갈색거저리를 이용한 식사 섭취에 따른 영양 섭취 및 영양상태 변화 : 수술 후 환자를 대상으로 (Change in Dietary Intake and Nutritional Status Using Mealworms as Hospital Meal in Postoperative Patie)

  • The purpose of this study was to verify the validity of Mealworms as a hospital meal with increased nutrition density. We provided a meal for postoperative patients and conducted analysis of dietary intake and nutritional status of patients and assessment of acceptability of the meal. This study was carried out as a randomized control trial. Patients were supplied either a hospital meal using Mealworms (Experimental group) or a regular hospital meal (Control group). We investigated the administration amounts of parenteral nutrition (PN) and food intake of patients after surgery and measured anthropometry, body composition, and blood tests before surgery and at hospital discharge. We included 34 postoperative patients who were admitted to Gangnam Severance Hospital from March to September. In the groups of patients not supplied with PN, the experimental group ($964.68{\pm}284.6kcal$, $38.82{\pm}12.9g$) had significantly higher dietary calorie and protein intake than the control group ($666.62{\pm}153.7kcal$, $24.47{\pm}4.9g$)(P<0.05). Additionally in the group of patients not supplied with PN, the experimental group (1.37%) showed a significantly higher increase in fat free mass index than the control group (-3.46%)(P<0.05). In all subjects, calorie density and protein density were significantly higher in the experimental group (P<0.001), and acceptability of calorie (P=0.036) and protein (P=0.001) was also significantly higher in the experimental group. Therefore, the results of this study support the validity of the introduction of hospital meals using Mealworms.

Do some patients receive unnecessary parenteral nutrition after pancreatoduodenectomy? Results from an international multicentre study

  • Backgrounds/Aims: After pancreatoduodenectomy (PD), an early oral diet is recommended; however, the postoperative nutritional management of PD patients is known to be highly variable, with some centers still routinely providing parenteral nutrition (PN). Some patients who receive PN experience clinically significant complications, underscoring its judicious use. Using a large cohort, this study aimed to determine the proportion of PD patients who received postoperative nutritional support (NS), describe the nature of this support, and investigate whether receiving PN correlated with adverse perioperative outcomes. Methods: Data were extracted from the Recurrence After Whipple's study, a retrospective multicenter study of PD outcomes. Results: In total, 1,323 patients (89%) had data on their postoperative NS status available. Of these, 45% received postoperative NS, which was "enteral only," "parenteral only," and "enteral and parenteral" in 44%, 35%, and 21% of cases, respectively. Body mass index < 18.5 kg/m2 (p = 0.03), absence of preoperative biliary stenting (p = 0.009), and serum albumin < 36 g/L (p = 0.009) all correlated with receiving postoperative NS. Among those who did not develop a serious postoperative complication, i.e., those who had a relatively uneventful recovery, 20% received PN. Conclusions: A considerable number of patients who had an uneventful recovery received PN. PN is not without risk, and should be reserved for those who are unable to take an oral diet. PD patients should undergo pre- and postoperative assessment by nutrition professionals to ensure they are managed appropriately, and to optimize perioperative outcomes.

영양집중지원에 따른 결과 비교 - 내과계 중환자실 대상으로 - (Effect of Nutrition Support Team Management - Focusing on Medical Intensive Care Unit Patients -)

  • Purpose : This study evaluated the nutritional status and effect of nutritional support team (NST) management in critically ill patients. Methods : From January 2015 to August 2017, the study retrospectively investigated 128 patients aged above 19 years admitted to a medical intensive care unit (MICU). The patients were divided into two groups: NST (n=65) and non-NST (n=63) groups. Nutritional status, classification of bedsore risks, incidence rate of bedsore and clinical outcomes were compared. Results : The study found a higher rate of the use of enteral nutrition in the NST group (${\chi}^2=45.60$, p < .001). The prescription rate of parenteral nutrition (PN) was found to be lower in the NST group (4.6%) compared to the non-NST group (60.3%). There was a higher PN of total delivered/required caloric ratio in the NST, compared to the non-NST, group (${\chi}^2=3.33$, p=.025). There were significant differences for higher albumin levels (t=2.50, p=.014), higher total protein levels (t=2.94, p=.004), and higher proportion of discharge with survival rates (${\chi}^2=18.26$, p < .001) in the NST group. Conclusions : Providing NST management to critically ill patients showed an increase in the nutrition support. Further, to achieve effective clinical outcomes, measures such as nutrition education and continuous monitoring and management for the provision of nutritional support by the systemic administration of a nutritional support team should be considered.

