체중 1.8 kg 5년령 중성화 수컷 치와와 개가 오른쪽 회음부 팽대 평가를 위해 건국대학교 부속 동물병원에 내원하였다. 회음부 팽대 외에 다른 증상이 없었으며 신체 검사에서 환납성 회음탈장임을 확인 하였다. 회음 탈장 교정을 위해 수정 된 전통 탈장 봉합술과 수정 된 내폐쇄근 변위 탈장 봉합술을 동시에 실시 하였다. 수술 한달 후 보호자가 왼쪽 회음부 팽대를 보고하였고 수의사의 신체검사에서 왼쪽 회음탈장이 확인 되었다. 수술 중 좌골, 등쪽, 뒤쪽 회음탈장의 동시 발생이 관찰 되었으며 회음 탈장 교정을 위해 위와 동일한 탈장 봉합술을 실시하였다. 정기 점진은 수의사의 신체 검사 혹은 보호자와의 전화 통화를 통해 실시 되었으며 양측 수술 부위에서 수술과 관련한 합병 증상이 없음을 확인하였다.

Two dogs (case 1:2-year-old intact male German Shepherd-mixed dog and case 2: 4-year-old intact female Jindo miked) with perineal hernia were referred to the Chonbuk Animal Medical Center, Chonbuk National University. In the both cases, there were unilateral perineal swelling involving the right side of the anus. In addition, in case 2, swelling was also present ventral to the anus. Rectal palpation of pelvic diaphragm revealed weakness of the muscles. Both cases were surgically treated using muscle transposition technique for hemiorrhaphy. In case 1, transposition of the superficial gluteal and internal obturator muscles and in case 2, transposition oi semitendinosus muscle was performed for hemiorrhaphy and reconstruction of the pelvic diaphragm. Both dogs became recovered after the surgical correction and no complications were observed during 10-month (casel) and 9-month(case2). Muscle transposition can be a useful technique for the treatment of perineal hernia in the dog.

체중 3.5 kg 11년령 요크셔 테리어가 대변실금, 뒤무직(이급후증), 회음부의 종괴를 동반하는 증상으로 내원하였다. 신체 검사상 비대 유선, 질 분비물과 정복이 가능한 회음부의 종괴가 발견되었다. 임상증상과 방사선검사를 바탕으로 배쪽 양측 회음 허니아와 자궁축농증으로 진단하였다. 자궁난소 적출술 후 양측 회음 허니아는 결장 고정술과 방광 고정술 후 반힘즐 근육 전위술을 이용하여 정복하였다. 수술 후 방사선과 컴퓨터 단층 촬영 검사상에 배쪽 양측 회음 허니아의 정복을 확인할 수 있었다. 술 후 10일째 정상적인 활동을 보였고 1년 동안에 검사상 수술과 관련된 부작용은 관찰되지 않았다. 배쪽 양측 회음 허니아는 결장 고정술과 방광 고정술 후 반힘즐 근육 전위술을 이용하여 성공적으로 치료 되었다.

회음부 팽대 병력을 보이는 세 마리 개가 건국대학교 부속 동물병원과 한강 동물병원에 내원하였다. 회음부 팽대 외에 다른 증상이 없었으며 신체 검사에서 두 마리는 환납성 한 마리는 비 환납성임을 확인 하였다. 일반 방사선 사진에서 골반강 또는 복강 내 장기가 탈장 낭 내에 없음을 확인하였다. 회음 탈장의 교정을 위해 내폐쇄근 변위 탈장 봉합술 실시 하였다. 한 마리에서 수술 후 지방을 함유하고 있는 대측 탈장이 확인되었다. 수술 후 정기 점진은 수의사의 신체 검사 혹은 보호자와의 전화 통화를 통해 실시 되었으며 세 마리 모두에서 수술과 관련한 좌골 신경 손상, 직장 탈출, 수술 부위 열개, 회음 탈장 재발과 같은 합병 증상이 없음을 확인하였다.

About 8 year-old castrated male Yorkshire terrier was presented for evaluation of dysuria, stranguria, hemtauria, and pollakiuria. On history taking, dysuria first was observed three months ago and these signs were waxed and waned. Physical examination revealed mild left perineal swelling. On routine laboratory examination, no significant findings were identified. Positive contrast urogram identified peritoneal herniation of urinary bladder. Urinalysis showed proteinuria and hematuria. Urine sediment revealed epithelial cells, white blood cells and rod-shaped bacteria. Pseudomonas aeroginosa was isolated from urine obtained through cystocentesis, and had resistance against fourteen antibiotics. Cystitis caused by P. aeruginosa concurrent with cystolithiasis and perineal hernia was diagnosed. Cystotomy, herniorrhaphy and EDTA-Tris solution lavage of bladder were performed. The patient was recovered to normal condition 2 days after treatment. Two weeks later, bacterial culture of urine was negative and any abnormality in ultrasonogram and urinalysis was not observed except calcium oxalate dihydrate crystals.

회음부 탈장은 나이든 수캐에서 골반강의 특발적인 약화로 일어난다. 14년령의 중성화되지 않은 수컷 시츄견이 우측 회음부 종창으로 건국대학교 부속 동물병원에 내원하였다. 3년간의 회음부 종창의 소견을 나타내는 병력을 가지고 있었으며, 방사선 검사에서 헤르니아 낭내의 직장과 결장의 확장이 관찰되었다. 회음부 탈장 교정술 중 바깥 항문조임근, 미골근, 그리고 항문 올림근이 약 3년간의 탈장으로 약화되어 있었고, 속폐쇄근의 변위만으로는 큰 결손부를 막기 어려워 polypropylene mesh를 이용하여 성공적인 수복이 이루어졌다.
