백서 두개관세포에서 Ipriflavone이 골조직 개조에 미치는 영향 (Effects of Ipriflavone on bone remodeling in the rat calvarial cell)

  • 본 연구는 ipriflavone(isopropoxysioflavone)의 투여가 백서 두개관세포의 증식과 골조직 개조에 미치는 영향을 알아보고자 시도되었다. 태령 20-21일째의 백서 두개관세포를 분리 배양한 후, $10^{-9}M$부터 $10^{-5}M$까지 농도의 ipriflavone을 투여하고 1일째와 3일째에 MTT분석을 시행하여 흡광도를 평가한 결과, 모든 농도에서 백서 두개관세포의 증식을 보이지 않았다. 한편 골조직 개조에 미치는 영향을 알아보기 위하여 14일째에 alizarin red 염색을 시행하여, 형성된 석회화 결절 면적을 측정하였을 때, $10^{-8}M,\;10^{-7}M,\;10^-6}M$농도를 투여한 경우 석회화 결절 형성이 유의하게 증가하였다 골아세포의 분화에 미치는 영향을 알아보기 위하여 ipriflavone을 투여하고 7일째와 14일째에 추출한 RNA를 역전사 중합효소 연쇄반응(RT-PCR)을 시켜 bone sialoprotein(BSP), type I collagen(COL I) osteocalcin (OCN) 유전자 발현을 관찰한 결과 BSP와 COL I 유전자는 배양 7일째 높은 발현을 보였고, OCN 유전자는 배양 14일째 높은 발현을 보였다. 이상의 연구결과 ipriflavone이 백서 두개관세포에서 석회화를 촉진시키고 골아세포의 분화에 관여하는 BSP, COL I 및 OCN 유전자 발현을 증가시켜 골조직의 개조를 빠르게 할 수 있음을 시사하였다.

클로르헥시딘이 초음파 치석제거기에 의해 발생된 에어로졸 내 세균 수에 미치는 영향 (The effect of chlorhexidine on reduction of viable organisms in aerosol produced by ultrasonic scaler)

  • Purpose: The aerosol generated by ultrasonic scaler can contain bacteria or virus which can penetrate into body through respiratory systems of dentists, dental hygienist or patients. The aim of this study is to evaluate the effect of chlorhexidine digluconate as preoperative mouthrinse or lavage for ultrasonic scaler on the reduction of viable organisms in aerosol produced during periodontal treatment using ultrasonic scaler. Methods: 30 patients with moderate chronic periodontitis were included and divided into 3 groups: Control (no preoperative mouthrinse and tap water as lavage), CHG (preoperative mouthrinse with 0.1% chlorhexidine digluconate and tap water as lavage), CHL (no preoperative mouthrinse and 0.1% chlorhexidine digluconate as lavage). Each patient received scaling or subgingival curettage for 30 min. In CHG group, mouthrinse with chlorhexidine digluconate was performed for 1 min. before treatment. Before, during and after scaling or subgingival curettage, air sampling was performed for 7 min. each (1000 L/7 min.) with trypticase-soy agar plate. Agar plates were incubated in $37^{\circ}C$ aerobically. The numbers of colony-forming units (CFU) were counted and compared. Results: The numbers of CFUs of the samples obtained during treatment were $97{\pm}14.0$ in control, $73.1{\pm}14.9$ in CHG group and $44.5{\pm}9.0$ in CHL group. The difference among the 3 groups was determined to be statistically significant (one-way ANOVA with Bonferroni's correction, p-value: 0.0003). In contrast, the numbers of CFU of samples obtained before and after treatment were not significantly different among the groups. Conclusions: Chlorhexidine digluconate used as preoperative mouthrinse or lavage for ultrasonic scaler can reduce the microorganisms in aerosol produced during periodontal treatment using ultrasonic scaler. Less number of microorganisms were detected when chlorhexidine was used as lavage for ultrasonic scaler.

