Factors Affecting the End-of Life Care Competency of Tertiary Hospital Nurses

  • Purpose: The purpose of this study was to investigate the levels of end-of-life care competency; knowledge, attitudes, and experiences regarding advance directives; perceptions of good death; and end-of-life care obstacles and supportive behaviors among tertiary care nurses. Methods: The participants were 150 nurses at a tertiary hospital in Jinju, Korea. The data collected using a questionnaire were analyzed using descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and stepwise multiple regression in SPSS for Windows version 24.0. Results: The mean (±SD) score of end-of-life care competency was 3.63 (±0.53) on a 5-point scale. A significant difference in end-of-life care competency was found according to whether nurses had experienced the death of a family member or acquaintance (P=0.029). According to stepwise multiple regression analysis, the factors affecting end-of-life care competency were the frequency of end-of-life care supportive behaviors (β=0.38, P<0.001), experience with advance directives (β=0.29, P<0.001), and marriage (β=0.15, P=0.039). This model had an explanatory power of 27.9% (F=18.87, P<0.001). Conclusion: In order to improve nurses' end-of-life care competency, it is important to strengthen end-of-life care supportive behaviors by exposing nurses to those behaviors and providing frequent experience with advance directives.

Exploring the Development of Public Health Care through Health Care Utilization Survey

  • Purpose: The purpose of this study was to provide comprehensive measures for the development of public health care through a survey on consumers' awareness of health care use from the point of view of local residents. Research design, data and methodology: For about one week from January 07 to January 14, 2021, questionnaires were distributed to 800 local residents and analyzed. For statistical analysis of collected data, frequency analysis and cross-analysis were performed. Results: Regarding public health service, 'providing medical services that can be used by all citizens and protect and promote health' had the highest response rate of 95.2% of total respondents. Regarding health care system satisfaction, 'Accessibility to general treatment' had the highest score with an average of 3.31 points. Regarding comprehensive measures for the development of public health care, 'Establishment of an infection and patient safety system' had the highest score with an average of 3.91 points. Conclusions: The direction of public health care and services should include management of infectious diseases during national disasters, reduction of gaps in medical use by region and class, improvement of access to emergency medical care, and quality improvement of specialized medical care.

영유아보육법의 간호학적 고찰 (Nursing Consideration of the Infant Care Act and Suggestion on Its Enforcement Decree and Regulations)

  • Purpose: This descriptive study was conducted to illuminate, from the nursing point of view, the infant care act, which was revised recently and to suggest a desirable direction of its enforcement decree and regulations. Method: This study was carried out through literature review and a collection of child care experts' opinions. Result: The most remarkable changes in infant care act and the suggestions for a desirable direction of its enforcement decree and regulations are as follows: It can be said that the law on 'health', 'nutrition' and 'safety' for a child care center was improved to the level of act from the level of enforcement regulation. In the enforcement regulation on the distribution of nurses in child care centers, it is desirable that nurse's aids are excluded. It is recommended that every child care center should have a nurse or a health care manager and/or an emergency caretaker. In the curriculum for child care, the subjects on child health care should be an essential subject, not elective. Conclusion: Child care act should be reformed under the discipline of first priority to child health. Every nurse and nursing professor should have interest and earnestness in child care and conduct related studies.

중환자실 간호사 근무환경과 환자가족 만족도의 관계 (Critical Care Nursing Work Environment and Family Satisfaction)

  • Purpose: The purpose of this study was to investigate the nursing work environment and family satisfaction in Korean intensive care units (ICUs). Methods: The study participants were 190 critical care nurses and 133 family members of ICU patients who were randomly chosen from four of the hospitals located in B city. The Korean Nursing Work Environment Scale was used to assess the work environment of critical care nurses. Family satisfaction was measured with the Korean version of the Critical Care Family Needs Inventory. Results: Critical care nurses reported moderate satisfaction with their work environment. The mean score for family satisfaction was 3.59 on a 5-point scale, and satisfaction with information provision received the highest score. Family satisfaction was higher in hospitals where the critical care nurses evaluated their work environment positively. Conclusion: This study revealed that the work environment of nurses affects family satisfaction in ICUs. Therefore, it is necessary to explore various methods of improving the critical care nursing work environment in order to provide the highest possible level of nursing care.

