Effects of an Integrated Case Management Program on Medication Adherence, Pain, Physical Function and Depression among Korean Medical Aid Beneficiaries with Osteoarthritis (의료급여 관절염환자의 복약순응향상을 위한 통합중재프로그램의 효과)

  • Purpose: The purpose of this study was to test the effectiveness of an eight session integrated case management program for improvement of medication adherence, physical function, pain, and depression among medical aid beneficiaries with osteoarthritis. Method: A nonequivalent control group pre-posttest design was employed. Participants were 55 medical aid beneficiaries who agreed to participate in this study, and were assigned to an experimental group (n=28) or control group (n=27). The framework of this research derived from Cox's Interaction Model of Client Health Behavior guided the overall intervention and the components. The program led by a case manager with a medication calendar, motivating interviewing and coaching strategies and collaboration with a pharmacist. Analysis included change in scores, ${\chi}^2$-test, and t-test. Result: The results showed significant increase in medication adherence, physical function and decrease joint pain, joint stiffness and depression in the experimental group compared to the control group. Conclusion: The eight session integrated case management program indicated an effect on medication adherence, pain, physical function, and depression. Partnership with a pharmacist is recommended for medication adherence and conduct of further studies will be needed in order to determine the long-term effect of an extended integrated program on health outcomes.

A Study on the Health Changes of Students in Long-Term Online Classes due to COVID-19

  • The COVID-19 pandemic has had a significant impact on the educational landscape for students across the globe, leading to a shift towards long-term online learning. This study aims to examine the changes in the health status of college and university students before and after the transition to online classes. We conducted a survey questionnaire among 200 students enrolled at K University in Gangwon-do, including participants from both the Department of Visual Optics and the Department of Physical Therapy. The survey employed a 5-point Likert scale to evaluate a range of health-related factors, including physical and mental well-being, alterations in lifestyle, and academic performance. Both male and female students experienced a decline in physical strength and exercise during the online class period, while mental health and overall happiness showed improvement, particularly among female students. Notable shifts in lifestyle emerged, including an increased usage of electronic devices and enhanced familial connections. The study also shed light on intriguing trends related to academic accomplishments and adherence to official quarantine guidelines. In sum, the findings of this study offer valuable foundational information for the maintenance of students' well-being during online learning, as well as the development of effective strategies for online education in future academic settings.

Development and Application of a Health Promotion Program for Marriage Immigrant Women (여성결혼이민자를 위한 건강증진 프로그램 개발 및 적용)

  • Purpose: This study was to develop a health promotion program for marriage immigrant women and to evaluate its efficacy. Methods: The health promotion program was comprised of eight 100-minute weekly sessions. Each session included understanding of health responsibility, nutrition, physical activity, interpersonal relations, stress management, and self actualization. The research was conducted under the principles of nonequivalent control group pretest-post test design. The outcome variables were health promoting behavior, health status, acculturation, self efficacy, and perceived barrier. The participants were 15 immigrant women in the experimental group and 16 in the control group. Data was analyzed using ${\chi}^2$ test, Fisher's exact test, Mann Whitney U-test and Wilcoxon signed rank test. Results: There was a significant improvement in health promoting behaviors (U=-3.08, p=.002), left shoulder flexibility (U=-3.02, p=.003), right shoulder flexibility (U=-3.02, p=.003), low back flexibility (U=-3.37, p=.001), social health status (U=-3.38 p=.001) and subjective health status (U=-2.17 p=.030) in the experimental group compared to the control group. Conclusion: The health promotion program for marriage immigrant women was an effective intervention for improving health promoting behavior, physical health status, social health, and subjective health status. Therefore, the developed health promotion program needs to be applied to married immigrant women in other kinds nursing care settings in future research.

  • Recent domestic findings indicate steep increase of geriatric diseases such as obesity, diabetes, and high blood pressure among children and adolescents. It is, therefore, necessary to establish a new system for constitution inspection that reflects domestic adolescents' health conditions by the introduction of feasible items and systematic methods in the inspect ion. The objective of this study is to grasp the problems and to present proper ways of improving the system. As the method of the research during the period from September 2001 to March 2002 we refer to the domestic and foreign literature, and ask the advice of the specialists in pediatrics, family medicine, endocrine, cardiology. As on-site research we us e survey of nationwide elementary, middle, and high schools, visit representative local schools and have an interview with school nurses. Through the interview we try to find practical problems of school heath care and find proper method of students physic al examination. The abstract of the problems of student examination system is as following. To the question about the extent of the result of physical examination 46. 46.4%(school doctor) and 42.6%(parents and students) of the respondents answered "don't know". It means neglectfulness about examination itself. 62% of the school nurses responded "unsatisfactory" people who take charge of practical affairs have complaint about pre sent examination method. Because of the short time of the examination, and numerous students, the examination was incomplete. Several questions, method of physical examination, content of physical examination and list of physical examination, were raised. Reform measures for physical examination system of students were presented by physician, school nurses, students and parents of student.

