Perceived Competency, Frequency, Training Needs in Physical Assessment among Registered Nurses

  • Purpose: The purpose of this study was to identify registered nurses learning needs about physical assessment. Specifically, what are the perceived competency, frequency of skill use and the unmet training needs. Methods: The study was an exploratory survey study. The sample was 104 registered nurses. Data were collected through three instruments: The Perceived Competency in Physical Assessment Scale, the Frequency of Physical Assessment Scale, and the Training Needs of Physical Assessment Scale which incorporated 30 core Physical Assessment skills. Descriptive statistics, t-test, and Pearson's correlation coefficient were used to analyze the data. Results: Auscultation of heart and lung sounds and inspection of the spine were rated by the subjects as physical assessment skills they feel least competent and also were less frequently performed. The most competent area for physical assessment was neurological system. The respiratory and abdominal system was identified as two systems that more education would be needed. Nurses with less than one year of working experience reported needing more training. Nurses with more than five years of clinical work experience performed physical assessment more frequently than nurses with less than five year of work experience. The perceived competency was positively related to the frequency of physical assessment. Conclusion: Continuing education is necessary to further train registered nurses regarding physical assessment skills and the program needs to be focused on the area which nurses are less competent for and have high training need.

Effectiveness of Self-directed Learning on Competency in Physical Assessment, Academic Self-confidence and Learning Satisfaction of Nursing Students

  • Purpose: Competency in physical assessment is an important component of nursing practice. However, some physical assessment skills are not being utilized within the current teacher-centered, content-heavy curriculum. This study was conducted to identify the effects of student-centered, self-directed learning in the physical assessment class. Methods: An experimental study with a post-test only control group design was used to compare an intervention group that was provided self-directed learning classes and a control group that was provided traditional lecture and practice classes. Competency in physical assessment, academic self-confidence, and learning satisfaction were evaluated. Collected data were analyzed using $x^2$-test (Fisher's exact test) and independent t-test. Results: Competency in physical assessment was significantly higher in the experimental group. However, academic self-confidence and learning satisfaction were not significantly different between the groups. Conclusion: The findings in this study indicate that self-directed learning can improve nursing students competency in physical assessment and that self-directed learning is a good education method to improve nursing students' competency in physical assessment during clinical practice and perform quality patient care by making active use of physical assessment skills.

웹기반의 건강사정 멀티미디어 컨텐츠 개발을 위한 실태조사연구 (A Preliminary Survey for the Development of Web-based Multimedia Contents on Physical Examination and Health Assessment)

  • Purpose: This descriptive survey was undertaken to find out the offering types of Physical Examination and Health Assessment course, student's ability to utilize web-based multimedia, and the basic knowledge on physical examination and health assessment in students attending baccalaureate(BSN) and associate degree(AD) programs in nursing, RN-BSN completion programs, and medical school. Method: 77 nursing programs in Korea were surveyed for the information on Physical Examination and Health Assessment courses, and the basic knowledge were tested on 169 nursing and medical students. Results: Majority of BSN(90.69%) and AD(85.29%) programs were offering Physical Examination and Health Assessment courses in students's sophomore years. Majority(95.58%) of programs were using face to face teaching method where as only 4.41% were using web-based multimedia programs. Basic knowledge on Physical Examination and Health Assessment were highest among students in medial school(14.04%) and RN-BSN(13.15%) compared to BSN(8.96%) or AD(8.06%) program. Conclusion: Web-based multimedia contents on Physical Examination and Health Assessment should be developed and should be integrated into the nursing curriculum.

