  • It will be possible to predict the success and failure of the prosthodontic treatment and prevent clinical complications if the oral environment including prostheses and their supporting teeth and periodontium can be monitored in real time. The aim of this report is to introduce the concept of a smart prosthesis, which monitors specific factors in the oral cavity, and investigate its feasibility through a literature review of MEMS (Micro-electro-mechanical System) and Biosensing.

Hard tissue augmentation to minimize a risk of buccal gingival recession in lower anterior teeth associated with orthodontic treatment: case report (교정 치료와 관련된 하악 전치부 협측 치은 퇴축의 위험성을 최소화하기 위한 골이식 : 증례보고)

  • Patients with malocclusion may present with preexisting mucogingival problems susceptible to attachment loss during or after orthodontic treatment. Lower anterior teeth especially show a high prevalence of gingival recession following orthodontic treatment. This case report demonstrates hard tissue augmentation of labially thin or deficient alveolar bone (dehiscences and fenestrations) to prevent attachment loss during or after orthodontic treatment. Three patients presented clinically prominent root surfaces and dehiscences and fenestrations on cone-beam computed tomography (CBCT) in lower anterior teeth. Labial hard tissue augmentation of lower anterior teeth was performed with deproteinized bovine bone mineral and collagen membrane. Six months later, hard tissue augmentation reduced root prominence and created a greater volume of hard tissue on lower anterior area in clinical and radiographic findings. Hard tissue augmentation using xenograft could prevent attachment loss associated with orthodontic treatment and maintain stability of healthy periodontium.

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

  • The purpose of this study was to determine the displacement of prosthesis & abutment and the stress distribution patterns induced in the periodontium by applying force to the fixed prosthesis. Two levels of periodontal support were compared using two-dimensional finite element stress analysis after placement of 3unit or 4 unit fixed partial denture(FPD) in case of missing of the lower first molar. Concentrated vertical load was delivered at the cusp tip of the second bicuspid or the central fossa of the pontic. The following results were obtained : 1. The greater the loss of alveolar bone in abutment teeth area, the greater the displacement of FPD and the stress concentration in alveolar bone around abutment. 2. The amount and direction of displacement and distribution of stress in the 4-unit FPD was better than those in the 3-unit FPD. 3. Multiple abutments reduced the amount of mesial and downward displacement of the weaked abutments and more uniformly distributed the stresses.

  • The aims of the present study are firstly to investigate the amount of bone loss around non-submerged implants placed in the posterior region and secondly to investigate the relationship between inter-implant and implant-tooth distance and peri-implant bone loss. Thirty-one subjects with 60 implants were selected consecutively from the implant patient pool at the department of Chonbuk National University Hospital. To be included in the study subject, the implant should have been functioned more than 6 months after loading. Inter-implant and implant-tooth distance, distance between implant shoulder and the first bone contact with the implant(DIB) were measured from the scanning image of the radiograph of each implant. The result showed that; 1. inter-implant distance has a statistically significant relationship with DIB in Pearson correlation analysis. 2. the DIB at the implant facing surface of the implant was greater than that of tooth facing surface of the implant. Within limitation of this study, it is suggested to place an implant not too closely to adjacent implants, and the presence of a tooth adjacent to an implant may keep the level of tooth-facing surface of the implant. Further studies with a prospective design are needed to elucidate the relationship between bone changes and various dimensions around implants.

  • The purpose of the present study was to evaluate the marginal discrepancy and topography of artificial crown on teeth extracted due to severe periodontal disease. Twenty specimens were invested into metamethylacrylate resin and cutted into vertical slices along with the long axis of tooth. The selected marginal discrepancy between the outer edge of the crown and the finishing line of abutment was examined by stereo- microscope(Olympus, PM-VSP-3, Japan) at magnification of up to 10, and the topography of finishing margin on crown was observed by stereomicroscopeat magnification of up to $70{\times}$. The results were as follows. (1) The mean marginal discrepancy between extracted tooth and artificial crown were $50.82{\mu}m$. (2) There was a considerable difference in the microstructure of finishing margins among specimens. Microscopic Structure on finishing margin showed indefinite line, poor fit (open, underextended and overextended), distorted margin, and surface roughness. This study suggested that there could be necessary to consider the response of periodontium to the emergence profile of natural tooth and precision of marginal geometry while establishing treatment planning for the reconsruction of the artificial crown.

