2001년 국민건강영양조사 자료를 이용한 한국 성인의 건강증진용 식품 사용실태 (Health Intended Food Use by Korean Adults: Data from the Korean National Health and Nutrition Examination Survey 2001)

  • Data from the 2001 Korea Health and Nutrition Examination Survey, a nationally representative, cross-sectional survey, were analyzed to assess prevalence of health intended food use overall and in relation to socio-demographic and lifestyle characteristics. Health intended food were classified into four broadly defined categories: vitamin and mineral supplements, health aid food, animal protein based restorative food and medicinal plants. Thirty three percent of adults aged 20 years or older reported taking health intended food in the past year : twenty four percent took vitamin or mineral supplements and medicinal plants were taken by 17 % of adults, health aid food by 15% and animal protein based restorative food by 6%. In multivariate logistic regression, female gender, older age, more education, regular exercise and non smoking were associated with greater use of vitamin or mineral supplements. Dietary quality was significantly associated with use of vitamin or mineral supplements or health aid food but not with use of medicinal plants or animal protein based restorative foods. Obese persons were less likely to take vitamin or mineral supplements. These findings suggest that epidemiologic studies of diet, demographic or lifestyle and health take health intended food use into account because of high prevalence of use of the food by the population and differential use of the food by socio-demographic and lifestyle characteristics.

The Health Examinees (HEXA) Study: Rationale, Study Design and Baseline Characteristics

  • Background: Korea has experienced rapid economic development in a very short period of time. A mixture of traditional and modern risk factors coexists and the rapid change in non-genetic factors interacts with genetic constituents. With consideration of these unique aspects of Korean society, a large-scale genomic cohort study-the Health Examinees (HEXA) Study-has been conducted to investigate epidemiologic characteristics, genomic features, and gene-environment interactions of major chronic diseases including cancer in the Korean population. Materials and Methods: Following a standardized study protocol, the subjects were prospectively recruited from 38 health examination centers and training hospitals throughout the country. An interview-based questionnaire survey was conducted to collect information on socio-demographic characteristics, medical history, medication usage, family history, lifestyle factors, diet, physical activity, and reproductive factors for women. Various biological specimens (i.e., plasma, serum, buffy coat, blood cells, genomic DNA, and urine) were collected for biorepository according to the standardized protocol. Skilled medical staff also performed physical examinations. Results: Between 2004 and 2013, a total of 167,169 subjects aged 40-69 years were recruited for the HEXA study. Participants are being followed up utilizing active and passive methods. The first wave of active follow-up began in 2012 and it will be continued until 2015. The principal purpose of passive follow-up is based on data linkages with the National Death Certificate, the National Cancer Registry, and the National Health Insurance Claim data. Conclusions: The HEXA study will render an opportunity to investigate biomarkers of early health index and the chronological changes associated with chronic diseases.

6세미만 도시 주변거주 어린이의 면역능력에 따르는 영양상태 판정에 관한 연구 (A Survey of Nutritional-Immunologic Interactions in the Children Under 6 Years Old in the Suburbs of Seoul)

  • Clinical and epidemiologic data point to a causal interrelationship between nutritional deficiency and infectious illness. Both are major contributors to childhood morbidity and mortality, particulary in underprivileged population groups. Protein-calorie malnutrition depress a variety of immune funtions. Delayed hypersensitivity and number of T-lymphocytes are consistently reduced. The interrelationship between nutritional status and immune response was studied in 80 children aged under 6 years. According to the anthropometric assessment based on weight for height for age, 36 children were classified as normal, 22 as morderate malnutrition, and 22 as severe malnutrition. The following determinations were made : hemeglobin, hematocrit, serum albumin, immunoglobulin G, complement 3, and WBC levels. Results indicate that levels of Hb, Hct, serum albumin, and C3 concentration were decreased in moderate and severe malnutrition children. However, levels of IgG and WBC were not affected by the nutritional status. It is suggested that nutritional status has more profound effects on complement system than humoral immunity.

