Transbronchial Needle Aspiration in the Diagnosis of Submucosal and Peribronchial Bronchogenic Carcinoma (기관지주위 병변과 점막하 병변을 보이는 폐암의 진단에 있어서 경기관지 침흡인술의 유용성)

  • Although exophytic endobronchial lesions can readily be diagnosed by routine forceps biopsy through the fiberoptic bronchoscope, submucosal or peribronchial tumor can be difficult to diagnose. So we evaluated the diagnostic utility of transbronchial needle aspiration (TBNA) through the fiberoptic bronchoscope in patients presenting with endoscopic abnormalities suggestive of submucosal or peribronchial tumor. Patients and Methods : Retrospective review of 120 lung cancer patients who were found to have the lesions suggestive of peribronchial and submucosal tumor during fiberoptic bronchoscopy was performed from Jan. 1994 to Dec. 2002 at Severance Hospital, Yonsei University College of Medicine. Results : Forcep biopsy was positive in 63 cases (52.5%) and TBNA in 91 (75.8%), which was significantly better than forcep biopsy (p=0.001). The combination of forceps biopsy and TBNA was positive in 106 cases (88.3%), which was significantly better than forceps biopsy alone (p=0.0001). The difference of TBNA yield according to cell type or bronchoscopic appearance of lesion was not significant, but it showed the relatively better result in small cell carcinoma. Conclusions : We concluded that TBNA significantly increase the yield over forcep biopsy alone in the detection of submucosal or peribronchial bronchogenic carcinoma.

  • Hamartoma had been considered as a congenital malformation of a tumerous lesion since the original description by Albrecht in 1904. Pulmonary hamartoma is able to classified as endobronchial and intrapulmonary hamartoma clinically, and is considered to benign mesenchymal neoplasms which originated in peribronchial connective tissue, and so called mesenchymoma by someone. We have experienced 3 cases intrapulmonary hamartoma which were discovered during routine chest film check up at all cases and are able to detailed histologic examination by electron microscopy at one case.

  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is becoming a standard method for invasive mediastinal staging and for the diagnosis of paratracheal and peribronchial lesions. It is essential to understand the technical aspects of EBUS-TBNA to ensure safe and efficient procedures. In this review, we discuss the practical aspects to be considered during EBUS-TBNA, including anesthesia, manipulation of equipment, understanding mediastinal ultrasound images, target selection, number of aspirations needed per target, sample handling, and complications.

Right middle lobe syndrome -Report of 4 cases- (폐 우중엽 증후군)

  • Atelectasis with chronic pneumonitis affecting the right middle lobe secondary to compression of the middle lobe bronchus by enlarged indurated or calcified peribronchial nodes is defined as middle lobe syndrome clinicopathologically. The lesion in this series of case reports including tuberculous lymphadenitis, lung cancer, atelectasis and lung abscess with organizing pneumonitis, had been treated by lobectomy. Postoperative courses were uneventful in all patients. Herewith, we report these four experiences with review of literature.

Comparative study of Korean White Ginseng and Korean Red Ginseng on efficacies of OVA-induced asthma model in mice

  • Background: Korean ginseng is a well-known medicinal herb that has been widely used in traditional medicine to treat various diseases, including asthma. Ginseng can be classified as white ginseng (WG) or red ginseng (RG), according to processing conditions. In this study, the authors compared the efficacies of these two ginseng types in a mouse model of acute asthma. Methods: To produce the acute asthma model, BALB/c mice were sensitized with ovalbumin (OVA) and aluminum hydroxide, and then challenged with OVA. WG and RG extracts were administered to mice orally. The influences of WG and RG on airway hyperresponsiveness (AHR), immune cell distributions in bronchoalveolar lavage fluid (BALF), and OVA-specific immunoglobulin E (IgE), IgG1, and IgG2a in serum were investigated. Cytokine production by lymphocytes isolated from peribronchial lymph nodes and histopathological changes was also examined. Results: In OVA-sensitized mice, both WG and RG reduced AHR and suppressed immune cell infiltration in bronchoalveolar regions. BALF OVA-specific IgE levels were significantly lower in RG-treated OVAsensitized mice than in the OVA-sensitized control group. WG and RG also suppressed inflammatory cytokine production by peribronchial lymphocytes. Histopathological findings showed reduced inflammatory cell infiltration and airway remodeling (e.g., epithelial hyperplasia) in WG- and RG-treated OVA mice compared with OVA controls. Conclusion: In this study, WG and RG showed antiasthmatic effects in an OVA-sensitized mouse model, and the efficacies of RG were found to be better than those of WG.

Chest CT Finding and Its Comparison with Bronchoscopic Finding in Endobronchial Tuberculosis (기관지결핵의 흉부전산화단층촬영소견 및 기관지경소견과의 비교)

