신종감염병 대응 비의료인 전담요원 개인보호구 착탈의 융합 교육 효과 (Convergence Education Effect of Donning and Doffing Personal Protective Equipment for Non-medical Personnel to the New Infectious Disease)

  • 본 연구는 신종감염병 대응 전담요원의 개인보호구 착탈의 융합 교육 효과를 확인하고자 시도되었다. Level D 개인보호구 착용 기준에 적합한 28명의 비의료인 신종감염병 대응 전담요원을 대상으로 2018년 3차에 걸쳐 교육 후 개인보호구 착탈의 점수를 수집하였으며 프리드만 검정(Friedman)과 윌콕슨 부호순위 검정(Wilcoxon's Signed-Ranks Test)으로 분석하였다. 연구결과, 교육횟수에 따른 분석에서 착의점수는 유의하게 상승하였고 탈의 점수는 유의한 차이를 보였다. 교육차수별 분석에서는 착의 점수는 1차보다 3차에서 유의하게 상승하였으며 탈의 점수는 1차보다 3차에서 점수가 상승하였으나 유의 하지 않았다. 본 연구의 대상자들은 비의료인이기 때문에 지식이 부족하지만 감염병에 접촉할 확률이 크기 때문에 교육이 필요한 점에서 의미가 크며 본 연구결과를 통하여 개인보호구 탈의 교육의 효과가 상대적으로 낮아 교육회수의 증가 혹은 주기적인 교육의 필요성이 중요하다고 파악되었다.

사례분석을 통한 객체검출 기술의 건설현장 적용 방안에 관한 연구 (A Study on the Application of Object Detection Method in Construction Site through Real Case Analysis)

  • 연구목적: 본 연구의 목적은 건설현장의 재해 예방을 위해 딥러닝기반의 개인보호구 검출 모델을 개발하고, 실제 건설현장에 적용하여 분석하는 것이다. 연구방법: 본 연구의 수행 방법은 실제 환경의 데이터를 구축하고, 개발된 개인보호구 검출 모델을 적용하였다. 개인보호구 검출 모델은 크게 근로자 검출 및 개인보호구 착용 분류 모델로 구성되어 있다. 근로자 검출 모델은 딥러닝 기반의 알고리즘을 실제 현장에서 획득한 데이터셋을 구축하여 학습 및 근로자를 검출하였고, 개인보호구 착용 분류 모델은 앞단에서 추출된 근로자 검출영역에서 학습된 개인보호구 검출 알고리즘을 적용하였다. 구축된 모델의 검증을 위해 건설현장 3곳에서 획득된 데이터를 통해 실험결과를 도출하였다. 연구결과: 데이터베이스 12,000장을 구축하여 정상검출 9,460장(78.8%), 오검출 1,468(12.2%), 미검출 1,072장(8.9%)으로 나타났으며 주요 원인은 영상에서의 객체 크기, 객체간 중첩(Occulusion), 객체 잘림, 그림자에 의한 오검출로 분류되었다. 결론: 개인보호구 검출모델은 현장 상황마다 다른 검출률을 확인할 수 있었고, 본 연구의 결과가 차후 현장적용을 위한 연구에 활용될 수 있을 것으로 여겨진다.

간호대학생의 병원감염관리 표준주의에 대한 인지도와 수행도 (Nursing Students' Awareness and Performance on Standard Precautions of Infection Control in the Hospital)

  • Purpose: The purpose of this study was to investigate nursing students' awareness and performance on standard precautions and to provide meaningful information for nursing students' education regarding hospital infection control. Method: Four hundred forty seven nursing students at 6 universities in D-city participated in this study. Data collection was conducted from March to April 2011. Students' awareness and performance on standard precautions of infection control in hospitals were measured using the modified 2007 CDC standard precautions guidelines. Data analysis was performed using the SPSS WIN 18.0 program, descriptive statistics, t-test, and ANOVA. Results: The level of students' awareness in the standard precautions was higher than performance. The higher levels of students' awareness and performance on standard precautions included patient care equipment, safe injection practices, and worker safety. The lower levels of students' awareness and performance on standard precautions included hand hygiene and personal protective equipment. There was no difference in the students' awareness and performance of standard precautions according to their characteristics. Conclusions: Based on the findings of this study, hand hygiene and personal protective equipment need to be stressed more within the program to improve nursing students' infection control techniques.

