멀리건 테이핑과 플로싱 밴드가 만성 발목 불안정성 환자의 한 다리 점프와 Q-각에 미치는 효과 (Effect of Mulligan Taping and Flossing Bands on Q-angle and Functional Movement in with Chronic Ankle Instability)

  • Background: This study investigated the effects of a single-leg hopping test and Q-angle when applying Mulligan taping, flossing bands, and a combination of Mulligan taping and flossing bands to chronic ankle instability (CAI) patients. Methods: The subjects of the study were 68 patients with chronic ankle instability, randomly divided into three groups receiving the following treatments: Mulligan taping (n=22), flossing bands (n=23), and a combination of Mulligan taping and flossing bands (n=23). The immediate effects of the treatments were evaluated using the single-leg hopping test and Q-angle measurement for functional movement before and after the intervention. Results: All three groups showed significant improvements in the single-leg hopping test after the intervention, and there was a significant difference between the performance of the simultaneous application group and the Mulligan taping group (p<.01). As a result of the post-hoc test, there was a significant difference in the simultaneous application group than in the mulligan taping group (p<.01). When measuring the Q-angle change during the activity of descending stairs, there was also a significant effect in all three groups, with no significant differences when comparing the changes between groups (p>.05). Conclusions: In conclusion, Mulligan taping and flossing bands were confirmed as effective interventions for the functional movement of the lower extremities in patients with CAI. This study can provide basic data on the effectiveness of interventions in patients with CAI.

편마비 환자의 환측 기능회복을 위한 연구 (The study of paralytic side functional recovery on hemiplegia)

  • This study was conducted to assess the changes in the functional levels of affected upper extremities after treating hemiplegic patients by applying constraint-induced movement therapy(CIMT). The subjects were selected from 20 hemiplegic patients with scores of 25 or more in Mini Mental State Examination(MMSE-k), transferred to the departments of physical therapy in two university hospitals in Busan from December, 2001 to march, 2002, and were divided into two groups. Eleven subjects with fixing unaffected arms by CIMT were assigned to the experimental patient group and the other 9 patients to control group without fixing unaffected arms. The function of upper arms for both groups were evaluated by using Actual Amount of Use Test(AAUT) and Motor Active Log(MAL) before and after physical therapy. The malts were as follows: The recovery rates of upper extremity by AOU(Activity of Use) and QOM(Quality of Movement) were 23.9% and 27.3% for CIMT treated group, and by 8.3% and 4.6% for the control group on the average, respectively, in AAUT after physical therapy, showing statistically significant differences between two groups. And in MAL, the average recovery rates were 27.3% by AOU and 22.6% by QOM for CIMT treated group while 3.1% by both AOU and QOM for the control group, and were significantly different between twogroups.

고령 여성의 신체활동량과 심박동수 변시성(HRV) 및 체력의 관계 (Effects of Physical Activity on the Heart Rate Variability and Fitness in Older Female Adults)

  • PURPOSE: This study examined the effectiveness of an active lifestyle on the HRV and fitness of the older female adults. METHODS: Eighty female older adults (72.68±3.78 years) were sampled from a senior wellness institute. Accelerometers were distributed to each participant to have the device attached to them for one week, and the physical activity data (Energy expenditure in Kcal) of each participant were analyzed. Participants were divided into 3 groups (<1,000 Kcal vs 1,000 to 1,500 Kcal vs ≥1,500 Kcal) based on the amount of weekly physical activity. Before and after data analysis, the HRV was measured at resting state and the Senior Fitness Test was performed. RESULTS: One way ANOVA showed a significant difference in the HRV between the three groups. Group A (≥ 1,500 Kcal) was superior in HF and HF/HF + LF whereas Group C (<1,000 Kcal) showed high level of LF, LF/HF, and LF/HF+LF. The same statistics revealed significant differences in fitness. Most of the fitness variables were superior in Group A and B compared to Group C. Significant correlations were observed between physical activity and both HRV and in fitness. CONCLUSION: Theses results suggest that an active lifestyle may be an effective intervention for the prevention of both cardiometabolic disorder and improvement of the fitness level in older female adults.

