Interim palatal lift prosthesis as a constituent of multidisciplinary approach in the treatment of velopharyngeal incompetence

  • The velopharynx is a tridimensional muscular valve located between the oral and nasal cavities, consisting of the lateral and posterior pharyngeal walls and the soft palate, and controls the passage of air. Velopharyngeal insufficiency may take place when the velopharyngeal valve is unable to perform its own closing, due to a lack of tissue or lack of proper movement. Treatment options include surgical correction, prosthetic rehabilitation, and speech therapy; though optimal results often require a multidisciplinary approach for the restoration of both anatomical and physiological defect. We report a case of 56 year old male patient presenting with hypernasal speech pattern and velopharyngeal insufficiency secondary to cleft palate which had been surgically corrected 18 years ago. The patient was treated with a combination of speech therapy and palatal lift prosthesis employing interim prostheses in various phases before the insertion of definitive appliance. This phase-wise treatment plan helped to improve patient's compliance and final outcome.

OBSERVATIONS ON EMBRYONAL DEVELOPMENT OF HUMAN LARYNX (사람 배아의 후두발달에 관한 형태학적 관찰)

  • 후두는 호흡기가 시작되는 부위로 후두의 태생학적 연구는 타장기에서와 같이 정상 후두의 해부학적 이해, 후두 기형의 병인의 이해 및 치료에 기초 자료가 된다. 그러나 사람 배아의 후두 발생에 관한 연구는 재료확보가 용이하지 않고 해부학적 난이성으로 매우 드물다. 이에 저자는 사람 배아의 후두 발달을 연구하고자 배령이 확인된 20례의 배아(배령 4 주에서 8 주 까지)의 연속절편에서 광학현미경하에 후두 발달의 형태학적 관찰을 시행하여 다음과 같은 결과를 얻었다. 1. 배령 4 주에 median pharyngeal groove, laryngotracheal sulcus 와 tracheoesophageal septum 이 관찰되었다. 2. 배령 5 주에 hypopharyngeal eminence, epithelial lamina of larynx와 arytenoid swelling이 관찰되었다. 3. 배령 6 주에 hyoid condensation과 tracheoesophagealfistula가 관찰되었다. 4. 배령 7 주에 epiglottis 가 확인되었고 hyoid bone, thyroid lamina 와 cricoid cartilage 의 condensation 및 muscular condensation 이 관찰되었다. 5. 배령 8 주에 laryngeal cartilage의 chondrification과 ventricle이 관찰되었으며, vestibulotracheal canal과 pharyngotracheal canal 의 교통은 관찰되지 않았다.

A Case of Unexpected Clinostomum complanatum Infection Initially Presenting as Foreign Body in Pharynx

  • A 46-year old man visited our outpatient clinic with complaint of foreign body sensation in throat after consuming raw freshwater fish 5 days ago. Laryngoscopic examination revealed a motile worm attached on posterior pharyngeal wall. The worm was removed using biopsy forceps under transnasal endoscopy and evidently identified as Clinostomum complanatum after microscopic examination. Patient's subjective foreign body sensation of throat and hyperemia of laryngeal mucosa remained for approximately 2 weeks post-removal, which were eventually resolved after administration of non-steroidal anti-inflammatory drug and anti-refluxant drug for 2 weeks. Treatment was ended at three weeks since the first visit. C. complanatum infections in humans are rare, and only four cases have been reported in Korea. Symptoms resembling pharyngitis or laryngitis occurs by consumption of raw, infected freshwater fish and treatment is done by mechanically removing the parasite.

Antitumor effects of octyl gallate on hypopharyngeal carcinoma cells

  • The antitumor effects of octyl gallate (OG) were investigated on FaDu human hypopharyngeal squamous carcinoma cells. At various concentrations, OG inhibited the proliferation of FaDu cells by suppressing cell cycle regulators and induced apoptosis by activating caspase 3 and its downstream poly (ADP-ribose) polymerase, thereby damaging DNA. Immunoblotting demonstrated that OG significantly suppressed the expression of integrin family proteins (integrin α4, αv, β3, β4), hindering cell adhesion. The reduced expression of integrins subsequently mediated the mitogen-activated protein kinase signaling pathway to stimulate the activation of extracellular signal-regulated kinases and c-jun N-terminal kinases, leading to apoptosis. Thus, OG demonstrated antitumor activity on hypopharyngeal squamous carcinoma cells by suppressing cell proliferation and inducing apoptosis.

First report and morphological description of two Acrobeloides species(Nematoda: Rhabditida: Cephalobidae) in South Korea

  • The genus Acrobeloides(Cobb, 1924) Thorne, 1937 are bacterial feeders and are one of the most abundant and widely distributed nematode groups in various terrestrial environments. Based on morphological and morphometric analyses, we found two Acrobeloides species reported in Korea for the first time: A. bodenheimeri (Steiner, 1936) Thorne, 1937 and A. tricornis (Throne, 1925) Thorne, 1937. These species exhibit morphological characters concordant with typical features of the genus Acrobeloides, such as a fusiform pharyngeal corpus with swollen metacorpus and lateral incisures extending to the tail terminus. However, A. bodenheimeri is distinguished from other acrobeloids by having its low and rounded labial probolae, distinct post-uterine sac and five lateral incisures. Acrobeloides tricornis is distinguished from its congeners by the following characteristics: its high labial probolae with acuate termini, inconspicuous post-uterine sac and five lateral incisures. Morphological characters and their measurements, and illustrations of A. bodenheimeri and A. tricornis are described in this study.