가금류 악성 전염병 감염축 처리자의 보호 장비 착용실태 조사 및 개선에 관한 연구 (A study on wearing practice and improvement of personal protective equipment for participant handling livestock carcass infected with virulent avian infectious agents)

  • This study was aimed to investigate factors that affected the status of wearing personal protective equipment (PPE) for handling poultry carcasses with potential exposure to virulent avian infectious agents. A large outbreak of high pathogenic avian influenza (HPAI) occurred in South Korea in 2014. Many public officers participated for euthanizing and handling livestock carcass. However, several safety issues with using PPE were revealed. Therefore, a questionnaire survey was conducted for 340 people who participated euthanasia and carcass disposal in the place where HPAI mainly occurred in 2014. It was found that 31.8% of the respondents had ever taken off their protective equipment during operations because of its inconvenience. The most inconvenient PPE was goggles (54.6%), followed by mask (20.2%), latex gloves (11.6%), shoe covers (5.9%) and protective clothing (5.3%). The main complaints about this individual PPE was unclear sight, damp emitted toward eye, sweating, tearing easily and sweating, respectively. Considering such problems of PPE, new possible directions for improvement of gloves and goggles were suggested. With newly developed rubber coating fabric gloves and conventionally using latex and fabric gloves, H3N2 influenza virus transmission experiment was conducted. Rubber coating fabric gloves showed similar efficiency for blocking virus transmission with latex laboratory gloves and were not easily torn by sharp claws of chicken. In addition, air flow control safety eyewear was suggested to minimize moisture formation. The air flow control system efficiently suppressed moisture formation inside the goggles. Therefore our study will provide more specific directions about new PPE development for safety protection of actual wearers.

  • 2019년말 발생한 COVID-19이 세계적 대유행으로 확산되면서, 선진국과 개발도상국을 가리지 않고 전세계의 의료 시스템을 동시에 무너뜨리는 초유의 사태로 나타나고 있다. 대부분의 시민 사회에서 개인의 보호장구의 부족, 클러스터가 된 지역사회의 격리, 진단과 치료에 필요한 의료시스템의 중단, 사회적 격리로 인한 교육 및 경제활동의 중단 등, 초연결 사회를 지향하던 시민사회가 경험해보지 못한 새로운 위기에 직면하고 있다. 효과적인 대응에 필수적인 개인 보호장비 (PPE), 제독 및 방역 도구, 신속하고 정밀한 대규모 진단, 환자 치료에 필요한 의료기기, 사회적 고립에서 활용될 수 있는 정보의 분석과 연결망 등은 선진국과 개발도상국을 가리지 않고 COVID-19 상황에서 필수적인 사회적 인프라임이 확인되고 있다. 본 Part 1에서는 최근 적정기술로 위기를 극복하기위한 개인보호장구와 개인 및 지역사회의 방역에 대한 사례, 그리고 ICT를 이용한 빅데이터 및 정보활용기술등을 적정기술의 테두리안에서 소개하고자 한다.

Veterinarians as a Risk Group for Zoonoses: Exposure, Knowledge and Protective Practices in Finland

  • Background: Veterinarians may encounter a variety of zoonotic pathogens in their work. Methods: We conducted two cross-sectional questionnaire studies among veterinarians in Finland. Participants were recruited during two Annual Veterinary Congresses. In 2009, 306 veterinarians participated in an extensive questionnaire study, and in 2016, 262 veterinarians participated in a more focused study that included two same questions. Results: In 2009, the majority (90.9%) of the participating veterinarians reported having been occupationally exposed to zoonotic pathogens. Zoonotic infections (15.0%), needle stick incidents (78.8%), bites (85.0%), as well as infected skin lesions (24.2%) were reported. In 2009, 8.2% of the participants fully agreed with the statement "I have good knowledge of zoonoses and their prevention"; in 2016, the proportion was 10.3%. The reported use of protective practices and personal protective equipment in connection with specific veterinary procedures indicated that there was room for improvement, particularly in protection from pathogens that are transmissible via inhalation and mucous membranes. Conclusion: The results confirm that veterinarians are commonly occupationally exposed to zoonotic pathogens. Education should aim to improve and maintain the knowledge of zoonoses and their prevention. Use of protective practices should be advocated.

중앙119구조본부 소방대원용 화학 보호복과 방열복의 동작적합성 평가 및 디자인 개선 (Enhancing Design and Evaluating Mobility of Firefighting Chemical and Flame Protective Clothing for the National 119 Rescue Headquarters)

  • The present study developed a test protocol for evaluating the mobility of firefighting chemical and flame personal protective equipment (PPE) for the National 119 Rescue Headquarters in Korea and suggested ergonomic design factors to improve their mobility and performance. Six types of PPE were employed, including three types for flame protective PPE (5 ~ 6 kg excluding the self-contained breathing apparatus), and three types for chemical and flame protective PPE (8 ~ 11 kg). These PPEs are used by the 119 Rescue firefighters. Three male firefighters (34.3 ± 1.2 y in age, 175 ± 8 cm in height, 81 ± 13 kg in body weight) participated in the mobility test and interview. A mobility test protocol consisting of 16 components (nine postures and seven motions including a dexterity test) along with a visibility test were developed based on pre-interviews and literature reviews. The findings indicated that the clothing microclimate humidity on the neck and chest exceeded 85%RH on average for all the six PPE conditions, with the chest area reaching as high as 98%RH. This high humidity caused fogging inside the visor and impaired visibility. The requirements for improving the PPE design in terms of mobility varied depending on whether it was the separated types or all-in-one types, particularly regarding the hood and gloves design. The findings of this study can be applied to improve the design of Level A_PPE for firefighters. The mobility test protocol and visibility test developed in this study can also be applied to other types of Level A impermeable PPE.

