Alloplastic materials used for orbital fracture reconstruction can induce complications, such as infection, migration, extrusion, intraorbital hemorrhage, and residual diplopia. Silicone is one of the alloplastic materials that has been widely used for decades. The author reports a rare case of spontaneous extrusion of a silicone implant that was used for orbital fracture reconstruction 30 years earlier. A 50-year-old man was admitted to the emergency room for an exposed substance in the lower eyelid area of the left eye, which began as a palpable hard nodule a week earlier. The exposed material was considered to be implant used for previous surgery. Under general anesthesia, the implant and parts of the fibrous capsule tissue were removed. Several factors hinder the diagnosis of implant extrusions that occur a long period after the surgery. So, surgeons must be aware that complications with implants can still arise several decades following orbital fracture reconstruction, even without specific causes.
The auricle is easy to be amputated in the face due to its protruding position. When facial parts are amputated, the mechanism of injury usually involves some form of avulsion, which damages these fragile vessels over a distance and renders them unsuitable for anastomosis. Replantation of the ear remains a challenging problem because of the tiny vessels and the paucity of adequate veins for anastomosis. Reattachment as a composite graft of the total or subtotal amputated ear is unreliable. Microsurgical replantation can be performed in a minority of cases because of technical difficulties and long operation time. In this article, the authors report two case of a successful ear replantation of completely amputated auricle. Only one artery and one vein were anastomosed in first case. In the other case one artery was anastomosed without vein. Instead of venous repair, multiple incision was done with leech application therapy. and the outcome was successful. In spite of the technical difficulties and long operation time, microsurgical replantation of amputated ear is better than other reconstructive method or reattachment without microsurgery.

Background Fascial free flaps have been widely used for reconstruction of the hand because they are thin. However, studies reporting objective data regarding the advantages of this approach are lacking. Thus, we report our experience with such flaps. Methods Forty-five cases of fascial free flaps between November 2006 and March 2014 were reviewed. Nine cases involving reconstructed dorsal or lateral defects were included. Four anterolateral thigh fascial free flaps and 5 lateral arm fascial free flaps were examined. Maximal flap contour was assessed by measuring reconstructed tissue thickness at the central area from the surface of the skin to below the bone in a vertical manner using ultrasonography and X-ray data. Contralateral regions were examined in the same manner and a comparative analysis was performed. A questionnaire survey regarding aesthetic satisfaction was also administered. Results All reconstructed parts had a thicker contour than the contralateral side. The average relative percentage of reconstructed tissue thickness was found to be 152% using ultrasonography and 143% using X-ray imaging. According to the aesthetic satisfaction survey, the average rate of satisfaction for patients was 62%, and satisfaction with the flap contour was 72%. Conclusions Using a fascial free flap, the reconstructed tissue was approximately $1.5{\times}$ as thick as the contour of the normal side, which led to positive responses regarding aesthetic satisfaction.
To fine out the concentration of $SO_2$ gas in plastic film house, the survey was conducted at 343 sites throughout the nation. The $SO_2$ concentration in plastic film house were similar both heating and non-heating, but $SO_2$ cocentration higher than 0.8ppm was detected at those with heating. The $SO_2$ concentration in plastic film house cultivated red-pepper higher than those of cucumber or tomato cultivation, and it was produced higher amounts at day time than night due to the higher temperature. In plastic film house with heating, $SO_2$ was leaked at brocken parts of heater and joint of stove pipe, sometimes, $SO_2$ discharged from smokestack was resucked with air. $SO_2$ concentration in the plastic film house with and without ventilation were 0.2 and 0.6ppm, respectively.
Shot peening is a cold surface treatment employed to induce residual stress field in a metallic component beneficial for increasing its fatigue strength. The experimental investigation of parameters involved in shot peening process is very complex as well as costly. The most attractive alternative is the explicit dynamics finite element (FE) analysis capable of determining the shot peening process parameters subject to the selection of a proper material's constitutive model and numerical technique. In this study, Ansys / LS-Dyna software was used to simulate the impact of steel shots of various sizes on an aluminium alloy plate described with strain rate dependent elasto-plastic material model. The impacts were carried out at various incident velocities. The influence of shot velocity and size on the plastic deformation, compressive residual stress and force-time response were investigated. The results exhibited that increasing the shot velocity and size resulted in an increase in plastic deformation of the aluminium target. However, a little effect of the shot velocity and size was observed on the magnitude of target's subsurface compressive residual stress. The obtained results were close to the published ones, and the numerical models demonstrated the capability of the method to capture the pattern of residual stress and plastic deformation observed experimentally in aluminium alloys. The study can be quite helpful in determining and selecting the optimal shot peening parameters to achieve specific level of plastic deformation and compressive residual stress in the aluminium alloy parts especially compressor blades.