  • Background : Endobronchial tuberculosis(ET) is still relatively common disease in Korea. We intended to evaluate the length of endobronchial lesion, peribronchial thickness, luminal irregularity and associated mediastinal lymph node enlargement with Chest CT to get information for such aggressive treatment as electrocautery, laser therapy and so on of bronchial stricture in ET, and also to compare the change of Chest CT finding with that of bronchoscopic finding after one month of anti-tuberculosis treatment. Method : We performed CT in 26 patients who were diagnosed as ET by bronchoscopy at Boramae Hospital from November 1991 to March 1996. After classifying ET into seven subtypes according to bronchoscopic finding, we analyzed the CT finding of each subtype. And we followed up the bronchoscopy, CT, and PIT after one month of anti-tuberculosis treatment, and compared the change of CT findings with those of bronchoscopic findings in nine patients. Results : Age of the patients was from 17 to 73 years old, and the ratio of male to female was 1 : 25 with absolute female predominance. The site(s) of bronchial involvement by tuberculosis is one in 14 cases, two in nine cases and three in one case, respectively, and the left main bronchus was the most frequently involved site (13 cases for multiple involvements and 7 cases for single involvement among 26 cases). The length of bronchial involvement by tuberculosis which was measured by CT was from 10 to 55 mm, and there was a tendency that the length of involved lesion in fibrostenotic type was shorter than that of actively caseating type. Bronchial stricture on CT was noticed in 25 (96%) cases and the range of severity was from total occlusion to near-normal and also showed wide variation even though the subtype of ET was same. The increase of peribronchial thickness which was measured by CT, was noticed in 21 cases (91%) among 23 cases (in which the measurement was possible), and there was no improvement of peribronchial thickness in those cases which showed little improvement in bronchial stricture despite anti-tuberculosis treatment. There was no difference in the luminal irregularity of involved bronchi on CT in relation to bronchoscopic subtypes. The mediastinal lymph node enlargement, defined as the diameter of lymph node was larger than 1cm on CT, was detected in 20 cases (77%), and right side was more frequently involved (L : R = 1 : 5.2). The CT finding usually showed extrinsic bronchial compression but showed direct invasion in two cases which were bronchoscopically classified as tumorous type. When follow-up bronchoscopy and CT was performed after one month of anti-tuberculosis treatment in nine patients, CT showed significant improvement in peribronchial thickness and mediastinal lymph node enlargement. Bronchial stricture was also improved in 6 cases but aggravated in 3 cases despite anti-tuberculosis therapy. In two cases which were classified as fibrostenotic type by bronchoscopy, CT showed significant improvement in bronchial stricture, interestingly. Conclusion : We concluded that the role of Chest CT was complimentary to bronchoscopy in ET, since CT was useful in evaluating the length of bronchial involvement, peribronchial thickness, and mediastinal lymph node enlargement.

Histologic Changes of the Transplanted Lung after Allotransplantation in Dogs (황견에서 폐이식 수술후 이식된 폐의 병리소견)

  • We have performed left lung transplantations in 15 dogs for one year and six months from June, 1990 to December, 1991 at the Repartment of Thoracic and Cardiovascular Surgery Yonsei University College of medicine, Seoul, Korea. These dogs were sacrificed at from operative day to post-operative 15 days when their general conditions were deteriorated. The gross findings of the transplanted lungs were thrombi in left atrium in three cases, partial occlusion of pulmonary artery or pulmonary veins in three cases, hemorrhage at pulmonary arterial anastomotic site in one case and bronchial anastomotic stenosis in two cases, bronchial anastomotic rupture in one case % no abnormal gross findings in four cases. The microscopic findings of the transplanted lungs were hemorrhagic infarction in one case, perivascular hemorrhage or pulmonary edema in two cases, peribronchial inflammatory cell infiltration & pneumonia in three cases, and alveolar type rejection with infiltration of type II pneumocytes and septal thickening in 3 cases. And also there were no abnormal findings including rejection or inflammatory evidences in six cases. The one among these six dogs survived to 15 days without evidence of rejection or inflammatory reaction & died due to postoperative care accident.

A clinical pathogenetic study of broncholithiasis (기관지 결석증의 임상적 연구)

  • Broncholithiasis is defined as a cor9ition in which a concretion is present within a bronchus or a cavity in the lung communicating with a bronchus. The usual causes of broncholithiasis are known as tuberculosis, histoplasmosis, silicosis, aspirated calculi, and a few fungal infections. It is generally accepted that the constant motion created by respiration and beating of the heart may cause the peribronchial calcified lymph node to erode into the tracheobronchial tree and to form broncholith. After the analysis of our 6 cases of broncholithiasis which were treated surgically in the Department of Thoracic Surgery, Seoul National University Hospital from 1960 to December, 1985, we could suggest that intrinsic formation of calculi should be regarded as the pathogenesis of broncholithiasis in addition to the extrinsic formation of calculi.

Surgical Treatment of Broncholithiasis (기관지 결석증의 외과적 치료)

  • I clinically analysed 13 cases of broncholithiasis which treated surgically and found only four cases of broncholithiasis caused by the erosion of the peribronchial lymph node named by extrinsic formation[Group II], and nine cases of broncholithiasis caused by intrinsic formation[group I] which indicates that no evidence of the erosion of the calcified per-ibronchial lymph node into the bronchial tree was found. This study is performed to see any differences between two groups regarding presenting symptoms, locations of broncholith, preoperative diagnosis, a history of pulmonary tuberculosis, a procedure of operation, and chemical compositions of broncholiths from both groups. There was no difference between two groups in the presenting symptoms, the procedure of operation, and chemical compositions of broncholiths. The broncholits from group two is located in the right middle lobe bronchus, and the broncholiths of group one were distributed at various bronchi, In group one, only five cases out of nine had preoperative diagnosis[55.5%], but in group two, all four had preoperative diagnosis. All cases had good postoperative courses without any complication and recurrences.

  • Pathological studies, on the lung of 27 cases of bovine parasitic bronchitis which were secured from abattoirs in Korea, were performed and discussed. The chief pathological findings were as follows. Grossly, the lungs were shown various degree of lobular consolidation, collapse and alveolar emphysema around the bronchi in which nematodes of Dictyocaulus viviparus were present. Emphysema and edema were observed in the interlobular septa. The lesions were encountered mostly in the caudal and ventral borders of the diaphragmatic lobes. In all the cased of parasitic bronchitis in which thickening of the visceral pleura and fibrinous deposition on the pleural surfaces were not seemed to be entirely due to the lungworm infection. Microscopically, bronchopneumonia, peribronchial and peribronchiolar lymphoid hyperplasia, lympho-reticular broncho-occlusive legion, and bleakthrough lesion were examined in the affected lungs.
