The Possibility of Expanding Pay-for-Performance Program as a Provider Payment System (성과연동지불제도의 확대 가능성 고찰)

  • This paper investigates the possibility of expanding pay-for-performance (P4P) program as a provider payment system, in terms of financial, economical, and political sustainability. In order to expand the sustainable P4P, P4P should have usefulness in terms of economic value as well as efficiency in the financial aspects of health care. More importantly, the P4P would be politically sustainable only when both providers and consumers can accept. Korea's healthcare system seems to have logical ground for the P4P program financially and economically. However, how well the P4P can work remains to be proven in its implementation. After 43 tertiary hospitals applied the P4P program for acute myocardial infarction (AMI) and C-section in 2007, the number of hospitals adopting the P4P program for AMI and C-section has increased to 316 in 2011, and an incentive for hospitals applying the P4P has risen to 2% from 1% of health insurance benefits. This shows that the P4P program introduced by Health Insurance Review and Assessment Service is quite successful. In addition, people are aware of the need for improved P4P program and policy alternatives have been already made. Therefore, it is very important to come up with politically supportable strategies that can make providers and consumers accept the P4P program while maintaining the governance of the existing health insurance policy. To this end, there are some tasks to be considered. First, the expansion of the P4P program should be placed on the agenda of the Health Insurance Policy Review Committee, the highest decision-making body, and a separate agency for P4P planning should be established. Second, for more efficient P4P program, the processes of review and assessment, currently carried out separately, should be integrated into a single process. Third, infrastructure to measure the quality of medical services should be sharply expanded. Fourth, the current paradigm for the assessment should be changed. Lastly, a P4P program for consumers should be considered. Given that the consumers in Korea can use medical services freely, the National Health Insurance Corporation could initiate the P4P program for consumers as a means of controlling excessive use of medical services and adjusting consumer's moral hazard.

On Restructuring of the Debates on Child Care Financing in Reflection of Changing Policy Surrounding in Korea: with Burchardt's Model (우리나라의 보육정책 환경 변화에 따른 재정지원방식 논쟁의 재구조화 - Burchardt의 모델을 중심으로 -)

  • This article attempts to restructurize the current arguments on how to change the method of financing child care programme, which is currently under discussion in Korea. There has been a series of changes in Korean child care policy, such as the amendment of Child Care Act, the transition of its responsible administrations, adoption the child care policy as a national agenda. In these changes, new needs for child care financing method comes out with a new direction of child care policy and a plan of expansion of finances. That's the transition from 'facility-centered subsidy' to 'child-centered subsidy'. This article redefine it as the arguments about 'supply-side subsidy' vs. 'demand-side subsidy'. To analyse the different traits of two branches of arguments, this article reviews the social welfare models, and has come to the conclusion that the Burchardt's model is appropriate to explain the financing debates. Thus, the analysis the traits of the two perspectives is based on three dimensions i. e. service provision, financing and decision making according to the Burchardt's model. This also examines how the two sides is connected to the discussion of publicity and quality improvement of child care service. Through this approaches and analysis, this helps us restructurize the debates on the method of financing from the present superficial arguments.

Public Health Nutrition Policies and the Role of the Government: International Examples and the Need for Action in the Republic of Korea

  • Many of the non-communicable diseases, which are now the major causes of death and disability worldwide, can be linked to our lifestyles, and thus to what eat The life-style related risk factors are - to a great extent - preventable. Public health nutrition (PHN) policies are means through which govemments can have an enornous impact on the reduction of nutrition-related non-communicable diseases, such as diabetes, hypertension, obesity, cancer and cardiovascular disease, by creating and supporting environments which enable healthier food choices and which are conducive to healthy nutrition behavior. More and more countries are developing nutrition policies. Nutrition policies are tools through which governments can intervene and control nutrition-related concerns throughout all levels of society. The need for more concerted action in the Republic of Korea is demonstrated, by showing the lack of priority for nutrition issues. Four recommendations for action are made; the first recommendation places emphasis on the need to implement a structure at the political level, through which nutrition concerns can be addressed, such as a nutrition unit within the Ministry of Health and Welfare. The second recommendation stresses the need for a strong nutrition advocacy strategy, to raise the awareness of the gains that can be achieved by promoting healthy nutrition. The third recommendation calls for more vigorous regulations and stricter enforcement of food and nutrition advertisement, and the fourth recommendation emphasizes the need for a settings-bsed approach to nutrition interventions. Acknowledging the developments that have already occurred in Korea, public health nutrition has yet to become a priority on the agenda of policy makers in Korea.