Evaluation of the Home-based Rehabilitation Program Based on CBR Model Through In-depth Interviews With Visiting Physical Therapists

  • Purpose: This study was conducted in order to explore home visiting therapists' self-perceived role, problems, and supplement points of the home-based rehabilitation (HR) program based on the community-based rehabilitation (CBR) model. Methods: Four home visiting physical therapists, who conducted the HR program, participated in this study. After completion of the HR program, in-depth interviews were conducted using a semi-structured questionnaire for participants to explore their self-perceived role, problems, and supplement points of the HR program. Results: Participants regarded the role of the HR program as for "the linkage between recipient & society", "maintenance & improvement of recipients' physical function", and "education of recipient about the way of self rehabilitation". The problems and supplement points were derived from all phases of the HR program, including "human & material resources", "training program for human resources for HR service", "selection of recipient of HR service", "contents of the intervention for HR service", and "duration of HR service". Conclusion: These findings indicate that participants well recognized the intention of the HR program based on the CBR model, and suggest that high-quality human resources with rich expertise and experiences, a training program for HR service led by experts, and selection of recipients led by experts are necessary for an effective HR program. In addition, strategies and capacities specified to HR service should be identified, and applied to the training program for human resources for HR service. The results of this study could provide useful information when the government decides on the next guideline for home visiting health service.

A Comparison of Physical Health, Anxiety and Depression between the Dan-Jeon Breathing Trained group and non-trained group (단전호흡 수련인과 비수련인의 신체건강, 불안, 우울정도 비교)

  • The purpose of this study is to compare the physical health, anxiety and depression of the Dan-Jeon Breathing trained group with those of a non-trained group and to analyze the relationship between the training period of Dan-Jeon Breathing method and physical health, anxiety, and depression. The number of people in the Dan-Jeon Breathing trained group over 3 months was 149 and in the non-trained group 142. Data were collected from August to October 1999. As tools, 35 physical symptom questions were chosen from The Cornell Medical Index modified by Nam(1965) and from the Symptom Checklist-90-Revision(SCL-90-R) by Kim(1984); 10 questions on anxiety and 13 questions on depression were used in the study. The statistics of the study were gathered by using SPSS Window; the analysis was made by applying $X^2$-test, t-test, Pearson correlation, ANOVA and Scheffe tests. The results were as follows. 1. The Dan-Jeon Breathing trained group had lower physical symptom score and showed less anxiety and depression than the non-trained group, which supported the 1st, 2nd, 3rd hypotheses. 2. The longer the training period of Dan-Jeon Breathing, the lower the physical symptom score, which supports the 4th hypothesis, but the 5th and, 6th hypotheses were not supported because anxiety and depression did not get lower as the training period of Dan-Jeon Breathing got longer. 3. The physical symptom score, anxiety and depression that were made by the general characteristics of Dan-Jeon Breathing trained group were analyzed. As a result, the physical symptom score of women was greater than that of men. Physical symptoms score and the rate of anxiety and depression were different by education levels. The degree of anxiety was different by age. Those who have a religion have a higher physical symtoms score than people who have no religion. There was no difference in physical health score, anxiety and depression according to marital status, economic status and occupations. The results suggest that the physical and mental health status of Dan-Jeon Breathing trained group is better than that of the non-trained group. Physical symptoms scores gets lower as the training period of Dan-Jeon Breathing gets longer which results in the improvement of physical health status.

Need of Dormitory Environment Improvement and School-life Stress in the Special-purpose high school according to Residential Satisfaction in Dormitory and Housing (기숙사와 자가의 주거환경 만족도에 따른 특목고의 기숙사 환경개선 요구도 및 학교생활 스트레스)