뇌졸중환자 평가를 위한 Chedoke-McMaster Stroke Assessment의 측정자간 신뢰도 (Inter-Rater Reliability of Chedoke-McMaster Stroke Assessment for Stroke Patients)

  • This study was performed to determine the inter-rater reliability of the Chedoke-McMaster Stroke Assessment translated in Korean. This measures the physical impairments and disabilities that impact on the lives of individuals with stroke. The purposes of this measure were 1) to stage motor recovery to classify individuals in terms of clinical characteristics, 2) to predict rehabilitation outcomes, and 3) to measure clinically important change in physical function. Twenty-two subjects from physical therapy unit were assessed by two physical therapists. The ratings were compared by Spearman's rank correlation The correlation between two raters ranged from 0.85 to 0.98. Inter-rater reliability coefficient for total scores ranged from 0.95 to 0.97. This study confirms that the Chedoke-McMaster Stroke Assessment yields reliable results.

전문 및 전담 간호사의 신체검진 수행정도 및 심화교육 요구 (Use of Physical Assessment Skills and Education Needs of Advanced Practice Nurses and Nurse Specialists)

  • Purpose: The study was done to investigate physical assessment skills used by, and educational needs of, advanced practice nurses (APNs) and nurse specialists in Korea. Methods: A total of 123 APNs and nurse specialists working in five major hospitals in Seoul were surveyed from July 15 to August 20, 2007. Results: Fourteen skills out of 126 items were reported as being performed on a regular basis by participants. The majority of these skills involved general observation. Forty-six skills were rarely used. Some participants showed a lack of confidence in certain assessment skills, such as in doing a rectal or pelvic exam, and the use of some assessment equipment. Over 90% of participants required in-depth education on health assessment provided by specialists or nursing professional organizations. Conclusion: More educational opportunities in physical assessment should be provided including education programs based on the nurses' skill levels and needs. This effort will help to increase confidence of APNs and nurse specialists in physical assessment skills, ultimately resulting in better nursing outcomes.

뇌졸중 환자에서 기능평가와 보행 및 균형과의 관련성 (The Relation Between the Fugl-Meyer Motor Assessment and Walking and Balance Ability in Stroke Patient)

  • Purpose : The purposes of this study were to find correlations among Fugl-Meyer Assessment Scale, walking velocity, walking asymmetry and balance ability. Methods : The study sample consisted of 50 stroke patients referred to the Department of Rehabilitation Medicine in the Sanggye Paik, Ilsan Paik, Seoul Paik, and Dobong Hospital. All subjects were ambulatory with or without an assistive device. All participants were assessed on Fugl-Meyer Assessment scale and walking velocity, walking asymmetry. The data were analyzed using independent t-test, ANOVA, and multiple regression. Results : The results revealed that upper extremity coordination, balance and pain items of Fugl-Meyer Assessment scale were significantly correlated with walking velocity and upper extremity and upper extremity motor and balance items of Fugl-Meyer Assessment scale were significantly correlated with walking asymmetry. Fugl-Meyer Assessment scale was not significantly correlated with Static Balance Index, Dynamic Balance Index and Weight Distribution Asymmetry Index. Their power of explanation regarding comfortable walking velocity and comfortable walking asymmetry were 60.3%, 42.5% respectively. Conclusion : These results showed that Fugl-Meyer Assessment scale is significantly correlated with walking velocity, asymmetry and not significantly correlated with balance ability. Therefore Fugl-Meyer Assessment scale is an appropriate assessment tool to predict walking ability of patients with stroke. Futher study about walking velocity and asymmetry by change of Fugl-Meyer Assessment scale is needed using a longitudinal study design.