치태형성 억제세균과 구강내 세균수와의 관계 (RELATIONSHIP OF THE BACTERIA INHIBITING PLAQUE FORMATION AND THE NUMBER OF ORAL BACTERIA)

  • 소아의 구강으로부터 분리한 비수용성 글루캔 형성 억제세균의 Streptococcus mutans 치태 형성에 대한 억제 정도와 분리 세균속의 타액내 농도가 타액내 전체 세균의 농도에 미치는 영향을 본 결과 다음과 같은 결과를 얻었다. 구강으로부터 분리한 비수용성 글루캔 형성 억제세균의 효과를 비커 와이어 검사로 본 결과, 교정용 와이어상에 형성된 치태의 무게가 Streptococcus mutans 단독배양시 152mg에서 Enterococcus durans, Lactobacillus acidophilus, Streptococcus oralis와의 혼합배양시 각각 4mg, 78mg, 72mg으로 감소되었다. Streptococcus mutans의 생균수는 단독배양시 ml당 $3.6{\times}10^8$ 개에서 Enterococcus durans, Lactobacillus acidophilus, Streptococcus oralis와의 혼합배양시 각각 ml당 $1.4{\times}10^6,\;5.6{\times}10^6,\;3.8{\times}10^6$으로 감소하였다. 소아로부터 얻은 타액을 BHI agar에 접종하였을 때의 생균수는 $4.8{\times}10^6$에서 $1.3{\times}10^9$이었으나, 이러한 전체 세균의 농도는 타액내 Enterococcus, Lactobacillus, Streptococcus mutans를 억제하는 Streptococcus의 농도와 관련이 없었다. 이상의 결과를 종합하면 소아의 구강으로부터 분리한 비수용성 글루캔 형성 억제 세균의 Streptococcus mutans 치태형성에 대한 억제는 Streptococcus mutans의 증식을 억제하여 일어났으며, Enterococcus, Lactobacillus, Streptococcus mutans를 억제하는 Streptococcus의 구강내 농도는 구강 즉 타액내 전체 세균의 농도에 영향을 미치지 않았다.

정상 치근과 치주질환에 이환된 치근면의 Electron Probe Microanalysis와 주사전자 현미경에 의한 연구 (Periodontally Diseased Root and Normal Root as Studied by Electron Probe Microanalysis & SEM)

  • Root surfaces affected by periodontal disease undergo various forms of changes. Cementum exposure from gingival recession may result in absorption of calcium, phosphorus, and fluoride and subsequent hypermineralization and increased radiodensity. Although some reports have suggested that inorganic content with root cementum might show various changes depending upon age or extent of periodontal disease, but no consensus can be reached regarding the the distribution of various elements. The present study examines the difference in mineral content between healthy and periodontal diseased roots by analyzing three areas per tooth along the root surface in cervico-apical direction using electron probe and scanning electron microscope. Healthy tooth that was extracted for orthodontic purpose was used as control. Experimental teeth include those with periodontal pocket depth exceeding 6mm and those with gingival recession and periodontal pocket depth of 2-4mm. Levels of Ca, P, Mg and Na were measured using wavelength dispersive x-ray spectrometer at three areas per tooth. The examined areas were located apical to cemento-enamel junction in control and periodontal ligament-depleted areas in experimental teeth. The corresponding areas were also examined with scanning electron microscope(x70) The results are as follows. 1. Minerals were detected in order of Ca, P, Mg and Na. In all root surfaces, levels of Ca and P were higher in dentin than in cementum. 2. Level of Mg was twice as high in dentin than in cementum. There was no significant difference in the level of Mg and Na between normal and periodontal diseased roots or between the various locations in the same root. 3. Level of Ca and P in the surface cementum showed no difference between normal and periodontal diseased root, although the areas in dentin with high level of either ion also showed high level of corresponding ion in cementum. 4. Difference in the Ca and P content between various locations within the same root was noted, although no coherent pattern existed. These results suggest that although the mineral content of the root cementum in periodontitis-affected tooth is affected by exogenous ions from saliva and food, but there was no difference in the mineral contents between normal and periodontally diseased root.