Drosophila melanogaster 한국지반에서 분리한 KP Elements의 DNA 염기서열 분석 (Molecular Analysis of KP Elements Derived from Korean Populations of Drosophifa mefanogaster)

  • KP elements derived from Korean populations (Seoul, Cheonan and Taegu) of Drosophila melunoguster were examined for their molecular structure. The entire 1.15 kb sequence of the three KP elements KC-137 (Cheonan), KS-95 (Seoul) and KT-99 (Taegu) have been obtained by PCR amplification using inverted repeat primers and DNA sequencing. The 1.15 kb fragments of KP elements were cloned into pCRmll vector plasmids, and subsequently sequenced. The sequence of the three KP elements in these populations suggested that there might have been derived from the complete P element by a 1753 bp internal deletion between positions 808 and 2560. Therefore these KP elements were confirmed to be identical to that isolated from M'10 strains widely distributed in most Eurasian populations of D. melanoguster. Sequence comparison with the 2.9 kb complete P element in pn25.1 revealed that KC-137 has only shown to be two base substitutions of A to G and G to A at positions 62 and 242, respectively. The retained sequence of the two KP elements KS-95 and KT-99 shows complete homology to the P factor in pn25.1. Based on this result, the two base substitutions in KC-137 might be due to Taq DNA pollunerase errors. Finally, it is suggested that the high copy numbers of KP elements provieds an explanation for the suppression of P-mediated hybrid dvssenesis in Korean population of D. melunogoster.

Dione Bisoxime 계통의 화합물에 대한 테크네슘표지 원리에 관한 연구 (Tc-99m Labeling of Dione Bisoxime Compounds)

  • Tc-99m Labeled hexamethylenepropyleneamineoxime ([$^{99m}Tc$]-HMPAO) is a famous amino-oxime compound and is widely used to construct SPECT images of cerebral blood flow. To investigate the relationship between chemical structure and radiolabeling in these kind of diamine-oxime compounds, we synthesized seven compounds by Schiff's base formation and successive reduction with sodium borohydride. They were (RR/SS )-4,8-diaza-3,6,6,9-tetramethylundecane-2,10-dione bisoxime (2), (RR/SS/meso)-4,8-diaza-3,9-dimethy-lundecane-2,10-dione bisoxime (4), (RR/SS/meso)-4,8-diaza-3,10-dimethyldodecane-2,11-dione bisoxime (5), (RR/SS/meso)-4,7-diaza-3,6,6,8-tetramethyldecane-2,9-dione bisoxime (8), (RR/SS/meso)-4,7-diaza-5,6-cyclohexyl-3,8-dimethyldecane-2,9-dione bisoxime (10), (RR/SS/meso)-3,4-bis(1-aza-2-methyl-3-oxime-1-butyl)-benzoic acid (12), and (RR/SS/ meso)-2,3-bis(1-aza-2-methyl-3-oxime-1-butyl) benzophenone (14). Chemical structures of all the synthesized compounds were identified by taking $^1H$ spectrum. Among them, 2 and 4 are propyleneamine oxime (PnAO), 6 is butyleneamine oxime (BnAO) and 8, 10, 12 and 14 are ethyleneamine oxime (EnAO). Each compound (0.5 mg) was incubated with stannous chloride (0.5 g - 8 g), carbonate-bicarbonate buffer (final concentration = 0.1 M, pH 7 - pH 10) and Tc-99m-pertechenate (1 ml). Tc-99m labeling of these compounds were checked by ITLC (acetone), ITLC (normal saline), reverse phase TLC (50 % acetonitrile) and ITLC (ethyl acetate). According to the results, EnAO's were not labeled by Tc-99m in any of above condition. About 11 % of maximum labeling efficiency was obtained with BnAO. However, 4 (PnAO) was labeled with Tc-99m to 85 % which is similar to the labeling efficiency of 2 (HMPAO). Hydrophilic impurity (9 % ) was the most significant problem with the labeling of 4, however, pertechnetate (3 % ) and colloid (3 %) were minor problem. In conclusion, we synthesized seven diamine blsoxlme compounds. Among them, four EnAO compounds were not labeled by Tc-99m. A BnAO was labeled poorly and two PnAO's were labeled well. These labeling can be explained by tertiary structure of their Tc-99m chelate.