The First Record of Leocratides kimuraorum (Annelida, Hesionidae) from Korea, with DNA Barcode Data

  • A hesionid species, Leocratides kimuraorum Jimi, Tanaka and Kajihara, 2017 is newly reported from the sublittoral zones (100 m depth) of the Korean coasts. This species is characterized by lateral antennae as long as the palps, peristomial membrane without papillose, peristomial dorsolateral tubercles with two round marginal lobes, and pharyngeal with terminal papillae. The intra-specific genetic distance among the cytochrome c oxidase subunit I(COI) sequences of L. kimuraorum specimens from Japan (type locality) and Korea (this study) was in the range of 0.002-0.005. The inter-specific genetic distance between L. kimuraorum and other hesionid species were 0.166-0.307. The present study is the first record of Leocratides species in Korean fauna. This paper also provides a morphological description and photographs of L. kimuraorum, with partial sequences of the mitochondrial COI based on Korean specimens.

Transoral Laser Excision of a Pyriform Sinus Cyst

  • Pyriform sinus cyst is a rare disease. This disease can be found by chance or appear as lymph nodes with cancer, infections, shortness of breath, or airway compromise, and can be an unusual cause of dysphonia. This paper presents a case of a pyriform sinus cyst in the pharyngeal wall of the left pyriform sinus, which presented as neck discomfort in a 63-year-old female. The plan was to remove this cyst with a transoral CO2 laser because a CO2 laser is a useful tool for removing cysts without bleeding and consuming time. The excision was performed successfully. Transoral laser pyriform sinus cyst excision is a simple, safe, and effective method for the treatment of pyriform sinus cysts. This procedure is likely to be used as a selective treatment when managing this condition.

  • The pterygomandibular raphe (PMR) is a tendinous structure connecting the bucinator and the superior pharyngeal constrictor muscles. With its implications in the spread of oral cancer, the proper treatment of obstructive sleep apnea, and dental procedures, it is important to obtain a thorough understanding of the PMR. We reviewed the existing literature to compile the published information regarding its anatomy, embryology, imaging, variations, functions, pathologies, and clinical relevance of the pterygomandibular raphe.

The effects of mandibular setback osteotomy on the oropharyngeal airway space in mandibular prognathic patients (하악전돌 환자에서 하악골 후퇴수술이 기도공간에 미치는 영향)

  • As a result of surgical orthodontic treatment of mandibular prognathism, changes take place in the skeletal and soft orofacial components. Although some investigators had stated that permanent reduction of airway space was observed after mandibular setback surgery, it was not clear that this permanent reduction was sustained during long-term follow-up. The purpose of this study was to assess the changes in oropharyngeal airway space and soft tissue orofacial component following the mandibular setback surgery and during the follow-up period. The correlation between the changes of the oropharyngeal airway space and the changes of other soft tissue orofacial component was also assessed. The findings of this study were as follows ; 1. The oropharyngeal airway space area decreased following mandibular setback surgery for mandibular prognathism and continued to decrease during the follow-up period(p<0.05). 2. The pharyngeal depth at Xi point level and the 2nd cervical vertebra point level decreased after the surgery and remained during the follow-up period(p<0.05). The decrease of these pharyngeal depth was correlated with the decrease of oropharyngeal airway space area(p<0.01). 3. The decrease of pharyngeal depth at the 3rd and 4th cervical vertebra point level was not significant after the surgery and during the follow up period. 4. The hyoid bone moved downward after the surgery(p<0.05), but returned to its original position during the follow-up period. 5. The length & height of tongue and the Position of epiglottis base did not change significantly(p>0.05). 6. The soft palate was displaced posteriorly after the surgery and remained to its changed position during the follow-up period(p<0.05) due to posterior displacement of tongue. The changes of soft palate were significantly correlated with the decrease of oropharyngeal airway space area(p<0.01). 7. The narrowing of oropharyngeal airway space was due to the posterior displacement of tongue above the level of epiglottis tip. The posterior displacement of tongue following mandibular setback osteotomy remained during the follow-up period.

Clinical Significance of Hypopharyngeal pH Monitoring in Gastroesophageal Reflux (위식도 역류에 대한 후두인두부에서 산도 검사의 의의)

  • Purpose: The aim of this study was to evaluate the significance of pH monitoring at two levels, hypopharynx and esophagus in gastroesophageal reflux. Methods: 29 patients with pathological gastroesophageal reflux were classified into two groups: Group I had recurrent respiratory symptoms and Group II had not recurrent respiratory symptoms. The ambulatory pH monitoring was performed at the hypopharynx and the esophagus simultaneously with two channel catheter for 18~24 hr. The pathological reflux was defined when the percent of time that pH was below 4.0 exceeding the 95th percentile of normal value. Hypopharyngeal reflux was defined as the pharyngeal pH drops below 4. Results: 39 patients were performed pH monitoring at the level of hypopharynx and esophagus for 24 hours. Among 7 patients with chronic respiratory symptom, 6 patients (85.7%) have pathological esophageal reflux. Among 32 patients without chronic respiratory symptom, 23 patients (71.8%) have pathological esophageal reflux. Thus 29 out of 39 patients (74%) have pathological esophageal reflux. In the Group I, all parameters except the longest episode showed significant differences between hypopharyngeal and esophageal pH monitoring. None of parameters showed significant differences between group I and II in the pharyngeal pH monitoring. Conclusion: Regardless of presence of respiratory symptoms, the pH monitoring at the pharyngeal level in patients with gastroesophageal reflux did not showed any differences compared with the esophageal pH monitoring. Therefore we may reconsider the usefulness of hypopharyngeal pH monitoring in patients with chronic respiratory symptoms.