환경부 소관 유해화학물질 취급자의 개인보호장구 착용 기준에 관한 소고 (A Brief Review of Regulations on Personal Protective Equipment for Hazardous Chemical Handlers as Regulated by the Ministry of Environment)

  • Objectives: Personal protective equipment (PPE) is an essential means for protecting workers against hazardous agents or risks that threaten their safety and health. Governmental organizations related to safety and health in the workplace regulate the PPE rules to protect workers and to minimize damage from hazardous agents. This study discussed current PPE rules overseen by the Korea Ministry of Environment and explores future perspectives on the matter. Methods: This study was based on a review of PPE regulations with which every stakeholder should comply in the workplace. Both South Korean regulations enforced by the Ministry of Employment and Labor and the Ministry of Environment and cases from other countries were reviewed. Results: Regulations related to the PPE required for handling chemical substances in the workplace are enforced by the Occupational Safety and Health Act, Enforcement Decree, Enforcement Rules, and Notification of Protective Equipment Certification under the Ministry of Employment and Labor. The Ministry of Environment also regulates the PPE standards for 97 substances requiring preparation for accidents and adjustment of work conditions, but a recent amendment (partially amended on September 30, 2022) loosened some unreasonable or excessive provisions. It requires workers simply to carry or otherwise keep PPE handy instead of wearing it for some tasks in which hazardous chemicals are not handled directly. Conclusions: It is important to regularly review provisions that need to be improved or supplemented to help all stakeholders. Considerations should be also made to build a reasonable regulatory system that can induce more mature safety management in each workplace.

119 구급대의 구급장비 감염 관리와 실태조사 (Infection management for emergency rescue equipment)

  • Purpose: This study investigated the use of infection management and the frequency of emergency equipment sterilization by emergency rescue crews to provide basic guidelines and suggestions for infection control. Methods: A self-reported questionnaire was completed by 160 emergency medical technician rescue crews in J area from May 7 to May 22 in 2015. The questionnaire consisted of 95 items, including the general characteristics of the subjects (8 items), the use of emergency equipment (33 items), the frequency of facility sterilization (33 items), infection control (12 items), vaccination (4 items), and the use of personal protective equipment (5 items). Results: In all, 97.3% of respondents were aware of the "Guidelines of Infection management for Rescue Operators' and 90.9% received training on infection control. A total of 72.7% and 47.3% of rescue centers were equipped with disinfection facilities and laundry rooms, respectively. The average frequency of sterilization was $3.17{\pm}0.75$, which significantly differed for teams with more clinical experience (p=.050) and teams with three members (p=.030). The average score for individual protective equipment supplies was $1.95{\pm}0.66$. Conclusion: For proper infection management of emergency equipment, our results suggest that the number of crew members should be increased for each ambulance, protective equipment and rescue products should be supplied, and the number of facilities for sterilization and laundry at rescue centers should be doubled.

일부 농업인에서의 농약살포방식 및 보호구 착용에 따른 유기인계 농약노출평가 (Evaluation of Exposure to Organophosphorus Pesticides According to Application Type and the Protective Equipment among Farmers in South Korea)

  • 본 연구는 국내 일부 농업인을 대상으로 유기인계 농약 노출수준과 농약살포 특성의 관련성을 규명하기 위하여 수행되었다. 소변시료를 채취하여 GC/MSD와 GC/MS/MS를 이용하여 분석하였으며, 소변 중 DMP, DMTP, DEP, DETP를 대상으로 검출률 및 노출수준을 평가 하였다. 검출률은 DMP, DMTP에서 캡이 없는 SS살포기가 캡이 있는 SS살포기보다 높았고, 보호구 착용률이 낮을수록 높았다. 노출수준은 동력 분무기가 가장 높았고, 캡이 있는 SS살포기에서 낮게 나타났다. 또한, 착용 보호구 개수가 증가할수록 노출수준이 낮아짐을 확인할 수 있었다. 검출률은 카이제곱, 노출수준 비교는 연령 및 살포방법을 보정한 GLM을 이용하여 분석하였다.
