엎침 스쿼트 운동이 안굽이와 무릎 주위 근육에 미치는 즉각적 효과 (Immediate Effects of the Pronation Squat on the Genu Varum and the Muscles Around the Knee)

  • Purpose : The purpose of this study was to compare the ankle pronation squat with the general squat and investigate the effect on the geun varum and the muscles around the knee. Methods : Subjects were chosen as the target for squat exercise with the distance between the knees more than 5 cm. The selected 30 students were randomly divided into 15 pronation squat group and 15 general squat group, and performed five sets movements 20 times. Global postural system (GPS) and digital goniometer were used to check the distance between the knees and the Q angle, and muscle activity was measured with EMG during squat exercise. Results: The result is as follows. First, as a result of analyzing the change in the distance between the knees, the distance between the knees decreased and the Q angle increased in the pronation squat group after exercise. Second, as a result of analyzing the change in muscle activity of the peroneus longus, the muscle activity increased in the pronation squat group after exercise, and it was more effective than the general squat group. Third, as a result of analyzing the change in the VMO (vastus medialis oblique) and VL (vastus lateralis) muscle activity ratio, the activity ratio of the pronation squat group increased after exercise, and the imbalance in the VMO/VL muscle activity was decreased. Conclusion: The pronation squat exercise applied to the ankle will greatly affect what is made into a balanced leg as decreased of the distance between knees, increased of Q angle, increased of the muscle activity of the peroneus longus and the ratio of VMO/VL.

허리 불안정성이 있는 허리통증 환자에게 실시한 자가-복합 운동프로그램이 통증과 기능, 심리사회적, 균형 능력 그리고 배가로근에 미치는 효과 (Effect of Self-Complex Exercise Program on Pain, Function, Psychosocial, Balance Ability, and TrA Muscle in Patients with Lumbar Instability: A Randomized Controlled Trial)

  • Purpose : Low back pain (LBP) is reported as a risk of experiencing musculoskeletal disorders due to muscle stiffness and hypokinetics. The lumbar spine in an unstable state causes imbalance and lumbar instability. Therefore, This study examined the effects of lumbar stabilization exercise and self-complex exercise program on pain, function, psychosocial level, static balance ability, and transverse abdominal muscle (TrA) thickness and contraction ratio in patients with lumbar instability. Methods : The design of this is a randomized controlled trial (RCT). Twenty-six LBP patients participated in this study. Screening tests were performed and assigned to the experimental group (n=13) and control group (n=13) using a random allocation program. Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the self-complex exercise program. All interventions were applied three times per week for four weeks. The quadruple visual analog (QVAS), the Korean version of the Oswestry disability index (K-ODI), Korean version of fear-avoidance belief questionnaire (FABQ), static balance ability, TrA thickness, and contraction ratio were compared to evaluate the effect on intervention. Statistical significance was set at 𝛼=.05. Results : Both groups showed significant differences before and after the intervention in QVAS, K-ODI, FABQ, static balance ability, and TrA thickness in contraction (p<.05). In addition, significant differences in K-ODI and FABQ were observed between the experimental group and control group (p<.05). Conclusion : A lumbar stabilization exercise and self-complex exercise program resulted in reduced dysfunctions, psychosocial stability in patients with lumbar instability. Therefore, Lumbar stabilization exercise and self-complex exercise program for patients with lumbar instability are effective method with clinical significance in improving the function and psychosocial stability.

뇌성마비 아동의 대동작 기능에 대한 가정중심치료 효과 : 체계적 고찰 (Effectiveness of home-based therapy on gross motor function in children with cerebral palsy: A systematic review)

  • Background: Although children with cerebral palsy (CP) are able to walk independently, gait imbalance occurs due to abnormal muscle tone, musculoskeletal deformity, loss of balance, and selective motor control impairment. Gait restriction in the community and school is a major problem of rehabilitation in CP. Home-based therapy (HBT) provides a variety of interventions in which the therapist and the parent work together to resolve the activities and problems caused by the child's body structure. Therefore, we investigate the effectiveness of home-centered therapy on gross motor function in CP and try to present the possibility of clinical application. Design: A Systematic Review Methods: Research papers were published from Jan, 2012 to Jan, 2022 and were searched using Medline and PubMed. The search terms are 'family-centered' OR 'home-based' AND 'cerebral palsy'. A total of nine papers were analyzed in this study. The paper presented the quality level based on Physiotherapy Evidence Database (PEDro) scores to assess the quality of randomized clinical trials studies. Results: The results showed that HBT for strengthening exercise in lower extremity has a positive effect on the isokinetic torque and gross motor function. home-based treadmill therapy in CP is effective to perform at least 12 sessions of treadmill HBP in which the therapist determines the treadmill speed every week and the child's own gait pattern is modified. Conclusion: These results suggest that it will be important data for founding evidence on the effectiveness of home-centered therapy on gross motor function in children with cerebral palsy to advance clinical protocols.

