Sonographic Diagnosis of Cervical Lymph Node Metastasis in Patients with Thyroid Cancer and Comparison of European and Korean Guidelines for Stratifying the Risk of Malignant Lymph Node

  • Objective: To evaluate the ultrasonography (US) features for diagnosing metastasis in cervical lymph nodes (LNs) in patients with thyroid cancer and compare the US classification of risk of LN metastasis between European and Korean guidelines. Materials and Methods: From January 2014 to December 2018, US-guided fine-needle aspiration was performed on 836 LNs from 714 patients for the preoperative nodal staging of thyroid cancer. The US features of LNs were retrospectively reviewed for the following features: size, presence of hilum, margin, orientation, cystic change, punctate echogenic foci (PEF), large echogenic foci, eccentric cortical thickening, abnormal vascularity, and cortical hyperechogenicity. A multiple logistic regression analysis was performed to identify the independent US features for the diagnosis of metastatic LNs. The diagnostic performance of independent US features was subsequently evaluated. LNs were categorized according to the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) and European Thyroid Association (ETA) guidelines, and the correlation between the two sets of classifications was assessed. Results: Absence of the hilum, presence of cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features of metastatic LNs. Cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity showed high specificity (86.8%-99.6%). The absence of the hilum had the highest sensitivity yet low specificity (66.4%). When LNs were classified according to the ETA guidelines and K-TIRADS, they yielded similar categorizations of malignancy risks and were strongly correlated (Spearman coefficient, 0.9766 [95% confidence interval, 0.973-0.979]). According to the ETA guidelines, 9.8% (82/836) of LNs were classified as "not specified." Conclusion: Absence of hilum, cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features suggestive of metastatic LNs in thyroid cancer. Both K-TIRADS and the ETA guidelines provided similar risk stratification for metastatic LNs with a high correlation; however, the ETA guidelines failed to classify 9.8% of LNs into a specific risk stratum. These results may provide a basis for revising LN classification in future guidelines.

99mTc-3PRGD2 SPECT/CT Imaging for Diagnosing Lymph Node Metastasis of Primary Malignant Lung Tumors

  • Objective: To evaluate 99mtechnetium-three polyethylene glycol spacers-arginine-glycine-aspartic acid (99mTc-3PRGD2) single-photon emission computed tomography (SPECT)/computed tomography (CT) imaging for diagnosing lymph node metastasis of primary malignant lung neoplasms. Materials and Methods: We prospectively enrolled 26 patients with primary malignant lung tumors who underwent 99mTc-3PRGD2 SPECT/CT and 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET)/CT imaging. Both imaging methods were analyzed in qualitative (visual dichotomous and 5-point grades for lymph nodes and lung tumors, respectively) and semiquantitative (maximum tissue-to-background radioactive count) manners for the lymph nodes and lung tumors. The performance of the differentiation of lymph nodes with and without metastasis was determined at the per-lymph node station and per-patient levels using histopathological results as the reference standard. Results: Total 42 stations had metastatic lymph nodes and 136 stations had benign lymph nodes. The differences between metastatic and benign lymph nodes in the visual qualitative and semiquantitative analyses of 99mTc-3PRGD2 SPECT/CT and 18F-FDG PET/CT were statistically significant (all P < 0.001). The area under the receiver operating characteristic curve (AUC) in the semi-quantitative analysis of 99mTc-3PRGD2 SPECT/CT was 0.908 (95% confidence interval [CI], 0.851-0.966), and the sensitivity, specificity, positive predictive value, and negative predictive value were 0.86 (36/42), 0.88 (120/136), 0.69 (36/52), and 0.95 (120/126), respectively. Among the 26 patients (including two patients each with two lung tumors), 15 had pathologically confirmed lymph node metastasis. The difference between primary lung lesions in patients with and without lymph node metastasis was statistically significant only in the semi-quantitative analysis of 99mTc-3PRGD2 SPECT/CT (P = 0.007), with an AUC of 0.807 (95% CI, 0.641-0.974). Conclusion: 99mTc-3PRGD2 SPECT/CT imaging may notably perform in the direct diagnosis of lymph node metastasis of primary malignant lung tumors and indirectly predict the presence of lymph node metastasis through uptake in the primary lesions.