Reliability and Validity Inquiry for Tele-assessment Based on Video Conferencing

  • Objective: The study examined the validity and reliability of tele-assessment of cervical range of motion (ROM) through video conferencing. Design: A cross-sectional study Methods: The study included 22 healthy adults as subjects and 10 physical therapists as evaluators. The subjects underwent both face-to-face and tele-assessment of cervical ROM. The CROM (Cervical Range of Motion) device was used for reference measurements in the face-to-face assessment. The evaluators assessed the subjects' ROM through visual inspection (VI) and using a Universal Goniometer (UG) in the tele-assessment. Results: The results showed a significant correlation between the face-to-face and tele-assessment measurements for all ROM contents. The correlation coefficients ranged from r=0.54 to 0.71 (p<0.05), indicating concurrent validity. In terms of inter-rater reliability, the ICC values for both VI and UG assessments in the tele-assessment were high, with ICCs of 0.93 and 0.92, respectively. Additionally, the study investigated the usability of the tele-assessment method and environment. The satisfaction of the participants and evaluators was assessed using the Telehealth Usability Questionnaire (TUQ), which evaluates perceptions, usability, and satisfaction with telehealth services. Conclusions: In conclusion, tele-assessment of cervical range of motion using video conferencing was valid and reliable. Also, investigated participant and evaluator satisfaction and opinions, providing insights into the clinical value of tele-assessment. It is important to note that the study was conducted only healthy adults, and further research may be needed to validate the tele-assessment method in populations with cervical dysfunction or neck pain. These findings support the potential effectiveness of telerehabilitation services in assessing and managing musculoskeletal disorders.

A Study on the Transition and Classification of Somatotyping

  • Experts of medicine, philosophy and psychology found that human somatotypes were related to personality or physical characteristics and classified somatotypes into various forms. This study explored the changes and types of somatotyping methods from Before Christ to present day and identified the status of studies of somatotypes in the area of physical therapy. This study covered the methods applied in various majors with a focus on those provided in books and papers of Heath BH. and Carter JEL. Based on the results, there are officially twelve assessment methods. Currently, the method of Heath & Carter is most widely applied. Somatotypes are studied in many areas. It is actively explored in the area of pain physical therapy, thermotherapy and integumentary physical therapy. Also, the soft tissue physical therapy area seeks interdisciplinary studies. This study found that there were various assessment methods in diverse areas. It is likely that continuous studies will develop new assessment methods. It is hoped that in the area of physical therapy, somatotypes shall be applied more amply.

하천정비에 의한 하천의 물리적 교란 평가 - 남강과 영천강을 대상으로 - (Assessment of Physical Stream Disturbances by River Improvement - Case Studies of Nam River and Youngcheon River -)

  • The objects of study is to propose criteria for physical river disturbance assessment and as case study to show the application results for river improvement. For this purpose, the river disturbance assessment method for past disturbance process and the present-day potential natural state of stream is proposed. To assess the disturbance of the Youngcheon River caused by river improvement, One ares of Nam River was selected for the reference reach and two areas of Youngcheon River were selected for the comparison reach. And these reaches were surveyed and analyzed according to applying criteria of the river disturbances assessment. The assessment indices were physical factors as like epifaunal (bottom), embededness, velocity/depth regime, sediment deposition, channel flow status, channel alteration, frequency of riffles, bank stability, vegetative protection and riparian zone etc. The results showed that physical river environment in Youngcheon River area was disturbed by artificial revetment and bed excavation, consequently this disturbance give rise to impact of ecosystem in river. Hereafter, the criteria for river disturbance assessment are needed to consider various river characters as bed materials and bed slop etc.

운동조절능력의 평가 (Assessment of Motor Control Ability)

  • The purposes of this review are that 1) what is the the concept of motor control, 2) what is the content of motor control assessment, and 3) which instruments or scales are used in clinical setting. Motor control is defined broadly the control of both movement and posture. And motor control focuses on understanding the control of movement already acquired. The purposes of assessment are screening, placement to apprpriate treatment program, program plan, evaluation, and progress assessment for individuals. The content of motor control assessment is included not only fexibility, tone, reflex & reaction, muscle strength, movement pattern, balance, gait, and functional ability, but also cognition, arousal, sensation, and perception. There ars many kinds of instruments or scales for assessing motor ability. Most of materials are tested the validity and reliability. But Korea has not own instrument for assessing motor ability. Therefore, Korea needs to develop the assessing tools for motor ability.