Epidemiologic Survey of Infantile Cancer in Iran based on the Data of the Largest Pediatric Cancer Referral Center (Ali-Asghar Children Hospital), 1996-2005

  • Background: Cancer in infants younger than one year of age represents a unique problem with distinct epidemiological, clinical and genetic characteristics compared with older age groups. No report is yet available from Iran regarding epidemiological and survival rate of cancers diagnosed in this age group. Materials and Methods: The population under study comprised of patients which were diagnosed and admitted to Ali-Asghar hospital between years 1996-2005. In total, 287 infants were included in the retrospective descriptive survey. Patient files were evaluated for age of patient at the time of diagnosis, sex, geographical residence, consanguinity of parents, histological diagnosis, site of cancer involvement, type of therapy, date of last follow-up and cause of death (if applicable). Results: The average age at the time of diagnosis was 7.2 months old. The most frequent malignancy was retinoblastoma (44%), followed by leukemia (19%) and neuroblastoma (10%), with five-year overall survival rates of 77.7%, 41% and 90%, respectively Parents of 40 infants (13.9%) had consanguinity relationships. Conclusions: Although we cannot make any conclusions regarding the incidence of infant cancer subtypes based on this study, survival rates for major types were similar to the developed countries, which signifies strict adherence to standards of care in Ali-Asghar hospital, the main infant cancer care centre in Iran. A Childhood Cancer Registry with high-resolution data collection and also advanced genetic testing is advocated for in-depth analysis of variation in incidence and survival.

Chocolate consumption and the prevalence of metabolic syndrome in the Korean adult population: an analysis based on the 2014-2016 Korea National Health and Nutrition Examination Survey

  • BACKGROUND/OBJECTIVES: Recent studies have reported chocolate consumption has beneficial effects on blood pressure, obesity, and dyslipidemia, which are components of metabolic syndrome (MetS). However, evidence regarding these relationships in Asia is limited. The aim of this study was to investigate relations between chocolate consumption and MetS in Korea. SUBJECTS/METHODS: Study subjects were 6,982 adults aged 19-64 years free of major chronic diseases that participated in the 2014-2016 Korea National Health and Nutrition Examination Survey. Information on chocolate consumption was obtained using food frequency questionnaires. For primary analysis, subjects were divided into two groups: non-consumer (n = 3,741, 53.6%); consumer (n = 3,241, 46.4%). In order to investigate dose-response effects on MetS, chocolate consumers were divided into quartiles based on amounts consumed: Q1 (n = 789, 24.3%); Q2 (n = 837, 25.8%); Q3 (n = 863, 26.6%); Q4 (n = 752, 23.2%) in second analysis. RESULTS: Chocolate consumers; 1) were more physically active(P < 0.0001); 2) less likely to smoke(P < 0.0001); 3) paid more attention to food nutrition labels (P < 0.0001), and 4) had healthier dietary patterns, that is, they consumed vegetables, fruits, and dairy products more frequently(P < 0.0001) than non-consumers. Furthermore, chocolate consumers had a 30% lower risk of MetS (odds ratio, 0.70; 95% confidence interval, 0.58-0.86; P = 0.0005). However, dose-response effects between chocolate intake and MetS was not found by secondary analysis after excluding non-consumers. CONCLUSIONS: In conclusion, chocolate consumers had healthier lifestyles and dietary patterns and a lower prevalence of MetS than non-consumers, as determined by our primary analysis. Although dose-response effects were not found to be significant among chocolate consumers during secondary analysis, the patterns were partially similar. This cross-sectional finding shows chocolate consumption does not increase the risk of MetS. The prospective studies and intervention trials in Korea are needed to confirm this cross-sectional finding.

인구역학적 요인에 따른 한국인의 사시 및 눈떨림 유병률 조사: 제5기 국민건강영양조사 자료 (An Epidemiologic Survey of Strabismus and Nystagmus in South Korea: KNHANES V)

