소아청소년정신과영역의 새로운 항우울제 (NEW ANTIDEPRESSANTS IN CHILD AND ADOLESCENT PSYCHIATRY)

  • 새로운 항우울제가 다량 개발되어 임상에 도입되면서 항우울제의 약리 작용에 대한 이해도 넓어졌다. 그에 따라 얻어진 새로운 정보와 이론을 소아청소년기 우울증 치료에 반영할 필요가 있다. 소아청소년기 최신지견에 따라 합리적인 우울증 치료 지침 마련을 위하여 고려하여야 할 주요 사항들과 새로운 항우울제의 특징들을 살펴보고자 하였다. 전자문헌 데이터베이스 검색 도구를 사용하여'new', 'antidepressant', 'children'의 검색어로 얻어진 97개 고찰 문헌 가운데 특히 치료 지침과 새로운 항우울제의 정신약리작용에 관한 것을 선별하여 중점적으로 고찰하였다. 아울러 새로운 항우울제 각 약물에 대한 치료 효과에 대한 문헌을 추가로 선택 참고하였다. 또 약물상호 작용과 안전성에 관하여 미국 식품보건국과 보건후생부의 공식사이트에서 제공되는 문헌을 참고하였다. 1) 우울 장애의 임상 경과, 치료 기간, 및 치료 효과는 성인기 치료 지침에서 제시된 바를 채택한 경우가 많았다. 2) 소아청소년기 우울증에 대한 항우울제의 효과에 대한 연구 결과는 TCA가 효과와 부작용에서 SSRI보다 뒤져서 소아청소년 우울장애의 일차 치료제로 권장 된다. 3) 새로운 항우울제는 아직 소아청소년에게 치료 경험과 임상 연구 결과가 부족하다. 4) SSRI와 새로운 항우울제들은 두 개 이상을 병용할 때 약동학적 및 약력학적인 상호작용이 있을 수 있다. 5) 소아청소년기 항우울제의 임상적 효과가 성인기와 다른 것은 발달적 측면에서 설명할 수 있겠으나 직접적인 증거가 좀더 쌓일 필요가 있다. 소아청소년기 우울증의 약물 치료 지침은 임상 연구 소견과 임상적 경험을 종합하여 세울 수 있다. 그러나 약물 치료 지침은 임상가가 합리적인 판단을 내릴 수 있도록 하는 참고 자료이며 그 목적을 다하기 위하여 새로운 연구 결과가 있을 때 마다 새로 개정 되어야 할 것이다. 중 외톨이-왕따에 해당되는 청소년의 어머니는 비외톨이 청소년의 어머니보다 자녀들의 사고 문제 및 우울/불안 등의 정서적 문제를 더 높게 평가하였다. 그러나, 자녀들에 대한 양육 태도에 대해서는 차이를 두지 않는 것으로 보인다. 향후 보다 많은 환자들을 대상으로 한 추가 연구가 필요할 것이다. 강압적인 양육행동을 변화시키기 위해서는 주의력결핍과잉행동장애 아동 부모의 양육 스트레스, 아동에 대한 역기능적 사고 및 양육 효능감을 다루어야 할 뿐 아니라, 부모의 우울감을 치료 시 고려해야 할 것이다.순차처리항목과 계산능력에서 유의하게 높았고(p<.05), KEDI-WISC를 이용한 평가에서는 ADHD- HI형은 대상수가 소수여서 비교할 수 없었으며, ADHD-C형과 ADHD-Ⅰ형 사이에 유의한 차이는 보이지 않았다. CPT, WCST, SST를 이용한 신경심리학적 실행기능의 비교에서 아형간 계량적인 차이는 있었으나 통계적으로 유의한 차이는 보이지 않았다. 결 론:결론적으로 ADHD 세 아형은 임상적으로 뚜렷한 차이를 보였지만, 실행기능상 유의한 차이를 발견할 수 없었다. 향후 보다 잘 고안 된 연구와 발달중인 아동에 적절한 신경심리 평가 도구의 개발을 통해 결과를 보완해야 할 것으로 사료된다.었으나, 주의력에서는 전두엽의 실행능력(executive function)과 관련되는 검사들에서 산소흡입이 특이한 효과를 보여준다는 것이 확인되었고, 기억능력에서는 단기기억능력 평가에서 산소흡입군이 대조군보다 유의한 효과를 보여주는 것으로 평가되었다. 이러한 연구결과는 산소흡입이 전두엽과 관련된 수행능력, 작동기억능력 향상에 도움이 될 가능성이 있음을 시사하는 결과라고 생각된다.증 1명(5%)이었다. 모든 대상 아동이 주 진단 이외의 2∼6개 이상의 다양한 공존진단을 보였다. 공존진단에는 주의력결핍-과잉운동장애, 우울병, 경계선지능 및 정신지체, 학습장애,