백두옹 추출물의 치주 병인균에 대한 항균효과 (The Antimicrobial Effect of Pulsatilla Koreana Extracts to Oral Micro-Organism)

  • Gingivitis and periodontitis are infectious diseases in that microorganisms are the primary extrinsic cause of the diseases. the occurrence of gingivitis has been associated clearly with the presence of microorganisms at the disease site, and the histologic nature of the tissue involved is indicative of an inflammatory response induced by microorganisms. additional evidence for the microbial etiology of periodontal disease is that numerous antimicrobial agents are effective in reducing plaque accumulation and periodontal diseases. the purpose of this article is to analyze the antimicrobial effects of Pulsatilla koreana. Well-dried Pulsatilla koreana purchased from herbs distributor was ground and extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform($CHCl_3$) and Butyl alcohol(BuOH). we have then applied each solution to the bacteria samples(Bacteroides forsythus, Streptococcus mutans, Streptococcus sanguis, Porphylomonas gingivalis, Actinobacillus actinomycetemcomitans, Eikenella corrodens, Prevotella intermedia, Actinomyces viscosus, Prevotella nigrescens , Rothia dentocariosa, Fusobacterium nucleatum, Pseudomonas aeruginosa, Staphylococcus aureus) collected from several organizations. To conduct susceptibility test(Kirby-Bauer method), plate contained each periodontopathic bacteria is spread extracted into methanol(MeOH), ethylacetate(EtoAc), chlorform($CHCl_3$) and Butyl alcohol(BuOH) and to measure the minimum inhibition concentration(MIC) of the bacteria against the solutions to ultimately determine antimicrobial effects of the solutions, insert bacteria sample into $20{\mu\ell}/{m\ell}$, $10{\mu\ell}/{m\ell}$, $5{\mu\ell}/{m\ell}$, $2.5{\mu\ell}/{m\ell}$ of each solution and control group(not contained solution) 1. Solution extracted into methanol did not show clear zone against all bacteria samples. Only P.nigrescens, S. mutans and S. sanguis in solution extracted into ethylacetate, S. mutans and S. anguis in solutions extracted into chlorform and Butyl alcohol showed clear zone against all bacteria samples. Solution extracted into Butyl alcohol showed clear zone against 13 types of bacteria, excluding P. gingivalis. 2. In Solution extracted into methanol, the bacteria samples grew in the highest concentrated plate, showing minimal variation from control group. 3. In Solution extracted into Butyl alcohol, S. aureus, P. intermedia, E. corrodens, A. actinomycetemcomitans, B. forsythus, P. gingivalis et al. showed decreased growth in the highest concentrated plate. P. auruginosa, R. dentocariosa, A. viscosus, P. nigrescens, S. mutans et al. showed decreased growth at MIC $20{\mu\ell}/{m\ell}$ and S. sanguis showed decreased growth at MIC $10{\mu\ell}/{m\ell}$. 4. By analyzing the MIC level through considering the results from Kirby-Bauer method, Solution extracted into methanol did not reveal any antimicrobial effects and Solution extracted into Butyl alcohol showed the highest antimicrobial effects In conclusion, it can be used the extracts of Pulsatilla koreana as wide spectrum antimicrobial agent.

Retrospective long-term analysis of bone level changes after horizontal alveolar crest reconstruction with autologous bone grafts harvested from the posterior region of the mandible

  • Purpose: The goal of this study was to evaluate the long-term success of horizontal alveolar crest augmentation of the retromolar region of the mandible with particulated bone, as well as factors affecting subsequent peri-implant bone loss. Methods: A total of 109 patients (68 female, 41 male) suffering from alveolar ridge deficiencies of the maxilla and mandible were included in this study. All patients were treated with particulated retromolar bone grafts from the mandible prior to the insertion of endosseous dental implants. Mesial and distal peri-implant crestal bone changes were assessed at six time points. Several parameters, including implant survival and the influence of age, gender, localisation of the implant, diameter, covering procedures, and time points of implant placement, were analysed to identify associations with bone level changes using the Mann-Whitney U-test, the Kruskal-Wallis test, and Spearman's rank-order correlation coefficient. Results: A total of 164 dental implants were placed in the maxilla (n=97) and in the mandible (n=67). The mean observation period was $105.26{\pm}21.58$ months after implantation. The overall survival rate was 97.6% after 10 years. Overall, peri-implant bone loss was highest during the first year, but decreased over time. The mean amount of bone loss after 10 years was 2.47 mm mesially and 2.50 mm distally. Bone loss was significantly influenced by implant type and primary stability. Conclusions: The use of particulated autologous retromolar bone grafts is a reliable technique for the horizontal reconstruction of local alveolar ridge deficiencies. Our results demonstrate that implants placed in augmented bone demonstrated similar bone level changes compared to implants inserted in non-augmented regions.