무선 데이터 통신에서 MRC 다이버시티 수신시의 DS/CDMA-Trellis Coded QPSK 신호의 오율 해석 (Error Performance Analysis of DS/CDMA-Trellis Coded QPSK Signal with MRC Diversity Reception in Wireless Data Communication)

  • 본 논문에서는 라이시안 페이딩, 다중 사용자 간섭, 다중경로 채널에서 MRC(Maximum Ratio Combining) 다이버시티 수신된 DS/CDMA-Trellis Coded QPSK 변조 신호의 패킷 오율 성능을 분석하였다. 그리고 직접 파 전력 대 반사파 전력의 벼($K_r$, , 다이버시티 가지의 수(M), 다중 사용자의 수(U), PN chip rate(PN), 다중 경로 채널의 수($L_p$), 패킷 길이(PL)를 함수로 하여 MRC 다이버시티 수신된 DS/CDMA-Trellis Coded Q QPSK 시스템의 성능과 용량올 구하였다. 해석 결과, 라이시안 페이딩($K_r$=6 dB), 5명의 다중 사용자 간섭,3개의 다중경로 채널에서 2가지(branch) 의 MRC 다이버시티를 적용한 DS/CDMA-Trellis Coded QPSK 시스댐은 2가지 (branch)의 MRC 다이버시 티를 적용한 부호화되지 않은 DS/CDMA BPSK 시스댐에 비하여 약 6 dB의 부호 이득이 있음을 알 수 있다. 그리고 AWGN 채널에 대해 디자인된 Trellis Coded QPSK 신호는 MRC 다이버시티 수신을 적용한 라이시안 페이딩 채널에서도 좋은 성능을 나타냉올 알 수 있다. 결과적으로 제안된 시스템 구조는 라이시안 때이딩, 다중 사용자 간섭, 다중경로 채널에서 무선 데이터 통신시스템으로서 적절하다고 예상된다.

티모시 건초 급여 한우 씨수소 정소상체 정자의 수정 효과 (Effect of sperm penetration of oocytes after in vitro fertilization (IVF) with cauda epididymal spermatozoa in Hanwoo bull after feeding of timothy hay)

  • 본 연구는 티모시 건초와 농후 사료 위주의 사료를 급여한 한우 씨수소 정소상체 정자 체외수정 효율 조사를 통해 정자의 활용 가능성을 조사하였다. 농후 사료는 체중의 1.8%를 급여하고 양질의 티모시 건초를 자유채식 시킨 14개월령 거세우의 정소에서 분리된 정소상체 미부의 정자를 회수하고 동결 흉해 후 체외수정을 실시한 결과는 다음과 같다. 웅성전핵과 자성전핵이 형성(2PN)된 난자는 정상수정으로, 1개의 전핵(1PN), Expanded Sperm Head (ESH), Polyspermy 형태는 비정상적인 수정의 형태로 평가하였다. 정상적으로 수정된 난자의 비율은 정소상체 정자의 경우 전체 침투율은 49.7% 그리고 정상적인 2PN을 가진 난자는 18.5%를 보였으며, 대조구 정자의 전체 침투율은 54.4%로서 정소상체 정자 보다 높은 결과를 보였으나 유의적인 차이를 보이지는 않았다. 정상적으로 2PN을 형성한 비율은 36.7%로서 정소상체 정자를 이용한 정자 보다 높았으나 유의적인 차이는 없었다. 체외수정 후 발달률 조사에서 정소상체 정자의 분할률은 81.2%, 대조구 정자는 82.7%로 유사한 결과를 보였으나, 배반포 발달률은 정소상체 정자 24.4%와 대조구 정자 12.2%로 정소상체 정자를 사용한 난자의 발달에서는 유의적으로 높았다(p<0.05).