  • The effective control of cost overrun is a crucial issue for construction companies to secure profitability. Especially in situations where cost pressures are significant due to factors such as rising raw material prices and increased financial costs due to high-interest rate policies, cost overruns resulting from project failures have a highly detrimental impact on the profitability of construction firms. The objective of this study is to analyze the current status of cost overrun control measures adopted by construction companies using the IPA technique and provide key characteristics and implications. The IPA analysis results showed that practitioners in general contractors exhibit a high level of interest and effort regarding cost overrun while the performance level is relatively low. Nevertheless, the measures considered important to control cost overruns generally show a high tendency for execution as well. Cost overrun control measures that show high importance but low execution are primarily related to collaboration and communication sectors. To effectively control cost overruns, enhancing collaboration and communication with construction supervisors/CM, headquarters, and regulatory authorities emerged as the most urgent need. Through this study, the current status and areas for improvement regarding cost overrun control measures in general contractors can be identified. This can be valuable for deriving directions and enhancements for future cost overrun control strategy development.

Effect of feeding frequency on reproductive performances and stress responses in gestating sows

  • The objective of this study was to investigate the influence of feeding frequency on a sow's reproductive performance and stress response during gestation. A total of twenty multiparous sows (Yorkshire × Landrace) were used in a completely randomized design based on their parity, body weight (BW), and backfat thickness (BFT), and the sows were allotted to two different feeding systems: 1) once daily feeding (OF) and 2) twice daily feeding (TF) in corn-soybean meal based diets. The gestation diet was formulated to contain 3,265 kcal of metabolizable energy (ME) / kg, 12.90% of crude protein (CP), and 0.75 % of total lysine. The lactation diet was formulated to contain 3,265 kcal of ME / kg, 16.80% of CP, and 1.08% of total lysine and provided ad libitum during lactation. In gestation, sow BFT and BF changes were not affected by feeding frequency, but higher BW and BW gain from day 35 to 90 and day 35 to 110 were observed in OF sow (p < 0.10). In lactation, feeding frequency did not influence on BW, BW gain, BFT, BF changes, average daily feed intake, and wean-to-estrus interval. Also, there were no differences in litter size, litter weight and piglet weight in lactating sows. OF sows had higher (p < 0.05; p < 0.10) protein, solid-not-fat, and total solid concentrations in colostrum compared to TF sows, while OF sows had a lower (p < 0.05) lactose concentration in colostrum compared to TF sows. Sows in OF showed significantly lower average daily water consumption (ADWC) from day 35 to 110 of gestation (p < 0.05). While there were no significant differences in stereotypic behaviors and salivary cortisol levels during gestation between treatments, the OF sows showed less time spending on the activity at day 105 (p < 0.05). In conclusion, reduced feeding frequency increased BW gain during gestation, decreased activation time, and changed the colostrum composition. This information may contribute to the understanding of the physiological and behavioral change of gestating sows by manipulating feeding frequency.

Identifying Personal Values Influencing the Lifestyle of Older Adults: Insights From Relative Importance Analysis Using Machine Learning (중고령 노인의 개인적 가치에 따른 라이프스타일 분류: 머신러닝을 활용한 상대적 중요도 분석 )

  • Objective : This study aimed to categorize the lifestyles of older adults into two types - healthy and unhealthy, and use machine learning to identify the personal values that influence these lifestyles. Methods : This cross-sectional study targeting middle-aged and older adults (55 years and above) living in local communities in South Korea. Data were collected from 300 participants through online surveys. Lifestyle types were dichotomized by the Yonsei Lifestyle Profile (YLP)-Active, Balanced, Connected, and Diverse (ABCD) responses using latent profile analysis. Personal value information was collected using YLP-Values (YLP-V) and analyzed using machine learning to identify the relative importance of personal values on lifestyle types. Results : The lifestyle of older adults was categorized into healthy (48.87%) and unhealthy (51.13%). These two types showed the most significant difference in social relationship characteristics. Among the machine learning models used in this study, the support vector machine showed the highest classification performance, achieving 96% accuracy and 95% area under the receiver operating characteristic (ROC) curve. The model indicated that individuals who prioritized a healthy diet, sought health information, and engaged in hobbies or cultural activities were more likely to have a healthy lifestyle. Conclusion : This study suggests the need to encourage the expansion of social networks among older adults. Furthermore, it highlights the necessity to comprehensively intervene in individuals' perceptions and values that primarily influence lifestyle adherence.