  • 목적: 사시 질환은 어느 연령대에서도 발병 가능하며 다양한 기능적 또는 비기능적 안과적 문제를 발생시킬 수 있다. 인구 기반 연구를 통해 다양한 인구역학적 요인에 따른 한국의 사시, 눈떨림 등을 포함한 사시 질환의 유병률을 확인하고자 한다. 대상과 방법: 2008년 7월부터 2011년 12월까지 실시된 국민건강영양조사(National Health and Nutrition Examination Survey, n=30,538)의 자료를 사용하여 분석하였다. 사시와 눈떨림의 유병률을 파악하고, 관련된 인구역학적 요인에 대해 평가하였다. 결과: 3세 이상에서 사시 및 눈떨림 환자의 전체 유병률은 $1.4{\pm}0.1%$ (95% confidence interval [CI] 1.3-1.6%, 남자 $1.5{\pm}0.1%$, 여자 $1.3{\pm}0.1%$)였다. 외사시(15프리즘디옵터 이상), 내사시(10프리즘디옵터 이상), 수직 편위, 기타 복잡한 사시 및 눈떨림의 유병률은 각각 1.0%, 0.2%, 0.2%, 0.1% 및 0.1%였다. 유병률은 6-15세 연령층에서 가장 높았으며($1.9{\pm}0.3%$), 40-49세($0.8{\pm}0.1%$)에서 가장 낮았다(p=0.005). 성별, 지역, 주거 지역, 가구 소득, 교육 수준 및 직업과 관련하여 통계적으로 유의한 차이는 없었다. 결론: 국민건강영양조사 자료를 분석한 결과 한국의 인구역학적 요인에 따른 사시 질환의 유병률은 연령 집단을 제외하고는 통계적으로 유의하지 않았다. 향후 사시 질환의 유병률과 관련된 추가적인 다른 보건 및 사회적 지표들의 검토가 유병률 연구에 도움이 될 것으로 판단된다.

Four years Incidence Rate of Colorectal Cancer in Iran: A Survey of National Cancer Registry Data - Implications for Screening

  • Introduction: Design and implementation of screening programs in each country must be based on epidemiological data. Despite the relatively high incidence of CRC, there is no nationwide comprehensive program for screening in Iran. This study was designed to investigate national CRC data and help to determine guidelines for screening. Methods: Incidence data used in this study were obtained from Iranian annual of National Cancer Registration report. Age standardized rates (ASR)were calculated using world standard population and were categorized by age, sex, anatomic subsite and morphology of tumor. Data were analyzed using SPSS.V.13 and Open Source Epidemiologic Statistics for Public Health software (OpenEpi v.2.3.1). Results: A quarter of cases were less than 50 years of age. The majority of tumors were detected in the colon. The overall ASR in the four years period was 38.0 per 100000 and was higher for men compared women (P<0.05). Incidence rate of colorectal cancer increased with age. Conclusion: Results of present study indicated that incidence of colorectal cancer is relatively high in Iran. Incidence of CRC in people under 50 years and in rectum were reported higher than other countries that related etiologic factors should be investigate in further studies. According to the increasing of ASR after age 50 years, it seems that onset of screening at age 50 would be appropriate.

Development and validation of prediction equations for the assessment of muscle or fat mass using anthropometric measurements, serum creatinine level, and lifestyle factors among Korean adults

  • BACKGROUND/OBJECTIVES: The measurement of body composition, including muscle and fat mass, remains challenging in large epidemiological studies due to time constraint and cost when using accurate modalities. Therefore, this study aimed to develop and validate prediction equations according to sex to measure lean body mass (LBM), appendicular skeletal muscle mass (ASM), and body fat mass (BFM) using anthropometric measurement, serum creatinine level, and lifestyle factors as independent variables and dual-energy X-ray absorptiometry as the reference method. SUBJECTS/METHODS: A sample of the Korean general adult population (men: 7,599; women: 10,009) from the Korean National Health and Nutrition Examination Survey 2008-2011 was included in this study. The participants were divided into the derivation and validation groups via a random number generator (with a ratio of 70:30). The prediction equations were developed using a series of multivariable linear regressions and validated using the Bland-Altman plot and intraclass correlation coefficient (ICC). RESULTS: The initial and practical equations that included age, height, weight, and waist circumference had a different predictive ability for LBM (men: R2 = 0.85, standard error of estimate [SEE] = 2.7 kg; women: R2 = 0.78, SEE = 2.2 kg), ASM (men: R2 = 0.81, SEE = 1.6 kg; women: R2 = 0.71, SEE = 1.2 kg), and BFM (men: R2 = 0.74, SEE = 2.7 kg; women: R2 = 0.83, SEE = 2.2 kg) according to sex. Compared with the first prediction equation, the addition of other factors, including serum creatinine level, physical activity, smoking status, and alcohol use, resulted in an R2 that is higher by 0.01 and SEE that is lower by 0.1. CONCLUSIONS: All equations had low bias, moderate agreement based on the Bland-Altman plot, and high ICC, and this result showed that these equations can be further applied to other epidemiologic studies.