내리막 경사로 트레드밀 걷기 훈련이 흉추 뒤굽음증의 흉추각도와 흉추기립근 활성도에 미치는 영향 (Immediate Effects of the Downhill Treadmill Walking Exercise on Thoracic Angle and Thoracic Extensor Muscle Activity in Subjects With Thoracic Kyphosis)

  • Background: In previous studies, changes in postural alignment were found when the slope was changed during walking. Downhill walking straightens the trunk by shifting the line of gravity backward. Objects: This study investigated the effect of the downhill treadmill walking exercise (DTWE) on thoracic angle and thoracic erector spinae (TES) activation in subjects with thoracic kyphosis. Methods: A total of 20 subjects with thoracic kyphosis were recruited for this study. All the subjects performed the DTWE for 30 minutes. A surface EMG and 3D motion capture system were used to measure TES activation and thoracic angle before and after the DTWE. Paired t-tests were used to confirm the effect of the DTWE (p<.05). Results: Both the thoracic angle and TES activation had significantly increased after the DTWE compared to the baseline (p<.05). An increase in the thoracic angle indicates a decrease in kyphosis. Conclusion: The DTWE is effective for thoracic kyphosis patients as it decreases their kyphotic posture and increases the TES activation. Future longitudinal studies are required to investigate the long-term effects of the DTWE.

Associations among Physical Activity, Comorbidity, Functional Capacity, Peripheral Muscle Strength and Depression in Breast Cancer Survivors

  • Physical inactivity may an important outcome in the prognosis of breast cancer. Physical activity levels decrease significantly for breast cancer patients following the treatment and remain low after oncology treatment is completed. The aim of this study was to determine physical activity levels and to examine associations among physical activity, comorbidity, functional capacity, peripheral muscle strength, and psychosocial status in breast cancer patients. Forty breast cancer survivors were included in this study. Demographic and clinical characteristics were recorded. Physical activity was assessed with a short form of the International Physical Activity Questionnaire (IPAQ). We defined comorbid diseases according to the Charlson Comorbidity Index (CCI). Functional capacity was evaluated with a six-minute walk test (6MWT). Peripheral muscle strength was measured for quadriceps femoris muscle with a hand-held dynamometer. Psychosocial status was measured with the Hospital Anxiety Depression Scale (HADS). Forty percent of patients were inactive. The IPAQ total score was significantly related with quadriceps muscle strength (r=0.492; p<0.001) and HADS depression score (r=0.341; p<0.05). Marked correlations were also observed between IPAQ walking score and quadriceps muscle strength (r=0.449; p<0.001), HADS depression score (r=0.341; p<0.05), and CCI (r=-0.433; p<0.001). The CCI score was also markedly associated with quadriceps muscle strength (r=-0.413; p<0.001). 6MWT distance was not significantly correlated with any of the parameters. Regression analyses revealed that psychosocial status and peripheral muscle strength were significant predictors of physical activity estimated with the IPAQ short form and when combined, they explained 35% of the variance. Comorbidities, peripheral muscle strength and psychosocial status partially explain the variability of physical activity level in breast cancer survivors. These results suggest that physical inactivity contributes to worsening health in breast cancer survivors.

Effect of Hamstring Eccentric Exercise on Pain and Functional Activity in Patients with Chronic Low Back Pain

  • Objective: The aim of this study is to investigate the effect of hamstring eccentric exercise on pain and functional activity in patients with chronic low back pain. Design: Randomized controlled trial Methods: Participants comprised 27 subjects with back pain persisting for 3 months. They were randomly assigned to the experimental group (Eccentric exercise: n=13) and the control group (Stretching: n=14). The experimental group performed gastrocnemius, iliopsoas, and quadratus lumborum stretching along with hamstring eccentric exercise, while the control group engaged in the same stretches along with hamstring stretching. The eccentric exercise for the hamstrings was Nordic curl exercise. Each group practiced for 1 hour a day, 3 times a week, for 4 weeks. Pain, disability index, balance, and hamstring length were measured. Results: Significant differences were observed in the comparison of changes in back pain, back disability index, and hamstring length before and after exercise within each group (p<0.05). The balance ability of the experimental group showed a difference before and after exercise (p<0.05), whereas no significant difference in the control group. There were no significant differences in back pain and hamstring length between the groups. Changes in back disability index and balance ability significantly increased in the experimental group compared to the control group (p<0.05). Conclusion: Hamstring eccentric exercise using Nordic curls has a positive effect on back pain, back disability index, balance ability, and hamstring length changes.

Recreational Physical Activity and Risk of Ovarian Cancer: a Meta-analysis

  • Our aim was to access the association between recreational physical activity (RPA) and risk of ovarian cancer (OC). The studies were retrieved from the PubMed and Embase databases up to February 20th, 2014. Risk ratios (OR) and 95% confidence intervals (CI) were used to estimate effect sizes. Random-effects or fixed-effects models were used to pool the data. The trim and fill method was applied for sensitivity analysis. Begg's rank correlation test and Egger's regression asymmetry test were employed to assess the publication bias. A total of 6 studies (435398 participants including 2983 OC patients) were included in this meta-analysis. The overall estimate indicated that there was weakly inverse association between RPA and OC risk (RR=0.90, 95%CI: 0.72-1.12, p=0.335). Meanwhile, for prospective cohort studies, a result consistent with the overall estimate was obtained (RR=1.12, 95% CI: 0.88-1.42, p=0.356). However, for case control studies, the pooled estimate of RR was 0.76 (95%CI: 0.64-0.90, p=0.002), indicating a clear significant association between RPA and OC risk. In addition, the sensitivity analysis indicated a significant link between RPA and risk of OC after removing Lahmann's study (RR=0.80, 95% CI: 0.68-0.93, p=0.004). No significant publication bias was found (Begg's test: p=1.00; Egger's test: p=0.817). In conclusion, our meta-analysis indicated a weakly inverse relationship between RPA and the occurrence of OC.