Impact of an Instrumental Daily Living Activities Occupational Therapy Program for Adults With Developmental Disabilities on Their Daily Living Activities and Quality of Life (성인 발달장애인 대상 수단적 일상생활활동 작업치료 프로그램이 일상생활활동 및 삶의 질에 미치는 영향)

  • Objective : This study aimed to determine the impact of an instrumental daily living activity program for adults with developmental disabilities on their daily living activities and quality of life (QOL). Methods : This study used a single-group pre-test-post-test experimental design. Participants comprised 17 adults with developmental disabilities from the Welfare Center for the Disabled. This program consisted of 10 sessions, including pre- and post-assessments, pertaining to training and education on grocery shopping, meal preparation and cleaning, home management, safety, and emergency management. The evaluation tools used in this study were the Quality of Life Questionnaire, Korean version of the Life Space Assessment (K-LSA), and Korean version of the Instrumental Activities of Daily Living (K-IADL). Results : There was a statistically significant difference in the QOL and K-IADL scores before and after the instrumental activities of daily living occupational therapy program, and there was no statistically significant difference in the K-LSA scores. Conclusion : This study confirmed that implementing an IADL program for adults with developmental disabilities had a positive impact on performance and QOL. For adults with developmental disabilities to participate in daily life and society, interventions for work participation, including training in activities of daily living, must be expanded.

Probability Map of Migratory Bird Habitat for Rational Management of Conservation Areas - Focusing on Busan Eco Delta City (EDC) - (보존지역의 합리적 관리를 위한 철새 서식 확률지도 구축 - 부산 Eco Delta City (EDC)를 중심으로 -)

  • In some areas of the Republic of Korea, the designation and management of conservation areas do not adequately reflect regional characteristics and often impose behavioral regulations without considering the local context. One prominent example is the Busan EDC area. As a result, conflicts may arise, including large-scale civil complaints, regarding the conservation and utilization of these areas. Therefore, for the efficient designation and management of protected areas, it is necessary to consider various ecosystem factors, changes in land use, and regional characteristics. In this study, we specifically focused on the Busan EDC area and applied machine learning techniques to analyze the habitat of regional species. Additionally, we employed Explainable Artificial Intelligence techniques to interpret the results of our analysis. To analyze the regional characteristics of the waterfront area in the Busan EDC district and the habitat of migratory birds, we used bird observations as dependent variables, distinguishing between presence and absence. The independent variables were constructed using land cover, elevation, slope, bridges, and river depth data. We utilized the XGBoost (eXtreme Gradient Boosting) model, known for its excellent performance in various fields, to predict the habitat probabilities of 11 bird species. Furthermore, we employed the SHapley Additive exPlanations technique, one of the representative methodologies of XAI, to analyze the relative importance and impact of the variables used in the model. The analysis results showed that in the EDC business district, as one moves closer to the river from the waterfront, the likelihood of bird habitat increases based on the overlapping habitat probabilities of the analyzed bird species. By synthesizing the major variables influencing the habitat of each species, key variables such as rivers, rice fields, fields, pastures, inland wetlands, tidal flats, orchards, cultivated lands, cliffs & rocks, elevation, lakes, and deciduous forests were identified as areas that can serve as habitats, shelters, resting places, and feeding grounds for birds. On the other hand, artificial structures such as bridges, railways, and other public facilities were found to have a negative impact on bird habitat. The development of a management plan for conservation areas based on the objective analysis presented in this study is expected to be extensively utilized in the future. It will provide diverse evidential materials for establishing effective conservation area management strategies.

Effects of stocking density on the homeostasis of uric acid and related liver and kidney functions in ducks

  • Objective: Stocking density (SD) is an important issue in the poultry industry, which is related to the production performance, intestinal health and immune status. In the present study, the effects of SD on the metabolism and homeostasis of uric acid as well as the related functions of the liver and kidney in ducks were examined. Methods: A total of 360 healthy 56-day-old Shan-ma ducks were randomly divided into the low stocking density (n = 60, density = 5 birds/m2), medium stocking density (n = 120, density = 10 birds/m2) and high stocking density groups (HSD; n = 180, density = 15 birds/m2). Samples were collected in the 3rd, 6th, and 9th weeks of the experiment for analysis. Results: The serum levels of uric acid, lipopolysaccharide and inflammatory cytokines (interleukin-1β [IL-1β], IL-8, and tumor necrosis factor-α [TNF-α]) were increased significantly in the HSD group. Serious histopathological lesions could be seen in both the livers and kidneys in the HSD group in the 9th week. The mRNA expression levels of inflammatory cytokines (IL-8 and TNF-α) and related pathway components (toll-like receptor 4, myeloid differentiation primary response gene 88, and nuclear factor-κB) were increased significantly in both the livers and kidneys in the HSD group. The mRNA expression levels of enzymes (adenosine deaminase, xanthine oxidase, phosphoribosyl pyrophosphate amidotransferase, and phosphoribosyl pyrophosphate synthetase 1) related to the synthesis of uric acid increased significantly in the livers in the HSD group. However, the mRNA expression level of solute carrier family 2 member 9, which plays an important role in the excretion of uric acid by the kidney, was decreased significantly in the kidneys in the HSD group. Conclusion: These results indicated that a higher SD could cause tissue inflammatory lesions in the liver and kidney and subsequently affect the metabolism and homeostasis of uric acid, and is helpful for guiding decisions related to the breeding and production of ducks.