Development Effectiveness of the Paris Declaration: An Empirical Evaluation (파리선언의 개발효과성: 실증적 평가)

  • This study aims to assess the development effectiveness of the Paris Declaration (2005). Using data collected by the OECD/DAC from 78 developing countries for the period 2005~2010, this study evaluates the role played by the Paris Declaration principles alone and in interaction with aid in promoting per-capita GDP growth. The analysis shows that the overall net impact of aid on promoting economic growth has been negative. However, aid effectiveness has been enhanced by the sound policies or institutions and some Paris Declaration (PD) principles. Of the five principles of the PD, only the alignment and, to some extent, mutual accountability principles of the PD did show a significant and positive role in making aid more effective for economic growth of aid recipient countries. Therefore, OECD's statement that the PD enhances aid effectiveness is supported only partially. These findings have significant implications for the importance accorded to sound policies and institutions in the growth literature, and for future international development cooperation agenda.

  • This paper analyses recent regional development policies being carried out in Japan and the U.S. Such policies are formed on the sustainability principle aiming at the realization of circula-flow economy and zero waste society. The study here illuminates Japan's Eco-town project and three cases of eco-industrial developments in Minnesota. Such projects have gone from improving industry competitiveness through constructing clean production systems on the company level to improving national competitiveness through constructing a 'circular society' on the government level. Japan included the realization of a 'circular society' as its government's top agenda because it recognized that environmental pollution stems from the inefficient use of natural resources. Eco-town project is a regional development policy specifically reflecting such recognition. The eco-efficiency based city development approaches in Minnesota are but small examples of the new wave in regional development in the developed countries. We need to go beyond emphasizing eco-friendly and ethical management to just the companies. The government itself needs to design national policies based on environmental sustainability.

  • Recent national policy agenda of 'Low-Carbon Green Growth' has been implemented as the important vision of the country. The government has set a target of entering the seven major world powers of Green Nation at 2020, and five major world powers at 2050. In order to achieve the goal, the reduction of greenhouse gases and the composition of green land and transportation has been promoted as an important policy. In a construction industry sector, policies and technologies for reducing greenhouse gas emissions and energy have been actively developed. In the case of developed countries, research for green road infrastructure has been conducted, which is the basis for certification system of green highway. This paper benchmarks the policies and cases of developed countries, verifies the applicability in the domestic road sector, and suggests the introduction of the certification system of green highway.

