A Study on Burnout of the Workers in Elderly Living Facilities (노인생활시설 종사자의 소진(消盡)에 관한 연구)

  • This study has been performed to investigate group differences and determining factors of burnout of the workers in elderly living facilities. In this study, 131 male and female workers composed of social workers, health practitioners, and other management workers were selected among 23 retirement and nursing facilities in Korea between 2003 and 2005. They were asked about their socioeconomic background, working environment, and their perceptions on burnout in the working places. As for main dimensions of the burnout, there were three elements such as emotional weariness, diminishing personal accomplishment, and depersonalized behaviors. Particularly in this study, the depersonalized behaviors were defined as aggressive or rude verbal and behavioral outbursts from anger. As a result of this study, the following results were found: Most workers have medium level of perception on their commitment and burnout, but there. are some risk factors; newly married young workers were less satisfied or fairly rewarded with their job and as a result, they have significantly higher level of emotional weariness but lower feeling of personal accomplishments. In addition, they occasionality practiced aggressive or rude behaviors such as yelling, threatening, and physically harassing As for the influence of determining variables, long-time workers of long established facilities were more satisfied and less burnout. In addition, there are some impacts of job category; health practitioners were more burnout than social workers. In this study, suggestions are made as follows: first, guiding and settlement programs should be developed for the newly started young married workers, particularly at early stage of their career. Second, future aspects of facility visions should be guided for the workers, so that they are relieved from stressful routines without any future plan. Third, medical workers such as physical therapists and nurses should be further monitored for main reasons of increasing their stress.

A Q-methodological Study on Nursing Students' Attitudes toward Nursing Ethics

  • Purpose. Professional nursing ethics is a living, dynamic set of standards for nurses' professional moral behavior. Furthermore, in daily clinical nursing training, nursing students are constantly confronted with decision-making that is moral in nature. The aim of this study was to identify the perceived ethical attitudes in the clinical training process of senior nursing students using Q-methodology to offer basic strategies for nursing ethics education and thereby improve patients' care. Methods. Q-methodology provides a scientific method for identifying perception structures that exist within certain individuals or groups. Thirty-seven participants in a university rated 38 selected Q-statements on a scale of 1-9. The collected data were analyzed using pc-QUNAL software. Results. Principal component analysis identified 3 types of ethical attitudes in nursing students in Korea. The categories were labeled Sacred-life, Science-realistic and Humane-life. Sacred-life individuals think that a life belongs to an absolute power (God), not a man, and a human life is a high and noble thing. Science-realistic individuals disagreed that allowing an induced abortion or embryo (human) duplication is unethical behavior that provokes a trend, which takes the value of a life lightly; most of them took a utilitarian position with respect to ethical decisions. Humane-life individuals exhibit a tendency toward human-centered thought with respect to ethical attitudes. Conclusion. This study will be of interest to educators of students of nursing and hospital nursing administrators. Also, the findings may provide the basis for the development of more appropriate strategies to improve nursing ethics education programs.

A Study on the Relationship Between Nursing Organizational Culture and ICUs Team Effectiveness (중환자실의 간호조직문화와 팀효과성에 관한 연구)

  • Purpose: The purpose of this research is, by investigating organizational characteristics, types of nursing organizational culture and team effectiveness in ICU, to ascertain the type of nursing organizational culture and the organizational characteristic that can improve the team effectiveness. Method: The research targeted 427 nurses from 33 ICUs of 14 general hospitals which have more than 250 beds and the data were gathered by using self-report questionnaires from April 10, 2003 to April 24, 2003. For this research, the following tools were used; the tool for measuring organizational characteristics and organizational cultures and the tool for measuring team effectiveness. Result: The most significant nursing organizational characteristic in ICU is the centralization. The organizational culture in ICU is generally rank-oriented culture. There was a significant difference (p<.01) in four types of organizational cultures; relation-oriented, innovation-oriented, rank-oriented and task-oriented. Verifying influence power of organizational cultures upon team effectiveness of ICU, relation-oriented culture had 49.2% of an influence upon team effectiveness, innovation- oriented and relation-oriented culture had 60.4% of an influence, and rank-oriented, innovation-oriented and relation-oriented culture had 61.2% of an influence. The organizational culture profiles according to the types of nursing organizational cultures in 33 ICUs were found by a cluster analysis. They were classified into five culture profiles; strong balance culture profile, weak balance culture profile, innovation-oriented and task-oriened culture profile, strong relation culture profile and strong rank culture profile(p<0.5). According to me organizational culture profiles, a significant difference of team effectivenesses(coworker satisfaction, team performance perception, team satisfaction and team commitment) was found(p<.01). The strong balance culture profile had the best team effectivenesses. Conclusion: For nursing culture management, a nursing administrator should identify the relevant nursing organizational culture at first by utilizing an innovative team-leader. After identifying the organizational culture, the administrator should make strategic plans and practices that can distinguish good organizational cultures to be expanded from ones to be sublated so that a strong balance culture can be developed.

