국내 노인들의 신체적 활동 유형별 참여와 건강 관련 요인 연구 (A Study on the Determinants among Types of the Korean Seniors' Physical Activity and Related Health)

  • 목적 : 본 연구에서는 노인들의 신체적 활동을 일 관련 신체활동, 운동 및 여가 신체활동, 이동 관련 신체활동, 앉아서 하는 활동으로 나누어 신체적 활동 유형 별 참여 여부가 노인들의 건강 관련 삶의 질과 정신건강에 어떤 관계가 있는지 알아보았다. 연구방법 : 본 연구는 2016년에 수행된 제7기 1차년도 국민건강영양조사(The 7th Korea National Health and Nutrition Examination Survey KNHANES VII-1)에서 건강 설문조사에 응한 65세 이상 978명을 대상으로 수행되었다. 신체활동유형, 건강관련 삶의 질(EQ-5D), 우울지수(PHQ-9), 스트레스 인지수준, 수면시간 등의 관계성을 복합표본설계선형회귀분석으로 알아보았다. 결과 : 신체활동은 건강 관련 삶의 질과 정신건강에 있어, 강도 높은 일이나 가사활동에 참여할 경우는 부정적인, 중강도 이상의 여가에 참여하는 경우 긍정적인 영향을 주고 있음을 알 수 있었다. 앉아서 하는 활동의 시간이 많을수록 건강 관련 삶의 질에 부정적 영향을 준다는 다른 연구 결과와는 다르게 노인들의 앉아서 하는 활동은 건강 관련 삶의 질과 정신건강에 긍정적 영향을 주었다. 결론 : 연구 결과, 우리나라 노인은 여가 관련 신체활동과 앉아서 하는 활동은 건강 관련 삶의 질에 긍정적이며, 일과 관련된 신체활동은 삶의 질을 낮추는 부정적 요인으로 작용한 것으로 해석할 수 있었다. 이러한 국내 노인들을 대상으로 한 신체활동의 수준과 건강 관련 결과를 바탕으로 노인 대상의 의미·목적있는 작업중심 활동 프로그램을 수립할 때 참고하도록 해야 할 것이다.

다양한 다리 꼬기 자세가 요통환자의 체간근 활성도에 미치는 영향 (The Change of Muscle Activities of Trunk Muscles during Various Leg-Crossing Positions in Low Back Pain Patients)

  • Purpose: Leg-crossing sitting is very common for men and woman. No solid evidence exists for either a beneficial or a detrimental effect of this posture. This study investigated the change of activities of trunk muscles between the normal group and the low back pain group during various leg-crossing positions. Methods: The subjects were consisted of 10 subjects who don't have low back pain and 10 subjects who have low back pain. In this study, we used electromyography(EMG) to evaluate the activities of both the trunk muscles (rectus abdominis, external oblique, internal oblique, and multifidus) during various leg-crossing positions (up-right, leg-crossing, tailor-crossing, and ankle-crossing). We analyzed the data by using repeated one way ANOVA. Results: In normal group, there were increased in EMG activities of trunk muscles, but no significant differences during leg-crossing positions. In back pain group, there were increased in EMG activities of right external oblique, left. internal oblique, and both multifidus muscles in leg-crossing and tailor-crossing position, but no significant differences during leg-crossing positions. There was no significant difference of muscle activity of trunk muscles between the back pain group and the normal group. Conclusion: We suggest that low back pain people who have weak muscles of rectus abdominis, external and internal oblique are often experienced in leg-crossing posture than normal. To compensate this unstability of trunk, leg-crossing posture is substituted passive structure for activities of active muscle.

