The Effects of M&L Trauma Psychotherapy on Impact of Events, Affection, and Quality of Life among Female Vicims of Family Violence (가정폭력 피해여성들의 사건충격과 정서 그리고 삶의 질에 대한 M&L 트라우마 심리치료 프로그램의 효과에 대한 임상연구)

  • Objectives: The purpose of this study is to investigate the effects of the M&L Trauma Psychotherapy Program on: Impact of Event Scale - Revised (IES-R-K); Euro QOL-5 Dimension (EQ5D); Five-facet Mindfulness Questionnaire (FFMQ); and psychological tests. Methods: Eight middle-aged women subjected to domestic violence participated in the two-day M&L Trauma Psychotherapy Program. The Program was executed 5 times 2 days for about 3 hours. We evaluated IES-R-K, EQ5D, FFMQ, Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), State-Trait Anger Expression inventory (STAXI) and SUDS before the Program and for four weeks afterwards, to analyze the effects of the M&L Trauma Psychotherapy Program. Results: The scores of IES-R-K, BDI, STAI-S, STAI-T, Hwa ST and Hwa CT decreased significantly (p<0.05). EQ5D scores increased significantly (p<0.05) and overall physical condition scale (VAS) increased. In the FFMQ scores, all five facet scores increased slightly. Scores of SUDS decreased significantly right after the program, and remained decreased four weeks later, rather than before the Program (p<0.05). Conclusions: The results suggested that the M&L Trauma Psychotherapy Program improved post-traumatic stress, quality of life and mindfulness skills, and had positive responses to psychological problems - depression, anxiety, anger and distress.

The Pathway Linking Childhood Trauma and Stress Perception: Mediating Role of Theory of Mind (아동기 외상과 마음이론이 스트레스 지각에 미치는 영향)

  • This study investigated the differential effects of childhood trauma, especially neglect, emotional abuse, and physical abuse on stress perception in relation to the ToM (theory of mind), among one-hundred and fifty five college students. For this purpose, self-reported childhood neglect, emotional abuse, physical abuse, and stress levels during the previous week were measured. Social cognition was measured using a ToM questionnaire which asked respondents to guess the intentions of the person in each vignette by providing additional hints. In order to examine the predicting role of childhood trauma and mediating role of ToM, two distinct structural models assuming full (Model 1) and partial (Model 2) mediational effects were compared using structural equation modeling. The partial mediational model was supported and the main results were as follows. First, childhood neglect indirectly predicted stress perception through the effect of the ToM. Second, childhood emotional abuse directly predicted stress perception. Third, childhood physical abuse had no distinct effect on stress perception. Finally, the limitations of this study and the implications for future studies are discussed.

Injury of Corticostriatal Tract between the Striatum and the Premotor Area in a Patient with Traumatic Brain Injury

  • Objectives: We investigated injury of corticostriatal (CStr) tract in patient with mild traumatic brain injury (mTBI), which was demonstrated by DTT. Method: A 44-year-old female with no previous history of neurological, physical, or psychiatric illness had suffered from head trauma resulting from a pedestrian car accident. She complained that could not quickly move the left hand with her intension. After three month's administration, her slowness movement of left hand recovered rapidly to the point that she was able to extend all fingers quickly. Results: On DTT configuration, the integrity of the left CStr tract was well-preserved, however the right CStr tract showed narrowing and partial tearing in the subcortical white matter on a DTT at 25 months after onset. Conclusion: Injury of the right CStr tract was demonstrated in a patient who developed mild motor control problems following mild TBI. We believe that the evaluation of the CStr tract from the secondary motor area for patients who showed unexplained motor control problem is necessary.

North-Korean-Refugee Women's Experience of Discrimination in South Korea and Reemergence of Trauma Experience (탈북여성들의 남한사회에서의 차별 경험과 트라우마 경험의 재현에 관한 탐색적 연구)

  • This study aims to explore the reexperience of trauma caused by experience of discrimination and stigma from South Korean. For this purpose of this study, focus group interview methods are used. The participants are five women who escaped from North Korea and they were from 20s to 50s. From the outcomes of this study, I find out that they had experienced to put in prison, to let arrest to let human traffic, and sexual or physical assault. Now they have PTSD(Post Trauma Stress Symptom) of nightmare related with past trauma, avoidance of interpersonal relationships and similar situations, and retrospection and memorization of negative experiences. In that situation, they also have experienced discrimination caused by lack of understanding of their suffering in South Korea. And almost of them suffer from poor situation economically and physically. Participants regret their entry to South Korea and perceive that their hope has gone. And they have anger of Korean's discrimination and stigma and they feel suicide impulse after experiencing very difficult situation. These negative experiences affect their PTSD and make reemergence of trauma experience. In conclusion, the outcomes of this study are discussed and I proposed the practical suggestions which have affected decreasing the PTSD.