A CONSERVATIVE APPROACH FOR THE NON-INFLAMMATORY GINGIVAL RECESSION IN MIXED DENTITION (혼합치열기 아동의 비염증성 치은퇴축에 대한 보존적 접근)

  • The occlusal disharmonies resulted from labially protruded or malpositioned teeth can damage the periodontium and induce the non-inflammatory gingival recession. For these cases, a conservative approach was performed by improving oral hygiene and correcting the axial and positional status of the gingivally recessed teeth and removing the prematurely contacted areas. In some cases, rapid remission of tooth mobility and gradual decrease of gingival recession was observed just after start of treatment. In cases of gingival recession in permanent lower incisors of the children with mixed dentition, the treatment of choice is non-surgical conservative approaches. In cases when the gingival inflammation can be controlled through reinforcing the oral hygiene, when attached gingiva have a potential to increase in width through growth (not more than 1 year after eruption or not yet arrived at adult level), and when the recession can be corrected by moving the teeth from labial cortical plate through orthodontic treatment, the conservative measures would be the first choice. On the contrary, when recession has exceeded beyond the level of CEJ, when the gingival inflammation existed with the cause of poor oral hygiene, when the attached gingiva have little potential to increase (for example, more than 8 years after eruption), and when the conservative measures yielded no benefit after 4-8 weeks of treatment, the surgical approaches should be sought.

Protective Effect of HP08-0106 on Ligature-induced Periodontitis in Rats

  • Periodontitis is an inflammatory disorder of the periodontium, characterized by destruction of the tooth supporting tissues including alveolar bone and mediated by various pro-inflammatory mediators. Here, we demonstrated that HP08-0106, composed of four crude drugs-Gardenia jasminoides Grandiflora, Angelica gigas Nakai, Rehmannia glutinosa, and Schizonepeta tenuifolia in a weight ratio of 2:2:1:2, perturbs inflammatory responses, osteoclast formation in LPS-induced RAW 264.7 cells and alveolar bone resorption in ligature-induced periodontitis. HP08-0106 decreased the protein level of iNOS and COX2 as well as the secreted level of IL-$1{\beta}$, indicating that HP08-0106 has antiinflammatory effects. HP08-0106 also inhibited the expression of genes associated with osteoclastogenesis including c-Fos, MMP-9 and TRAP. Moreover, HP08-0106 exhibited a protective effect from alveolar bone loss in ligature-induced periodontitis animal models. Our results strongly suggest that HP08-0106 represent an important therapeutic tool to treat inflammatory disorders associated with bone loss such as periodontitis.

S대학 신입생의 구강위생상태 및 치주조직질환의 이환율에 관한 연구

  • In order to appreciate the school dental services of the primary, middle, and high school and to get the basic data for the university dental service programs in Korea, the author had examined the oral health status of 1,300 freshmen in S University. Then, the followed results were obtained from the calculation and evaluation of DEMT index, DT index, MT index, FT index, DT rate, MT rate, FT rate, debris deposit rate, calculus deposit rate, periodontal disease rate, and abnormal jaw relation rate. 1. DMFT index in the freshmen was higher than that of the Korean youth in the same age. 2. It seems the above result to be appeared from the influence of the intake of the more carbohydrate foods in the freshmen. 3. In the freshmen, MT rate was lower, and FT rate higher than in the Korean youth in the same age. 4. The oral hygiene condition was good in 3.15% of the freshmen. 5. Calculus was not deposited in 8.69% of the freshmen. 6. In 9.69% of the freshmen, the periodontium was healthy. 7. In Korea, the school dental services in the primary, middle, and high schools must be improved. 8. There was the abnormal jaw relation in 12.92% of the freashmen.

Clinical effectiveness of combining platelet rich fibrin with alloplastic bone substitute for the management of combined endodontic periodontal lesion

  • The term "endo-perio" lesion has been proposed to describe the destructive lesion resulting from inflammatory products found in varying degrees in both the periodontium and the pulpal tissues. In most of the cases, clinical symptoms disappear following successful endodontic therapy. However failure after conventional root canal treatment calls for surgical intervention. A 35 year old male patient with endo-perio lesion in right maxillary lateral incisor was treated with platelet rich fibrin (PRF) and alloplastic bone substitute after conventional endodontic therapy. At the end of 6 months there was gain in clinical attachment, increased radiographic bone fill and reduction in probing depth which was maintained till 18 month follow-up. Present case report aims to evaluate the efficacy of PRF and alloplastic bone substitute in the management of intrabony defect associated with endo-perio lesion in maxillary lateral incisor because the healing potential of PRF and bone graft has not been widely studied in endodontics. The use of PRF allows the clinician to optimize tissue remodelling, wound healing and angiogenesis by the local delivery of growth factors and proteins. The novel technique described here enables the clinician to be benefited from the full regenerative capacity of this autologous biologic material.
