Correlations between HER2 Expression and Other Prognostic Factors in Breast Cancer: Inverse Relations with the Ki-67 Index and P53 Status

  • Background: Overexpression or amplification of human epidermal growth factor receptor-2 (HER2) is associated with grade of malignancy and a poor prognosis in breast cancer (BC). The aim of this study was to evaluate of value of HER2 as a prognostic marker, and to analyze associations with common histopathological parameters in BC cases. Materials and Methods: Between of 2007 to 2014, 260 patients with BC referred to Oncology Clinic provided cancer tissue samples which underwent immunohistochemistry (IHC) for markers. ER and PR positivity was defined as ${\geq}10%$ positive tumor cells with nuclear staining. HER2-positive was defined as either HER2 gene amplification by fluorescent in situ hybridization (FISH) or scored as 3+ by IHC. For HER2 (2+), FISH was performed to determine HER2 positivity. Results: The mean age at diagnosis for the patients with HER2-negative was significantly higher than in HER2-positive cases. Also, there were significant correlations between histological grade, nuclear grade, lymph node metastasis, tumor size, ER status, PR status, p53 overexpression and Ki-67 index with HER2 expression. HER2-negative lesions were of higher grade and more likely to be ER-negative, PR-negative, p53-positive, lymph node metastasis, with a tumor size<2cm and also $Ki-67{\geq}20%$ as compared to the HER2-positive group. Conclusions: Contrary to the results of other studies, HER2-positive tumors in our study had a lower Ki-67 index and were p53-positive. Also, Ki-67 proliferation index ${\geq}20%$ in more studies was associated with p53-positive.Therefore, tumors which are HER2-positive and have a Ki-$67{\geq}20%$ had a more aggressive behavior compared to HER2-positive and Ki-67<20% lesions.

Silk Dyeing Method in Natural Pigments - In Case of Korean Colored Rice Bran - (한국산 유색미 속겨의 안토시아닌 색소에 의한 견직물염색)

  • This research was conducted to establish the efficient use of the colored rice bran fur dyeing textiles. To investigate the fabric dyeability of the colored rice bran extract, the anthocyanin pigments were extracted with water of different temperature ranges of 40 ~ 8$0^{\circ}C$ and were dyed on silk fabrics with different dyeing temperature $25^{\circ}C$~6$0^{\circ}C$, at acidic pH and neutral pH, respectively. Aluminum chloride was preheated with mordant K/S value and dyeing fastness of dyed silk fabrics were examined. The anthocyanins of the colored rice bran were stable and red color at acidic pH, red purple or purple blue at neutral pH, but unstable, blue color at alkaline pH. If extracting temperature and dyeing temperature of dyeing solution were higher, the dyeability was high, but the color of dyed fabric showed red tone. When extracting temperature was 8$0^{\circ}C$ and dyeing temperature of dyeing solution was 6$0^{\circ}C$, the dyeability was best. Without mordant, the dyeability of silk fabrics was higher in acidic pH than in neutral pH solution. With mordant, the dyeability was higher than without mordant, and also higher in acidic pH. Pretreatment of aluminum chloride resulted in the increase of color intensity and stability. The laundering fastness of dyed fabrics was good from grade 5 to grade 3-4. Because of the anthocyanins sensitivity on light radiation, the light fastness of dyed fabrics was poor from grade 3 to grade 1-2.

The Role of Lumbar Drainage to Prevent Shunt-Dependent Hydrocephalus after Coil Embolization for Aneurysmal Subarachnoid Hemorrhage in Good-Grade Patients