체중심의 보조 방법에 따른 계단 오르기 시 안쪽넓은근과 가쪽넓은근의 근활성 변수들과 개시시간에 미치는 즉각적인 영향 비교 (Comparison of the Immediate Effects of the Assisting Method for Center of Body Weight on Vastus Medialis Oblique and Vastus Lateralis Muscle Activation Variables and Muscle Onset Time During Stair Ascending)

  • Purpose: The aim of this study is to compare the immediate effects of weight-assisting methods on vastus medialis oblique (VMO) and vastus lateralis (VL) muscle activation, on the VMO/VL muscle activation ratio, and on muscle onset time in healthy subjects when ascending stairs. Methods: Healthy participants were randomly assigned to the belt group (n = 11), hand group (n = 11), and control group (n = 11). In the belt group, a belt was wrapped around the sacrum and pulled forward with both hands, moving the center of weight forward, while ascending stairs. The hand group grasped the hips with both hands and climbed stairs, assisting their weight from the rear and moving the center of weight backward, and the control group climbed the stairs without any intervention. Results: Muscle activation of the VMO decreased significantly after the intervention in the belt and hand groups, and activation of the VL muscle in both groups showed a greater decrease than that of the VMO muscle. Further, the VMO/VL muscle activation ratio increased significantly, with an improvement shown in the order of the belt group, hand group, and control group, while muscle onset time also improved in the order of the belt group, hand group, and control group. Conclusion: The belt group demonstrated the greatest effect across all dependent variables, confirming that in clinical practice, these two weight-assisting methods are more effective interventions during stair ascent for patients with knee joint instability, pain, and imbalance than no assistance.

Comparison of Rotator Cuff Muscle Strength With and Without Scapular Dyskinesis in Adolescent Baseball Players

  • Background: Scapular dyskinesis (SD) is closely related to imbalance of the rotator cuff (RC) muscles. However, previous studies have only focused on isometric strength. To our knowledge, there has been no study examining potential differences in concentric and eccentric strength and functional strength ratio (FSR) of the RC muscles based on functional parameters related to throwing in with or without SD. Objects: This study aimed to determine whether there was a difference in the RC muscle strength and FSR between the dominant shoulder with SD and the non-dominant shoulder without SD in adolescent baseball players. Methods: Forty adolescent baseball players participated and classified types of SD based on movement patterns using the SD test by two examiners. The isokinetic concentric and eccentric peak torque of the internal rotation (IR) and external rotation (ER) were measured and quantified as peak torque to body weight (PT/BW). Also, the FSR was obtained by calculating the strength ratios of eccentric IR to concentric ER (IRecc/ERcon) and eccentric ER to concentric IR (ERecc/IRcon), respectively. Results: There was a significant decrease in the IR and ER PT/BW in the dominant shoulder with SD compared to the non-dominant shoulder without SD (p < 0.05), regardless of contraction types. However, no significant difference was observed in the FSR in both IRecc/ERcon and ERecc/IRcon ratios. Conclusion: The findings indicate that the isokinetic concentric and eccentric PT/BW of the IR and ER muscles were significantly lower in the dominant shoulder with SD than in the nonthrowing shoulder without SD. Therefore, when establishing a strategy for addressing RC muscle weakness in adolescent baseball players with SD, it is essential to consider an approach that accounts for scapular kinematic recovery.

