Three cases of atypically presented group A streptococcal infections (전형적인 전구 증상 없이 발현된 A군 연구균 감염증 3례)

  • Streptococcus pyogenes, which is classified to Group A streptococcus (GAS), is one of the most common bacterial pathogens of the childhood infection. This organism can cause acute bacterial pharyngitis, impetigo, peritonsilar abscess or scarlet fever. It can also cause severe invasive diseases such as toxic shock syndrome, sepsis, septic arthritis, necrotizing pneumonia or necrotizing fasciitis. Usually, invasive GAS infections are accompanied by systemic symptoms and signs. Necrotizing pneumonia presents with acute fever, pleuritic chest pain and cough. The progress of disease is usually rapid and typically, pleural effusion develops in the early course of disease. Necrotizing fasciitis is relatively rare but once it has developed, it may be life threatening and cause necrosis of adjacent soft tissues with rapid progress. Clinical manifestations of parapharyngeal abscess are fever, dysphagia or bulging of pharyngeal wall. We experienced three cases of GAS infections which were presented atypically.

The Effect of Proprioceptive Neuromuscular Facilitation(PNF) on Swallowing Function of the Stroke Patients (고유수용성신경근촉진법이 뇌졸중 환자의 연하기능에 미치는 효과)

  • The purpose of this study was to identify an effects of Proprioceptive Neuromuscular Facilitation(PNF) on swallowing function of the stroke patients. For this purpose, subjects were 9 experimental group and 9 control group of the stoke patients. The experimental group has been proceeded total 12 times based on 3 times per 30~50 minutes at a supplied place in the PNF program. The results of this study were as follows. First, the dysphagia scale of the experimental group that was provided with PNF, had a considerable statistic difference. Secondly, the oral pharyngeal functions of the experimental group that was provided with PNF, had a considerable statistic difference. Thirdly, the swallowing speed of the experimental group that was provided with PNF, had a statistic difference in the swallowing speed reduced of the first set, in total. Therefore, PNF is informed to have a effect in advancing the swallowing function of the stroke patients. And in the future, it is considered that researches that it develops the motor program in which the various techniques are combined and it can contribute to the swallowing functions for patients of stroke.

A Study of Environmental Factors and Self-efficacy in Female College Student Smokers (여대생 흡연과 환경적 요인 및 자기효능감에 대한 연구)

  • For the past 10 years, the number of male smokers has dwindled. On the other hand, the number of female smokers has increased abruptly. Cigarette smoking is more hazardous to a woman than to a man because it can affect her health and her fetus during pregnancy. Many studies show that cigarette smoking is one of the most important risk factors determining the mortality and the morbidity of various kinds of diseases such as lung cancer and pharyngeal cancer. But, it is also known to be one of the risk factors we can handle and prevent easily. The subjects of this study consisted of 263 female nursing students in Seoul and Chonbuk Province. Using questionnaires, we were collected data from December 1 to December 10 1998. The questionnaire included 24 questions related to environmental factors and 14 questions related with self-efficacy. The statistical analysis was done with SPSS ver. 8.0(SPSS Inc. U.S.). The results of this study are as follows. 1) Cigarette smoking in women was influenced by her family or friends. That is to say, the higher number of her friends or family members who smoked cigarettes, the more likely a woman was to become a smoker. Economic status and knowledge level about the risks of cigarette smoking were also related to women smoking. So, women with lower levels of economic status and less knowledge about the risks of cigarette smoking were the more likely to become smokers. 2) Woman with higher levels of self-efficacy were less likely to become smokers. 3) The factors influencing a woman's smoking were whether her friends smoked, whether her mother smoked and her self-efficacy in that order. Environmental factors, self-efficacy were found to be important factors influencing smoking in women. It is suggested a quit-smoking program for women college students, be based on consideration of these environmental factors and self-efficacy.

  • Matured adults of chum salmon were collected from Namdae-chun River, Yangyand-gun, Kangwon-do, Korea in december of 1989 and 1990. Artificial insemination was made in the field. Hatching and neasing were cassied out in laboratories of the Yangyand Fisheries Institute and Korea Ocean Research and Development Institute : Ossification of bones of alevin, fry and smolt were investigated : 1. The newly hatched alevins, 1.80 to 2.56mm in total length, were ossified partly on parasphenoid, maxillary, dentary and opercle bones. 2. The alevin stage, the visceral bones connected with feeding and respiratory function were ossified earlier than cranium and vertebra. At swim up stage, the fry attained 3 to 4cm in total length, most of bones except cranium were ossified. 3. Teeth on the jaw bones, palatine, vomer, upper and lower pharyngeal bones were increased conspicuously when the body length were ranged from 3 to 4cm. 4. Caudal fin of hatched larvae had already seventeen occified rays and developed earlier than any other fins.

