Implantation of a permanent pacemaker is a widely accepted procedure for the patient with complete heart block.As a result of these device, the prognosis for patients with Adams-Stokes syndrome caused by complete A-V block and other cardiac arrhythmia have become much more optimistic. Permanent pacemaker implantation by means of a transvenous approach has made the operative risk much less and the procedure simpler. However, a number of complications have been reported in the literature regarding transvenous endocardial pacemaker implantation during the last a decade. The patient presented in this paper is a 26-year old girl who was implanted with a permanent pacemaker at 14 years of age because of a congenital A-V block. Following first exchange of pulse generator, the electrode (lead) was fractured, so that by the pulse generator, a change to the transvenous technique of implantation was made, After this, there were episodes of recurrent wound infection on three occasions, even though the site of pulse generator implantation was exchanged to the contralateral side of chest wall, massive doses of antibiotics were administered and sensitivity tests for coagulase positive staphylococcal infection were performed. Though there was no definite evidence of blood stream infection by blood culture, we decided not to use the transvenous technique and not to implant the pulse generator in the chest wall because the venous system and the entire anterior chest wall appeared to be diseased or contaminated by virulent pyogenic organisms. Finally this intractable systemic and local wound infection was successfully controlled by myocardial lead implantation via a subxiphoid approach and implantation of the pulse generator far down in the abdominal wall. The causes and routes of recurrent wound infection and possible blood born infection in this particular patient are still obscure. We strongly believe that myocardial pacemaker implantation is much safer than transvenous endocardial pacemaker implantation & myocardial pacemaker implantation is a definite method for controlling such an intractable wound infection. following transvenous pacemaker implantation.
Two instrumented full scale tieback walls in sand were constructed at to Geotechnical Experimentation Site located on the Texas A 51M University Riversic Measurements were obtained from the one row anchor wall and from the two row at different times during construction. The measured performance of the tieback walls is presented and investigated. Shear Connector should be used to fix the PHC pile with extension wall in order to utilize PHC-W retaining wall as permanent wall. The pullout behaviours on shear connectors anchored into PHC-W pile were observed as two modes. The first type behaviour showed that after reaching the maximum pullout resistance, the anchorage was broken and shear connector was pulled out abruptly. The second type behaviour showed that even after arriving the maximum pullout resistance, the anchorage was not destroyed and there was a progressive increase in pullout displacement. The maximum pullout resistance of the steel anchor shear connector is larger than that of deformed bar shear connector. The larger the diameter and the longer the embedment length of shear connector, the higher the maximum pullout resistance would be. The pullout displacements corresponding to the maximum pullout resistance of the shear connector showed various ranges regardless of the materials, the diameters and the anchoring lengths. A-D20 shear connectors showed a pull-out displacement of about 8~10 mm. A-D16, D-D19 and D-D16 shear connectors exhibited a pulling displacement of about 14~20 mm, but a pulling displacement of about 6~10 mm when the anchoring lengths were 50 and 80 mm.

Recently, there have been numerous attempts to expand the traditional temporary soil nailing system into a permanent wall. Two reasons for this include the soil nailed system's advantage of efficient and economic use of subgrade space and its ability to decrease the total construction cost. However, the systematic and logical design approach has not been proposed yet. The permanent soil nailing wall system, which utilizes precast concrete from soil nailing system, is already used in many countries, but the study of cast-in-place concrete lacing or rigid walls in bottom-up construction of traditional soil nailing walls is imperfect and insufficient. In this paper, various laboratory model tests have been carried out to investigate the influence of parameters, including stiffness of the rigid wall to the soil nailing structure with respect to failure mode, displacement patterns and tensile forces at the nail head in several levels of load. Then, the variation of earth pressure distribution on the soil nailing wall, built with a rigid front plate, is sought through different levels of surcharge load and tensile forces at the nail head.
It is essential to take consideration of long-term deformation characteristics of mechanically stabilized earth wall user sustained and repeated loads for design and construction, especially for use as part of permanent structures. This paper presents the long-term performance of a full-scale geogrid reinforced segmental retaining wall results based on the measured strains in geogrids for three years. The results indicate that the reinforcement tensile strains tend to continuously increase after wall completion with the increase being more pronounced in the reinforcement layers in the lower tier. It can be concluded that the long-term deformation should be taken in account for walls constructed as part of permanent structures for which wall deformation should be controlled.