전방머리자세와 중립자세에 대한 인위적 자세변화가 호흡에 미치는 영향 (Effect of the Changes in Forward Head Posture and Neutral Head Posture on Respiratory)

  • Purpose : Forward head posture is typical neck disorders occur in all people. And this attitude causes a shortening and weakening of the muscles in the body. It also causes excessive extension acts as a reward. This attitude has to change if the pain occurs around the neck and shoulders, and are subjected to unusual stress. Patients with chronic neck pain associated with forward head posture was found to be the more severe the fall of the respiratory, forward head posture poor quality of much breath. The purpose of this study was to compare the effect of changes in forward head posture and neutral head posture on respiratory. Method : Forty volunteers were participated in study and divided into two groups [forward head posture group (n=20) and neutral head posture group (n=20)]. We measured cervical alignment with global postural system to find out a forward head posture. Respiratory function was measured with a SPIROVIT SP-1 equipment and we found out a forced vital capacity (FVC), forced expiratory volume at one second (FEV1) and FEV1/FVC. A pared t-test was used to determine a statistical significance for the pulmonary function variation and a independent t-test was used to determine a statistical significance in the two groups. Results : In the experimental group, FVC and FEV1 were significantly higher in the artificial neutral head posture than in the forward head posture. In the control group, FVC and FEV1 were significantly higher in the neutral head posture than in the forward head posture. FVC, FEV1, and FEV1/ FVC were significantly higher in the neutral head posture of control group than the artificial neutral head posture of experimental group and higher in the artificial forward head posture of control group than the forward head posture of experimental group. Conclusion : In conclusion, neutral head posture is considered to be an important factor in correct posture and improvement of lung function and continuous study of posture correction program for posture imbalance will be needed.

만성 뇌졸중 환자에서 경직에 따른 족저근막의 두께 변화에 관한 연구 (A Study on Change of Plantar Fascia Thickness in Chronic Stroke Patient Based on Spasticity)

  • 본 연구의 목적은 불균형 상태의 근 긴장도와 무리한 보행훈련에 자주 노출되는 만성 뇌졸중 환자에서 경직에 따른 족저근막의 두께 변화를 알아보고자 하였다. 연구대상은 만성 뇌졸중 환자 54명에서 Group I(MAS G0) 18명, Group II(MAS G1) 18명, Gropup III(MAS G3) 18명으로 선정하였다. 측정방법으로는 임상증상 및 이학적 검사, MAS(Modified Ashworth Scale), 초음파영상 촬영장치, ROM(Range of Motion), VAS(Visual Analogue Scale), TUG(Timed Up and Go test)로 측정하였다. 연구결과, 각 그룹 간 정상측과 마비측의 족저근막의 두께는 통계학적으로 유의하게 두꺼워졌다(p<.001). 각 그룹 간 정상측과 마비측의 족관절 배측굴곡 ROM은 통계학적으로 유의하게 작아졌으며(p<.001), VAS(p<.001), TUG(p<.001)는 통계학적으로 유의하게 커졌다(p<.001). 본 연구는 족저근막에 대한 병리역학(pathokinesiology)적 내용을 제시함으로서 뇌졸중 환자의 보행훈련 시 고려해야 할 내용 중 하나임을 제시하였다.

코로나19 전후 청소년의 식생활에 대한 관심과 인식 변화 연구 (A Study on Changes in Interest and Awareness of Adolescents' Dietary Habits Before and After COVID-19)

  • 본 연구는 네이버, 다음, 구글, 유튜브와 트위터 등을 대상으로 코로나19 국내 확진자 발생이 공식 발표된 2020년 1월 19일 기준으로 전후 2년씩, 총 4년 동안 텍스톰(TEXTOM)을 사용하여 청소년의 식생활에 대한 관심과 인식의 변화를 분석하여 향후 가정 교과의 식생활 관련 단원에서 올바른 식생활 교육을 할 수 있도록 식생활 교육 프로그램을 개발할 수 있는 계기를 만들고자 하였으며, 본 연구를 통해 얻어진 결과는 다음과 같다. 첫째, 코로나19 이전 가장 동시 출현 빈도가 높은 키워드는 '영양'과 '상담'이었고, 다음 키워드는 '영양'과 '교육'으로 나타났다. 코로나19 이후 가장 동시 출현 빈도가 높은 키워드는 '영양'과 '교육'이었고, 다음 키워드는 '식품'과 '안전'이었다. 둘째, 동시 출현 빈도 네트워크 분석 결과 코로나19와 상관없이 영양과 상담에 대해 높은 관심을 가지고 있었고 코로나19 이후에는 안전과 건강에 대한 관심이 더 증가함을 알 수 있었다. 셋째, CONCOR 분석을 통한 군집 형성으로 코로나19 이전에는 '식사 및 신체활동', '피부 및 질병', '건강 및 음식', '영양 및 섭취'로 범주화하였고 코로나19 이후에는 '영양, 섭취 및 코로나19', '식사 및 신체활동', '피부 및 질병', '생체리듬 불균형 및 질병'으로 범주화하였다. 넷째, 코로나19 전후 청소년의 식생활 관련 키워드 군집 분석 네트워크 결과 코로나19 이전에는 '식사 및 신체 활동' 그룹의 키워드들은 '건강 및 음식'과 '영양 및 섭취' 그룹의 키워드들과 강하게 연결되어 있었고, 코로나19 이후에는 '식사 및 신체활동' 그룹의 키워드들은 '영양, 섭취 및 코로나19' 그룹의 키워드들과 강하게 연결되어 있었다.

