A Study About the Formative Process of So Yang-In(少陽人)'s prescriptions (소양인(少陽人) 신정처방(新定處方)의 형성과정(形成課程)에 대(對)한 고찰(考察))

  • After studying about the formative process and theory concerning So-Yang In(少陽人)'s prescriptions, Some principles were founded. Those were summarized as follows. 1. Lee Jae Ma(李濟馬) established the theory and remedy about the So Yang In's diseases, and the formation of So Yang In's prescriptions was based on those theories and remedies 2. As the emprical prescription of So Yang In's diseases, ten pre-scriptions of Discussion of Cold-Induced Disorder(傷寒論) and nine of Won Myung dynasty period(元明時代) were described in Dong Eui Su-Se Bo Won(東醫壽世保元), and I could confirm that the formation of So Yang In's presciptions was based on those emprical prescriptions. 3. The pathological characteristics of emprical prescriptions of So-Yang In's diseases in Discussion of Cold-Induced Disorder(傷寒論) were completely corresponded with the principle of So Yang In's diseases, and we could know that emprical presciptions of diseases that were the base of So Yang In's prescriptions were selected by the principle of So Yang In's diseases. 4. In the serious cases of So Yang In's diseases, So Yang In's pre-scriptions were mostly made up of tonifying and nourishing yin medicine(補陰藥物) that dirrectly rufill the damaged Eum Chung Zi Ki(陰淸之氣). According to the above results, it could be concluded that Lee Jae-Ma(李濟馬) made So Yang In's prescriptions with emprical pre scri-ptions that were based on the theory of So Yang In's diseases and concluded that So Yang In's prescriptions were made up of tonifying and nourishing yin medicine(補陰藥物) in the serious cases of diseases.

Comparisons of Adherence, Efficacy and Price between Sitagliptin/Metformin Fixed-dose Combination Tablets and Concomitant Administration of Sitagliptin and Metformin in Type 2 Diabetes Mellitus Patients (제2형 당뇨병 환자에서 시타글립틴과 메트포민 고정 복합제 투여와 단일제 병용간의 복약 순응도, 효과 및 약가에 대한 비교 연구)

  • Objective: This study was conducted to compare the adherence, clinical and economical utility of fixed-dose combination tablets of sitagliptin/metformin with concomitant administration of sitagliptin and metformin in patients with type 2 diabetes mellitus. Methods: Adherence was measured as the medication possession ratio (MPR) of ${\geq}80%$, and MPR was calculated as the number of total prescription days divided by the total treatment period. Hemoglobin $A_{1C}$ ($HbA_{1c}$) differences between baseline and predetermined periods were analyzed. Proportions of patients who achieved $HbA_{1c}$ less than 6.5% for three or more consecutive times were compared. To evaluate cost-effectiveness, prices of sitagliptin, metformin and sitagliptin/metformin tablets were investigated. Results: More than 90% of patients showed adherence in both groups (92.0% in fixed-dose combination group vs 95.9% in concomitant administration group), and there was no statistically significant difference (P = 0.113). Proportion of patients with HbA1c less than 6.5% for three or more consecutive times tended to be somewhat higher in fixed dose combination group than in concomitant administration group without a statistically significant difference (32.6% vs. 28.0%, P = 0.344). Total price of metformin and sitagliptin was cheaper up to 222 KRW in the case of fixed-dose combination tablets compared to the case of concomitant administration. Conclusion: The sitagliptin/metformin fixed-dose combination tablet had a similar patient adherence and was not significantly different in efficacy to the concomitant administration of each component. In terms of drug prices, fixed-dose combination tablets were cheaper than concomitant administration of each tablet.