비골신경 마비에 따른 비복신경 손상의 정도 (Sural nerve involvement accompanying peroneal nerve palsy)

  • Backgrounds: The pathway of the sural nerve (SN) is variable, but usually divided into medial and lateral sural branches joining the posterior tibial nerve (PTN) and the peroneal nerve (PN). The sural nerve may be affected by PN palsy. The frequency or the severity of SN involvement in peroneal palsy is not known. The purpose of the study is to investigate the frequency and the severity of the SN involvement by the peroneal nerve palsy. Methods: Total 85 patients were included with peroneal palsy. Amplitudes of distal peroneal, sural, and superficial peroneal nerves (SPN) were compared between normal and paralyzed sides. The frequency and severity of SN involvement by peroneal palsy were investigated. Results: Mean age was $48.4{\pm}17.4$ years old at the time of the test. Peroneal palsy was right side in 32, left in 38, and bilateral in 15 patients. Mean amplitudes of affected distal PN, SPN, and SN were $1.51{\pm}1.64mV$, $3.50{\pm}4.86{\mu}V$, and $10.42{\pm}6.59{\mu}V$ in right side, and $1.19{\pm}1.57mV$, $4.38{\pm}5.67{\mu}V$, and $11.06{\pm}6.87{\mu}V$ in left side, respectively. Sensory nerve action potential (SNAP) amplitude of the SN in the affected side was average $73.7{\pm}33.1%$ of normal, which was significantly lower than that in the normal side(p<0.01). The decrease of the sural SNAP amplitude was more than 15% in 39 out of 70 patients with unilateral peroneal palsy. Peroneal compound muscle action potential (CMAP) amplitude was not correlated with the amplitude of the sural SNAP. By complete peroneal palsy, SN SNAP amplitude was decreased to 4% of SNAP and $57.7{\pm}31.8%$ of that in normal side. Conclusions: PN injury without PTN involvement may induce reduction of sural SNAP amplitude. Because of the anatomic variation of SN, the electrophysiological findings are variable. It should be considered to interpret the location of the PN lesion.

Effects of the plane of nutrition during the latter grower and entire finisher phases on grow-finish pig performance in summer

  • The present study was undertaken to investigate the effects of the plane of nutrition (PN) for growing-finishing pigs on growth performance and meat quality in summer. One hundred and two barrows and 102 gilts weighing approximately 44 kg were placed on a high-, medium-, or low-plane grower diet (HPG, MPG, or LPG) with ME and lysine concentrations ranging from 3.33 to 3.40 Mcal/kg and 0.93% to 1.15%, respectively, for 29 days in 6 replicates (pens) in total. Pigs from each grower pen were divided into two finisher pens and provided with a high-plane finisher diet (HPF) containing 3.40 Mcal ME and 9.5 g lysine/kg and a low-plane finisher diet (LPF; 3.25 Mcal ME and 8 g lysine/kg), respectively, up to approximately 110 kg, and slaughtered. Growth performance of the pigs, including average daily gain (ADG), average daily feed intake (ADFI), and gain:feed ratio, was not influenced by the grower-phase PN during any of the grower phase, a 31-d finisher phase I, and ensuing phase II. However, both the ADG and gain:feed ratio were greater (p < 0.05) for the HPF group than for the LPF group during the finisher phase I (748 vs. 653 g with SEM = 13 g and 0.333 vs. 0.299 with SEM = 0.008, respectively). The ADG, but not gain:feed ratio, was greater for the HPF group vs. LPF during the finisher phase II (673 vs. 623 g with SEM = 15 g for ADG and 0.322 vs. 0.323 with SEM = 0.005 for the gain:feed ratio). The carcass backfat thickness (BFT) was greater for the LPF group vs. HPF within the pigs which had been placed on LPG during the grower phase, but not within the pigs from the HPG or MPG group. Physicochemical characteristics of the longissimus dorsi muscle (LM) and sensory quality attributes of fresh and cooked LM were not influenced by either the grower-phase or finisher-phase PN. In conclusion, high PN is necessary for finishing pigs during the hot season to minimize the reduced rate of weight gain and also to prevent the increase of BFT that could result from low PN.

ON QUANTUM CODES FROM CYCLIC CODES OVER A CLASS OF NONCHAIN RINGS

  • In this paper, we extend the results given in [3] to a nonchain ring $R_p={\mathbb{F}}_p+v{\mathbb{F}}_p+{\cdots}+v^{p-1}{\mathbb{F}}_p$, where $v^p=v$ and p is a prime. We determine the structure of the cyclic codes of arbitrary length over the ring $R_p$ and study the structure of their duals. We classify cyclic codes containing their duals over $R_p$ by giving necessary and sufficient conditions. Further, by taking advantage of the Gray map ${\pi}$ defined in [4], we give the parameters of the quantum codes of length pn over ${\mathbb{F}}_p$ which are obtained from cyclic codes over $R_p$. Finally, we illustrate the results by giving some examples.