Reconstruction of Hypopharynx and Cervical Esophagus : Choice of Flap (하인두 및 경부식도 결손의 재건 : 재건술의 선택)

  • Background and Objectives: Various flaps are using for reconstruction of hypopharyngeal and esophageal defect. However, complication and indication of each flap are not fully analyzed. Patient and Methods: Records of 52 hypopharyngeal cancer patients who had surgical treatment and 13 other head and neck cancer patients who underwent hypopharyngeal and/or esophageal reconstruction with flap were retrospectively analyzed. Eighty three percent(54 cases) of patients needed reconstruction other than primary pharyngeal closure. Five split thickness skin graft, 1 pectoralis major myocutaneous flap, 20 forearm free flap, 13 jejunal free flap, 15 gastric pull up were used. Result: Flap failure was noted in 2 cases who had subsequent gastric transposition. Wound dehiscence and fistula were most common problem of forearm free flap. Most fistulas were developed in patients with conduit type reconstruction of forearm flap while there wasn't any fistula in patient with patch type reconstruction. Stenosis of lower anastomosis was the frequent problem of jejunal transfer. Gastric pull-up has frequent com-plication of stomal stenosis. All but three patients had reached oral feeding postoperatively. Conclusion: Based on this study, forearm flap is effective in partial hypopharyngeal defect while jejunum is the choice for circumferential defect. Gastric pull-up is for combined esophageal defect.

Clinicopathologic Predictors and Impact of Distant Metastasis from Adenoid Cystic Carcinoma of the Head and Neck (두경부 전양낭성암종에서 원격전이와 관련된 임상적, 병리학적 예측 인자)

  • Background and Objectives: Adenoid cystic carcinoma (ACC) is a unique tumor characterized by frequent and delayed distant metastasis (DM) with uncommon regional lymph node metastasis. We evaluated the factors affecting DM of ACC and survival after appearance of DM. Materials and Methods: Medical records, radiographs and pathologic slides were reviewed for 94 patients from 1979 through 2001. Results: DM of ACC occurred in 46 patients, and developed more frequently in patients with tumors of the solid histologic subtype than in patients with tubular or cribriform subtypes. DM occurred less frequently in the sinonasal tract, and development of DM was not affected by tumor stage. Disease-specific 5- and 10-year survival rates were 88% and 72% for patients without DM, respectively and 76% and 48% for those with DM(p=0.02). Regarding the site of DM and its impact on outcomes, 30 patients had lung metastasis alone, 5 patients bone metastasis alone and 6 patients developed both lung and bone metastasis. Median survivals after appearance of DM among patients with isolated lung metastases and those with bone metastases with or without lung involvement were 54 and 21 months, respectively (p=0.04). Conclusions: Development of DM in ACC is predicted by solid histologic subtype, and major salivary gland or oral/pharyngeal rather than sinonasal primary site. Those patients with bone involvement with our without lung metastases had worse outcomes than those with pulmonary metastasis only.

Eagle's Syndrome Presenting Unilateral Facial Nerve Palsy: Case Report (편측 안면신경 마비를 동반한 Eagle 증후군 치험례)

  • Eagle's syndrome is the manifestation of elongated styloid process that is not a common entity in dental clinics. Without the knowledge of this syndrome misdiagnosis can be made and therefore differential diag-nosis with other diseases of oral and maxillofacial area is required. The symptoms of elongated styloid process are cervical pain, foreign body sensation, dysphagia and pharyngeal pain. A 27 years old male vis-ited our hospital. He had cervical pain with unilateral facial nerve palsy. Along with meticulous clinical examinations, 3D-CT, Plain radiographic studies were used to make the diagnosis. In computed tomogra-phy, Lt. styloid process was elongated and fractured. Patient have undergone surgical resection of elongat-ed styloid process and have been carefully observed. Characteristically, preoperative facial nerve palsy showed rapid postoperative recovery. The other clinical symptoms that were present in the initial visit were diminished. The purpose of this article is to report a case of Eagle's syndrome presenting unilateral facial nerve palsy that was treated with transoral surgical approach with literature review.