인지능력과 상지기능이 만성 뇌졸중 환자의 일상생활동작에 미치는 영향 (Cognition and Upper-extremity Function Influence on Performance of Activities of Daily Living in Patients with Chronic Stroke)

  • PURPOSE: This study compared the scores of the Mini-Mental State Examination-Korean version (MMSE-K) and Korean version Montreal Cognitive Assessment (K-MOCA), which assess cognitive function, the Fugl- Meyer Upper-Extremity Assessment (FMA), which assess the upper extremity function; and Modified Barthel Index (MBI), which that assesses activities of daily living among patients receiving inpatient treatment following a diagnosis of stroke to determine the correlations among MMSE-K, K-MOCA, FMA, and MBI. METHODS: The study assessed the cognitive function using the MMSE-K and K-MOCA, upper extremity function using FMA, and activities of daily living using MBI. The data were analyzed using Pearson's correlation analysis and multiple regression analysis. RESULTS: An analysis of the differences in the MMSE-K, K-MOCA, MBI, and FMA scores according to the affected side, revealed differences in the, K-MOCA scores according to the affected side, where patients with right hemiplegia showed better cognitive function (p <.05). Correlation analysis among MMSE-K, K-MOCA, FMA, and MBI Showed significant correlations (p <.05). The results indicate that those with higher cognitive and upper extremity functions had higher performance of activities of daily living. CONCLUSION: The cognitive and upper extremity functions were correlated with the activities of daily living in stroke patients. Accordingly, applying physical therapy with a focus on improving the cognitive function and training activities of daily living could assist in the functional recovery of stroke patients significantly.

이중과제운동이 뇌졸중 환자의 호흡과 균형 및 일상생활활동에 미치는 영향 (Effects of Dual-Task Exercise on Breathing, Balance, and Activity of Daily Living in Stroke Patients)

  • PURPOSE: This study aimed to provide basic clinical data by investigating the impact of Activities of daily living-related dual-task intervention on lung function, balance, and Activities of daily living of stroke patients. METHODS: After sampling 40 stroke patients who met the selection criteria, this study randomly assigned 20 patients who received dual-task exercise intervention to the experimental group and 20 patients who received single exercise intervention to the control group by drawing lots. Next, the study pre-tested their lung function, balance, and activity of daily living. All interventions were conducted for 30 minutes, 3 times a week for 4 weeks, and when all interventions were completed after 4 weeks, lung function, balance, and activity of daily living were re-measured in the same way as the pre-test. RESULTS: In comparing changes in lung function, balance, and activity of daily living within each of the experimental and control groups, statistically significant improvement were found in the experimental group only (p < .01). Statistically significant improvement were also found in lung function, balance, and activities of daily living between the groups (p < .05) (p < .01). CONCLUSION: A statistically significant improvements were found in lung function, balance, and activities of daily living only in the experimental group and statistically significant differences were found between groups. Because they take arm exercises by maintaining balance in a standing position on a labile surface and through dual-task exercise such as folding a towel, moving a cup, and throwing and catching a ball, muscles related to lung function were stimulated and lung function and balance were improved. This helped activities of daily living to be improved. Thus, it is considered that dual-task exercise should be utilized for stroke patients' smooth everyday life.

가정방문물리치료가 재가장애인의 일상생활동작과 기능에 미치는 영향 (Effects of Home Visiting Physical Therapy on Activites of Daily Living and Function in Disabled Persons Living at Home)

  • Purpose: The purpose of this study was to provide basic data for the effectiveness and the necessity of home visiting physical therapy through examining performance ability and functional changes in daily life activities by investigating the life of disabled persons living at home. Methods: After selecting 100 people with physical disabilities greater than level 3 and brain damage related disabilities living in Youngam-Gun, we conducted mobility tests according to a Modified Bathel Index (MBI), the Berg Balance Scale (BBS), and floor and bed movements? for Activities of Daily Living evaluation. We did this before the experiment and after doing home visiting physical therapy for 90 minutes at a time, once a week for 6 months Results: 1) Persons who live in a bed are higher than those who live in a floor. There was a significant difference between persons who live in a floor and those who live in a bed in the change in MBI and BBS scores after home visiting physical therapy. 2) Those with a musculoskeletal disorder had a significantly different change in MBI and BBS according to the type of disease and kind of disability. Those with neuropathy had a significantly different change in MBI. Those with physical disabilities showed a significant difference in MBI and BBS. Conclusion: A home visiting physical therapist can cause an improved performance ability and lead to the ability of disabled persons living at home to carry out activities of daily livingin a large part and the study for brain lesion disabilities except physical disabilities and stroke which occupy large proportion of disabled persons should be done.