The Clinical Study of Keumsuyukunjeon on the Dyspnea (금수육군전(金水六君煎)이 호흡곤란(呼吸困難)에 미치는 임상적(臨床的) 고찰(考察))

  • In the Oriental Medicine, the breathing problem is defined as one of the symptoms such as Cheon-Jeung, Hyo-Jeung and chronic respiratory organs disease. Its source is heard to be the functional reduction of descending of Lung and Kidney. The organs for breathing are known as Lung, Spleen and Kidney. In this research, some remarkable results are referred which were detected by measuring the variations of the breathing volume of 20 patients after taking Keumsuyukunjeon. Picrometer is used for the measurement of the volume. The investigation had been performed since from January 1 st to August 31th of 1998. The patients for the experiment were mainly composed of patients who had trouble in breathing due to the pneumonectasis, sthmas, pneumonias. The percent of men is 67% in sex distribution and the ratio of persons over 50's was 85%. After Keumsuyukunjeon was taken to the patients, the enhancement ratio of breathing volume was appeared as 7.7%. The analysis based on an age was that the patients of 40's show the highest volumetric advancement. The ratio of breathing volume was the aged patients whose lung or kidney is weak and it can be used as the prescription for supplement of body and lung. From the relations between the breathing volume before treatment and the enhancement ratio, the increase of the enhancement ratio and the better response to the medicine were shown to the more serious patients. The period of treatment was 27.5 days average. The enhancement ratio of smoker was 23. and that of non-smoker was 50. At the test of relation between the trouble rate in breathing and the enhancement ratio. Grade Ⅲ shows the highest enhancement value 50%. From the experimental results, It is found that Keumsuyukunjeon gives a noticeable benefit for the patients whose main symptom was breathing problem. Long-term treatments for the serious and aged patients will make much more efficient to the reduction of the symptoms.

Study of the Drugs Prescribed on Alzheimer's Disease: from the Insurance Claims Data of Korea National Health Insurance Service (우리나라 건강보험 청구자료를 이용한 알츠하이머성 치매 치료제의 사용현황 분석)

  • Objective: The aims of this study are to investigate the total volume of prescribed medicines against Alzheimer's disease (AD) and the trends of usage by analyzing the claims-data from the Korea National Health Insurance Service. Method: The demographic and claims-data were included the major AD treating medicines such as donepezil, galantamine, rivastigmine and memantine, and analyzed during the period of 2010~2012. The assessing criteria were gender, age, habitation, types of medical institution, code of ingredients, outcomes of treatment, volume and amount of claims, and the numbers of patients with dementias. After trimming the data, it were analyzed by the market size, demographic traits, characteristics of medical service, characteristics of each anti-AD medicine, etc. Results: Among the chosen 4 medicines, donepezil had the top prescription volumes. Most prevalent prescribing preparations of donepezil were conventional types. However, among the non-conventional types, oro-dispersible formulation is the fast increasing one in both volume and growth rate. This specialized preparations to improve both toleration and adherence, tend to being prescribed generally at the tertiary medical institutions. While the younger patients with mild-to-moderate AD mostly treated by expensive medicines in resident at the tertiary hospitals, the rest older patients with severe AD have been treated non-expensive one at long-term care facilities. Conclusion: AD is a chronic illness therefore, long-term use of therapeutic medications are highly important. If an anti-AD treatment was applied steadily in the earlier stages, it would be achieved not only improving the quality of life of patient but also reducing the expenses in the medical and nursing cares. As the socioeconomical impacts of AD is expanding, healthcare professionals need to aware the importance of pharmacotherapy and to improve sociopolitical fundamentals.

  • This is a fundamental study for bibliometric analysis investigating the general, research, copyright activities and cited literature related to the field of Oriental Medicine, The data used for this study included theses published by the Journal of Korean Oriental Medicine Society over a period of nine years, from 1990 to 1998, We investigated the productivity of national authors, distribution of cited literature and subject analysis about Oriental Medicine and its authors quantitatively, So we were able to reach several conclusions: 1. The rule of Lotka is applied considerably to analyze the productivity of authors in the field of Oriental Medicine, When it comes to the analysis of national authors it was found that most authors in this field of study published 2 to 10 volumes, likewise other studies. And the rate of co-authorships was high, because most surveys are clinical. 2. When it comes to distribution of cited literatures, independent volumes were cited more than journals. This represents that the citing style of Oriental Medicine has a tendency to follow the methodology of historical research. 3. Scholars in the field of Oriental Medicine mainly tend to cite self-subjects among cited literatures. After analysing self-subjects in this field, the result is that the order of frequency used as cited literatures is as follows: Internal Medicine, Herbology and Oriental Prescription, Acupuncture & Moxibustion. Acupuncture point, and subjects with the circulatory system diseases were mainly cited as far as subordinary subjects of Internal Medicine.

