Since the introduction of pit and fissure sealants in the early 1970's, many studies have been published amply documenting the efficacy of these materials as caries-preventive agents. However, the acceptance of the technique by dentist has been questionable. This survey was investigated the attitudes toward and use of pit and fissure sealants by dentist in Chonbuk area. The results were as follows: 1. Utilization of pit and fissure sealants is very high. Only 15% of dentists never use pit and fissure sealants, whereas 85% generally use pit and fissure sealants. 2. Most dentists reported that their knowledge of pit and fissure sealants was gained through dental schools education. 3. Concern about difficult to sell' to patient, poor retention of pit and fissure sealants and unsubstantiated by research were given as the main reasons why pit and fissure sealants was not used. 4. Patient demands and various educate program were given as important factors that would lead to increase use of pit and fissure sealants.
The purpose of this study was to grasp the benefits from pit and fissure sealants and to analyze the factors of influencing the benefit amount by using the raw data of the Korea National Health and Nutrition Examination Survey for 2007~2013 targeting the subjects aged from 6 years in full to 18 years. The analysis was used STATA 11.0. As a result of the analysis, the following conclusions were obtained. It rose up to 1.24 pieces after carrying out the national health insurance coverage from averagely 0.93 piece until the year in 2007~2009 (up to November) when is before enforcing the national health insurance coverage in the pit and fissure sealants. The benefits from pit and fissure sealants depending on general characteristics were surveyed to be higher in the higher income level, in the more subscription to health insurance and private insurance, and in the more toothbrushing frequency per day. The factors that have influence upon the benefits from pit and fissure sealants were shown to include whether or not to have the national health insurance coverage in the pit and fissure sealants, income level, health insurance type and private insurance subscription appearance, and one-day toothbrushing frequency. Synthesizing the results, a rise in the benefits from pit and fissure sealants is shown in 2010 based on December 2009 when the national health insurance coverage in the pit and fissure sealants was implemented, but is showing the tendency of declining again from 2011. To increase the benefits from pit and fissure sealants, it is thought that the schemes will need to be discussed such as reinforcing publicity on the national health insurance coverage in the pit and fissure sealants, expanding a support for low-income bracket, and differentiating the outpatient cost sharing according to socio-economic level.
Purpose: To evaluate reliability of Q-ray view (Aiobio Inc,. Seoul, Korea) for assessing retention status of pit and fissure sealants. Methods: Pit and fissure sealants of 58 permanent molars from 15 third-grade students were examined. Posterior teeth with ≥1 pit and fissure sealants applied to the occlusal surface for >6 months were examined. The teeth were examined using traditional visual-tactile assessments and combined Q-ray view. Pit and fissure sealants were evaluated by assessing marginal plaque, marginal discoloration, marginal integrity, retention, and presence of caries. Fleiss kappa and Cohen's kappa values were calculated to compare inter- and intrarater agreements between visual-tactile and combined Q-ray view assessments. Results: Regarding interrater agreement in visual-tactile assessments, K values of Cohen's kappa for marginal plaque, marginal discoloration, and presence of caries were 0.22-0.57, 0.36-0.57, and 0.43-0.61, respectively, and agreements ranged from slight to moderate. When combined with Q-ray view, the values were 0.81-0.89, 0.69-0.88, and 0.80-0.90, respectively, and agreements ranged from substantial to nearly perfect level, indicating statistical significance. Marginal plaque (0.81-0.83), marginal discoloration (0.57-0.89), and presence of caries (0.69-0.91) showed higher agreements in combined Q-ray view than in visual-tactile assessments, and kappa values of marginal plaques were significantly higher in combined Q-ray view than in visual-tactile assessments. Conclusion: Evaluating retention status of pit and fissure sealants using Q-ray view showed higher reliability than using visual/tactile assessments for marginal plaque, marginal discoloration, and presence of caries. Therefore, Q-ray view may be used to assess the retention status of pit and fissure sealants.
Objectives: The purpose of this study was to interpret regional disparities in the number of teeth sealed with pit and fissure sealants, identify the factors that affect these disparities and find solutions for the same. Methods: Data were collected from the National Health Insurance Service and Korean statistical information service using metropolis-city-rural area dental health infrastructure variables, regional health behavior variables, and local finance-related variables. Results: In 2015, the number of teeth sealed with pit and fissure sealants per 100 people was higher in the metropolis or city than in the rural area. There was a positive correlation between the number of teeth sealed with pit and fissure sealants and the number of dentists, dental hygienists, dental institutions, standardization rate of subjective awareness of well-being, standardization rate of brushing after lunch, and the proportion of welfare budget in the general budget. There was a negative correlation with the annual standardization rate of health institution use, the standardization rate of unused medical services, and the local government's financial independence. According to the final model of the multiple regression analysis, while the impact of infrastructure on dentistry was not statistically significant, the statistical significance of standardization rate of brushing after lunch, the local government's financial independence, and the proportion of welfare budget in the general budget were maintained. Conclusions: To reduce regional disparities in the volume of use of pit and fissure sealants, it was concluded that it may be effective to select regions with a consideration of the level of regional economic power, implement separate and appropriate policies and projects, and improve the awareness in residents.

Pit and fissure sealants has been proven as safe and effective method of caries prevention. But improper application of pit and fissure sealant may masking the caries process in occlusal fissure. Dentists have to understand the correct and exact application method of pit and fissure sealant for he good result of caries prevention. A key factor to enhanced the effectiveness of caries prevention is retention of pit and fissure sealant. Deep penetration of material into fissure and least marginal leakage around the fissure orifice are the major concerns for dentists to achieve the successive application of pit and fissure sealants. This paper reviewed the literature on the pit and fissure sealants under the following subtitles for enhanced retention : (1) application timing, (2) Indication, (3) Occlusal prophylaxis, (4) Materials, (5) Penetration, (5) Recall check Dental profession must perform the exact application of pit and fissure sealant because this procedure is one of the most technique-sensitive one in dental field.