Inhibitory Effects of Hwao-tang on the Atherosclerosis and the Venous Thrombosis

  • The inhibitory effects of the traditional herbal medicine Hwao-tang on the progression of the atherosclerotic lesions were studied using the spontaneous familial hypercholesterolemia (FH) model, Kurosawa and Kusanagi-hypercholesterolemic (KHC) rabbits. Hwao-tang is activate blood circulation, vital energy and regulate menstruation, etc. Nowadays, Hwao-tang is mainly used for the treatment of inflammation, hyperlipemia and arteriosclerosis. However, pharmacological mechanisms of Hwao-tang on lipid metabolism and atherosclerosis formation are poorly understood. We have investigated the pharmacological effects of Hwao-tang on hypercholesterolemia and atherosclerosis using a spontaneous experimental model. In conclusion, the protection of extracts of HOT and its herbs on the ischemic infarction induced artificially might be related to their inhibitory effects on DIC, platelet coagulation and thrombic action. These suggest that Hwao-tang has inhibitory effects on the development of atheromatous plaque formation in spontaneous FH model rabbits. It is possible that the anti oxidative effects of Hwao-tang on LDL led to the beneficial effects observed in this study.

Effects of Silsosangami-extract and Its Seven Herbs on Endotoxin-induced Experimental Thrombosis in Rats

  • According to the Chinese and Korean medicinal and herbal literature, SSG(Silsosangami) is effective for the treatment of inflammation, hyperlipemia and arteriosclerosis. The pharmacological action of SSG has been limitedly studied in regard to ischemic infarction. This herbal medicine has been shown to express diverse activities such as immunomodulating, anti-infarction, anti-allergic and anti-inflammatory effects. Antisclerotic effects of SSG in experimentally induced atherosclerosis in rabbits have also been reported. However, pharmacological mechanisms of SSG on lipid metabolism and atherosclerosis formation are poorly understood. The present paper reports the effect of extracts obtained from SSG on endotoxin-induced experimental DIC in rats. Also, these were tested for their effect on endotoxin-induced blood platelet aggregation, thrombin-induced conversion of fibrinogen and fibrinolysis in vitro experiments with aspirin as a positive agent. The anti-thrombic properties of SSG were also investigated by means of analytical parameters of bood composition. The extracts of SSG and its seven herbs, except Cnidii Rhizoma and Carthami Flos, inhibited the endotoxin-induced DIC and thrombosis in rats. Also the extract inhibited the endotoxin-induced decrease in blood platelets and fibrinogen, and endotoxin-induced increase in fibrin degradation products (FDP) on disseminated intravascular coagulation in normal rats.

골다공증이 유발된 쥐 정강이뼈에 적용한 부분 진동자극 효과 (The Effects of Partial Vibration on Tibia of Osteoporosis Induced Rat)

  • The pharmacological therapies and whole body vibration as non-pharmacological therapies were known to have adverse side effects. Therefore, partial stimulation was suggested and its effects were evaluated. This study aimed to evaluate the site-specific effects of partial stimulator for treatment of osteoporosis induced by estrogen deficiency. Sixteen virginal Sprague-Dawley rats (12 weeks old) were divided into 2 groups(no stimulation, stimulation groups). All rats were ovariectomised to induce osteoporosis. After 3 weeks of operation, the right tibiae in rats of stimulation group (frequency: 10Hz, cycle: 1500, strain on bone surface: $2000{\mu}{\varepsilon}$) were stimulated perpendicularly at right tibia by using partial stimulator for 6 weeks (3days/week). The right tibiae in rats were scanned, before stimulation (0 week) and at 6 weeks after stimulation by using in-vivo micro computed tomography. For investigation of changes in morphological characteristics, structural parameters were measured and calculated. At 6 weeks the morphological characteristics (relative value) in stimulation group were significantly enhanced than those in no stimulation group (p<0.05). In this study, we find that after 6 weeks of partial stimulation, the morphological characteristics of tibia trabecular bone were enhanced. Thus, we concluded that partial stimulation could be used to treat osteoporosis.

