Effect of Sho-Seiryu-To (XIAO-QING-LONG-TANG) on skin itching and peripheral eosinophil level in three elderly patients

  • Sho-Seiryu-To is known to be effective against allergic diseases, but its effect on skin itching has not been reported. We observed the effect of Sho-Seiryu-To on three elderly patients who, desoite using an anti-allergic drug, has severe chronic skin itching with peripheral eosinophilia. All three patients decreased their peripheral eosinophilia and improved their skin condition within eight weeks. We conclude that Sho-Seiryu-To could be capable of treating elderly patients with chronic skin itching and peripheral eosinophilia.

  • 저자들은 흉부 외상 2개월 후 말초 혈액 호산구증가증이 동반된 호산구성 흉수가 발생한 환자에게서, 가능한 다른 원인들을 감별하고 특별한 치료 없이 경과 관찰하여 호전된 외상 후 호산구성 흉수를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

  • 저자들은 복통과 구토에 동반된 전신 부종을 주소로 내원하여 저알부민혈증과 말초혈액 호산구증가증의 임상 소견으로 단백 소실성 위장증을 동반한 호산구성 위장관염으로 의심되었으나, 내시경검사 소견, 조직 소견, 방사선 소견과 저절로 좋아지는 일과성 임상 경과 등을 종합하여 말초혈액의 호산구 증가증과 조직 내에서 호산구의 심한 침윤을 보인 CMV 감염 소아 Menetrier병 1례를 보고하고자 한다. CMV 감염과 관련된 소아 Menetrier병의 특징적인 임상 증상과 소견을 염두에 두어야 하며, 향후 CMV 감염과 말초 혈액 호산구증가증과 조직내 호산구 침윤의 관련성에 대하여 임상적 해석이 필요하리라 판단된다.

Partial cross-resistance between Strongyloides venezuelensis and Nippostrongylus brasiliensis in rats

  • Rats were immunized through an initial infection with 1,000 filariform larvae (L3) of Nippostrongylus brasiliensis and after complete expulsion of worms they were challenged with 1,000 L3 of Strongyloides venezuelensis to investigate whether cross-resistance developed against a heterologous parasite. Nippostrongylus brasiliensis immunized rats developed a partial cross-resistance against S.venezuelensis migrating larvae (MSL3) in the lungs and adult worms in the small intestine. The population of MSL3 in the lungs were significantly lower (p<0.05) in immunized rats($22.0{\;}{\pm}{\;}7.4$) compared with controls ($105.0{\;}{\pm}{\;}27.6$). The populations of adult worms, egg output and fecundity were initially decreased but from day 14 post-challenge they did not show any significant difference between immunized and control rats. However, the length of worm in immunized rat was revealed as retardation. Peripheral blood eosinophilia was significantly decreased (P<0.05) on day 7 post-challenge and then gradually increased which peaked on da 42 post-challenge when most of the worms were expelled. these results suggest that peripheral blood eosinophilia is strongly involved in the worm establishment and expulsion mechanisms.

Peripheral Eosinophilia and Clinico-radiological Characteristics among Health Screening Program Recipients

  • Background: Eosinophilia is well recognized in specific conditions. The objective of the present study was to determine clinico-radiologic characteristics of eosinophilia and changes in prevalence over 10 years in recipients of private health screening program at a tertiary hospital in Korea. Methods: Data of private health screening program recipients at the health promotion center of Chung-Ang University Hospital from 2004 to 2013 were collected. Health-related questionnaires and laboratory findings of private health screening program with possible relation with eosinophilia were reviewed. Results of enzyme-linked immunosorbent assay (ELISA) for parasite, chest computed tomography, and pulmonary function test were also reviewed. Results: The cumulative prevalence of eosinophilia was 4.0% (1,963 of 48,928). Prevalence of eosinophilia showed a decreased trend from 2004 to 2013. Most cases (96.6%) had mild degree of eosinophilia. Eosinophilic subjects were older and male-predominant. They showed lower levels of forced expiratory volume in 1 second ($FEV_1%$), forced vital capacity (FVC%), and $FEV_1/FVC$ than those without eosinophilia. Eosinophilic subjects showed higher positive rate for common parasite in ELISA than those without eosinophilia. On radiologic findings, consolidation and ground glass opacities were positively associated with the degree of eosinophilia. When eosinophil was classified based on severity, statistically significant correlation between the severity of eosinophil and radiologic abnormalities was found. Conclusion: Eosinophilia is uncommon in healthy population. It usually occurs at a mild degree. Eosinophilic patients have more radiologic abnormalities compared to those without eosinophilia. Such radiologic abnormalities are associated with the severity of eosinophilia.

  • Hypereosinophilic syndrome is a rare hematologic disorder with sustained eosinophilia. Peripheral blood eosinophilia without an underlying etiology and with organs dysfunction has been designated hypereosinophilic syndrome. We report a rare case of symptomatic spinal infiltration of eosinophils at osteolytic T5 in patient with idiopathic hypereosinophilic syndrome.

Toxocariasis and Ingestion of Raw Cow Liver in Patients with Eosinophilia

  • Ingestion of raw animal liver has been suggested as a possible mode of infection of human toxocariasis, We evaluated the relationship between toxocariasis and the ingestion of raw meat in patients with eosinophilia of unknown etiology. The study population consisted of 120 patients presenting with peripheral blood eosinophilia (> $500\;cells/{\mu}l$ or > 10% of the white blood cell count). They were divided into 2 groups: 104 seropositive patients based on a Toxocara excretory-secretory IgG ELISA and 16 seronegative patients. While 25.0% of seronegative patients had a recent history of eating raw cow liver, 87.5% of seropositive patients had this history. Multivariate statistical analysis showed that a recent history of eating raw cow liver was related to an increased risk of toxocariasis. Collectively, it is proposed that raw cow liver is a significant infection source of toxocariasis in the patients with eosinophilia of unknown etiology.

  • Angiolymphoid hyperplasia with eosinophilia(ALHE) is a benign subcutaneous lesion that primarily affects head and neck region. It is characterized by single or multiple nodules in the subcutaneous tissue associated with eosinophilia in the peripheral blood. Kimura's disease, originally reported by Kimura et al., is similar lesion with ALHE in the clinical and histopathological aspects. There has been considerable controversy about the relation between Kimura's disease and ALHE. In Korea, 26 cases of Kimura's disease and ALHE have been reported since 1975. We present a case of recurred ALHE occurring on the left cheek in a 58-year-old woman. Including this case, we summarized all cases that were reported as Kimura's disease or ALHE in Korea through literature review. We also give an outline of clinical and histopathological characteristics of cases reported in Korea.