Due to the various reason, sealing of pit & fissure might be imperfect. One of these reason can be the fracture of sealant material because of the low hardness value of sealing material. The purpose of this in vitro study was to evaluate the microhardness of two different curing type pit and fissure sealants: Dual-cure and Light-cure. The result from the present study can be summarized as follows: 1. All pit and fissure sealants that used in this study showed statistically significant difference in their microhardness of upper and lower surface. (P <0.05) 2. Except of lower surface of teethmate, microhardness of 40-second curing sealant was statistically higher than that of 20-second curing sealant. (P <0.05) 3. In comparison of sealants, microhardness of dual-cure sealant was statistically higher than that of light-cure sealant. Above results suggest that the use of dual-cure sealant and longer curing time are recommended.
This paper reviewed the following subheadings and a few selected references in each section were discussed: ${\cdot}$ Sealant placed over caries; is it possible? Mechanical preparation with fissurotomy or resin polishing bur is one of the most effective method to clean the pit and fissure. ${\cdot}$ Glassionomer cement as a sealant; GIC, wheather it released fluoride or not, cannot be as cost-effective as resin-based sealants. ${\cdot}$ Sealant products; Color(white vs opaque), fluoride(containing vs not), filler component(filled vs non-filled) do not influenced the quality and retention of sealants. ${\cdot}$ Use of intermediate bonding agent to improve retention; Intermediate bonding may increase the retention rate of sealants ${\cdot}$ Penetration method of sealants; Several methods including waiting before light curing are recommended.
Background: This study aimed to explore the overall perception of pit and fissure sealants and suggest methods to revitalize their current stagnation. Methods: To determine the social perception of the change in coverage policy for pit and fissure sealants, we categorized them into five time periods. The first period (December 1, 2009 to November 30, 2010), the second period (December 1, 2010 to September 30, 2012), the third period (October 1, 2012 to May 5, 2013), the fourth period (May 6, 2013 to September 30, 2017), and the fifth period (October 1, 2017 to December 31, 2022). We utilized text mining, an unstructured big data analysis method. Keywords were collected and analyzed using Textom, and the frequency analysis of the top 30 keywords, structural features of the semantic network, centrality analysis, QAP correlation analysis, and co-occurrence analysis were conducted. Results: The frequency analysis showed that the top keywords for each time period were 'Cavities', 'Treatment', and 'Children'. In the structural features of the semantic network of pit and fissure sealants by time period, the density index was found to be around 1.00 for all time periods. The QAP correlation analysis showed the highest correlation between the first and second periods and the fourth and fifth periods with a correlation coefficient of 0.834. The co-occurrence analysis showed that 'cavities' and 'prevention were the top two words across all time periods. Conclusion: This study showed that pit and fissure sealants are well accepted by the society as a preventive treatment for caries. However, the awareness of health education related to these sealants was found to be low. Efforts to revitalize stagnant pit and fissure sealants need to be strengthened with effective education.
Objectives: This study aimed to evaluate the relationship between pit and fissure sealants of the molars, and caries prevalence and experience to assess the effect of sealants on caries prevention. Methods: Data from 16,119 people were extracted from the seventh Korean national health and nutrition examination survey, which was conducted from 2016 to 2018. Demographic, socioeconomic, oral health-related behavioral, and oral examination survey data were collected as independent variables. SAS statistics for complex samples and multiple logistic regression analysis (Windows ver. 9.4) was used for the statistical analyses. Results: Caries prevalence and experience were significantly associated with age, sex, daily toothbrushing frequency, interdental cleaning, and having molar sealant. Multiple logistic regression analysis revealed a significant association between having molar sealants and the caries prevalence and experience among participants under 30 years of age after adjusting socio-economic variables and toothbrushing-related factors. Conclusions: This study confirmed the effect of molar sealants on caries prevention in Korea. Our results support that the application of molar sealants in children, adolescents, and young individuals is effective in preventing caries.

The aim of this study was to identify and quantify the major or detectable monomers released from any of five commercially-available, light-cured pit and fissure sealants with three different light sources : conventional halogen light curing unit, plasma arc light curing unit and LED curing unit. After curing, specimens were immediately immersed in distilled water for different time intervals. The time related release of monomers were analyzed by high performance liquid chromatography(HPLC). Identification and quantitative analysis of monomers were performed by the comparison of the elution time and the absorption peak height of the eluates with those of the authentic sample. The result of this study can be summarized as follows. 1. Standard solution peaks with retention times of 2.3, 3.2, 5.6, 6.5, 10.4 minutes were identified as BPA, TEGDMA, UDMA, Bis-GMA, Bis-DMA, respectively. 2. None of the chromatograms of the tested sealants displayed peaks with the same retention time as that of the standard solution, except for TEGDMA. 3. The highest release rate of TEGDMA was observed during the 12hr period for all samples and declined thereafter. 4. The elution of TEGDMA from curing with Halogen curing unit for 20 second and LED for 10 second was less than that resulting from curing with Plasma arc for 3 second. 5. TEGDMA was detected at much lower levels in eluates from the Pit & Fissure $Sealant^{TM}$ than other sealants. The elution of TEGDMA from the $Helioseal^{(R)}$ F cured with Halogen light curing unit, the $Concise^{TM}$ cured with Plasma arc curing unit and the $Teethmate^{(R)}$ F-1 cured with LED curing unit were higher than other sealants.