TRAUMATIC FALSE ANEURYSM OF THE LINGUAL ARTERY - A CASE REPORT- (설동맥에 발생한 외상성 위동맥류)

  • False aneurysms(Pseudoaneurysms) and arteriovenous fistulas have only rarely been reported in the facial region. In this region the false aneurysm arises most frequently in the superficial temporal and facial artery, but other branches of the external carotid are sometimes involved, including the maxillary and lingual artery. False aneurysms can be occurred by blunt trauma that either laceration or rupture the full thickness of the arterial wall. The diagnosis of a false arterial aneurysm can be often made solely on the basis of physical examination. Angiography is helpful for conformation, for delineating the lesion and its vascular supply, and for ruling out the presence of associated vascular lesions such as arteriovenous fistulas. Ultrasonography may also be useful in delineating lesions that are not easily accessible for physical examination. Treatment of false aneurysms is excision, ligation, and arterial embolization. This is a case of false aneurysm of the lingual artery after facial trauma caused by traffic accident. The lesion was successfully treated by embolization and ligation of the lingual and facial branches of the external carotid artery.

The Study of Muscle Fatigue and Risks of Musculoskeletal System Disorders from Text Inputting on a Smartphone

  • Objective: This study aims to analyze and evaluate fatigue in upper trapezius muscles of adults when inputting text on a smart phone, thereby examining the risk of cumulative trauma disorders in the musculoskeletal system of the upper extremity. Method: Forty-five healthy adults were divided equally into three groups: a smartphone group, a notebook group, and a desktop group. A surface electromyogram was placed on the upper trapezius muscle of each participant, after which they inputted text for ten minutes within an ergonomic setting; their side posture was recorded on video and analyzed. Results: The spectral edge frequency values for the smartphone group, as recorded from the upper right trapezius muscles on the surface electromyogram, showed a significant decrease(p<.05). Regarding the RULA, all three groups had results indicating "action level 2". Conclusion: All three groups had results indicating "action level 2" according to the RULA, which means that additional research on the posture of the participants while performing tasks is necessary, and their posture needs to be corrected. In particular, the smartphone group has the highest levels of fatigue in the upper trapezius muscles, suggesting that the task of inputting text on a smartphone is a high risk for causing cumulative trauma disorders.

  • Thoracic outlet syndrome is actually a collection of syndromes brought about by abnormal compression of the neurovascular bundle by bony, ligamentous or muscular obstacles between the cervical spine and the lower border of the axilla. First of all a syndrome is defined as a group of signs and symptoms that collectively characterize or indicate a particular disease or abnormal condition. The neurovascular bundle which can suffer compression consists of the brachial plexus plus the C8 and T1 nerve roots and the subclavian artery and vein. The brachial plexus is the network of motor and sensory nerves which innervate the arm, the hand, and the region of the shoulder girdle. The vascular component of the bundle, the subclavian artery and vein transport blood to and from the arm. the hand. the shoulder girdle and the regions of the neck and head. The bony, ligamentous, and muscular obstacles all define the cervicoaxillary canal or the thoracic outlet and its course from the base of the neck to the axilla or arm pit. Look at the scheme of this region and it all becomes more easily understood. Compression occurs when the size and shape of the thoracic outlet is altered. The outlet can be altered by exercise, trauma, pregnancy, a congenital anomaly, an exostosis, postural weakness or changes. Thoracic outlet syndrome has been described as occurring in a diverse population. It is most often the result of poor or strenuous posture but can also result from trauma or constant muscle tension in the shoulder girdle. The first step to beginning any treatment begins with a trip to the doctor. Make a list of all of the symptoms which seem to be present even if the sensations are vague. Make a note of what activities and positions produce or alleviate the symptoms and the time of day when symptoms are worst. Also, note when the symptoms first appeared. This list is important and should also include any questions one may have.