Background Patients who have undergone enucleation during infancy due to retinoblastoma can develop microorbitalism due to the decreased growth stimulation from the eyeball and the surrounding soft tissues. Anatomically, the orbit consist of parts of the frontal bone superiorly, the maxilla inferiorly, the ethmoid bone medially, and the zygoma laterally. Considering the possibility of surgically expanding the orbit using tripod osteotomy, in this study we conducted tripod osteotomy on adult patients with microorbitalism of retinoblastoma. Methods Tripod osteotomy was conducted to expand the orbital volume in adult patients with microorbitalism due to enucleation in infancy for retinoblastoma. The orbital volume was measured using the Aquarius Workstation ver. 4.3.6 and the orbit width was measured with preoperative and postoperative 3-dimensional facial bone computed tomography (CT) imaging. Preoperative and postoperative photographs were used to visualize the difference produced by the surgery. Results The orbital volume of the affected side was 10.3 $cm^3$ before and 12.5 $cm^3$ after the surgery, showing an average increase in volume of 2.2 $cm^3$ (21.4%). The increase in the obital width was confirmed by the preoperative and postoperative 3-dimensional facial CT images and aesthetic improvement was observed by the preoperative and postoperative photographs. Conclusions Tripod osteotomy, which realigns the orbital bone, zygoma, and maxilla, is used to correct posttraumatic malunion as well as non-traumatic congenital abnormalities such as that seen in facial cleft. We applied this procedure in microorbitalism secondary to enucleation for retinoblastoma to allow orbital expansion and correct asymmetry.

Recently, plastic products in air-intake parts of automotive engines have become very popular due to advantages that include reduced weight, constricted cost, and lower intake air temperature. However, flow-induced noise in air-intake parts becomes a more serious problem for plastic intake-manifolds than for conventional aluminum-made manifolds. This is due to the fact that plastic manifolds transmit more noise owing to their lower material density. Internal aerodynamic noise from an Idle Speed control Actuator (ISA) is qualitatively analyzed by using a scaling law, which is expressed with some flow parameters such as pressure drop, maximum flow velocity, and turbulence kinetic energy. First, basic flow characteristics through ISA passage are identified with the flow predictions obtained by applying Computational Fluid Dynamics techniques. Then, the effects on ISA passage noise of each design factors including the duct turning shape and vane geometries are assessed. Based on these results, the preliminary low noise design for the ISA passage are proposed. The current method for the prediction of internal aerodynamic noise consists of the steady CFD and the scaling laws for the noise prediction. This combination is most cost-effective, compared with other methods, and therefore is believed to be suited for the preliminary design tool in the industrial field.

The lower extremity injuries are extremely increasing with the development of industrial & transportational technology. For the lower extremity injuries that result from high-energy forces, particularly those in which soft tissue and large segments of bone have been destroyed and there is some degree of vascular compromise, the problems in reconstruction are major and more complex. In such cases local muscle coverage is probably unsuccessful, because adjacent muscles are destroyed much more than one can initially expect. Reconstruction of the lower extremity has been planned by dividing the lower leg into three parts traditionally The flaps available in each of the three parts are gastrocnemius flap for proximal one third, soleus flap for middle one third and free flap transfer for lower one third. Microvascular surgery can provide the necessary soft tissue coverage from the remote donnor area by free flap transfer into the defect. Correct selection of the appropriate recipient vessels is difficult and remains the most important factor in successful free flap transfer. Vascular anastomosis to recipient vessels distal to the zone of injury has been advocated and retrograde flow flaps are well established in island flaps. Retrograde flow anastomosis could not interrupt the major blood vessels which were essential for survival of the distal limb, the compromise of fracture or wound healing might be prevented. During 5 years, from March 1993 to Feb. 1998, we have done 68 free flap transfers in 61 patients to reconstruct the lower extremity. From analysis of the cases, we concluded that for the reconstruction of the lower extremity, free flap transfer yields a more esthetic and functional results.

This paper aimed to investigate the damage process of steel parts experiencing failure under strong repeated loading. Likewise, a damage index using various factors related to the damage was proposed. An analysis method for evaluating the damage state was also developed. The damage assessment method focused on the local strain history at the cross-section of the heaviest concentration of deformation. Cantilever-type steel parts were analyzed under uniaxial load combined with a constant axial load, considering horizontal displacement history, Loading patterns and steel types were considered as the main parameters in analyzing the models. The effects of the parameters on the failure modes, deformation capacity, and damage process as seen from the analysis results were also discussed. Each failure process was compared as steel types. In addition, the failure of steel parts under strong repeated loading was determined according to loading. Results revealed that the state of the failure is closely related to the local plastic strain.
