Suggested Clinical Reasoning Strategies Using a Mnemonic Device for Patients with Neurological Disorders (연상법을 이용한 신경계 환자의 임상적 추론 전략 제안)

  • Purpose: This study suggests clinical reasoning strategies for therapists with little experience in clinical reasoning for the evaluation and treatment of patients with neurological disorders. Methods: The suggested method was the mnemonic PT STRESS whose initials represent the body structure and functions that can affect the activity limits and the items that can cause problems at the functional level in patients with neurological disorders. Results: PT STRESS stands for pain (P), ability of the trunk (T), sensation (S), tone (T), range of motion (R), emotion and endurance (E), muscular strength (strength), and stability (S). It tests and measures problems in the body structure and functions. Conclusion: This study suggests easy clinical reasoning strategies that can be used by therapists who have insufficient experience in the evaluation or treatment of patients with neurological disorders. However, more factors need to be considered in the future with regard to clinical reasoning of the diverse problems of patients with neurological disorders.

Suggested Strategies for Observational Gait Analysis Using a Mnemonic Device for Patients with Stroke (연상법을 이용한 뇌졸중 환자의 관찰적 보행 분석 전략 제안)

  • Purpose: This study suggests observational gait analysis (OGA) strategies for therapists with limited experience in clinics in evaluating patients with stroke during walking. Methods: The proposed model was the mnemonic STEP APP, whose initials refer to the process of OGA for clinical reasoning and decision-making by identifying problems during walking in patients with stroke. Results: STEP APP stands for step (S), tibia (T), events (E), phases (P), ankle (A), problems (P), and priority (P). It serves as a procedural guide for OGA in patients with stroke walking on the ground. Conclusion: This review suggests a simple evaluation of gait using OGA that can be used by therapists who have less experience and difficulty in evaluating patients with stroke during walking. However, it is important to consider the front and back views of motion as well as motion in the transverse plane in order to analyze problems accurately. Furthermore, small joint problems, such as those in the foot, should be considered when evaluating patients with stroke during walking.

The research of the recognition rate about physical therapy among the high school teachers working in Busan and KyoungNam (부산.경남지역의 일부 고등학교 교사들의 물리치료에 대한 인식도 조사)

  • This study randomly chose 10 humanity high schools, which are located in Busan and KyoungNam province. We then examined the recognition rate of physical therapy among 378 teachers who were and still are working in the schools. 1. The recognition grade of physical therapeutics between men and women was significantly different with results of 80.7% of men and 69.7% of women(p<0.05). In the case of experiencing physical therapy, 83.6% of people said they knew the physical therapeutics and in the case of the opposite side, the result was also significantly different with a result of 67.8%(p<0.05). 2. We ranked the people on how much they knew about the physical therapeutics among men and women. With men, the actual experience of physical therapeutic came before what they heard from other people. In the case of women, what they heard from other people came first and the actual experience of physical therapeutic came after. According to the public information from Inje University, the recognition grade is recorded in the 3rd grade department as 21.5% while in the case of experiencing treatment from a physical therapist, the result was 39.1%. In the other department (the 1st grade department), what men and women heard from mass media was recorded at 17.5% and what they heard from other people, it was recorded at 33.8%(p<0.05). 3. The research about the recognition of academic ability of being a physical therapist was that 55.6% of teachers thought that 3-year-college was the best, which made the results different from the recognition grade of the experience of education(p<0.05). 4. In the recognition grade of physical therapeutics, there were significantly differences from 50.2% of men and 45.2% of women were expecting about 1mil ion won to 1.99million won. In case of 25.2% of people who have experience less than 15years at work, they expected about 2million to 2.99million won. The figures were higher than 10.1% of people who have experiences over 15years. About the recognition grade of physical therapists social level in the future is 48.8% of people who had experienced of physical therapy and 63.8% of people who never had experienced of it said it would be better in the future. From these results, the recognition about physical therapeutics and the view of its social status are optimistic and they know well about the level of academic ability of physical therapists. On the other hand, most teachers don't know exactly about physical therapists work. So we have to inform them aggressively for enhancing the recognition of physical therapy because high school students are well affected by their teachers when they face their future or major in university.