Predictors of Depression In Middle-School Girls (일 여자중학교 학생의 우울 예측요인)

  • Purpose: The purpose of this study was to explore the levels of depression and predictors of depression in middle-school girls. Method: A self-report survey was conducted with 2nd and 3rd grade students in a girls'middle school (N=401) in Pusan. Data were analyzed with descriptive statistics, Pearson correlation, and simultaneous multiple regression using the SPSS program. Results: The mean score for the Center for Epidemiologic Studies-Depression Scale (CES-D) was 20.63. The level of depression was negatively related to problem-solving ability, self-esteem, total household income, school achievement, self-perception of body-image, and satisfaction in relationships with siblings, parents, and friends. A multivariate approach showed that predictors explained 61% of variance in depression. Significant predictors of depression were self-esteem (${\beta}$=-.38), problem-solving ability (${\beta}$=-.34), and satisfaction in relationships with friends (${\beta}$=-.14) and parents (${\beta}$=-.08). Conclusion: The findings suggest that it is important to develop educational programs to increase self-esteem and problem-solving abilities in middle school girls. Considering the high levels of depression in middle school girls, school nurses play an important role in detecting and reducing emotional tension among these students. Nursing interventions, including art therapy, problem-solving counseling, and bibliotherapy could be useful in enhancing self-esteem, problem-solving abilities, and satisfaction in relationship with friends, siblings and parents.

A Meta-Analysis on Factors Related to Quality of Life in Heart Transplant Recipients (심장이식 수혜자의 삶의 질 관련 요인에 대한 메타분석)

  • Purpose: This study was a systematic review and meta-analysis to explore the factors related to quality of life in heart transplant recipients. Methods: To identify studies that suggested the factors related to the quality of life in heart transplant recipients, we searched the articles published from 1974 to November 2018 using Six databases, PubMed, CINAHL, EMBASE, Cochrane, KMBASE and RISS. A total of 22 studies were selected out of 5,234 for the systematic review and meta-analysis on the basis of the PRISMA flow. The quality of study was assessed by assessment tool form the NIH and meta-analysis was performed using the 'R 3.5.2' version to analyze the correlated effect sizes. Results: Factors related to quality of life in heart transplant recipients were categorized into six domains based on the health-related quality of life model introduced by Ferrans: individual, environmental, biological function, symptoms, functional status, and general health perception. In the meta-analysis, 34 factors were used and 17 factors having significant effect sizes were as follows: self-efficacy, demoralization, perceived control, current occupational status, age, marital status, health promotion life style in the individual characteristics; stress in environmental characteristics; physical function status, creatinine level, left ventricular ejection fraction (LVEF) in biological function; anxiety, depression, symptom frequency and distress in symptoms domain; coping, self-care compliance in functional status. Conclusion: The findings indicate that the multi-dimensional factors influencing the quality of life in heart transplant recipients and provide the evidence for developing effective interventions for improving the quality of life of recipients.

Analysis of the Contents of Visiting Nursing Articles on Domestic Portal Sites Using Topic Modeling: Focusing on the Comparison Before and After Coronavirus Disease (토픽 모델링을 이용한 국내 포털사이트 방문간호 기사 내용 분석: 코비드-19 이전과 이후 비교를 중심으로)

  • Purpose: This study aimed to explore the social perception of visiting nursing before and after coronavirus disease (COVID-19). Methods: This survey-based study used online big data for comparative analysis by classifying the keywords related to visiting nursing searched on domestic portal sites before and after COVID-19. Results: According to the results of analyzing the Intertopic Distance Map based on Latent Dirichlet Allocation in this study, four topics were extracted, two each before and after COVID-19. The first topic before the COVID-19 period was termed "the expansion of visiting nursing subjects and services visiting nursing," while the second was termed "visiting nursing," which is related to customized welfare. The first topic after the COVID-19 period was termed "the suspension and resumption of visiting nursing services," while the second was "the development of a non-face-to-face home visit healthcare system". Conclusion: The results of this study can be used as useful reference data to contribute to future medical service delivery system reform policies starting at the end of COVID-19 and the revitalization of community care for visiting nursing.

Social Support, Depression and Health Status in Heart Transplantation Recipients (심장이식 수혜자의 사회적지지, 우울 및 건강상태)

  • Purpose: As the number of cases of heart transplantation (HT) and the survival rate have increased, concerns regarding post-HT life have grown. This study was conducted to explore the relationships of perceived social support to the depression and health status of HT recipients. Methods: This descriptive study targeted all recipients who had undergone HT at S University Hospital since 1994, and 32 recipients were recruited. Results: 90.6% of the recipients were men, and the average age at the time of surgery was $47.4{\pm}13.4$ years. The ejection fraction was increased from $18.47{\pm}5.63%$ to $63.25{\pm}7.57%$, and 31 cases of New York Heart Association (NYHA) class III and IV improved to class I or II after HT. The patients reported mild depression even with high perceived social support and improved health status after HT. Among those resource persons who offered support, they reported their spouse, doctors, and nurses as most meaningful to them. Conclusion: This study confirmed the positive effects of HT on the recipients' physical improvement. It also added the discovery that the importance of recipients' subjective perception of social support might be critical for their more successful recovery and adjustment to post-HT life. Focusing on a different approach to social support and types of social resources in the recovery phase is suggested for future studies on quality of life after HT.

