Factors Predicting Medication Compliance among Elderly Visitors of Public Health Centers (보건소 방문보건 대상 노인들의 투약 지시이행에 미치는 영향요인 분석)

  • Purpose: The purpose of this study is to investigate compliance with prescribed medication in the elderly visiting public health centers. Method: Data were collected from 665 elders living in Seoul and the Gyeonggi Province during the period from February 21 to June 30 in 2006. The data were collected through individual interviews and were analyzed using correlation and multiple regression analysis with the SAS 9.1 program. Results: The mean of medication compliance was 2.97(${\pm}.68$) on a 5-point Likert scale. Specifically, compliance 3.14(${\pm}.70$) for medication dose, 2.94(${\pm}.77$) for medication frequency, and 2.84(${\pm}.79$) for medication time. The elderly with a higher level of education (${\beta}=.095$, p<.001), with health insurance (${\beta}=.208$, p=.0009) and with a higher level of family support (${\beta}=.040$, p=.0306) showed a higher level of mediation compliance. Female elders (${\beta}=.142$, p<.001) kept higher medication compliance than male ones. Conclusion: These findings suggest that people with low education, low socioeconomic status and less family support need more education before medication. The evaluation of medication compliance needs to be encouraged before starting medication to distinguish those who may not comply with medical prescription. Predictive factors identified in this study must be considered when designing interventions, program development and education for appropriate medication management for the elderly.

A Feasibility Study of an Exercise Intervention Based on Self-efficacy Theory among Overweight and Obese College Students in Korea (과체중 및 비만 대학생의 체력증진을 위한 자기효능이론 기반 운동프로그램의 효과검증)

  • Purpose: This study explored the feasibility of a 12 week self-efficacy based exercise intervention for overweight and obese college students. Method: A repeated measure, 4-group randomized controlled design was used. Students were recruited from two universities. in Seoul. Korea using: (1) flyers either on the campus bulletin boards or in campus restrooms: (2) advertisement on campus e-board: and (3) self-referral. Inclusion criteria were: (1) Korean college students aged 18 to 29: (2) BMI >23: (3) viscerally obese: and (4) accessible to a phone and a computer. Participants completed three self-reported questionnaires: socio-demographic questionnaire (at the baseline), Exercise Self-Efficacy Scale (at the baseline and $12^{th}$ week), and Physical Fitness sub-subscale (at the baseline and $12^{th}$ week). Additionally. physiological data (height, weight, blood pressure) were collected at the baseline and $12^{th}$ week. Descriptive statistics and a two-way mixed ANOVA were performed using SPSS12.0. Results: No significant group difference was observed. However. students with increased exercise self-efficacy during the program showed better physical fitness in the $12^{th}$ week. When students' physical fitness was enhanced. the physiological factors were improved. Conclusions: Self-efficacy based exercise intervention may be applicable and acceptable to the college students. It is necessary to conduct a replicated study with a larger sample and an elongated intervention period.

The Rapid Drug Susceptibility Testing of Mycobacterium tuberculosis by GenoType$^{(R)}$ MTBDRplus in Contaminated Specimen

  • There are several methods currently being used to diagnose tuberculosis in patients, such as smear, PCR, tuberculosis culture and X-ray. For a proper medical treatment, antimicrobial susceptibility test and rapid drug susceptibility testing have been operated. Tuberculosis bacilli usually need 3~8 weeks of culture period because of delay in RNA synthesis and require 15~22 hours for generation. After a germ raises in culture, we initiated antimicrobial susceptibility test for a proper treatment. It has some difficulties to give a proper prescription for a tuberculosis patient because antimicrobial susceptibility test requires 4 weeks. To supplement this, we are practicing drug susceptibility testing which allow us to know the sensibility of RMP and INH after 2 or 3 days. But this is only possible when more than 2 positive germ. Therefore, we should practice rapid drug susceptibility testing with culture test. But if media is contaminated by other germs except Mycobacterium tuberculosis, it's hard to interpret result about culture test and to practice antimicrobial susceptibility test and rapid drug susceptibility testing. Because we have to practice again smear, culture test after extracting specimen from the patient, time is consumed and proper patient treatment is postponed. To address these problems and quick patient treatment, rapid drug susceptibility testing is practiced by using GenoType$^{(R)}$ MTDRplus method. As a result of this method we detected sensibility 10 and 7 cases and resistance 0 and 3 cases using RIM and INH respectively with other 1 case toward medicals out of the total 11 test. In conclusion rapid drug susceptibility testing can be used from the contaminated specimen after elimination of contaminated source from culture and proved that it can be practiced for rapid examination of a tuberculosis patient.