구선 활인심법이 여성 암 환자의 삶의 질과 불안 및 우울에 미치는 영향 (Coo-Sun activity a man of mind rule and its effect on the quality of life of female cancer patients suffering from anxiety and depression)

  • Purpose: This study aimed to investigate a clinical effect of Coo-Sun activity focusing on improvement of quality of life in female cancer patients. Methods: The subject of study consisted of 5 female cancer patients at the East-West Cancer Center of OO Oriental hospital. The survey methods of this study are Functional Assessment of Cancer Therapy-General (FACT-G) and State-Trait Anxiety Inventory (STAI). Patients performed Coo-Sun activity 6 times a week and filled out FACT-G and STAI questionnaire after every 3 times training. Results: In FACT-G questionnaire, physical well-being and emotional wellbeing was improved significantly after Coo-Sun activity. And change of social/family well-being and functional well-being score were not significant statistically. In STAI questionnaire, STAI1 was improved significantly after Coo-Sun activity. And change of STAI2 score was not significant statistically. Conclusion: After Coo-Sun activity, quality of life (QOL) in female cancer patients was improved and anxiety was decreased. But this study have limitation to prove effects of Coo-Sun activity completely in female cancer patients. So well designed prospective cohort study will be needed to prove its benefits.

척추보조기 착용과 일어서기 운동전략이 지면반발력과 하지근활성도에 미치는 영향 (Influence of Spine Orthosis and Sit-to-Stand Motor Strategies on Ground Reaction Force and Lower Extremity Muscle Activity)

  • The purpose of this study was to assess the influence of spine orthosis and sit-to-stand motor strategies on ground reaction force (GRF) and lower extremity muscle activity. Twenty healthy adult men participated, and subjects randomly performed sit-to-stand motions in three different conditions: Momentum-transfer strategy (MTS); MTS with spine orthosis; and zero-momentum strategy (ZMS) with spine orthosis. GRF data, onset time, and muscle activity were determined and compared using force plate and electromyography. Data were statistically analyzed by the SPSS version 13.0. One-way repeated analysis of variance (ANOVA) was used to determine the statistical significance, and least significant difference was used as a post hoc test. The level of significance was .05. The results of this study were as follows: 1. Peak GRF and relative time to peak GRF were not significantly different in the three different conditions (p>.05). 2. Onset time of four muscles, tibialis anterior, gastrocnemius, biceps femoris and rectus femoris, in the three different conditions were significantly different (p<.05). 3. The tibialis anterior and rectus femoris muscle activity before hip-off and tibialis anterior, gastrocnemius, and rectus fermoris muscle activity after hip-off were significantly different in the three different conditions (p<.05).

현수보조장치를 이용한 과제 지향적 훈련이 경직형 양하지 뇌성마비아동의 체간 안정성과 대동작기능에 미치는 영향 (The Effects of Task Oriented Training with Suspension Device on Trunk Stability and Gross Motor Function of Children with Spastic Diplegia Cerebral Palsy)

  • PURPOSE: The purpose of this study was to examine the effect of using a suspension device for arm reaching activity on trunk stability and gross motor function of children with spastic diplegia cerebral palsy. METHODS: The subject in this study consisted of 11, GMFCS(Gross Motor Function Classification System) III~IV children with spastic diplegia cerebral palsy, all of whom agreed to participate in the study. All subjects were divided into two groups: the experimental group using a suspension device, and the control group using no suspension device. For each group, a thirty-minute intervention was done twice per week during 8 weeks. Before and after intervention, each test was measured using TIS(Trunk Impairment Scale), GMFM (Gross Motor Function Measure) and PRT(Pediatric Reaching Test) to change trunk stability, gross motor function and arm reaching activity. The data were analyzed with the Wilcoxon signed rank test. RESULT: All two groups had a meaningful increase in GMFM-Sit data measured before and after intervention. The experimental group had a significant increase from an average of 78.83 to an average of 84.83 in GMFM-Crawling. For both groups, there was a substantial increase in the change in sitting position and arm reaching. CONCLUSION: According to the results of this study, the arm reaching activity using suspension device had an effect on trunk stability and gross motor function and it changed arm reaching activity.
