Osteoporosis (골조송증)

  • Osteoporosis is a major underlying cause of bone fracture in postmenopausal women and older person in general, and so it is a major public health problem. It is a condition in which bone mass decreases, causing bones to be more susceptible to fracture. A trivial trauma can easily cause one or more bones to break in a person with severe osteoporosis. Physicians and patients alike are concerned with the optimum approach to the treatment and prevention of osteoporosis. The diagnostic method and proper use of agents, such as calcium, vitamin D, estrogen, and fluorides, as well as the role of exercise are issues that have generated major research efforts. So these are to be mentioned in this paper.

An Ergonomics Evaluation on the Shovels of Korean Standard (삽에 대한 인간공학적 평가)

  • We Propose an improved shovel design to minimize physical discomfort and the risk of cumulative trauma disorder at shoveling tasks. The specifications of shovels in Korean standard data were scrutinized to check if they met the anthropometric specification of Korean population in 1997. We also applied "Body Map" pictograph and self-conscious checklist to field survey to analyze the discomfort of shoveling task. Thirty male subjects whose age were ranging from 18 to 65 voluntarily participated in the experiment. The measurement results show that shoveling workers were exposed to hazards of CTD's. Especially, when workers were involved in ditch digging and construction their trunk vend more than 120 degree. In order to compare the new designed shovel and the traditional K.S shovel, an analysis of variance was performed and we obtained very strong evidence that the new designed shovel was better in reducing physical discomfort. According to the results from electromyograrn experiment the new shovel contributed to improving subjective comfort level and reducing low back muscle fatigue. For futher study, the research may be extended to determine the relationship among shovel sharpness, type, digging motion and digging force.ing force.

Diagnostic imaging features of traumatic subgaleal hematoma in a dog: a case report

  • Subgaleal hematomas are accumulation of blood between periosteum and galea aponeurosis. A 2-year-old male Chihuahua was presented with a severe head swelling after trauma. Radiography and computed tomography (CT) showed a massive swelling encircling the entire calvarial vault, extending toward the cervical neck and crossing the suture line. It was heterogeneously, mild hyperdense fluid to soft tissue attenuating with contrast enhancement on CT images. On day 4, physical and imaging examination showed resolution of the calvarial swelling. Subgaleal hematoma should be considered as a differential diagnosis when there is a massive soft tissue swelling over the skull on physical and imaging examinations.

Clinical studies on neck pain 4 cases associated with kyphotic cervical curvature (Kyphotic cervical curvature로 인한 항통(項痛) 4례(例)에 대한 임상적(臨床的) 고찰(考察))

  • Kyphotic cervical curvature, straghtening is commonly caused by trauma, muscle spasm without trauma and wrong posture, etc. Objective : This study is performed to evaluate the clinical effect of neck pain associated with Kyphotic cervical curvatre on cervical x-ray lateral view. Methods : One of the many causes, We examined the patients with neck pain & upper back pain who visited to Department of Acupuncture & Moxibustion, Gumi Oriental Hospital of Kyung-San University from 16th June 1999 to 22th June 2000. Pre and post treatment, We evaluated the cervical angle, Jochumsen's method, VAS(visual analogue scale) and effective score of treatment. Results & Conclusion : 1. Kyphotic cervical curvature is mainly caused by wrong posture during long term, sudden trauma, etc. therfore, postcervical muscles and tendon are injuryed by strong stress. So, muscle imbalance and pain is occured. 2. On these cases, The improvement index for pre/post treatment showed 28/42, 10/15, 9/30, 28/42 degree in cervical angle. Jochumsen's method showed -1/+2, -9/-3, -5/-2, -1/+2mm. Afer treatment VAS is 2, 1, 1, 1 and effective score of treatment is above good. The results suggest that treatments of Oriental Medicine(Acupuncture & Moxibustion, Chu-Na, Cupping and Physical therapy) are effective methods for neck pain with kyphotic cervical curvature on cervical x-ray lateral view.