A Study on the Trend in Korean Medicine Research of Irritable Bowel Syndrome in Korea, Japan and China (한국, 일본, 중국 3국의 과민성 대장 증후군에 대한 한의학적 연구동향 분석)

  • Objectives : The purpose of this study was to analyze the papers from 2011 to 2020 to study on the trends of irritable bowel syndrome (IBS) in Korea, Japan and China. Methods : In order to analyze the research trends on IBS in Korea, Japan and China, domestic and foreign databases were selected to provide academic resources to the common use of telecommunication lines represented by the Internet. The latest research trends were determined by limiting the 10-year period from January 2011 to December 2020. Results : In the past decade, six IBS-related papers have been searched in Korea, six in Japan and 458 in China. Although not many studies have been found in Korea and Japan to understand the research trends, there have been many clinical reports, and researchers are more interested in herbal medicine than acupuncture treatment. In China, where many papers were searched, there were more papers related to herbal medicine than acupuncture treatment. Acupuncture treatment was highly interested by researchers in certain acupoints and stimulation methods, and herbal medicine was found to seek diversity in drug prescription and use. Conclusions : In this study, we were able to understand the research trends of Korea, Japan, and China, and provided information that could be useful as a basic data for establishing the direction of Korean medical research on IBS in the future.

Preparation and Hypoglycemic Effect of Reconstituted Grain Added with Selected Medicinal Herb Extract (한방 생약재 추출물을 첨가한 재성형 당뇨쌀의 제조 및 혈당강하 효과)

  • Hot-water extract from selected medicinal herbs of diabetes prescription was mixed with brown rice and pearled barley flours and extruded to prepare a reconstituted grain for diabetes millitus patients. Even though the cooked reconstituted grain containing medicinal herb extract was somewhat different from cooked ordinary rice in textural properties measured by texture analyzer, it was estimated to be organoleptically acceptable in sensory parameters. The reconstituted grain added with medicinal herb extract reduced the rate of dialysis of glucose evaluated by in vitro dialysis experiment. The effects of reconstituted grain diet on blood glucose levels in diabetes patients were studied during 3-week period. Compared to normal diet, the reconstituted grain diet for 2 weeks significantly decreased the fasting and 2-hr postprandial blood glucose levels in diabetes patients by 14% and 10%, respectively. These results suggested that the reconstituted grain could be used as an effective therapeutic diet for the control of diabetes mellitus.

  • In this study, the normal group and possible sarcopenia(PS) group were divided by the criteria for sarcopenia diagnostic based on the 7th period data of the National Health and Nutrition Examination Survey. An independent t-test and logistic regression was performed for the difference between the two groups. As a result, the PS group was older, and height, weight, and BMI were all lower. The waist circumference was significantly lower in men but significantly higher in women. According to the quality of life by domain, the frequency of impairment in daily activities in exercise capacity was 8.28 fold in men and 15.52 fold in women. In addition, aerobic exercise regularity was 19% and 26% lower in the PS, and strength exercise regularity was practiced was 52% and 43% lower.