간호사의 태움 개념분석 (Concept Analysis of Tae-um in Nurses)

  • 본 연구는 간호사의 태움 개념분석연구로 개념의 속성을 확인하고 명확한 개념 정의를 내리고자 시도되었다. Walker and Avant의 개념분석 방법을 사용하였다. 간호사의 태움의 속성은 조직문화, 피해자인 미숙한 신규 간호사, 가해자인 선임 간호사, 가르침의 변질, 언어적, 신체적 폭력경험, 심리적, 신체적 증상으로 도출되었다. 간호사의 태움의 선행요인은 간호업무 과다, 높은 노동 강도, 직무 스트레스, 인력 부족, 열악한 근무 환경, 신규 간호사의 업무 능력이 기대에 미치지 못함, 조직의 위계질서, 힘의 불균형이었으며 결과는 신체적, 심리적 증상, 간호업무 효율 저하, 불신 증가, 이직의도 증가, 환자 간호의 질 하락으로 나타났다. 간호사의 태움의 선행요인을 해결하기 위해 활동 간호인력 확충을 위한 근로조건 개선, 사회적지지 프로그램 제공, 의사소통과 상호협력 관계를 지향하는 공동체 문화가 조성되어야 한다. 이후 간호사의 태움에 대한 더 많은 질적 연구와 태움의 속성이 잘 반영된 도구개발 연구, 태움 문화 개선을 위한 프로그램 개발 연구를 제언한다. 본 연구는 태움의 선행요인을 제시하여 해결방안을 제안함으로써 신규간호사 적응 프로그램을 위한 이론적 근거를 제공했다는 점에서 의의가 있다.

Kyphotic cervical curvature로 인한 항통(項痛) 4례(例)에 대한 임상적(臨床的) 고찰(考察) (Clinical studies on neck pain 4 cases associated with kyphotic cervical curvature)

  • Kyphotic cervical curvature, straghtening is commonly caused by trauma, muscle spasm without trauma and wrong posture, etc. Objective : This study is performed to evaluate the clinical effect of neck pain associated with Kyphotic cervical curvatre on cervical x-ray lateral view. Methods : One of the many causes, We examined the patients with neck pain & upper back pain who visited to Department of Acupuncture & Moxibustion, Gumi Oriental Hospital of Kyung-San University from 16th June 1999 to 22th June 2000. Pre and post treatment, We evaluated the cervical angle, Jochumsen's method, VAS(visual analogue scale) and effective score of treatment. Results & Conclusion : 1. Kyphotic cervical curvature is mainly caused by wrong posture during long term, sudden trauma, etc. therfore, postcervical muscles and tendon are injuryed by strong stress. So, muscle imbalance and pain is occured. 2. On these cases, The improvement index for pre/post treatment showed 28/42, 10/15, 9/30, 28/42 degree in cervical angle. Jochumsen's method showed -1/+2, -9/-3, -5/-2, -1/+2mm. Afer treatment VAS is 2, 1, 1, 1 and effective score of treatment is above good. The results suggest that treatments of Oriental Medicine(Acupuncture & Moxibustion, Chu-Na, Cupping and Physical therapy) are effective methods for neck pain with kyphotic cervical curvature on cervical x-ray lateral view.