  • Objective : To evaluate the role of lumbar drainage in the prevention of shunt-dependent hydrocephalus after treatment of ruptured intracranial aneurysms by coil embolization in good-grade patients. Methods : One-hundred-thirty consecutive patients with aneurysmal subarachnoid hemorrhage in good-grade patients (Hunt & Hess grades I-III), who were treated by coil embolization between August 2004 and April 2010 were retrospectively evaluated. Poor-grade patients (Hunt & Hess grades IV and V), a history of head trauma preceding the development of headache, negative angiograms, primary subarachnoid hemorrhage (SAH), and loss to follow-up were excluded from the study. We assessed the effects on lumbar drainage on the risk of shunt-dependent hydrocephalus related to coil embolization in patients with ruptured intracranial aneurysms. Results : One-hundred-twenty-six patients (96.9%) did not develop shunt-dependent hydrocephalus. The 2 patients (1.5%) who developed acute hydrocephalus treated with temporary external ventricular drainage did not require permanent shunt diversion. Overall, 4 patients (3.1%) required permanent shunt diversion; acute hydrocephalus developed in 2 patients (50%). There was no morbidity or mortality amongst the patients who underwent a permanent shunt procedure. Conclusion : Coil embolization of ruptured intracranial aneurysms may be associated with a lower risk for developing shunt-dependent hydrocephalus, possibly by active management of lumbar drainage, which may reflect less damage for cisternal anatomy than surgical clipping. Coil embolization might have an effect the long-term outcome and decision-making for ruptured intracranial aneurysms.

The Clinical Study of Lumbar Spinal Stenosis in Oriental Medical Hospital (요추관협착증의 한의학적 보존적 치료에 대한 임상적 고찰)

  • This clinical study was carried out 15 cases with lumbar spinal stenosis patients, who had been admitted and diagnosed by Computed Tomography from June, 1999 to May, 2000, in the department of acupuncture and moxibustion. Hanseo University Oriental Medical Nospital. The results obtained from this study were as follows; 1. We investigated 10 female and 5 male patients. The age distribution was from early 20's to 70's, among which 30's and 50's were most common. 2. The most common duration of symptom was less than a year(46.7%). 3. Regarding to the number of the involved levels, one was 12 cases(80%), two was 3 cases(20.0%). In the one level cases, L4~5 was the most common involved site. 4. Regarding to the clinical symptoms, low back pain was the most common, followed by lower extremity radiating pain, intermittent claudication, lower extremity paresthesia. 5. In orthopaedic examinations, positive sign in Milgram test was 80.0%, positive sign in straight leg raising test was 40.0%, and positive sign in Braggard test was 33.3%. Inneurologic examinations, sensory loss was seen in 60.0%, motor weakness and decreased deep tendon reflex were 26.7% in each. 6. According to clinical symptom scale proposed by Chae(1989), grade I1I was the most common (53.3%), followed by grade II, grade IV, grade I . 7. According to treatment outcome scale proposed by Chae(1989), Good was the most common (73.3%), followed by Excelleat, Fair and Poor.

Oral health literacy of upper grade elementary school children (초등학교 고학년 아동의 구강건강문해력 평가)

  • Objectives: The purpose of this study was to examine oral health literacy in a sample of Korean of 5th and 6th grade elementary school children. Methods: Data were obtained from a convenience sample of 274 $5^{th}$ and $6^{th}$ grade children from two elementary schools located in Seoul and Uijeongbu. The oral health literacy assessment tool for children of Korea (OHLC-K) takes approximately 11 minutes to complete, and consists of 20 self-report questions with a score range of 0 to 20. Item analysis, descriptive statistics, correlation and multiple linear regression were performed using SPSS 23.0 and testAn 1.0. Results: The lowest and highest percentage of correct answers were found in "Definition of periodontal disease (19.3%)" and "Definition of halitosis (92.7%)," respectively. The mean score for oral health literacy was found to be $14.95{\pm}2.83$, corresponding to a score of approximately 75% correct answers. The correlation coefficient for Korean and OHLC-K scores was high (r=.73, P<.01). In the multiple linear regression, oral health literacy was associated with grade and gender. Conclusion: The present findings revealed that a quarter of the subjects were likely to misunderstand oral health information. Oral health education is indispensable to all children and, therefore, it is necessary to pay greater attention to children who exhibit poor oral health literacy.

  • Glioma is a type of brain tumor that occurs in glial cells and is classified into two types: high hrade hlioma with a poor prognosis and low grade glioma. Magnetic resonance imaging (MRI) as a non-invasive method is widely used in glioma diagnosis research. Studies to obtain complementary information by combining multiple modalities to overcome the incomplete information limitation of single modality are being conducted. In this study, we developed a 3D CNN-based model that applied input-level fusion to MRI of four modalities (T1, T1Gd, T2, T2-FLAIR). The trained model showed classification performance of 0.8926 accuracy, 0.9688 sensitivity, 0.6400 specificity, and 0.9467 AUC on the validation data. Through this, it was confirmed that the grade of glioma was effectively classified by learning the internal relationship between various modalities.

