Changing Prevalence of Helicobacter pylori Infections in Korean Children with Recurrent Abdominal Pain

  • Purpose: The aim of this study is to investigate the changing prevalence rate of Helicobacter pylori infection in children, of different age groups, with recurrent abdominal pain over a 10-year period. Methods: Children with recurrent abdominal pain who visited the pediatric outpatient clinic at university hospital were screened for H. pylori. Children were divided into 3 age categories of 4-5, 6-11, and 12-16 years. To study the changes in the annual prevalence rates of H. pylori infection, the study period was divided into 3 time periods: 2004-2007, 2008-2010, and 2011-2014. Urea breath test was performed for all children aged 4-16 years, with a cut-off value of 4.0‰ for children aged ${\geq}6$ years and 7‰ for children aged <6 years. Results: A total of 2,530 children (1,191 boys) with a mean age of $10.0{\pm}3.0years$ (range, 4.0-16.9 years) were included in the study. The total prevalence of H. pylori infection was 7.4% (187/2,530). The prevalence rate of H. pylori infection in children with recurrent abdominal pain was 8.0% (70/873) in 2004-2007, 7.7% (51/666) in 2008-2010, and 6.7% (66/991) in the 2011-2014. Nevertheless, a significant difference was observed in the prevalence rate between children <12 years old and ${\geq}12$ years of age (p=0.018). Conclusion: The prevalence of H. pylori infection in Korean children with recurrent abdominal pain was 7.4%, showing no significant decrease in the last 11 years; however, the prevalence rate in children <12 years old was significantly lower than that in those ${\geq}12$ years old.

  • Purpose: The purpose of this study was to investigate the effects of the professional nurse's postpartum breast-feeding support on breast-feeding prevalence for mothers who delivered in Baby-Friendly Hospitals (BFH). Methods: This quasi experimental study was designed with a nonequivalent control group post test. The subjects of this study were 55 mothers who were hospitalized in the delivery room of a university hospital which was selected as a BFH in Daegu from October 1, 2005 to June 30, 2006. Twenty nine mothers were assigned to the experimental group and 26 mothers to the control group. Postpartum breast-feeding support by nurses' telephone calls to the experimental group was provided once a week for 4 weeks postpartum and then once a month for 16 weeks postpartum. Four post tests were given at postpartum week 8, 12, and 16. The control group was given a telephone call at postpartum week 4, 8, 12, and 16. Results: The breast-feeding prevalence of the experimental group was significantly higher than that of the control group for each period. Conclusion: Postpartum breast-feeding support by nurses may be a useful intervention to increase breast-feeding prevalence.

A Survey of Chicken Coccidiosis in Slaughtered Chickens (도계장 출하닭에 대한 구포자충 감염 실태 조사)

  • The present study was conducted to investigate the prevalence of Eimeria infection in 1,300 slaughtered chickens from 130 farms in Gyeongnam, Jeonnam, Jeonbuk, Gyeongbuk and Chungnam during the period from October 2002 to August 2003. The results obtained are summarized as follows: 1. The prevalence of Eimeria infection in 130 chicken farms was 76.9% and Eimeria infection rates of Gyeongnam, Jeonbuk, Gyeongbuk, Jeonnam and Chungnam were 80%, 75%, 70%, 42.9%, respectively. That of Tongyoung was the highest infection(94.4%) than others districts in the Gyeongnam. 2. The prevalence of oocysts infection level in 1,300 slaughtered chickens from 5 districts was 33.4% and that of Jeonbuk was the highest rate(42%) and Gyeongbuk has the lowest rate(20%). Prevalence of OPG level of slaughtered chicken in Tongyoung was the highest rate(42.8%) in Gyeongnam. 3, Mixed infection rate of over types 2 was 79% and triple infection was shown the highest infection(33%) among the infection types. 4. Kinds of E. spp. were E. acerulina, E. mivati, E. necatrix, E. maxima, and E. tenella. E. tenella has the highest infection rate(78%) and E. necatrix has the lowest infection rate(36%) among the Eimeria spp. 5. The monthly prevalence of Eimeria infection was shown higher in July(94.4%) compared with other months and Jun was shown lower(57.9%). But, level of OPG was shown higher in summer season(July, August; over 40%) than that of winter season(January, February; 23.1%, 16%).

Menstrual Cycle Characteristics and Premenstrual Syndrome Prevalence Based on the Daily Record of Severity of Problems in Korean Young Adult Women

  • Purpose: This descriptive study aimed to identify the menstrual cycle characteristics and premenstrual syndrome (PMS) prevalence in Korean young adult women using the retrospective and prospective Daily Record of Severity of Problems (DRSP). Methods: In the first stage, participants included 151 nursing students studying in a university located in Seoul. Data were collected from April 20 to June 2, 2017, using the questionnaire on menstrual characteristics, pictorial blood assessment chart, and retrospective DRSP. In the second stage, participants included 17 students with PMS, based on the screening conducted in the first stage. Data were collected using the prospective DRSP from May 29 to 2 September 2, 2017. Results: Of the study sample, 104 participants (68.9%) had regular periods. Those with regular periods had 11.97 periods annually with a menstrual cycle of 29.38 days and a period duration of 5.72 days. Fifty-five participants (37.4%) showed menorrhagia. Sixty-four participants (42.4%) were found to have PMS based on their retrospective DRSP. When the ratio of women (52.9%) with PMS shown in the prospective DRSP was used as a positive predictive value, the estimated PMS prevalence was 22.4%. Conclusion: This study provides clinically significant PMS prevalence among Korean young adult women, positive predictive value of the retrospective DRSP, and valid data to basically understand the menstrual cycle characteristics experienced by these women.