  • 본 연구의 목적은 한국에서 시도된 국가지리정보체계(NGIS)구축의 1단계사업에 대한 정책적인 측면에서 해외의 NGIS와 비교하여 2, 3단계 사업의 성공화를 도출하는데 있다. 본 연구의 첫 장에서는 Masser(1999)에 의한 세계 NGIS 추진 10개국의 정책과 핵심분야를 한국의 NGIS와 비교 분석하여 장단점을 제시하였다. 2장에서는 한국의 NGIS정책에 대하여 구체적으로 분석하여 3장에서 한국 NGIS의 핵심5대 분야와 연계한 결과 및 평가의 구체성과 4장에서 한국의 미래 NGIS의 방향을 동시에 제시하였다. 세계적으로 90년대 후반에 시작된 국가지리정보체계의 구축사업 중에서 11개국에 대한 선행정책연구 분석에서 볼 때 한국이 포함된 7개 국가에서 정부주도형의 하향식(Top-down)형식으로 수행하였으며, 3개 국가는 기존 수행체계를 발전시킨 상향식(Bottom-up) 형식이었고 나머지 한 국가는 절충형의 후발주자에 속하여, 한국의 NGIS 1단계사업은 국가주도형의 정책이 주종을 이루는 하향식 정책의 국책사업으로 밝혀졌다. 한국의 1단계 NGIS사업의 정책과 분야는 1995년부터 정부주도 정책에 따라서 2억 $(중앙정부: 64%, 지방정부: 36%)의 예산으로 2000년까지 11개 중앙부처가 참여하고 5개 분과위원회(총괄분과, 지리정보분과, 토지정보분과, 기술개발분과, 표준화분과)에서 10개 사업(지형도전산화사업, 주제도 전산화사업, 지적도전산화사업, 지하시설물도전산화사업, 지하시설물 관리체계개발시범사업, 공공GIS활용체계개발사업, GIS기술개발, GIS전문인력육성, GIS표준화사업, 국가GIS지원연구사업)을 수행한 대규모 국책연구개발사업이었다. 따라서 한국의 미래 NGIS 정책방향은 국가공간정보기반조성을 통한 디지털국토구축으로 국가 Agenda인 'u코리아'를 실현시키고 선진형 대민서비스를 제공하는 것으로 정해질 것이다.

A Study upon the Formation of Techno-surplus Society and Its Specificities (국내 기술잉여사회의 형성과 특수성 연구)

  • There have been technologically distorted naturalization and overzealous digital culture in the formation and development of digital society in Korea. While the suppressive aspects of the 'neo-'authoritarian control and regulation have been excessively centered on the Internet, the autonomous actions of online users from below, with regards to their roles in agenda-setting function, have been evolved as the political. This paper aims to investigate the specificities in the developmental mode of digital technology in Korean society since the mid-90s. In this paper, 'techno-surplus' depicts the state that the abnormal is embedded within a technological artifact beyond its receptive ability. 'Techno-surplus society' designates such an extreme case of specifying technological surplus. In fact, the term of 'techno-surplus society' can be used for a metaphor symbolizing our society, in which social distortion and abnormality caused by 'techno-surplus' have been quite frequently happening, in its comparison to a degree of normality in the institutional politics. This paper explores the local specificities of 'techno-surplus society', in which the regressive aspects have stand out as being more different from the technological developments in China, Japan and the U.S.

Application of Sentiment Analysis and Topic Modeling on Rural Solar PV Issues : Comparison of News Articles and Blog Posts (감성분석과 토픽모델링을 활용한 농촌태양광 관련 이슈 연구 : 언론 기사와 블로그 포스트 비교)

  • News articles and blog posts have influence on social agenda setting and this study applied text mining on the subject of solar PV in rural area appeared in those media. Texts are gained from online news articles and blog posts with rural solar PV as a keyword by web scrapping, and these are analysed by sentiment analysis and topic modeling technique. Sentiment analysis shows that the proportion of negative texts are significantly lower in blog posts compared to news articles. Result of topic modeling shows that topics related to government policy have the largest loading in positive articles whereas various topics are relatively evenly distributed in negative articles. For blog posts, topics related to rural area installation and environmental damage are have the largest loading in positive and negative texts, respectively. This research reveals issues related to rural solar PV by combining sentiment analysis and topic modeling that were separately applied in previous studies.

Search tweets, retweets, while the formation of policy issues, the agenda of Twitter users to listen to the statements of the candidates using the Internet or a smartphone. Content during the progressive tweets core keywords you do not often discussed, followed by the negative information increases the number of tweets has become a policy issue. Top retweets was to evaluate the process of debate, regardless of the issue. Tweeter complements the TV so Twitter has made public opinion. Smart phones and SNS Twitter, combined with the TV and the participation and direct democracy, voters vote one instrument was realized. Should forward approval ratings, real-time Twitter subtitles on the TV screen in TV debate Twitter influence in the election will be greatly expanded.