Arterial Spin Labeling MRI for Quantitative Assessment of Cerebral Perfusion Before and After Cerebral Revascularization in Children with Moyamoya Disease

  • Objective: To determine the correlation between cerebral blood flow (CBF) on arterial spin labeling (ASL) MRI and the degree of postoperative revascularization assessed on digital subtraction angiography in children with moyamoya disease (MMD). Materials and Methods: Twenty-one children (9 boys and 12 girls; mean age, 8.4 ± 3.6 years; age range, 3-16 years) with MMD who underwent both pseudocontinuous ASL MRI at 1.5T and catheter angiography before and after superficial temporal artery encephaloduroarteriosynangiosis were included in this retrospective study. The degree of revascularization in the middle cerebral artery (MCA) territory was evaluated on external carotid angiography and was graded on a 3-point scale. On ASL CBF maps, regions of interest were manually drawn over the MCA territory of the operated side at the level of the centrum semi-ovale and over the cerebellum. The normalized CBF (nCBF) was calculated by dividing the CBF of the MCA territory by the CBF of the cerebellum. Changes in nCBFs were calculated by subtracting the preoperative nCBF values from the postoperative nCBF values. The correlation between nCBF changes measured with ASL and the revascularization grade from direct angiography was evaluated. Results: The nCBF value on the operated side increased after the operation (p = 0.001). The higher the degree of revascularization, the greater the nCBF change was: poor revascularization (grade 1), -0.043 ± 0.212; fair revascularization (grade 2), 0.345 ± 0.176; good revascularization (grade 3), 0.453 ± 0.182 (p = 0.005, Jockheere-Terpstra test). The interobserver agreement was excellent for the measured CBF values of the three readers (0.91-0.97). Conclusion: The nCBF values of the MCA territory obtained from ASL MRI increased after the revascularization procedure in children with MMD, and the degree of nCBF change showed a significant correlation with the degree of collateral formation evaluated via catheter angiography.

Comparative Analysis of the Efficacy and Safety of Oxaliplatin Plus 5-Fluorouracil/Leucovorin (Modified FOLFOX6) with Advanced Gastric Cancer Patients having a Good or Poor Performance Status

  • Background: Combination chemotherapy of 5 fluorouracil (5-FU) and leucovorin (LV) with oxaliplatin, mainly FOLFOX regimens, has shown considerable antitumor activity and a tolerable toxicity profile in gastric cancer. The goal of this study was to retrospectively compare the efficacy and toxicity of modified FOLFOX-6 (mFOLFOX6) regimen in advanced gastric cancer (AGC) patients with good and poor performance status (PS). Materials and Methods: AGC patients receiving the mFOLFOX6 regimen including oxaliplatin $85mg/m^2$, bolus of 5-FU $400mg/m^2$ and LV $400mg/m^2$ on the first day, followed by $2400mg/m^2$ of 5- FU as a continious infusion over 46 hour for first-line treatment were eligible for the study. Results: A total 58 patients with a median age of 59.5 (32-81) were included. The median follow up of the study was 9.2 months. Thirty patients (51.7%) with an ECOG PS 0-1 were assigned to the good PS arm, while 28 patients (48.3%) with ECOG PS 2 were in the poor PS arm. Overall response rates were 36.6 and 28.8%, respectively (p=0.91). Median PFS was 6.7 and 6.3 months in good PS and poor PS arms (p=0.50) and median OS was 9.6 and 10.4 months (p=0.55). As compared with good PS arm, poor PS arm was associated with more grade 3-4 neutropenia and anemia. Dose reduction and dose delays were also significantly higher. Conclusions: In this study, mFOLFOX6 was similarly effective in both arms. Although hematologic toxicity was significantly higher in patients with poor PS, it remained manageable. Our results suggest that this regimen may be an effective treatment option for AGC patients with poor PS.