Bone-level implants placed in the anterior maxilla: an open-label, single-arm observational study

  • Purpose: This study assessed marginal bone remodeling and soft tissue esthetics after the loading of single bone-level implants in the anterior maxilla. Methods: An open, single-arm observational clinical trial with 3 years of follow-up was performed, including 22 implants. The patients presented with a single tooth gap in the anterior maxilla (tooth positions 14-24), with natural or restored adjacent teeth. An implant was placed at least 8 weeks post-extraction and healed submerged for 6 weeks. After the second-stage operation, a fixed provisional prosthesis was provided. The final restoration was placed 6 months after the provisional restoration. The time of the provisional crown connection was considered to be the baseline in this study. Esthetic parameters and the marginal bone level were assessed at 6, 12, 24, and 36 months. Results: All implants were well integrated in the bone. A statistically significant increase was found in the mean implant stability quotient between the time of the provisional prosthesis and the time of the final prosthesis. Most implants (95.5%) revealed marginal bone resorption (<0.5 mm), and just 1 implant (4.5%) showed a change of 2.12 mm from baseline to 36 months (mean $0.07{\pm}0.48mm$), while the crestal bone level decreased significantly, from $2.34{\pm}0.93mm$ at baseline to $1.70{\pm}1.10mm$ at 36 months. The facial gingival margin and papilla were stable and the esthetic scores indicated high patient and dentist satisfaction. Conclusions: Platform-switching bone-level implants placed in maxillary single-tooth gaps resulted in successful osseointegration with minimal marginal bone resorption. The peri-implant soft tissue was also esthetically satisfying and stable.

타액검사기의 신뢰도 평가 (Evaluation of the Reliability of Salivary Testing Instrument)

  • 타액검사기는 방법이 간단하고 단시간 내에 검사를 할 수 있는 장점이 있다. 하지만 타액 채취가 용이하고 장치의 사용이 간단하나 그 결과의 신뢰성에 영향을 미치는 요인에 대해서는 검증해 볼 필요가 있다. 이번 연구는 타액검사기의 측정 결과에 영향을 미칠 수 있는 요인 중 외부변수인 시간적 요인을 분석하기 위해 측정 시간에 따른 검사 결과의 차이를 비교하였다. 또한 내부변수를 파악하기 위해 타액검사기 측정값 사이의 관련성을 분석하였다. 나이, 성별, 질환의 유무에 상관없이 무작위적으로 선정한 20명의 사람들을 대상으로 하여 타액을 채취하였다. 평균 나이는 46.6세였고, 남자 10명, 여자 10명이었다. 채취한 타액을 바로 타액검사기로 측정한 것을 I군으로 하였다. 또한 채취한 타액을 종이컵에서 10분간 공기중에 방치한 후 타액검사기로 측정한 것을 III군으로 하였다. 그리고 I군으로 사용한 것을 30분 후 재 측정하여 II군으로, III군으로 사용한 것을 30분 후 재 측정한 것을 IV군으로 하였다. 실험 결과 cariogenic bacteria, acidity, buffer capacity, blood, leukocyte, protein and ammonia 중 buffer capacity만 제외하고 나머지는 모두 II, IV군에서 유의한 변화가 있었다. 또한 측정값 중 cariogenic bacteria는 leukocyte, protein과 상호관련성이 있었고 buffer capacity는 acidity, protein과, 그리고 protein은 buffer capacity, leukocyte와 연관성이 있었다. 결론적으로 외부변수인 측정 시간에 따른 측정결과 값의 차이는 각각 다르게 나타났으며, 이는 타액검사 시 시간요인의 중요성을 의미하는 것이다. 또한 내부변수인 측정값 간의 상호관계를 이용하여 임상적으로 접근할 경우 그 연관성에 대한 고려가 필요하다.