한국 성인의 수정체 혼탁 유형에 따른 백내장의 유병률 (Prevalence of Cataract with Different Type of Lens Opacity in the Korean Population)

  • 목적: 40세 이상 한국 성인남녀에서 수정체의 혼탁 종류에 따른 백내장의 유형별 유병률을 산출하고 교정시력을 알아보았다. 방법: 제4기 국민건강영양조사(NHANES) 중 2009년 자료를 이용하여 인구 기반 단면 연구로 디자인하였다. 40세-95세까지 총 4,977명에 대해 연령과 성별에 따라 층화하여 백내장의 유병률과 수정체 혼탁의 종류에 따른 유병률을 조사하였다. 전체 대상자의 평균 연령은 $58.28{\pm}12.07$세 였고, 남자가 2,142명(43.0%), 여자가 2,835명(57.0%)이었다. 교정시력은 0.63 미만인 경우를 산출하였다. 결과: 전체대상자 4,977명 중에서 현재 백내장이 있는 경우는 40.6%(남자 17.5%, 여자 23.1%)로 나타났다. 백내장의 유형별 유병률은 핵형(56.3%), 피질형(23.4%), 혼합형(16.2%), 전낭하(3.0%), 후낭하(1.1%) 순으로 나타났다. 유형별 남녀의 차이는 없었고, 모든 연령대에서 핵형이 가장 많았다. 전낭하 백내장은 대부분의 경우에서 교정시력이 0.63 미만으로 낮았다. 결론: 핵형 백내장의 유병률이 가장 높았고, 수정체의 혼탁을 고려한 역학 연구와 예방 정책의 수립이 필요할 것으로 사료된다.

전립선염 증상지수를 이용한 전립선염 증상의 규모와 삶의 질에 미치는 영향 (Size of Prostatitis Symptoms Using Prostatitis Symptom Index(PSI): The Effect of Prostatitis Symptoms on Quality of Life)

  • Objectives : To determine the prevalence of prostatitis symptoms in the general population by questionnaire survey and to measure the effect of prostatitis symptoms on quality of life(QOL). Materials & Methods : A cross sectional community-based epidemiologic study was performed on 2,034 men, living in the Seoul metropolitan area using stepwise random sampling. Out of 2,034 interviewees, 1,356 men who were older than 40 and provided sufficient information were selected for this study. The questionnaires were completed by well trained interviewers. Contents of the questionnaires included demographic data, the Prostatitis Symptom Index(PSI), the International Prostate Symptom Score(IPSS), a general health questionnaire section and a sexual health questionnaire section. The PSI was composed of a sum of the scores from three questions about dysuria, penile pain and perineal pain and it ranged 0 to 12. Incidence of prostatitis symptoms was defined by a score of 4 or more and the reference group was defined as consisting of those with a score of 3 or less. The rate of incidence of prostatitis symptoms was assessed according to age and the difference of QOL between the prostatitis symptoms group and the reference group. Results : The overall positive rate of prostatitis symptoms measured by the PSI, in men older than 40, living in the Seoul metropolitan area, was 4.5%(61/1,356), adjusted to 4.8% by the relative proportion of this age group in the general population of the Seoul metropolitan area as compared to Korea and the World. The proportion of the group with prostatitis symptoms assessed by the PSI did not increase with age although the proportion of participants with moderate to severe lower urinary tract symptoms (LUTS) did increase with age. The group with prostatitis symptoms suffered from a much greater incidence of LUTS compared to the reference group (p<0.05). The QOL scores of the IPSS, and the general health and sexual health status of the group with prostatitis symptoms, were worse than those of the reference group.(p<0.05). Conclusions : The positive rate of prostatitis symptoms in men older than 40, living in the Seoul metropolitan area, was 4.8% and it didn't increase with age. The general QOL of the group with prostatitis symptoms was much worse than that of the reference group.