Study on the Prescribed Doses and Dispensing Patterns of Warfarin and Digoxin Tablets (Warfarin 및 Digoxin 정제의 처방 용량 실태 및 조제 양식의 고찰)

  • Drugs with a narrow therapeutic index (NTI) require very precise dosing. Warfarin and digoxin are the examples of NTI-drugs and dosing of them varies widely for different patients. However, in South Korea, only two strengths of warfarin and one of digoxin are commercially available. This is a big barrier for the precise dispensing and has potential safety risks to patients, particularly to elderly patients. To find a potential solution to the problem, an analysis of the prescribed doses and dispensing patterns of those drugs was performed. Data were collected by computer-facilitated prescription review in a university hospital. The period screened was from May 1st, 2012 to April 30th, 2013. All the prescriptions with either warfarin or digoxin tablets were selected for this study and dispensing patterns were analyzed according to the prescribed doses. A total of 17,017 warfarin prescriptions were analyzed; 8,148 for inpatient prescriptions, 8,869 for outpatient prescriptions, respectively. Of the 23 kinds of prescribed doses, 2 mg (19.9%) was most frequent, followed by 3 mg (13.2%) and 2.5 mg (11.7%). By analyzing the dispensing patterns, 60.3% (10,253) of the prescriptions required pill splitting and 72.0% of them were for the patients 65 years old and over. On the other hand, 4,350 digoxin prescriptions were included in this study. Of the 6 kinds of prescribed doses, 0.125 mg (71.2%) was most frequent, followed by 0.0625 mg (20.2%). Among the prescriptions for digoxin, 92.0% (3,998) should be split and 65.7% of them were for the patients aged 65 years and over. Despite limitations of strengths, various doses of warfarin and digoxin were prescribed. Furthermore, more than half of the prescriptions that required pill splitting were for elderly patients. The results from this study suggest that different strengths of warfarin and digoxin should be provided for accuracy of dispensing and safety for patients receiving them.

Hypertension Treatment: A 118-Case Series in a Korean Medicine Clinic (일개 한의원의 고혈압 한의치료 118례에 대한 고찰)

  • Objectives: The aim of this study was to report a 118-case series demonstrating the treatment of hypertension in a Korean medicine clinic and to evaluate the effect of Korean medicine. Materials & Methods: From 2006 to 2018, patients who visited a Korean medicine clinic for hypertension were investigated by studying changes in blood pressure before and after treatment with herbal medicine, acupuncture, and Uwhangchungsim-won. The average treatment period was 81.6 days. Results: After treatment with Korean medicine, the mean systolic blood pressure (SBP) and diastolic blood pressure (DBP) of all patients decreased significantly (p<0.001). The SBP decreased from $148.9{\pm}10.3$ to $133.8{\pm}13.9$, and the DBP decreased from $91.3{\pm}7.5$ to $82.5{\pm}9.0$. After treatment with acupuncture or Uwhangchungsim-won, the patient showed decreased blood pressure but the differences were not statistically significant when compared with the non-treated group. Conclusions: This study shows the real situation of hypertension treatment in Korean medicine and indicates that Korean medicine could be one of the primary treatment for hypertension. However, this study had limitations, such as variations in the treatment periods, the frequency of acupuncture treatments, and the type of antihypertensive drugs. For further evaluation of the effectiveness of treatment for hypertension using Korean medicine, a well-designed study should be undertaken.