  • This article reviews the concepts of motor ability and motor skill and the models, theories, and treatment approaches of motor contorl. Reflex, hierarchical, and systems models of motor control are discussed. And muscle reeducation, neurodevelopment, motor relearning program, and contemporary task-oriented approach is also discussed. We consider that the concept of motor control is being changed and the treatment approach also is being changed. There are a number of reasons, the first is traditional trteatment approach has limitation. Second is theories of motor control is contineously developed. Third is new approach is coming to us. therefore, the therapists shoulder strive for acquisition of motor skill and motor ability. This article hope the successful integration of models and treatment approaches and its application in physical therapy practice.

  • 21C's physical therapy in going to be changed by the new needs of medical demanders. Therefore we have to ready to treat these things. 1. The resolute alteration of curricula and scientific development of Korea physical therapy are urgent. 2. The levels of educational system have to be changed like developed country in short time and the master's, doctor's courses have to be formed in university. 3. The field of study has to be seperated by the needs and alterations of the times. And the association has to present vision for physical therapy's developing. 4. The university has to invest and support for training of great capacity's therapists.

  • Lateral epicondylitis is characterized by pain at the lateral aspect of the elbow, commonly associated with resisted wrist or finger extension and gripping activities. Lateral epicondylitis is also known such as lateral epicondylosis, tennis elbow, or tendonitis of the wrist extensor muscles, especially extensor carpi radialis brevis. Lateral epicondylitis is a common musculoskeletal lesion in the clinical states, so physical therapist usually meet a patients who has the lateral epicondylitis. But many of physical therapists are treating patients based on a self know-how, so I want to describe a systematically about lateral epicondylitis and give a objective information on the lateral epicondylitis. And now I want that they will be treat patients with lateral epicondylitis not a self-experience but based on a objective information.

  • Purpose : The purpose of this study was to understanding of a hemophiliac with hemarthrosis and provided information of rehabilitative therapy. Methods : The General understanding and rehabilitative therapy of a hemophiliac with hemarthrosis considered the matter carefully using a related publications and a internet searching investigation. Results : From the physical therapeutic method for pain alleviation, decreasing edema, ability of muscular strength and control, range of motion(ROM) in joint, prevention of damage on hemorrhaged joint in the hemophiliac with hemarthrosis, rehabilitative therapy for exercise of ROM, muscular strength, proprioceptive sense was most effective for patients. Conclusion : Physical therapists known the rehabilitative therapy should helped a hemophiliac with hemarthrosis to prevent from permanent change.

Inter-Rater Reliability of Chedoke-McMaster Stroke Assessment for Stroke Patients (뇌졸중환자 평가를 위한 Chedoke-McMaster Stroke Assessment의 측정자간 신뢰도)

  • This study was performed to determine the inter-rater reliability of the Chedoke-McMaster Stroke Assessment translated in Korean. This measures the physical impairments and disabilities that impact on the lives of individuals with stroke. The purposes of this measure were 1) to stage motor recovery to classify individuals in terms of clinical characteristics, 2) to predict rehabilitation outcomes, and 3) to measure clinically important change in physical function. Twenty-two subjects from physical therapy unit were assessed by two physical therapists. The ratings were compared by Spearman's rank correlation The correlation between two raters ranged from 0.85 to 0.98. Inter-rater reliability coefficient for total scores ranged from 0.95 to 0.97. This study confirms that the Chedoke-McMaster Stroke Assessment yields reliable results.

  • The proportion of the aged people in Korea has increased rapidly by more than 7% in 2000. A survey says that the changing rate from the aging society to the aged is the highest among the OECD countries. The prevalence rate of the aged population is on the increase. Chronic diseases such as arthritis and strokes limits their body's functions and activities of daily living. It is necessary that functional training. ADL training and continuous treatment be provided by Physical or Occupational Therapists. Discontinuance of treatment can cause the aged people to be in a bed state. The aged people need a variety of home care services at their convenience. Occupational and Physical therapist should play a major role in providing the rehabilitation-base home care services. Further they are expected to contribute to the extension of health care services such as home care and nursing home programs.

Analysis of the Present Status in Taegu and Kyung-Pook Physical Therapists (대구.경북지역의 취업 물리치료사 현황분석)

  • The following analysis is conducted to the 108 employed physical therapist males 46, female 62 in Taegu and Kyung-Pook for the purpose of research on their present status from August in to September 17 in 1994. The percentage of the female polled is 57.4 % and the age of 21 to 30 is 79.7 %. The number of people who have worked over five years in general hospital is 8(7.4 %) and in local clinic 5(4.6 %). This report is researched in to the proportion of the males to females, age, career period, job classification, and salary range.