It is necessary to work on a vertical plane of workpiece in order to produce a large structure like a ship. These works can be automated by using the robot with permanent magnet wheels. We developed the permanent magnet wheel which can be used by a mobile robot and easily detached. We enhanced an adhesive power by restricting the occurrence direction of magnetic flow. And we also developed a method which weakens adhesive magnetic force by changing magnetic flow with metal pins. We used the load cell and the gaussmeter to measure the characteristics of the adhesive force and magnetic force. We obtained the result that the adhesive power is reduced to 1/3 of normal state by using 4 inducing pins.

This paper provides an overview of seismic design considerations for vertical construction joints of a slurry walls used as a permanent basement walls.
Most orbital surgeons believe that it's difficult to restore the primary orbital wall to its previous position and that the orbital wall is so thin that cannot be firmly its primary position. Therefore, orbital wall fractures generally have been reconstructed by replacing the bony defect with a synthetic implant. Although synthetic implants have sufficient strength to maintain their shape and position in the orbital cavity, replacement surgery has some drawbacks due to the residual permanent implants. In previous studies, the author has reported an orbital wall restoring technique in which the primary orbital wall fragment was restored to its prior position through a combination of the transorbital and transantral approaches. Simple straight and curved elevators were introduced transnasally to restore the orbital wall and to maintain temporary extraorbital support in the maxillary and ethmoid sinus. A transconjunctival approach provided sufficient space for implant insertion, while the transnasal approach enabled restoration of the herniated soft tissue back into the orbit. Fracture defect was reduced by restoring the primary orbital wall fragment to its primary position, making it possible to use relatively small size implant, furthermore, extraorbital support from both sinuses decreased the incidence of implant displacement. The author could recreate a natural shape of the orbit with the patient's own orbital bone fragments with this dual approach and effectively restored the orbital volume and shape. This procedure has the advantages for retrieving the orbital contents and restoring the primary orbital wall to its prior position.

For the reasonable analysis of design problems for agricultural facilities, considered the reinforcing effect of thin-wall. The most of agricultural structure is constructed small scale and have many purposes. Thus it has been designed temporary rather than permanent structure, and has relatively large slenderness ratio, small section and semi-rigid condition. Therefore many agricultural facilities are consist of relatively strong frame with weak wall at the viewpoint of stiffness and have not been reflected in the design. But the tension field influences to collapse of structure have already known. Therefore, we need quantification the effect of tension field at structural analysis. In this study, present the method of quantification the effect of tension field that came out thin-plate surrounded by high stiffness frame. The numerical results show that the effect of tension field effect for thin-wall is about 5% of the sectional area of frame in study agricultural facilities.

The coercivity mechanism in permanent magnets was analyzed according to the effects of domain nucleation and domain wall pinning. The coercivity mechanism of a TbCo thin film with high perpendicular magnetic anisotropy was considered in terms of the local inhomogeneity in the thin film. The initial magnetization curves of the TbCo thin films demonstrated domain wall pinning to be the main contributor to the coercivity mechanism than domain nucleation. Based on the coercivity model proposed by Kronmuller et al., the inhomogeneity size acting as a domain wall pinning site was determined. Using the measured values of perpendicular anisotropy constant ($K_u$), saturation magnetization ($M_s$), and coercivity ($H_c$), the inhomogeneity size estimated in a TbCo thin film with high coercivity was approximately 9 nm.