Analysis of Prevalence and Risk Factors for Latent Tuberculosis Infection among Healthcare Workers (의료기관 종사자의 잠복결핵감염 유병률과 위험요인 분석)

  • Purpose: The purpose of this study was to provide basic data on the infection prevention management program, which is one of the infectious disease control program by identifying the prevalence and risk factors of latent tuberculosis infection (LTBI) in healthcare workers. Methods: We surveyed a total of 3,046 LTBI test results, including those of 2,269 existing staff and 777 new employees. An interferon-gamma release assay (IGRA) for the diagnosis of LTBI was performed using QuantiFERON®-TB Gold In-Tube (QFT-IT). The risk factors of LTBI were analyzed using logistic regression analysis. Results: The overall prevalence of LTBI was 16.0% (487/3,046). The prevalence of LTBI in the existing staff was 17.9% (406/2,269) and the prevalence of LTBI in new employees was 10.4% (81/777). Multivariate logistic regression analysis revealed that the risk factors of latent tuberculosis infection among the existing staff were gender, age and work period wheres, the risk factor amongst the new employees depended on their age. Conclusion: The LTBI was not related to the type of occupation and work unit. Therefore, while establishing an infection control program for the prevention of tuberculosis infection at medical institurions, institutional heads and infection control experts should encompass a policy for all the employees.

Prevalence and Epidemiological Characteristics of Endoscopically Proven Reflux Esophagitis in Children in Korea

  • Purpose: The prevalence of reflux esophagitis (RE) has increased recently in Korea. Little is known concerning the prevalence and characteristics of RE in pediatric patients. This study investigated the prevalence and influence of risk factors in endoscopically proven RE in Korea in pediatric patients over a period of 14 years. Methods: A retrospective chart review of all patients between the ages of 1 month and 20 years who underwent esophagogastroduodenoscopy at Samsung Medical Center between 2001 and 2014 was carried out. Univariate and multivariate analyses were conducted to identify independent risk factors for RE. Results: The prevalence rate of endoscopically proven RE in this study was 28.7% (978/3,413). The prevalence of RE increased from 11.8% from 2001 to 2007 to 37.7% from 2008 to 2014. Multivariate logistic regression analysis revealed that residency in the Greater Gangnam area (odds ratio [OR], 1.21; 95% confidence interval [CI], 1.02-1.44) and age (OR, 1.13; 95% CI, 1.11-1.15) were significant predictive factors for the presence of RE. Conclusion: The prevalence rate of endoscopically proven pediatric RE has increased over the past 14 years. Residency and older age are more important independent risk factors for pediatric RE in Korea.

Combination Tendency Analysis on Herbal Formula to Treat Insomnia Focused on Zizyphi spinosi Semen (불면증에서 산조인의 유무에 따른 약물배오의 경향성 고찰)

  • Objectives : Zizyphi spinosi Semen is widely used for treating insomnia in korea and other oriental country. This study is analyzing combination tendency of Zizyphi spinosi Semen in herbal formula for treating insomnia. Methods : Herbal formulas for treating insomia were searched in "The Prescription Dictionary of Traditional Chinese Medicine", and checked in each literary source. Then, the herbal formula was divided in two groups : FCZ(The formula group containing Zizyphi spinosi Semen) and FWZ(The formula group without Zizyphi spinosi Semen). FCZ and FWZ were compared by following criteria; i) Prevalence of herbal formula by period ii) Prevalence of medicinal material's effects iii) Prevalence of medicinal material iv) Prevalence of the nature of midicinals v) Prevalence of flavor of medicinals vi) Prevalence of meridian entry. Results : 116 Herbal formula met our criteria, and herbal formulas were divided in two groups : FCZ(n=59), FWZ(n=57). The result of prevalence in FCZ is following; Herbal formula prevalence increased as time passed to the Qing(淸) dynasty. Tonifying and replenishing medicinal(補益藥), tranquillizing medicinal (安神藥), and inducing diuresis to drain dampness medicinal(利水滲濕藥) are the three most prevalent types of materia medica. In specifically, Panax ginseng radix(人蔘), Liriopis Tuber(麥門冬), Angelicae Gigantis Radix(當歸), Poria Cocos(茯神), Glycyrrhizae Radix(甘草), Poria cocos(茯神), Polygalae Radix(遠志), Thuja Orientalis L.(柏子仁), Paeonia Japonica(白芍藥), and HgS(硃砂) are combined frequently. The most prevalent nature of medicinals were 'warm microwarm(溫 微溫)' and flavor of medicinals was normal(平). Meridian entrys that frequently combined were heart(心), spleen(脾), and liver(肝). Conclusion : We found combination tendency of Zizyphi Semen in herbal formula for treating insomnia.

Relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora (유지치주치료기간과 치은연하세균 출현율의 관계)

  • This study was performed to evaluate the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora. The subgingival plaques from 108 patients were used in the study. Control group were the patients with no periodontal treatment and test groups were assigned into 3 groups according to the period of recall check: group 1; 1-2 months, group 2; 3-4 months, group 3; 6months or more. The polymerase chain reaction (PCR) used for direct identification of periodontal pathogens (P. gingivalis, T. forsythensis, T. denticola) in subgingival plaque. The results of this study were as follows. 1. The prevalence of P. gingivalis, T. forsythensis, T. denticola in control group were 100%, 87%, 90%. 2. In clinical parameters such as plaque index, gingival index, bleeding on probing, control group was not significant different with group 1 but Significant different with group 2, group 3. 3. In group 1, the majority of P. gingivalis had type II fimA. 4. When group 3 were compared with group 1, the prevalence of P. gingivalis increased. But the prevalence of P. gingivalis with type II fimA, which have the virulence factor, decreased. 5. We were unable to find the correlation between P. gingivalis with type IV fimA and periodontal disease. 6. The prevalence of T. forsythensis, T. denticola in test group were 85%, 93% or more. From the above results, we were able to find the relation between the interval of supportive periodontal therapy and the prevalence of the subgingival microflora and the need of the strict supportive periodontal therapy to prevent recurrence of periodontal disease, because there were high prevalence of periodontal pathogens.

The Prevalence and Genotype of Human Papillomavirus from Patients with Genital Warts in Eastern Guangdong Province

  • Background: Low-risk human papillomavirus (LR-HPV) infection is the main cause of genital warts. LRHPV genotypes 6 and 11 are associated with genital warts, but there have only been a few published studies about the genotype-specific prevalence of HPV in genital warts in China. The objective of our study was to assess the prevalence of HPV genotypes for clinical cases involving both men and women and to evaluate the potential benefit of a quadrivalent (genotypes 6, 11, 16, and 18) HPV vaccine in eastern Guangdong province of China. Materials and Methods: A total of 696 eligible patients with genital warts were enrolled during the period Aug 2009 through Oct 2014. Specimens were collected from genital warts, the HPV GenoArray test was used for HPV detection and genotyping, which could detect 21 HPV genotypes, including genotypes 6, 11, 16, and 18. Results: Among the 696 cases, 675 samples were successfully genotyped. The median age of patients was 32.1 years (range, 16-67 years). The most prevalent genotypes were HPV-6 (285/675, 42.2%), HPV-11 (265/675, 39.3%), HPV-52 (52/675, 7.7%), HPV-16 (51/675, 7.56%), HPV-81 (50/675, 7.40%) and HPV-58 (37/675, 5.48%). Low-risk genotypes predominated, with a prevalence of 96.59%. The cumulative prevalence of genotypes 6 and 11 was 78.7% (531/675), the cumulative prevalence of genotypes 16 and 18 was 11.6% (78/675), and the cumulative prevalence of genotypes 6, 11, 16, and 18 was 82.5% (557/675). Conclusions: Our results provide strong evidence that, in eastern Guangdong, different from Western countries, the most prevalent low risk HPV genotypes in patients with genital warts are 6, 11 and 81. The quadrivalent HPV vaccine could prevent 82.5% of genital warts in eastern Guangdong.

Prevalence of Posttraumatic Stress Disorder Among North Korean Defectors During Preparation for Early Settlement

  • Objective : This study examined the prevalence of posttraumatic stress disorder (PTSD) among North Korean defectors during preparation for early settlement. In addition, the correlation between PTSD and other mental disorders with suicidal ideation was assessed. Methods : A total of 639 North Korean defectors aged 19-65 years were included from resettlement and training centers. PTSD was assessed using the PTSD Diagnostic Scale (PDS). The prevalence of other mental disorders and suicidal ideation were assessed using the Korean version of the Psychiatric Diagnostic Screening Questionnaire (K-PDSQ). Results : A total of 6.8% of North Korean defectors had a PDS score >23, which was suggestive of a diagnosis of PTSD. Prior repatriation to North Korea was significantly associated with PTSD. The defectors with PTSD had a higher prevalence of other mental disorders (major depressive disorder prevalence: adjusted odds ratio [AOR], 14.5; 95% confidence interval [CI], 6.7-31.2, panic disorder prevalence: AOR, 9.5; 95% CI, 4.6-20.0, generalized anxiety disorder prevalence: AOR, 5.6; 95% CI, 2.4-13.2). No significant association was found between suicidal ideation and PTSD. Conclusion : North Korean defectors had a relatively high prevalence of PTSD and suicidal ideation during preparation for early settlement. Cohort studies are needed to assess the lasting effects of PTSD on North Korean defectors' settlement in Korea for a longer period.
