를 이용한 Cine mode 촬영은 횡격막의 움직임에 따라 유동성이 큰 폐와 간의 경우나 자세의 안정성이 불안한 직장암 환자의 경우 추가 선량 없이 실시간으로 종양의 위치를 확인 할 수 있다는 장점이 있어 최적의 방사선 치료 구현이 가능하리라 사료된다.

A Comparison Study of Volumetric Modulated Arc Therapy Quality Assurances Using Portal Dosimetry and MapCHECK 2

  • A Varian Portal Dosimetry system was compared to an isocentrically mounted MapCHECK 2 diode array for volumetric modulated arc therapy (VMAT) QA. A Varian TrueBeam STx with an aS-1000 digital imaging panel was used to acquire VMAT QA images for 13 plans using four photon energies (6, 8, 10 and 15 MV). The EPID-based QA images were compared to the Portal Dose Image Prediction calculated in the Varian Eclipse treatment planning system (TPS). An isocentrically mounted Sun Nuclear MapCHECK 2 diode array with 5 cm water-equivalent buildup was also used for the VMAT QAs and the measurements were compared to a composite dose plane from the Eclipse TPS. A ${\gamma}$ test was implemented in the Sun Nuclear Patient software with 10% threshold and absolute comparison at 1%/1 mm (dose difference/distance-to-agreement), 2%/2 mm, and 3%/3 mm criteria for both QA methods. The two-tailed paired Student's t-test was employed to analyze the statistical significance at 95% confidence level. The average ${\gamma}$ passing rates were greater than 95% at 3%/3 mm using both methods for all four energies. The differences in the average passing rates between the two methods were within 1.7% and 1.6% of each other when analyzed at 2%/2 mm and 3%/3 mm, respectively. The EPID passing rates were somewhat better than the MapCHECK 2 when analyzed at 1%/1 mm; the difference was lower for 8 MV and 10 MV. However, the differences were not statistically significant for all criteria and energies (p-values >0.05). The EPID-based QA showed large off-axis over-response and dependence of ${\gamma}$ passing rate on energy, while the MapCHECK 2 was susceptible to the MLC tongue-and-groove effect. The two fluence-based QA techniques can be an alternative tool of VMAT QA to each other, if the limitations of each QA method (mechanical sag, detector response, and detector alignment) are carefully considered.

Verification of Extended Source-To-Imager Distance (SID) Correction for Portal Dosimetry

  • This study aimed to evaluate and verify a process for correcting the extended source-to-imager distance (SID) in portal dosimetry (PD). In this study, eight treatment plans (four volumetric modulated arc therapy and four intensity-modulated radiation therapy plans) at different treatment sites and beam energies were selected for measurement. A Varian PD system with portal dose image prediction (PDIP) was used for the measurement and verification. To verify the integrity of the plan, independent measurements were performed with the MapCHECK device. The predicted and measured fluence were evaluated using the gamma passing rate. The output ratio was defined as the ratio of the absolute dose of the reference SID (100 cm) to that of each SID (120 cm or 140 cm). The measured fluence for each SID was absolutely and relatively compared. The average SID output ratios were 0.687 and 0.518 for 120 SID and 140 SID, respectively; the ratio showed less than 1% agreement with the calculation obtained by using the inverse square law. The resolution of the acquired EPIDs were 0.336, 0.280, and 0.240 for 100, 120, and 140 SID, respectively. The gamma passing rates with PD and MapCHECK exceeded 98% for all treatment plans and SIDs. When autoalignment was performed in PD, the X-offset showed no change, and the Y-offset decreased with increasing SID. The PD-generated PDIP can be used for extended SID without additional correction.

섬광판을 사용하는 조사문영상기구의 기본적인 선량반응성 (Basic Dose Response of Fluorescent Screen-based Portal Imaging Device)

  • 목적 : 본 연구의 목적은 섬광판을 사용하는 전자조사문영상장치의 기본적인 선량반응성을 연구하는 것이다. 대상 및 방법 : 팬텀의 두께와 구성 및 팬텀과 영상장치와의 거리를 변화시킴으로 영상장치에 입사하는 방사선질을 변화시켜가면서 조사면평면에서 신호를 읽었다. 또한 섬광에 의한 신호가 전체신호에 기여하는 상대적인 정도를 모사하였다. 결과 : 본 연구는 조사면평면상에서 선량분포가 팬텀과의 거리 및 팬텀의 두께의 함수임을 보였다. 팬텀의 구성에 따라 조사면영상장치에 의한 선량의 오차는 크게는 $7\%$ 에 달하는 것을 발견했다. 영상장치 내부에서 발생하는 섬광의 산란이 오차의 중요한 원인이 됨을 또한 발견했다. 결론 : 본 연구에서 우리는 조사면영상장치의 선량반응성의 기본적인 특성을 알았다. 또한 섬광판을 기초로 한 영상장치는 정확한 선량측정시스템이 되지 못함을 보았다.

EPID의 적정관리 도구로서의 유용성에 관한 연구 (The useage of the EPID as a QA tools)

  • Purpose : The aim of this study is to conform the possibility of the liquid type EPID as a QC tools to clinical indication and of replacement of the film dosimetry. Aditional aim is to describe a procedure for the use of a EPID as a physics calibration tool in the measurements of radiation beam parameters which are typically carried out with film. Method & Materials : In this study we used the Clinac 2100c/d with EPID. This system contains 65536 liquid-filled ion chambers arranged in a $256{\times}256$ matrix and the imaging area is $32.5{\times}32.5cm$ with liquid layer thickness of 1mm. The EPID was tested for different field sizes under typical clinical conditions and pixel values were calibrated against dose by producing images using various thickness of lead attenuators(lead step wedge) using 6 & 10MV x-ray. We placed various thickness of lead on the table of linear accelerator and set the portal vision an SDD of 100cm. To acquire portal image we change the field size and energy, and we recorded the average pixel value in a $3{\times}3$ pixel region of interest(ROI) at field center was recorded. The pixel values were also measured for different field sizes in order to evaluate the dependence of pixel value on x-ray energy spectrum and various scatter components. Result : The EPID, as a whole, was useful as a QA tool and dosimetry device. In mechanical check, cross-hair centering was well matched and the error was less than ?2mm and light/radiation field coincidence was less than 1mm also. In portal dosimetry the wider the field size the the higher the pixel value and as the lead thickness increase, the pixel value was exponentially decreased. Conclusions : The EPID was very suitable for QA tools and it can be used to measure exit dose during patients treatment with reasonable accuracy.