한국 일부 농촌지역 노인의 생활기능상태 결정요인의 구조적 관계 연구 (A Structural Relational Analysis of the Determinants of Functional Status in Korean Rural Elderly People)

  • This study was conducted to develop a theoretical model for the determinants of activities of daily living in Korean rural elderly people. The theoretical model was derived from Lawton's Ecological Model to explain human behavior. The model includes determinant variables for functional status such as age, sex, physical status, cognitive status, physical environment, social environment, and depression. Data for this study was collected from 189 elderly people in a rural district near Chonju City. The data was analyzed with SPSS PC/sup +/4.1 and LISREL 8. The analysis of the data showed that the first proposed theoretical model did not fit the collected data well. In order to increase the fit of the model to the data, the model was modified by deleting the exogenous variables of sex, physical environment, and social environment. The fit of the modified model to the data was increased and was move suitable. The results of this study show that the determinants of the activities of Korean rural elderly people were age, perceived physical condition, cognitive status, and depressional status. Age has a significant direct and indirect effect on the activities of daily living of rural elderly people. Perceived physical condition has a significant indirect effect through depression on activities of daily living. Cognitive status also has a significant indirect effect through depression on functional status. Depression has a significant direct effect on the functional status. Physical environment and social environment showed reversal relationships to functional status. In conclusion, the results of this study show that personal characteristic factors are more important than environmental characteristic factors for functional status of elderly people. However, the importance of depression to functional status in Korean rural elderly people and the reverse relationship between social environment and functional status, reflect Korean specificity. Korean nursing needs to focus more on this Korean specificity in order to develop nursing knowledge and practice appropriate to Korean people.

중심 안정성 운동을 적용한 교각운동 시 지지면 불안정성이 체간 및 하지의 근 활성도에 미치는 영향 (The Influence of Unstability of Supporting Surface on Trunk and Lower Extremity Muscle Activities During Bridging Exercise Combined With Core-Stabilization Exercise)

  • The aim of this study was to investigate the effect of supporting surface instability to trunk and lower extremity muscle activities during bridging exercise combined with core-stabilization exercise. Thirty young healthy adults (15 males and 15 females) voluntarily participated in this study. Each subject was asked to perform bridging exercise combined with core-stabilization exercise on three different supporting surfaces (stable ground surface, the wooden balancing board, and the air cushion). The muscle activities were measured using surface electromyography (EMG) during performing exercise. To test statistical significance, one-way ANOVA with repeated measures was used with the significance level of .05. The findings of this study are summarized as follows. (1) There were significant differences in muscle activities on internal oblique, external oblique, gluteus medius, semitendinosus, biceps femoris, medial gastrocnemius and lateral gastrocnemius during exercise (p<.05). (2) The biceps femoris and lateral gastrocnemius showed significantly higher muscle activity on the wooden balancing board rather than on the ground, and semitendinosus, biceps femoris, medial gastrocnemius and lateral gastrocnemius showed significantly higher muscle activity on the air cushion rather than on the ground (p<.05). Therefore, it is concluded that muscle activities in the trunk and the lower limbs during bridging exercise combined with core-stabilization exercise was affected with instability of supporting surface. Further researches are needed to investigate the long term effect of bridging exercise on muscle activity with patient group.

