양측 거골에 발생한 골연골종 (Osteochondromas of the Bilateral Tali)

  • Benign and malignant tumors are found in the foot, although the incidence is low. The most common bone tumor in the foot is osteochondroma, which is thought to develop in bones that form through the process of enchondral ossification. In particularly, osteochondromas in the foot mostly occur in metatarsal bones and phalanges. It is seldom found in talus. It is usually confused with osteophyte or enthesis. We report an osteochondroma case confirmed by roentgenographical and pathological investigation, after removing the masses form bilateral tali of a 19-year-old male patient presented with bilateral ankle joint pain for three years.

수부 원위지골 원위부 조갑하에 발생한 유전성 다발성 외골종 - 1례 보고- (Hereditary Multiple Exostosis at Distal Tip of Distal Phalanx -A Case Report-)

  • 골연골종은 수부에서는 매우 드문 질환이지만 유전성 다발성 외골종의 경우는 수지골, 특히 성장판 부위의 기저부에 빈번하게 발생하며 수지관절주위에 호발한다. 하지만 원위지골 원위부에 발생한 경우는 매우 드문 것으로 이에 대한 보고는 거의 없다. 저자들은 다발성 외골종의 가족력이 있는 10세 남아가 여러 관절 주위의 골성 종괴를 주소로 내원, 시행한 단순방사선 검사상 수부 원위지골의 원위부에 외골종이 관찰되어 이를 수술적 치료로 제거하고 문헌고찰과 함께 보고하고자 한다.

  • 골연골종은 가장 흔한 양성 골 종양으로, 성장판에서 기원한 명확하게 구별되는 연골모를 가지고 있다. 호발 연령은 20세 이하의 환자에서 주로 발생하며, 성장에 비례하여 종양의 크기가 커지다가 골격계의 성숙과 함께 종양의 성장이 멈춘다. 골연골종의 호발 부위는 주로 장 관골의 골간단부와 편평골에서 발생하나, 족부에서는 낮은 발생 빈도를 보인다. 족부에서는 대개 중족골과 족지골에 발생하며 종골에서의 발생은 매우 드문 것으로 알려져 있다. 본 교실에서는 15세 남아에서 발생한 종골 내측 족저부 조면에 발생한 단발성 골연골종을 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

진심통(眞心痛)에 관(關)한 문헌적(文獻的) 고찰(考察) (The Literatural Study of True Heartache)

  • The true heartache is a condition of severe heartache corresponding to angina pectoris, as recorded from Hwang Jae Nai Kyung. According to the literatural study of true heartache, some results can be acquired, such as follows. 1. The site of the true Heartache, can be divided into two categories, first, its superficial and conscious area is the chest as same as the other heartache. But its inner lesion is the Heart-Meridian as others occupied at the Pericardium-Meridian in stead of the Heart-Meridian. 2. The etiological classification of true heartache, are Cold-evil, Heat-evil, Wind-evil, Blood stasis etc. But its major factor is Cold-evil, more than anything else. 3. The symptomatic signs of true heartache, consist of cyanotic change from hands and feet to phalanges; severe heartache pale complexion with cold breathing and its extreme state can manifestate unceased sweating called as Yang exhaustion.

Numerical study of 10-year-old child forearm injury

  • Forearm fractures in children are very common among all pediatric fractures. However, biomechanical investigations on the pediatric forearm are rather scarce, partially due to the complex anatomy, closely situated, interrelated structures, highly dynamic movement patterns, and lack of appropriate tools. The purpose of this study is to develop a computational tool for child forearm investigation and characterize the mechanical responses of a backward fall using the computational model. A three-dimensional 10-year-old child forearm finite element (FE) model, which includes the ulna, radius, carpal bones, metacarpals, phalanges, cartilages and ligaments, was developed. The high-quality hexahedral FE meshes were created using a multi-block approach to ensure computational accuracy. The material properties of the FE model were obtained by scaling reported adult experimental data. The design of computational experiments was performed to investigate material sensitivity and the effects of relevant parameters in backward fall. Numerical results provided a spectrum of child forearm responses with various effective masses and forearm angles. In addition, a conceptual L-shape wrist guard design was simulated and found to be able to reduce child distal radius fracture.

  • Brachydactyly type C is a limb malformation characterized by shortening of the second, third, and fifth middle and/or proximal phalanges, but it has variable phenotypic expressivity. Mutations in the growth differentiation factor-5 (GDF5) gene cause isolated brachydactyly C. Herein, we report a familial case with isolated brachydactyly type C characterized by brachymesophalangy of both second and third digits, with a GDF5 missense mutation, and discuss the phenotypic variability of the condition. Identifying more cases with genetic confirmation will help elucidate the clinical and genetic characteristics of this condition in the Korean population.

  • The abnormal female fowl (Arbor Acres broiler) of 40-day-old, which was polymelia with polydactyly was observed macroscopically and radiographically. An extra pelvic limb was attached at the left pelvic region. The extra pelvic limb consisted of an underdeveloped and malformed Os coxae like bone, femur like bone, tibia and fibula like bones, tarsal and metatarsal like bones, and five digits. The tarsal bones of two normal pelvic limbs and one extra pelvic limb were not fused with tibia and metatarsal bones respectively. The metatarsus of extra pelvic limb consisted of a single bone derived from several components. In a case of normal, first metatarsal bone remains independent. However in this case. all of metatarsal bones were fused. The extra pelvic limb was polydactyly. The digits consisted of the first and extra digit of three. the second and third of four. and the fourth of five phalanges. In each toe the last phalanx was pointed and formed the claw. The first and the second digits were polyphalangia [hyperphalangia]. The extra digit was microdactylia and brachydactyly.

종골에 발생한 내연골종: 증례 보고 (Enchondroma of the Calcaneus: A Case Report)

  • 내연골종은 주로 수부에 발생하는 양성 골 종양으로 족부에 발생하는 경우는 흔하지 않다. 족부에서 발생할 경우에도 대개 족지나 원위 중족골이며 종골에서는 발생이 매우 드문 것으로 알려져 있다. 수개월간 지속된 좌측 종골 부위 통증으로 내원한 44세 남자 환자에서 시행한 방사선 검사에서 내연골종 의심되어 골 소파술 및 골 이식술을 시행하였고 최종 병리검사에서 확진되었다. 이에 본 저자들은 종골의 내연골종을 1예 경험하였기에 문헌 고찰과 함께 보고 하는 바이다.

Nailbed Epithelial Inclusion Cysts in Two Dogs

  • A 5-year-old, 6.2 kg male mixed dog was presented to local animal hospital with a 6-month history of swelling, pain, inflammation, and lameness in the 5th digit of right hind limb. And a 7-year-old, 2.7 kg male Maltese dog was also presented to animal hospital with a 2-month history of nail deformities in the 5th digit of left hind limb. Abnormal growth or degeneration of the distal phalanges was observed at the 5th digit of hind limb in two dogs using radiographic examination. The masses in the digit were excised completely under local anesthesia. On histological examination of the digit masses, large well-circumscribed, unencapsulated round or irregular cystic neoplasms with/without inflammation were occupied in or adjacent area of the distal phalanx. These cysts were lined by stratified squamous epithelium that occasionally had a prominent granular cell layer. Based on the history, clinical signs, radiographic, gross and histopathologic features, these cases were diagnosed as nailbed epithelial inclusion cysts in the digit of dogs.