Ultrasonographic Assessment of the Cutaneous Changes Induced by Topical Use of Novel Peptides Comprising Laminin 5

  • Background Laminin 5, which is found in the basement membrane of dermal-epidermal junction (DEJ), is a major adhesive component and associated with proliferating and migrating keratinocytes. In this study, we hypothesized that the topical application of the skin care products containing the novel peptides might restore the DEJ structure by deriving deposition of laminin 5 and promoting the keratinocyte migration. Here, we evaluated the restoration of DEJ by measuring the skin thickness. Methods Single-center retrospective analysis was performed on a total of 13 patients who underwent skin care using Baume L.C.E. (France, Laboratories d' Anjou) between January and March 2021. All patients applied the skin care agent for 2 weeks only on their left hand dorsum. Before the initiation of the application and after 2 weeks, both their hands were evaluated on photography and ultrasound. And the patients were asked to rate their satisfaction with the questionnaire after 2 weeks. Results There was no obvious improvement in photographic assessment and questionnaire. The post-pre difference of skin thickness in ultrasound images was, in left hand, 0.1 ± 0.37 in distal point and 0.1 ± 0.35 in proximal point; and, in right hand, 0 ± 0.17 in distal point and 0 ± 0.15 in proximal point, respectively. The pre-post difference was statistically significant in proximal point (p = 0.035). Conclusion Topical application of novel peptide derivative comprising laminin 5 demonstrated cutaneous changes including skin thickness, as assessed by ultrasound. Further studies using other modalities including dermal density measurement, three-dimensional photography, optical coherence tomography, or skin biopsy would be helpful to determine the skin-improving effects.

일부 지역 임산부의 구강보건관리에 대한 인식 및 실천에 관한 연구 (A study on the awareness and practice of the pregnant women about oral health care)

  • The purpose of this study examines the level of awareness and practice for the oral health care of the pregnant women, finding problems and proposing the problem point regarding hereupon and an improvement point by analyzing the result. Data for this study are collected by the questionnaire method from pregnant women who attend gynecology clinic located Gwangju provide for the period between December, 2007 and January, 2008. The obtained results were as follows : 1. The experience of oral health education for pregnant women were 'none' 94.8%, 'yes' 5.2%, the majority no received oral health education. 2. The awareness score of dietary treatment was 34.09 and practice score was 29.73, the awareness score of periodontitis prevention was 33.31 and practice score was 29.04. The awareness score of subsidiary oral hygiene articles was 32.19, practice score was 17.95. The awareness score was higher than practice score. 3. The relationship between the awareness and practice of oral health care was positively correlated: dietary treatment(r=0.483, p<0.01), periodontitis prevention (r=0.531, p<0.01), subsidiary oral hygiene articles (r=0.515, p<0.01), oral health care (r=0.531, p<0.01). Conclusion of these survey showed that it is crucial to increase the level of practice of oral health care. It is also necessarily to increase the level of awareness of the oral health care.

요양보호사의 노인에 대한 지식 및 태도가 직무스트레스에 미치는 영향 (The Effect of Care Giver's Knowledge and Attitude toward the Elderly on Job Stress)

  • The purpose of this study was to identify the level of care-givers' knowledge, their attitude towards the elderly and their stress levels within the job, and to apprehend the cause which has had an influence on job stress. The research subjects were 230 care-givers who work in a sanitarium for the elderly in the East of Jeollanamdo. The tool used to measure their knowledge of the elderly was FAQ1 and the semantic differential scale was used to determine the attitude. Job stress was measured by the tool which was used by sun-ju Park. The data analysis was carried out by SPSS Win 13.0. The results of this study are as follows; The care-givers' knowledge level point towards the elderly was relatively low at 44.6%. The average point of attitude toward the elderly was 3.91, which is relatively moderate. The percentages of job stress was generally low. The results showed us that there was no correlation with the knowledge level of the elderly, the attitude towards the elderly and job stress. Since the results indicated that care-givers' knowledge level for the elderly was low, we reached the conclusion that further education where care-givers can get general knowledge for the elderly and aging is necessary. Moreover, research for strengthening positive attitudes towards the elderly is also necessary.

노인요양병원 입원실의 실내디자인 특성에 대한 사용자 평가 - 간호사와 간병인 및 보호자를 대상으로 - (Users' Evaluation of Interior Design Features of Patients Rooms in Geriatric Hospital - From the perspectives of Nurses and Care-Givers -)

  • Demands for geriatric hospital have increased in an era of rapidly aging population. Most of aged patients tend to stay in institutions for long terms. This means that the patient rooms of geriatric hospital should be given different considerations from those of normal hospital in designing interior features. They should be a homelike places for the aged patients and designed to take care of specific needs of the aged. However, most of geriatric hospitals are designed with little attention to such point. They appear almost same to normal ones. This study attempts to examine how users evaluate patients' rooms. The users are nurses, care-givers and family members of aged patients in six geriatric hospitals in Busan. They rated 12 features of patient rooms from 0 point to 100 points and described reasons why they rated in that way. Also, the walk-through was done for these six hospitals. 12 features are sizes of patient rooms, sizes and fixtures of bathrooms, sizes and locations of windows, bed layout, numbers and types of chairs, sizes and types of closet, lighting, color scheme, finishes of floor and wall, and interior design tone. Followings are findings : The users evaluated patients' rooms relatively positive. However, extra chairs for visitors, closet in patients rooms and storage in bathroom, and sizes of patients rooms and bathrooms were evaluated relatively negative.

중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석 (A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.