Reirradiation with Robotic Stereotactic Body Radiotherapy for Recurrent Nasopharyngeal Carcinoma

  • Background: Recurrent nasopharyngeal carcinoma (NPC) after previous radiotherapy is challenging. There is no standard approach for salvage treatment. Here we present toxicity and treatment results for recurrent NFC patients who underwent fractionated stereotactic radiotherapy (FSRT) as second line radiotherapy (RT). Materials and Methods: Between April 2009 and July 2012, 24 patients, with a male to female ratio of 3:1, were treated with CykerKnife$^{(R)}$ FSRT for recurrent NFC in our institution. Seven out of 24 patients had metastatic recurrent disease. Median age was 53 years (range, 20-70 years). Initial RT dose was 70Gy. The time period between initial RT and FSRT was a median of 33.2 months. The median prescription dose for FSRT was 30Gy (range, 24-30 Gy) in a median of 5 fractions (range, 4-6). Results: The median follow-up for all patients was 19.5 months (IQR: 12.2.-29.2 months). The locoregional control; progression free survival and overall survival (OS) rates for 1-, 2- and 3-year were 64%, 38%, 21%; 60%, 30%, 17% and 83%, 43%, 31%, respectively. Median OS for the entire cohort was 22 months (95% CI: 16.5-27.5). On multivariate analysis recurrent tumor stage was the only prognostic factor for OS (p=0.004). One patient exhibited grade III temporal lobe necrosis. One died because of grade IV mucositis and overlapping infection. Conclusions: The treatment of recurrent NPC is controversial. Fractionated stereotactic radiotherapy is promising. However, the published trials are heterogeneous with respect to the selection criteria and treatment details. Prospective studies with long term follow-up data are warranted.

The Study on the Internet Addiction Influencing Factor and Coping Strategies for Juvenile (청소년 인터넷중독에 영향을 미치는 요인과 대처방안에 관한 연구)

  • The purpose of this study was to investigate the relationship between internet addiction in adolescent and influencing factor traits. In addition, the relationship between internet addiction and environmental coping strategies were examined. To examine the main purpose. analyzed as followed. According to the results of study. First, What have influencing factors on internet addiction? Second, How about were relationship between multidimensional factors correlation and multiregression analysis? Third, What about were prevention and coping strategies on internet addiction in adolescent period? According to the analysis with coping strategies researches, the results were presented as follows. Results showed that have positive correlation with internet use time, escape from reality, impulsivity, depression, anger control by examined results of Young addiction measure. Significant variables have influencing internet addiction with internet use time, escape from reality internet use, depression, anger control impulsivity. Based on these study results, This paper made a suggestion about prevention prescription and coping strategies of influencing factors on internet addiction in adolescent.

A Comparative Study of Knowledge and Attitude on Oral Contraceptive between Korean and Japanese University Students (한.일 대학생의 경구피임약에 관한 지식과 태도 비교 연구)