A CLINICAL STUDY ON PULMONARY FUNCTION AFTER INTERMAXILLARY FIXATION (악간고정이 호흡기능에 미치는 영향에 대한 임상적 연구)

  • Intermaxillary fixation is routine procedure to oral and maxillofacial area in jaw bone fracture, surgical correction of jaw deformity, osseus reconstruction of jaw. After transoral surgery, accompanied by intermaxillary fixation, dysphagia or airway obstruction may be followed due to blood clot, vomitus, or laryngeal spasm resulting from irritation by blood or secretions. Lingual or pharyngeal edema is other contributing factors of airway obstruction. In addition, intermaxillary fixation itself may cause obstruction of airway. In this study, pulmonary function test and arterial blood gas analysis were evaluated before and after intermaxillary fixation in 30 patients suffered from mandibular fractures. Comparative analysis was performed by estimated values. The results were as followed. 1. The spirometric values of FEV1, FEV1/FVC and FEF25-75% without intermaxillary fixation were reduced from 97.57%, 85.1%, 98.3,% to 71.7%, 66.5%, 61.2% with intermaxillary fixation, indicating the presence of obstructive pulmonary impairment. 2. Spirometric value of MVV, as the most influencing value of sensitive to extrapulmonary factors, was changed from 84.5% to 46.48%. 3. After intermaxillary fixation, the spirometric value of FVC, as indicator of restrictive pattern of pulmonary function, was not reduced significantly as measured from 94.47% to 89.97%. 4. $O_2$ saturation of arterial blood gas analysis without intermaxillary fixation was 97.86%. While intermaxillary fixation, $O_2$ saturation was 97.47%. The results indicate that careful airway management is mandatory undergoing intermaxillary fixation of various oral and maxillofacial surgery.

Update of Head and Neck Cancer Staging in the 8th Edition Cancer Staging Manual of the American Joint Committee on Cancer (두경부암 병기 설정의 최신 변화: AJCC 암 병기설정 매뉴얼8판)

  • The recently released the $8^{th}$ edition of the American Joint Committee on Cancer (AJCC) Staging Manual introduces significant modifications from the prior $7^{th}$ edition. In this paper, the contents of the new changes in the decision of cancer of the head and neck is summarized except changes in staging of skin and thyroid cancer. In addition to the 8th edition, 1) Addition of extracapsular involvement in metastatic lymph nodes (N category) 2) Oral cancer T classification change, 3) Staging of the pharyngeal cancer was divided into 3 chapters: high-risk human papilloma virus (HR-HPV) associated oropharyngeal cancer (OPC), non HR-HPV associated OPC and hypopharynx cancer (HPC), and nasopharynx cancer (NPC) 4) Changes in T and N classification in NPC, 5) In the case of cancer of unknown primary, P16-positive case is defined as HR-HPV related OPC, and EBV-positive case is defined as NPC. The process that led to these changes highlights the need to collect high-fidelity cancer registry-level data that can be used to confirm prognostic observations identified in institutional data sets. Clinicians will continue to use the latest information for patient care, including scientific content of the 8th Edition Manual. All newly diagnosed cases through December $31^{st}$ 2017 should be staged with the 7th edition. The time extension will allow all partners to develop and update protocols and guidelines and for software vendors to develop, test, and deploy their products in time for the data collection and implementation of the 8th edition in 2018. The 8th edition strikes a balance between a personalized, complex system and a more general, simpler one that maintains the user-friendliness and worldwide acceptability of the traditional TNM staging paradigm.

Malondialdehyde Level by Ethanol Exposure in Mouse According to the ALDH2 Enzyme Activity

  • Excessive alcohol consumption is associated with increased risks of many diseases including cancer. Individuals who regularly consume excessive quantities of alcohol have a greater risk of developing head and neck cancers such as esophageal, pharyngeal and oral cavity cancers if they are deficient in ALDH2 expression compared to normal populations. We evaluated lipid peroxidation in Aldh2 +/+ and Aldh2 -/- mice after they had been subjected to acute ethanol exposure. Malondialdehyde(MDA) level in liver tissue was evaluated as a biomarker of oxidative lipid peroxidation. Although the ethanol treatment did not increase the hepatic MDA level both in Aldh2 +/+ mice and in Aldh2 -/- mice, the MDA level was significant higher in the Aldh2 -/- mice than in the Aldh2 +/+ group. The MDA level was also significantly correlated with olive tail moment in blood and the level of 8-OHdG in liver tissue. This is a strong evidence to support our hypothesis that oxidative stress is more intense in Aldh2 -/- mice than in Aldh2 +/+ mice. Our results suggest that ALDH2-deficient individuals may be more susceptible than wild-type ALDH2 individuals to ethanol-mediated liver disease, including cancer.