Effects of Shinchubogun-tang and Common Korean Medicine Treatment on Symptom Reduction in Patients with Intervertebral Lumbar Disc Herniation (신추보건탕 및 한방공통치료가 요추추간판탈출증 환자의 증상 호전에 미치는 영향)

  • This retrospective observational study was aimed to assess that Shinchubogun-tang (Shenzhuibujian-tang ; SBT) improves the symptoms related to lumbar disc herniation. We analyzed the medical records of 36 patients, who were satisfied with inclusion and exclusion criteria. The patients were diagnosed with lumbar disc herniation and prescribed with SBT in addition to Korean medical common treatments at Dunsan Korean Medicine Hospital of Daejeon University from January 1st, 2013 to November 30th, 2015. The effects of SBT was evaluated by comparing before and after taking SBT through 0-10 visual analogue scale(VAS) of the symptoms including lower back pain, radiating pain, and lower extremity numbness. The average period of common Korean medicine treatment before SBT prescription was 9.78 days and then the patients received SBT along with common Korean medicine treatment for 13.17 days on average. SBT plus common korean medicine treatment group significantly decreased VAS score of lower back pain from 5.40±1.62 to 3.28±1.70 (p<0.01), radiating pain from 5.60±1.42 to 2.35±1.79 (p<0.01), lower extremity numbness from 5.77±1.52 to 2.55±1.85(p<0.01). These results demonstrated that SBT might have a potential on improvement of lumbar disc herniation by reducing the symptoms of lower back pain, radiating pain, and lower extremity numbness.

A study on the Clincal Records of 『The daily Records of Royal Secretariat of Chosun Danasty』 (숙종의 두창(痘瘡)에 관한 『승정원일기(承政院日記)』의 의안(醫案) 연구)

  • Smallpox and measles might be the typical epidemics through Choson Danasty. Especially Sookjong was the first king who suffered from smallpox during the period of his reign. This study was examined critically focused on the Clinical Records about Smallpox which occurred in the 9th year of Sookjong's reign. The clinical records in Seungjeongwon Ilgi("承政院日記") was the main source and the Annals of the Joseon Dynasty was reffered. It tells us that Sookjong caught infectious disease, smallpox spread in town and the whole process of smallpox. Seungmagalgeuntang(升麻葛根湯) and hwadoktang(火毒湯) were given at the early term for treating Sookjong's smallpox, but later on, Bowontang(補元湯) was given for the treatment of smallpox. It showed us the medical view which placed emphasis on the vital force of human body. A dermatologist, Yoo Sang had joined since the beginning of the treatment and Siyakchung(侍藥廳) was established for Sookjong. Sasunghoichuntang(四聖回天湯) which was Chunyoohyung(全有亨)'s prescription showed us enough clinical experience and the ways of treatments. At the same time, the medical culture at that time was well known through the fact that smallpox was taboo among the royal family.

Analysis of the Hospital Pharmacists Turnover after the Separation of Prescribing and Dispensing Practice (의약분업 후 종합병원 약사의 이직요인분석)

  • The aim of this study was to examine the current turnover status of hospital pharmacists and to analyze the factors which affected the turnover of them after the separation of prescribing and dispensing practice. We surveyed 19 managers of hospital pharmacies and 154 hospital pharmacists. Results are as follows. Pharmacist manpower of hospital pharmacies was only $63.99\%$ in tertiary hospitals and $76.78\%$ in general hospitals respectively of the number of pharmacists before the separation of prescription and dispensing practice. The ratio of those who left hospital pharmacies during the period of January 2000 and October 2001 was $80.23\%$ for tertiary hospitals, and $100.84\%$ for general hospitals. Decrease in the number of pharmacists brought the increase of work load and night duty. Major factors which affected the turnover of hospital pharmacists were found as following: income gap between hospital pharmacists and community pharmacists, increasing workload especially at night and on holidays, infrequent chance for the promotion, and low chance to provide clinical pharmacy services after the separation of prescribing and dispensing practice. Adequate manpower is the basic factor for providing hospital pharmacy services and improving clinical pharmacy services. The study suggested that proper number of hospital pharmacists is to be ensured through strengthening the legal requirement for the hospital pharmacists and improving health insurance reimbursement rate for the pharmaceutical services at hospital.