  • The purpose of this study was to investigate knowledge and attitude about oral contraceptive between Korean and Japanese university students in order to provide better sex education programs and direcrion. Korean subjects of this study were 337 university students in M city, during the period from April 1 to April 20, 2001 and Japanese subjects 245, during the period from June to August, 2001. Collected data were analyzed using descriptive statistics, t-test, Pearson correlation coefficients with SPSS package. The results from this study were summarized as follows : 1. The mean age of Koreans and Japanese students was $21.0{\pm}3.2$ and $19.6{\pm}3.2$ years old. The number of Japaneses youths having the parter with sexual intercourse was larger than that of Korean university students. 2. Comparison of knowledge and attitude about oral contraceptives between Korean and Japanese university students ; 1) Comparison of sexual differences : Oral contraceptives related knowledge of Korean university students marked $55.7{\pm}7.5$ of male students and $56.7{\pm}6.2$ of females with a range of 15 to 75. The level of female students' knowledge was higher than that of male's but there is not statistically a significant difference (p=.080). Oral contraceptives related attitude of Korean university students marked $81.1{\pm}12.2$ of male students and $76.9{\pm}10.3$ of female's with a range of 24 to 120. The level of male students' attitude was higher than that of female's and there is statistically a significant difference(p= 002). Oral contraceptives related knowledge of Japanese university students marked $55.3{\pm}6.7$ of male students and $57.0{\pm}6.3$ of female students. The level of female students' knowledge was higher than that of male's but there is not statistically a significant difference (p=.159). Oral contraceptives related attitude of Japanese university students marked $80.3{\pm}10.1$ of male students and $80.4{\pm}9.9$ of female students. The level of female students' attitude was higher than that of male's and there is not statistically a significant difference(p= .928). 2) Comparison between the country : Oral conceptives related knowledge of Korean university students marked $56.2{\pm}6.8$ and $56.7{\pm}6.4$ of Japanese university students with a range of 15 to 75. The level of Japanese university students' knowledge was higher than that of Korean's but there is not statistically a significant difference(p= .361). Oral conceptives related attitude of Korean university students marked $78.9{\pm}11.4$ and $80.4{\pm}9.9$ of Japaneses with a range of 24 to 120. The level of Japanese university studentss' attitude was higher than that of Korean's and there is not statistically a significant difference(p=.100). 2. Wanted age of oral contraceptives taking medicine and age was correlated positively (r=.178, p=.004) and total knowledge score of oral contraceptives and total attitude score were correlated positively(r=.467 p= .000) in Korean university students. Wanted age of oral contraceptives taking medicine and age was correlated positively (r=.289, p=.004), age and total attitude score were correlated positively(r=.196 p=.002) and total knowledge score of oral contraceptives and total attitude score were correlated positively (r=.671 p=.000) in Japanese university students. 3. Korean university students lifted side effect by the greatest factors in investigation about leading person that disturb work oral contraceptive, and the following appeared by knowledge insufficiency, sexual feeling inflammation worry, social prejudice, sexual morality decline, supernumerary prescription being not right, other person reverse and economical burden. Japanese university students can know that it is appearing by side effect, supernumerary prescription being not right, knowledge insufficiency, sexual feeling inflammation worry, economical burden, social prejudice, sexual morality decline and other person reverse. Think that this is result by dissimilar health medical system and cultural difference between two countries.

Effect of Boxing Aerobic Dance on Body Composition, Blood Component and Vascular Compliance in Obese Middle Aged Women (복싱에어로빅 참여가 비만 중년여성의 신체조성, 혈액성분 및 혈관탄성에 미치는 영향)

  • This study was conducted to test the effects of participation in 12-week program of boxing aerobics by obese middle-aged women on their body composition, blood constituents, and vascular compliance The samples are the middle-aged women in their forties or more, who has 30% or more body fat percentage, but has no medical history in cardiovascular disorders or metabolic diseases. The samples are divided into 8 of exercising group, and 8 of control group by random assignment. The intensity of boxing aerobics was HRmax 50% for the initial 4 weeks, HRmax 60% for the 5th to 8th week, and HRmax 70% for the 9th to 12th week. Each session took 60 minutes. The result is as follows. First, as a result of participating in the boxing aerobics program, body weight, body fat percentage, and muscle showed significant differences depending on the measuring period and the interacting term of the group and measuring period(p<.001), and the result of t-test on the sample matched to each group's measuring time also showed the significant increase or decrease in the exercising group(p<.001). Second, as a result of participating in the boxing aerobics program, the exercising group's TC, TG, HDL-C, and LDL-C, all showed significant differences in accordance with each group, measuring time, and the interaction term between groups and measuring time(p<.01, p<.001), and the result of t-test on each group's samples matched to the measuring time also shows significant increase or decrease in the exercising group(p<.01). Third, as a result of participating in the boxing aerobics programs, the vascular compliance of right hand, left hand, right hand, and left hand showed significant differences in accordance with each group, measuring time, and the interacting term between the measuring time and the group(p<.001), the t-test results of the samples matched to the each group's measuring time also showed significant differences in the exercising group(p<.001). To summarize the results above, it is suggested that the 12-week boxing aerobics program can improve body composition, blood constituents, and the blood circulation, which may prevent or enhance relevant diseases such as cardiovascular disorders.