An Analysis of Nurse's Perception of Internal Marketing Activities Affecting on Nurse's Turnover Intention, Nursing Task Performance and Nursing Productivity (간호사가 지각하는 내부마케팅활동 정도가 간호사의 이직의도, 간호업무수행 및 간호업무생산성에 미치는 영향)

  • Purpose: The purpose was to analyze the effects of internal marketing activity factors on nurse's turnover intention, nursing task performance and nursing productivity. Methods: The subjects were 355 nurses who were working at the 3 universities hospital over 1 year. The instruments were used of internal marketing activity factors(Lee, 2001), turnover intension(Lee, 1995), nursing task performance(Park, 1988) and nursing productivity(McNeese-Smith, 1996). Results: The mean score of internal marketing activity factors was 2.79, education and training 2.97, individualization 2.93, communication 2.87, promotion 2.76, work environment 2.63, reward system 2.62, and management vision for employee 2.61. The turnover intention was 3.12, nursing task performance 3.49, and nursing productivity 3.38. The internal marketing activity factors were negatively correlated with turnover intention(r=-0.37, p<0.0001), and positively correlated with nursing task performance(r=0.29, p<0.0001) and nursing productivity(r=0.30, p<0.0001). The key predictor of turnover intension was reward system, education and training, communication, and salary. They explained 35.0% of the total variance. In nursing task performance, communication, management vision for employee, salary and unit explained 26.0% of the total variance. In nursing productivity, communication, reward, education and training, salary, and position explained 24.0%. Conclusions: To increase nurse's nursing task performance and nursing productivity and to decrease turnover intention, it is necessary to concentrate on improving communication and reward system in the internal marketing activity factors. Through these activities, the effectiveness of internal marketing strategies will be enhanced and finally, nursing organizational outcome will be increased.

Development and Validation of a Measurement to Assess Person-centered Critical Care Nursing (중환자실 간호사의 인간중심 간호 측정도구 개발)

  • Purpose: The purpose of this study was to develop a scale to measure person-centered critical care nursing and verify its reliability and validity. Methods: A total of 38 preliminary items on person-centered critical care nursing were selected using content validity analysis of and expert opinion on 72 candidate items derived through literature review and qualitative interviews. We conducted a questionnaire survey with 477 nurses who worked in intensive care units. The collected data were analyzed using exploratory factor analysis (EFA) and confirmative factor analysis (CFA) with SPSS and AMOS 24.0 program. Results: EFA was performed with principal axis factor analysis and Varimax rotation. The 15 items in 4 factors that accounted for 50.8% of the total variance were identified by deleting the items that were not meet the condition that the commonality should be .30 or more and the factor loading over .40. We named the factors as compassion, individuality, respect, and comfort, respectively. The correlation coefficient between this scale and the Caring Perception Scale was r=.57 (p<.001), which determined concurrent validity. The item-total correlation values ranged from .39 to .63, and the internal consistency for the scale was Cronbach's ${\alpha}=.84$. Conclusion: The reliability and validity of the 15 item person-centered critical care nursing scale were verified. It is expected that the use of this scale would expand person-centered care in critical care nursing.

Impact of Depression on Medication Adherence of Patients with Systemic Lupus Erythematosus: Focusing on Mediating Effect of Self-Efficacy and Belief about Medication (전신성 홍반성 루푸스 환자의 우울이 복약순응도에 미치는 영향: 자기효능감과 약물에 대한 신념의 매개 효과를 중심으로)

  • Purpose: The purpose of this study was to determine the mediating effects of self-efficacy and the belief about medication on the association between depression and medication adherence in patients with systemic lupus erythematosus. Methods: 128 patients aged ${\geq}19years$, who were regular outpatients or admitted patients diagnosed with systemic lupus erythematosus at a tertiary hospital in B city, participated in this study. Data were collected by using a self-administered questionnaire. Testing of mediating effects was analyzed by a parallel redundant mediated model using the PROCESS macro for SPSS version 3.3. Results: They scored an average of $16.71{\pm}11.13$ for depression, $694.14{\pm}170.68$ for self-efficacy, $3.05{\pm}4.60$ for the belief about medication, and $90.14{\pm}15.37$ for medication adherence. The direct effect of depression on medication adherence was not statistically significant, but the indirect effects of depression mediated with self-efficacy and belief about medication were statistically significant. Conclusion: It is necessary to develop and apply a nursing intervention program that can not only relieve depression but also promote self-efficacy and the belief about medication with the objective of improving medication adherence among patients with systemic lupus erythematosus.