중고등학생의 정신건강과 신체활동과의 관련성 (Relations Between Mental Health and Physical Activities of Middle to High School Students)

  • Objective: The study analyzed degrees of participation in physical activities, demographic and sociological characteristics of middle and high school students, and the relations to mental health related factors, based on "Korea youth risk behavior web-based survey." Therefore, the study can be used as preliminary data for establishing mental health promotion plans for youth and setting contents and directions of health education. Methods: The study analyzed the data, collecting from 75,066 middle and high school students, regarding their demographic and sociological characteristics, mental health, and relations to physical activities by using SPSS Win 15.0. Results: 19.1% of the students answered they had seriously contemplated suicide during the past 12 months, and 43.2% said they are usually under enormous pressure. 55.3% answered that academic achievements and the pressure from future paths are major factors of their stress. The students showed noteworthy differences in whether they think about suicide depending on whether the students live with their father or mother. The study found that the students whose parents' levels of education are low, or their academic records are low, or economic conditions are poor are more likely to think about suicide. The students tend to think more about killing themselves if they spend more time sitting to study than being engaged in physical activities, or spend little time walking. And they are more likely to think about committing suicide if they spend little time exercising, or are not engaged in physical activities on the playground or a gym in PE classes.

상심자(Morus alba)의 운동능력 향상과 스트레스 개선효과 (Anti-stress and Promoting Effect of the Fruit of Morus alba)

  • 상심자 추출물이 운동에 의한 체내 monoamine oxidase (MAO) 활성 변화에 미치는 영향을 연구하여. 상심자 추출물을 경구투여(0.3g/kg body weight) 한 흰쥐의 뇌와 간에서 MAO-A와 MAO-B의 활성에 중요한 영향을 미치는 사실을 확인하였다. 각 효소활성은 serotonin과 benzylamine을 기질로 이용하여 측정하였다. 운동 전 후 운동의 유형에 따라 효소활성의 변화경향이 서로 다른 경향을 나타내었다. 뇌에서 측정한 MAO-A 활성은 운동에 의해 효소활성이 현저히 감소하였으며 반면, 간에서 측정한 MAO-B의 활성은 운동이 끝나고 60분이 경과할 때까지 증가된 상태를 유지하고 있었다. 운동 시 체내 변화의 지표효소인 혈중 LDH의 활성 변화와 혈중 lactate의 농도변화를 함께 관찰함으로서 MAO 활성과의 상관관계를 비교하였다. 상심자 추출물을 경구투여 하고 운동을 한 동물의 MAO-A 활성은 증가하였고 MAO-B, LDH 활성과 lactate level은 감소하는 것을 확인하였다. 결과적으로 모은 지표들이 운동 전의 정상상태로 회복되는 것으로 확인되었다. 이 연구의 결과들로부터 상심자 추출물이 운동 전후의 MAO 활성을 조절함으로써 운동능력을 향상시키고 피로를 회복하는 효능을 갖는 것으로 추정되며 이러한 기능성을 갖는 건강기능식품의 소재로 활용이 가능할 것으로 생각한다.

Changes of abdominal muscle activity according to trunk stabilization exercises using a Swiss ball

  • Objective: The purpose of this study was to compare the activities of the abdominal muscles according to trunk stabilization exercises using Swiss ball in normal individuals. Design: Cross-sectional study. Methods: Ten healthy university students participated in this study. The subjects were required to complete the following three exercise positions: Exercise position 1, sitting on a Swiss ball and lifting the legs; Exercise position 2, pushing to a plank position from an ending position; and Exercise position 3, push-up posture with the legs on a Swiss ball. Changes in the trunk muscle activities were estimated using Biometric electromyography (EMG). Activities of the dominant side internal oblique muscle (IOM) and external oblique muscle (EOM) were estimated in all participants. The maximal voluntary isometric contraction (MVIC %) was measured to standardize the EMG signals for the IOM and EOM during maximum resistance when sitting up with each shoulder pointing towards the contralateral pelvis with knees bent and both arms crossed on the chest for 5 seconds. Results: There was a significant difference in the activity of the internal and external oblique muscles between Exercises 1 and 2 and Exercises 1 and 3 (p<0.05). Furthermore, the IOM/EOM activity ratio was the greatest during Exercise 3 and the smallest during Exercise 1. IOM and EOM activities were the greatest during Exercise 2 with greater EOM activity. Conclusions: In future studies, it will be necessary to investigate muscle activities by supplementing the above-mentioned limitations during the stabilization exercise. The results of this study may be used as a basis for controlling the intensity and frequency of exercise while prescribing trunk stabilization exercises.