  • The purpose of this study is to investigate the influence the need for improvements to dormitory environments and to study the stress from school-life according to residential satisfaction in dormitories and housing. For the purpose of this study, a survey was conducted including a questionnaire consisting of a residential satisfaction scale, a need of dormitory environment improvement scale, and a school-life stress scale. The subjects were 453 special purpose students in Daegu and Gyeongsangbuk-do who all live in a dormitory. Factor analysis, Cronbach' $\alpha$ and MANOVA were employed for data analysis, and Scheffe test was used for post-hoc analysis. First, the lower group the satisfaction of a physical residential environment in a dormitory, the more likely it is that the dormitory environment need improvement. The result of this study suggests that dormitory students are much more sensitive to conditions relating to physical residential satisfaction than to sociopsychological residential satisfaction. Also, the residents believe that change to these physical residential conditions facilitate an improvement to sociopsychological residential satisfaction. Moreover, when the satisfaction of a physical residential environment is lower group, more perceptive stress occurs from academic, health and personal factors while a lower satisfaction for the sociopsychological residential environment results in a more perceptive stress from the environment. The result of this study suggests that dormitory conditions indicate the necessity of improving the environment to adapt well to school-life, thus reducing stress. Also when simply providing a supplementary physical residential environment the sociopsychological residential environment should be considered, as this can cause conflicts between roommates. Second, the lower group the satisfaction for a physical residential environment in housing, the more likely it is that the dormitory environment need improvement. The result of this study suggests that for environments of inadequate housing there is a strong urge towards needing more independence and a better living environment. Third, when group feel there is a more substantial difference in the physical residential environment between dormitories and housing, it is more likely it is that the dormitory environment need to be improved. Also, when they feel there is more substantial difference in the sociopsychological residential environment between dormitories and housing, a more perceptive stress is detected from the environment. The result of this study suggests that dormitory students want to have the same level of conditions as those provided in housing. Also, dormitory students want to need more physical environment improvement than sociopsychological environment improvement.

Effects of Backward Walking Training with Task Orientation on the Functional Gait of Children with Spastic Hemiplegia

  • Purpose: This study examined the effects of backward walking training with task orientation on the functional walking ability of children with cerebral palsy. Methods: This study was a single-blinded, randomized controlled trial with a crossover design conducted at a single rehabilitation facility with cross-over to the other intervention arm following a two-week break. For a total of 12 children with spastic hemiplegia cerebral palsy, the forward walking training group (n=6) underwent training three times a week for three weeks, 40 minutes a day, and the backward walking training group (n=6) was also trained under the same conditions. To identify the functional walking ability, variables, such as the walking speed, stride length, and step length, were measured using a walk analyzer (OptoGait, Microgate S.r.l, Italy). Results: Both groups showed significant increases in walking speed, stride length, and step length (p<0.01). The backward walking group showed more significant improvement in the walking speed from pre- to post-test (p<0.05). The gait characteristics were similar in the two groups (stride length and step length) but the walking speed in the backward walking group showed a mean difference between the positive effects higher than the forward walking group. Conclusion: Task-oriented backward walking training, which was conducted on the ground, may be a more effective treatment approach for improving the walking functions of spastic hemiplegia children than forward walk training.

Improvement of Upper Extremity Function and Leisure Satisfaction of Children with Brain Lesions through Sports Stacking Activities: A Case Study

  • Purpose: This study aimed to investigate changes in upper extremity joints and leisure satisfaction in children with brain lesions through sports stacking activities. Methods: A sports stacking program was conducted on three children with brain lesions who had upper extremity joint limitations and joint range of motion lower than the normal range. It was conducted 10 times, 1 to 2 times a week, 40 minutes each time. Upper extremity joint angles were measured using a goniometer in the order of shoulder, elbow, wrist, and fingers, and leisure satisfaction was measured using a smile evaluation. Results: As a result of measuring the upper extremity joint angles, all three children showed slight angle changes in the shoulder, elbow, and wrist areas. Differences in joint angles appeared differently for each child. Smile evaluation results were evaluated in various psychological, educational, and physical aspects. Only child A was evaluated for Smile Evaluation No. 1. Conclusion: Sports stacking activities changed the upper extremity function of children with brain lesion disorders and showed differences in psychological, physical, and educational aspects of leisure satisfaction. As this is a short-term study result, the change in upper extremity function is minimal, but if sports stacking activities are continued, it will be a rehabilitation program that can prevent upper extremity dysfunction and improve physical strength. Accordingly, continuous attention should be paid to increasing accessibility and enjoyment of daily life according to individual characteristics and level.

The Effects of Quadriceps Setting Exercise and Electrical Stimulation on Improvement of Quadriceps Muscle Strength (대퇴사두근 등척성 운동(QSE)과 전기자극(ES)이 대퇴사두근의 근력 증진에 미치는 효과)

  • The purpose of this study was to analyze the effect of quadriceps setting exercise(N= 10) and electrical stimulation(N= 10) on improvement of quadriceps muscle strength. The experiment were performed for 6weeks. The subject of this study was 20 college students(male) who had not any medical problems. The quadriceps muscle strength was evaluated by make use of the KIN COM(Isokinetic dynamometer). The electrical stimulator was used to stimulate the quadriceps muscle. The results were as follows: 1. The mean increment ratio was 29.6% for concentric muscle strength and 36.4% for eccentric muscle strength after quadriceps setting exercise(p<0.05). 2. The mean increment ratio was 21.2% for concentric muscle strength and 24.3% for eccentric muscle strength after electrical stimulation, but did not significantly higher than pre-electrical stimulation In this study, we have found that quadriceps setting exercise on left side affect the improvement of the muscle strength in college students(male). However, electrical stimulation did not affect on the muscle strength.