Effect of Andrographis paniculata supplementation during the transition period on colostrum yield, immunoglobulin G, and postpartum complications in multiparous sows during tropical summer

  • Objective: This study evaluated the effect of Andrographis paniculata (A. paniculata) supplementation in sow diets before and after farrowing on the sow and piglets' performances during early postpartum period and on sows' backfat and longissimus muscle losses during lactation. Methods: Seventy Landrace×Yorkshire sows and their offspring (1,186 piglets) were distributed into three groups: control (n = 31), treatment-250 (n = 18), and treatment-1000 (n = 21). From 110.2±0.7 days of gestation until farrowing (5.8 days) and throughout the lactation period (25.2 days), sows in the control group were given the conventional lactation diet, while sows in the treatment-250 and treatment-1000 groups received supplements of 250 ppm and 1,000 ppm of A. paniculata, respectively. Results: In sows with parity 3-5, piglets from the treatment-1000 group had higher colostrum intake than the control and treatment-250 groups (p<0.05), but not in sows with parity 6-9. Colostrum immunoglobulin G (IgG) increased in treated sows versus controls for parity 6-9 (p<0.05), but was consistent for parity 3-5. Piglet performance until day 3 postnatal was similar across groups (p>0.05). Treatment-250 sows had higher feed intake post-farrowing than treatment-1000 sows (p<0.05). Longissimus loss was less in both treatment groups than control (p<0.05), but backfat loss was similar across groups (p>0.05). Post-partum complications were consistent across groups (p>0.05). Farrowing duration and piglet birth intervals in sows with parity 6-9 were prolonged in the treatment-1000 group. Conclusion: Supplementing with 1,000 ppm A. paniculata for 5.8 days pre-farrowing and 25.2 days post-farrowing enhanced sow colostrum IgG and piglet colostrum intake, while also reducing longissimus loss in sows. However, for sows of parity 6-9, this supplementation led to prolonged farrowing, increased intervals between piglet births, increased stillbirth, and reduced piglet birth weight. These effects should be considered when using A. paniculata supplementation.

Preoperative Prediction for Early Recurrence Can Be as Accurate as Postoperative Assessment in Single Hepatocellular Carcinoma Patients

  • Objective: To evaluate the performance of predicting early recurrence using preoperative factors only in comparison with using both pre-/postoperative factors. Materials and Methods: We retrospectively reviewed 549 patients who had undergone curative resection for single hepatcellular carcinoma (HCC) within Milan criteria. Multivariable analysis was performed to identify pre-/postoperative high-risk factors of early recurrence after hepatic resection for HCC. Two prediction models for early HCC recurrence determined by stepwise variable selection methods based on Akaike information criterion were built, either based on preoperative factors alone or both pre-/postoperative factors. Area under the curve (AUC) for each receiver operating characteristic curve of the two models was calculated, and the two curves were compared for non-inferiority testing. The predictive models of early HCC recurrence were internally validated by bootstrap resampling method. Results: Multivariable analysis on preoperative factors alone identified aspartate aminotransferase/platelet ratio index (OR, 1.632; 95% CI, 1.056-2.522; p = 0.027), tumor size (OR, 1.025; 95% CI, 0.002-1.049; p = 0.031), arterial rim enhancement of the tumor (OR, 2.350; 95% CI, 1.297-4.260; p = 0.005), and presence of nonhypervascular hepatobiliary hypointense nodules (OR, 1.983; 95% CI, 1.049-3.750; p = 0.035) on gadoxetic acid-enhanced magnetic resonance imaging as significant factors. After adding postoperative histopathologic factors, presence of microvascular invasion (OR, 1.868; 95% CI, 1.155-3.022; p = 0.011) became an additional significant factor, while tumor size became insignificant (p = 0.119). Comparison of the AUCs of the two models showed that the prediction model built on preoperative factors alone was not inferior to that including both pre-/postoperative factors {AUC for preoperative factors only, 0.673 (95% confidence interval [CI], 0.623-0.723) vs. AUC after adding postoperative factors, 0.691 (95% CI, 0.639-0.744); p = 0.0013}. Bootstrap resampling method showed that both the models were valid. Conclusion: Risk stratification solely based on preoperative imaging and laboratory factors was not inferior to that based on postoperative histopathologic risk factors in predicting early recurrence after curative resection in within Milan criteria single HCC patients.