The Effects of Combined Exercise Program to Intra-City Bus Driver With Chronic Neck Pain : Case Report (만성 경부통을 가진 시내버스 운전기사에게 복합 운동프로그램의 효과 : 사례연구)

  • Background: Although many of those who drive for a living are exposed to repetitive and awkward posture and whole body vibration which cause them cumulative trauma disorder, therapeutic studies related to the problems from the transport unit are still somewhat inadequate. The purpose of this case report is to identify the effect of combined exercise program on patients suffering chronic neck pain caused by long hour of intra-city bus driving. Method: A total of 4 subjects are selected as patients, all of whom have more than 10 years of experience in intra-city bus transportation services. These people have suffered from neck pain for years. We ran the combined exercise program 3 times a week for 4 weeks and respectively evaluated the results after the 2nd and 4th week. We implemented conventional physiotherapy for 40 minutes, another 40 minutes of combined exercise program, and then educated the patients to enable themselves to do active stretching program as a home program. The combined exercise program contained 3 different stages. 1st stage: active stretching program, 2nd stage combined stabilization exercise and strengthening exercise, 3rd stage: proprioceptive exercise Result: After 4 weeks of intervention, there was enhance in the range of motion. $12^{\circ}$ increase in flexion, $10^{\circ}$ in extension, $6^{\circ}$ in lateral flexion respectively, and $10^{\circ}$ in rotation on average. VAS(visual analgue scale) decreased by 33% on average, NDI(neck diability index) by 28% and fatigability by 23%. Conclusion: There were improved results in the range of motion, NDI, VAS, and in fatigability after applying combined exercise program to intra-city bus drivers exposed to whole body vibration and cumulative trauma disorder.

TRAUMATIC TMJ INJURY (외상성 악관절 손상)

  • Mandibular trauma is developed due to traffic accident, fall down, industrial injury, and others. TMJ disorder is usually also developed after facial traumatic injury. Many authors suggested that disc displacement or tearing, acute synovitis, TMJ ankylosis, traumatic arthritis, or effusion are developed after facial trauma. It is still very controversible what is the best treatment of TMJ injury such as condylar fracture and meniscal injury. In TMJ injury, synovial inflammation is developed and pain mediators such as prostaglandin E2 or leukotriene B4 are released from the synovial membrane. This can be a cause of TMJ disorder. I present a variety of experimental study about the condylar fracture and meniscal injury and enzyme-immunoassay of synovial fluid after mandibular trauma that have been studied since 1992 and establish the treatment criteria of traumatic TMJ injury. I think that the treatment option of condylar fracture depends upon the surgeon's criteria exclusively. There are no significant differences between conservative and surgical treatment. If the aggressive functional physical therapy and long-term followup be performed, the favorable functional recovery of TMJ can be obtained. And I think that the initial surgical management of meniscus of TMJ is unnecessary in condylar fracture. And also arthrocentesis can be available to release the patient's subjective symptoms and improve the healing of injured TMJ.

A Solitary Neurofibroma of the Small Finger Associated with Trauma

  • Neurofibroma may present as a solitary lesion or as multiple lesions. Although there is no site of predilection for solitary lesions, occurrence on the hand is rare. Plexiform neurofibroma can develop in isolation or more commonly as a part of neurofibromatosis type 1. In those that apper in isolation, trauma has been suggested as a precipitating factor. A 68-year-old male farmer had experienced repetitive prior episodes of trauma in the involved finger. He presented with a painless mass on the dorsal aspect of the fifth finger. Physical examination showed a protruding mass measuring approximately $15{\times}20mm$ which was not tenderness to palpation and any skin changes or pigmentation. Ultrasonography showed a cystic mass on the dorsal aspect of the middle phalanx. Microsurgical dissection was applied in order to seperated the lesion from the ulnar side of the dorsal branch of the digital nerve. Pathologic examination of the specimens revealed neurofibroma. At three-month follow-up, motor and sensory function were intact, and range of motion was fully recovered. Traumatic solitary neurofibroma is a rare tumor of the hand, especially in the finger. Hand surgeons should be aware of the diagnostic possibilities of this tumor based on examination, history taking and imaging studies.