Verruciform xanthoma in the hard palate: a case report and literature review

  • Oral verruciform xanthoma (OVX) is an uncommon lesion that appears on the oral mucosa. The aim of this paper was to discuss the probable etiopathogenesis of OVX in the hard palate, reinforcing the importance of including this lesion in the differential diagnosis of verrucous lesions. A 43-year-old male smoker presented with a painless lesion with a verrucous surface and erythematous spots on the hard palate. Excisional biopsy revealed oral mucosa consisting of hyperkeratosis, acanthosis, and elongated rete pegs. Subjacent connective tissue showed numerous foam cells with clear cytoplasm and pyknotic nucleus, negative on periodic acid-Schiff staining. Immunohistochemical analysis revealed foam cells positive for anti-CD68 antibody, while anti-KI-67 antibody was restricted to the basal layer of the oral epithelium. A final diagnosis of OVX was established. The patient showed no signs of recurrence after seven months of follow-up. Physical trauma and smoking habits can be directly related to the etiology of verruciform xanthoma because the lesion is chronic and inflammatory with slow growth, and sites if high trauma are more often affected by such a lesion. The hard palate is the second most commonly affected site, and local trauma caused by smoking can be a cause of this type of lesion.

Simultaneous Surgery on Jejunum perforation with Pelvic Ring Fracture: A Case Report

  • Patients with pelvic bone fractures with gastrointestinal perforations are reported in 4.4% of the cases and in very rare cases jejunum (0.15) is involved. However, intestinal perforations are often undiagnosed on the first examination before peritonitis is evident. We are presenting a report where a patient with anteroposterior compression injury, who was expected to undergo an internal fixation procedure, did not show any jejunum perforations on abdominal CT or other physical exams but was found on abdominal CT 1 week after right before surgery, therefore excision and anastomosis surgery, pelvic open reduction and internal fixation was simultaneously done with favorable results. In our case, we present a 61 year old male patient with liver trauma, adhesion at the abdominal cavity, with a past history of gallbladder excision, but without abdominal pain, fever, or infection symptoms. Therefore, this was a case that was difficult to initially diagnose the patient with jejunum perforation and peritonitis. The diagnosis was further supported during laparotomy when peritonitis around the area of intestinal perforation was observed. Generally, it is understood that pelvic bone fracture surgery is not immediately done on patients with peritonitis. However, this kind of patient who had peritonitis with intestinal adhesion and other complications could undergo surgery immediately as infection or other related symptoms did not coexist and the patient was rather stable, and as a result the treatment was successful.

Experiences with Pancreaticoduodenal Injuries at a Single Institute (단일 기관에서 치료한 췌 십이지장 손상에 대한 임상경험 보고)

  • Purpose: Although the duodenum and the pancreas are protected by surrounding organs and have a low probability of injury during trauma, the mortality and the morbidity due to complications is high. This report includes the pancreaticoduodenal injuries we observed that were treated at a single institute. Methods: The medical records of patients admitted to our institute between 2001 and 2012 for pancreaticoduodenal injury were retrospectively reviewed. Results: In our hospital, between 2001 and 2012, 15 patients were admitted for a pancreaticoduodenal injury. All patients experienced blunt trauma, 6 of whom were involved in traffic accidents and 9 of whom received injuries from physical assault. Most of the patients were men(13 of 15 patients, 86%) with a mean age of 23 years (range, 5?39 years). All patients were admitted to the emergency center and managed by the surgeons on duty. The mean value of the injury severity score was 22. The mortality rate was 6%(1 of 15 patients). Seven of the fourteen surviving patients(50%) had duodenal injury, 6 patients(42%) had a pancreatic injury, and 1 patient(7%) had a combined pancreaticoduodenal injury. The surgical procedures were targeted at damage control. Conclusion: In conclusion, we believe that damage control surgery is the optimal management for a pancreaticoduodenal injury.