발광 다이오드(Light Emitting Diode) 전동칫솔의 치은염에 대한 효과: 무작위 배정 임상시험 (The effect of Light Emitting Diode electric toothbrush on gingivitis: a randomized controlled trial)

  • 목적: 이 논문의 목적은 발광 다이오드(Light Emitting Diode(LED))의 유무에 따른 전동칫솔의 항치태 및 항치은염 효과를 치은염 및 경도의 치주염 환자에서 관찰하는 것이었다. 연구 재료 및 방법: 30명의 환자를 실험군, 대조군으로 각각 15명씩 나누어 실험을 진행하였다. 실험군은 정상 작동의 LED 전동칫솔을 사용하였고 대조군은 LED가 나오지 않도록 개조된 동일 제품의 전동칫솔을 사용하였다. 기준(Baseline), 2주, 4주 후의 임상지표($L{\ddot{o}}e-Silness$ gingival index (GI), Quigley-Hein plaque index (PI))를 측정하여 Wilcoxon signed rank test, Mann-Whitney test를 통해 통계적 분석을 시행하였다. 결과: 실험군과 대조군 간의 시간에 따른 GI의 변화는 두 군 모두 시간에 따른 감소를 나타내었으나, 2주, 4주차에서 유의한 차이를 나타내었다. PI는 4주차에서 실험군에서 대조군보다 낮은 수치를 나타내었는데, 통계적으로 유의한 차이는 아니었다. 결론: 본 연구에서 LED가 장착된 전동 칫솔은 단기간의 연구에서 치은염의 감소효과를 보였고, 장기간의 관찰 시 치태의 축적을 감소시킬 수 있다 추측할 수 있었다.

테이퍼드 직선형 SLA 임플란트의 안정성 평가 (Evaluation of the stability of sandblasted, large-grit, acid-etched implants with tapered straight body design)

  • 목적: 임플란트 표면의 특성과 임플란트 디자인은 성공적인 초기 고정을 얻는데 중요한 변수이다. 이 연구의 목적은 치유기간 동안 테이퍼드 직선형 디자인을 가진 SLA (Sandblasted and Acid-etched) 임플란트의 ISQ (Implant Stability Quotient) 값을 측정하고 임플란트의 안정성에 미치는 영향을 평가하는 것이다. 연구 재료 및 방법: 임플란트의 안정성을 측정하기 위하여 26명의 환자(여자 13명, 남자 13명)의 44개 임플란트에 대한 자기공명주파수분석(Resonance Frequency Analysis)을 실시하였다. 골질 및 골량에 대한 임상적 평가는 Lekholm & Zarb (1985)의 기준에 따라 시행하였다. 시간경과에 따른 임플란트 안정성변화(총 ISQ 값)를 고정체 식립 시 그리고 12주후 치유지대주 연결 시 측정하고 RFA와 식립토크, RFA와 골질, RFA와 상, 하악골 사이의 상관관계를 평가하였다. 결과: 임플란트의 평균 ISQ값은 임플란트 식립 당시(기준선) $69.4{\pm}10.2$, 치유지대주연결에서(두번째수술) $81.4{\pm}6.9$였다(P < 0.05). RFA와 골질, RFA와 상하악골 사이에 유의한 차이가 있었다(P < 0.05). RFA와 식립토크, 식립부위, 고정체 직경, 임플란트 길이 사이에는 유의한 차이가 없었다(P > 0.05). 결론: 본 연구의 제한된 범위내에서 테이퍼드 직선형 디자인을 가진 SLA 임플란트의 ISQ값은 고정체 식립시와 치유지대주 연결 시 모두 높은 값을 보였다. 따라서 테이퍼드 직선형 디자인을 가진 SLA 임플란트는 초기안정성 및 2차 안정성을 향상시킬 수 있으며, 즉시 또는 초기하중을 적용할 수 있을것으로 사료된다.