영골(靈骨).강압혈(降壓穴) 자극(刺鍼)이 혈압(血壓)에 미치는 영향(影向) (Effect of Acupuncture on Young-gol and Gang-ap-hyul on the Blood Pressure)

  • Objective : The purpose of this study is to evaluate the clinical effect of the non-pharmacological anti-hypertension method, acupuncture, for hypertensive patients. Methods : 26 patients diagnosed with hypertension were each treated by one of three methods: an anti-hypertension drug(adalat soft capsule), venepuncture, and acupuncture. In cases of rising blood pressure, patients were treated by their one of the three methods, and blood pressure was measured at regular intervals. Result : As for the group of acupuncture treated patients on Young-gol(ling-gu) and Gang-ap-hyul(jiang-ya-xue), readings dropped 18.0mmHg in systolic blood pressure and 6.50mmHg in diastolic blood pressure. This was a greater effect than that seen in the venepuncture treated patients. Blood pressure checked one hour after acupuncture treatment showed a greater difference than blood pressure checked after thirty minutes. Conclusion : Safe reductions in blood pressure were observed through the non-pharmacological method of acupuncture on Young-gol(ling-gu) and Gang-ap-hyul(Jiang-ya-xue).

Chronic postsurgical pain: current evidence for prevention and management

  • Chronic postsurgical pain (CPSP) is an unwanted adverse event in any operation. It leads to functional limitations and psychological trauma for patients, and leaves the operative team with feelings of failure and humiliation. Therefore, it is crucial that preventive strategies for CPSP are considered in high-risk operations. Various techniques have been implemented to reduce the risk with variable success. Identifying the risk factors for each patient and applying a timely preventive strategy may help patients avoid the distress of chronic pain. The preventive strategies include modification of the surgical technique, good pain control throughout the perioperative period, and preoperative psychological intervention focusing on the psychosocial and cognitive risk factors. Appropriate management of CPSP patients is also necessary to reduce their suffering. CPSP usually has a neuropathic pain component; therefore, the current recommendations are based on data on chronic neuropathic pain. Hence, voltage-dependent calcium channel antagonists, antidepressants, topical lidocaine and topical capsaicin are the main pharmacological treatments. Paracetamol, NSAIDs and weak opioids can be used according to symptom severity, but strong opioids should be used with great caution and are not recommended. Other drugs that may be helpful are ketamine, clonidine, and intravenous lidocaine infusion. For patients with failed pharmacological treatment, consideration should be given to pain interventions; examples include transcutaneous electrical nerve stimulation, botulinum toxin injections, pulsed radiofrequency, nerve blocks, nerve ablation, neuromodulation and surgical management. Physical therapy, cognitive behavioral therapy and lifestyle modifications are also useful for relieving the pain and distress experienced by CPSP patients.

Buxuletin의 가토에 대한 이뇨작용 (A Study on the Diuretic Action of Buxuletin)

  • Buxus leaves(Buxus microphylla var. koreana Nakai) have been used as a folk medicine in treating woman's disease. The chemical constituents, which have teen identified from Buxus sempervirens L. in Europe, are so far known in isolation of buxus alkaloids, cyclobuxine, buxamine, norbuxamine and buxaminol. But But any study on the pharmacological action of Buxus microphylla var. koreana Nakai has not yet teen rallied out, however, active substances have not been identified. As an attempt to isolate the biologically active substances from this plant, the examination of chemical constituents was undertaken. In the experiment, the crystalline component$(C_{10}H_8O_4)$ obtained from Buxus microphylla var. koreana Nakai was identified as a 6-methoxy, 7-hydroxy coumarin by chemical and physical methods. We named this crystal buxuletin. Therefore, we intended to screen the pharmacological action of buxuletin, especially, its diuretic effects in rabbits. Buxuletin was intraveneously injected to the rabbit in the dose of 20 mg/kg, 10 mg/kg and 5 mg/kg. The variations of urine volume and minerals $(Na^+,\;K^+\;and\;Cl^-)$ in urine were measured at an interval of 15 minutes for 4 hours after the treatment of buxuletin. The observed results are as follows: 1) With the administration of buxuletin (20 mg/kg, 10 mg/kg), the remarkable increment of the excreted urine volume was observed during the relatively long period. Excretion rates of urinary minerals $(Na^+,\;K^+\;and\;Cl^-)$ were also increased. 2) In a dose of 5 mg/kg, the slight increment on the diuretic actions of rabbits was observed. From the above results, authors name this crystal buxuletin and buxuletin shows the diuretic action.