Sitting Posture Associated With Carpal Tunnel Syndrome: A Literature Review (앉기 자세와 수근관 증후군의 상관관계에 관한 고찰)

  • The objectives of this study was to investiage the effects of sitting posture on carpal tunnel syndrome. Carpal tunnel syndrome (CTS) continues to be one of the most widely publicized maladies of the cumulative trauma disorder. Many studies have reported a positive association between CTS and highly repetitive work, high force, and poor posture. High force and repetitive work have especially been associated with CTS, but the evidence for work being a primary cause of CTS is strongest when these factors are combined. In addition to carpal tunnel syndrome, hand, wrist, and other disorders are attributed to these work-related movements. Such disorders are referred to as repetitive stress injuries, cumulative trauma disorder, overuse syndromes, and chronic upper limb pain syndrome. Incorrect posture also may play a role in the development of CTS in people who work at a computer and other types of keyboards. The tendency to roll the shoulders forward, round the lower back, and thrust the chin forward can shorten the neck and shoulder muscles, compressing nerves in the neck. This, in turn, can affect the wrist, fingers, and hand. The treatment and prevention of carpal tunnel syndrome continue to be approached with a segmental view of the human body. For example, the most common ergonomic solution for carpal tunnel syndrome associated with keyboard use is to keep the wrists in a neutral position by using a wrist rest in front of the keyboard and good sitting posture.

The Effect of Standard Keyboard and Fixed-Split Keyboard on Wrist Posture During Word Processing (문서입력 작업 시 컴퓨터 키보드 유형이 손목관절의 운동학적 특성에 미치는 영향)

  • There were two purposes of this study. The first was to research the effects of standard and fixed-split keyboards on wrist posture and movements during word processing. The second was to select optimal computer input devices in order to prevent cummulative trauma disorder in the wrist region. The group of subjects consisted of thirteen healthy men and women who all agreed to participate in this study. Kinematic data was measured from both wrist flexion and extension, and wrist radial and ulnar deviation during a 20 minute period of word processing work. The measuring tool was an electrical goniometer, and was produced by Biometrics Cooperation. The results were as follows: 1. The wrist flexion and extension at resting starting position were not significantly different (p>.05), however the angle of radial and ulnar deviation were significantly different in standard and split keyboard use during word processing (p<.05). 2. In the initial 10 minutes, the dynamic angle of wrist flexion and extension were not significantly different (p>.05), however the dynamic angle of radial and ulnar deviation was significantly different in standard and split keyboard use during word processing (p<.05). These results suggest that the split keyboard is more optimal than the standard keyboard, because it prevented excessive ulnar deviation during word processing.

Extensor Pollicis Longus Tendon Rupture Following Local Steroid Injection (국소 스테로이드 주사 후에 발생한 장무지신건 파열)

  • Spontaneous extensor pollicis longus tendon rupture is commonly caused by attrition of the tendon from trauma or inflammatory processes. We experienced a patient with extensor pollicis longus tendon rupture after steroid injection, in which the rupture may have been caused by the effects of steroid itself as well as direct damage from the needle. A 51-year-old woman complained of inability to extend her right thumb at the first metacarpophalangal & interphalangeal joint level. The patient had a history of local steroid injection into the dorsal & radial side of wrist on two occations, and had no history of trauma or rheumatologic disease. After a physical examination of the patient, we decided to explore the wrist. The patient agreed with operation. Intraoperatively, an incision was made into the wrist and the proximal and distal ends of the ruptured extensor pollicis longus tendon were identified. The defect between the proximal and the distal end was measured to approach 8cm, and a palmaris longus tendon graft was performed. After three months of rehabilitation, the first metacarpophalangal & interphalangeal joint recovered the normal range of motion. Steroid injection has been widely used in various musculoskeletal disorders such as rheumatoid arthritis and osteoarthritis. However, inadvertent steroid injection into the extra or intra articular spaces may lead to tendon rupture. Steroids reduce tensile strength by decreasing tenocyte activity and collagen synthesis. Also, the physical effect of direct needle-stick injury into the mesotenon and blood vessels around the tendon may cause damage. In addition, hematoma and edema may increase pressure around the tendon and compromise blood supply, leading to tendon degeneration and subsequent rupture. When injecting steroid into an articular area, all physicians should have a complete understanding of the surrounding anatomy and always keep in mind the hazards of such procedures.