Comparisons of Food Preference and Nutrient Intake of Students of Elementary School and Middle School Providing School Food Service in Nam Jeju Gun (남제주군 학교급식대상 초.중등학생의 음식 기호와 영양소 섭취량의 비교)

  • The purpose of this study was to investigate differences of food preference and nutrient intake of students between elementary school and middle school in the same area and to provide data for better school food service. The subjects were 486 students, third to sixth grade of 3 elementary schools and first to second grade of 1 middle school in Nam jeju gun, Jeju, and the survey was conducted during June 1999. Food preference was assessed using questionnaires and 24-hour food intake was assessed using dietary record method. Data of weight and height were obtained from annual physical examination conducted at schools in May 1999. All the variables were compared among 3 groups in each gender: third and fourth grade elementary school(ES3,4), fifth and sixth grade elementary school(ES5,6), and first and second grade middle school(MS1,2). The results were summarized as follows. The average height, weight and BMI for the 3th grade boys in elementary schools met the national averages, but those of the others are below the national averages. Although general pattern of food preference looked similar among groups, food preference scores were significantly different among groups in 38 kinds foods for boys, and 27 kinds of foods for girls. MS1,2 group showed significantly lower food preference scores for most of foods as compared to those of ES3,4 and ES5,6 in both genders. Students of higher grade took more starch foods such as instant noodle, stewed rice cake and snacks. Average energy intakes of all the groups except MS1,2 girls were lower than recommended dietary allowances(RDA), and average intakes of protein, vitamins B1 and C met RDA, but the other nutrients were taken less than RDA and especially the intakes of iron, calcium and vitamins B2 were poor. Most of nutrients taken by school food service meal provided a major proportion of intakes. In conclusion, students of middle school were more particular about their foods served at school food service and marked lower food preference score than elementary school children and more conscious about their weight and appearance. These points should be reflected in planning food service menu at middle school.

The Study on Natural Dyeing with Artemisia (쑥을 이용한 천연염색에 관한 연구)

  • Recently the interest in natural dyeing has been increased because of the color clarity, natural grace and reactionism in fashion. Indigo dyeing, safflower dyeing and Gal-ott in Cheju-Do become generally known, so the study about the natural dyeing is continued with national intrust and support. In this study, 1 used artemisia for various dyeing tests because we can get it easily. 1 tested the dyeability in wool as well as cotton and silk with wormwood in natural dyeing material. I also dyed nylon with the same material for the comparison of the molecular structure. The mercerization and the chitosan treatment were done in cotton to improve the low dyeability in the natural dyeing. The result of this study are as follows; We have to dye repeatedly to get deep color in natural dyeing, and mordant treatment brought good result in color difference and dyeing fastness. 1. Compared with silk, wool and nylon, the dyeability of cotton was the worse. The color difference of cotton was 18.81 without mordants , and 24.05 with mordant. The dyeability of cotton was much increased by mordants such as potassiumdichromate, copper sulfate, iron sulfate and salt water. The color of cotton was turned into yellow-green in potassium dichromate and yellow-green with deep green in copper sulfate. The mercerization and the chitosan treatment of cotton made the improvement in color, dyeability, laundering fastness, abrasion fastness. 2. The color difference of silk was 3 times as high as cotton. It showed the similar degree with the chitosan treated-cotton. That is to say, silk had good dyeability because it contains amino group and carboxyl group. The dyebility of silk was increased by a mordant such as iron sulfate, potassium dichromate and salt water, Drycleaning fastness showed 5 grade, abrasion-fastness was high over 4~5 grade and sunlight fastness showed 1 grade in all case. 3. The color difference of wool was the best among four fabrics because of 18 kinds of amino acids. It is considered that the sulfate of wool has the polarity and help metal- mordants get the better dyeability such as potassium dichromate, iron sulfate, copper sulfate, and aluminum chloride. The color was reddish-green in potassium dichromate, yellow.greenin iron sulfate and copper sulfate, and yellow in the rest mordants. Drycleaning fastness of wool showed over 4~5 grade, abrasion fastness 5 grade. Sunlight-fastness 1 grade. 4. The dyeability of nylon was almost same because of the similar molecular structure with silk. The clarity of color was poor. The color was yellow-green in copper sulfate and yellow in the rest mordants. Laundering fastness and abrasion-fastness of nylon was good. Sunlight- fastness represented 1 grade.