  • Casticin (3', 5-dihydroxy-3, 4', 6, 7-tetramethoxyflavone) is an active compound isolated from roots, stems, leaves, fruits and seeds of a variety of plants. It is well known for its pharmacological properties and has been utilized as an anti-hyperprolactinemia, anti-tumor, anti-inflammatory, neuroprotetective, analgesic and immunomodulatory agent. Recently, the anticancer activity of casticin has been extensively investigated. The resulkts showed that it exerts protective potential by targeting apoptosis, considered important for cancer therapies. In this article, our aim was to review the pharmacological and therapeutic applications of casticin with specific emphasis on its anticancer functions and related molecular mechanisms. Chemotherapeutic effects are dependent on multiple molecular pathways, which may provide a new perspective of casticin as a candidate anti-neoplastic drug. This review suggests that additional studies and preclinical trials are required to determine specific intracellular sites of action and derivative targets in order to fully understand the mechanisms of its antitumor activity and validate this compound as a medicinal agent for the prevention and treatment of various cancers.

Evaluating Pharmacological Effects of Two Major Components of Shuangdan Oral Liquid: Role of Danshensu and Paeonol in Diabetic Nephropathy Rat

  • Shuangdan oral liquid (SDO) containing radix Salviae miltiorrhizae (Chinese name Danshen) and cortex moutan (Chinese name Mudanpi) is a traditional Chinese medicine using for treating vascular diseases. Danshensu (DSS) is a main effective monomer composition derived from radix Salviae miltiorrhizae and paeonol (Pae) from cortex moutan. Although the two herbs are widely used in traditional Chinese medicine, the pharmacological functions of their active compositions were not reported. Therefore, the research of DSS and Pae in mechanisms and pharmacodynamics interaction can provide scientific evidence to support clinical application. The diabetic nephropathy (DN) rats which were induced by streptozotocin (STZ) were treated with SDO, DSS, Pae, and DSS+Pae for eight weeks. The positive effects on DN animal models were investigated by detection of physiological and biochemical indexes and oxidative stress markers, within five treatments: SDO, DSS, Pae, DSS+Pae and insulin group. Compared with the model group, the DSS+Pae group improved the renal function, blood lipid metabolism and blood viscosity, increased the vitality of T-SOD or T-AOC and decreased the level of MDA or NO after the treatment. The study was successfully showed that the DSS+Pae group could delay the process of DN, especially in the renal injury part of histopathology changes. Our results suggest that the co-administration of DSS and Pae significantly may play a protective role in DN rats through decreasing the oxidative stress and improving the blood lipid metabolism mechanisms.

참소리쟁이의 특성을 이용한 염색성 연구 (Dyeability using Characteristics of Curly Dock)

  • In this research, the curly dock was used in the process of dyeing for fabrics of the inner wear & the patient wear. Since the curly dock has a pharmacological effect on dermatosis, this study focused on the variety of color and functions of the inner wear fabrics & patient wear fabrics to make the best use of the pharmacological effect of curly dock. With regards to giving a variety of colors and functions in the inner wear, patient wear fabrics, the curly dock dye was used in each treatment conditions on the cotton & silk fabrics. After dyeing, the dyeability, color change, light fastness, washing fastness, perspiration fastness, antibiosis, far infrared emissivity and emission power were evaluated. The evaluation results are as follows; The dyeablity increased from repeated dyeing and, by using the mordant, variety of colors such as skin, mustard, greyish-brown and dark earth colors were conformed to the naked eye. Fe mordant was better than Al on the lightfastness and the washing fastness. The repeated dyeing was found out to have less effect on neither lightfastness nor washing fastness. Both silk and cotton fabrics were graded $3{\sim}4$, since their degree of degradation appeared to be the same in alkali perspiration and acidic perspiration. In the case of silk fabrics mordanted by Al, the rate of declining in both Staphylococcus aureus ATCC 6538 and Klebsiella pneumonia ATCC 4352 were 99.9%. In addition, the antibiosis was enhanced when the mordant was used. The far infrared was 86.6% of emissivity, $3.34{\times}10^2\;W/m^2{\cdot}{\mu}m$ emission power.
