종격동섬유종증의 방사선학적 소견 (Radiologic Findings of Mediastinal Fibromatosis)

  • The fibromatosis is a rare tumorous with local invasion, but is not metastasized distantly. This term should not be applied to nonspecific reactive fibrous proliferations that are part of an inflammatory process of are secondary to injury of hemorrhage and have no tendency toward growth or recurrence. It arises principally from the connnective tissue of muscle and overlying fascia or aponeurosis(musculaponeurotic fibromatosis), and chiefly affects the muscle of shoulder, pelvic girdle, and extremity. The term 'aggressive fibromatosis' is also employed to describe this disease, but it is impossible to predict the clinical course in the individual case. The fibromatosis arising in the mediastinum is very rare, and the report about it is nearly absent. The plain radiography shows merely mass with soft tissue density. The CT demonstrates a poorly defined homogenous or heterogenous mass, isodense with skeletal muscle on precontrast-images, and slightly hyperdense to muscle on postcontrast-scan. Accurate delineation between the tumor & surrounding tissue is vague or frequently impossible. The authors experienced one case of the mediastinal fibromatosis recently and report the case with review of concerned literature.

Late Occurrence of Multiple Bone Metastasis in Patient with Well Controlled Advanced Pancreatic Cancer

  • A 67-year-old male was admitted due to abdominal pain. Abdominal CT scan performed in a local clinic showed about 2 cm sized pancreatic tail mass with extensive liver and multiple regional lymph node metastasis. Histology of liver biopsy revealed poorly differentiated adenocarcinoma. He underwent chemotherapy with gemcitabine and erlotinib for 5 cycles followed by 8 cycles of second line chemotherapy with 5-fluorouracil and cisplatin. At 12 months after diagnosis, follow-up abdominal CT scan revealed marked reduction of tumor mass in the liver and pancreas with small residual tumor. After one month of last chemotherapy, he complained radiating pain along left leg. Blood chemistry revealed isolated elevation of alkaline phosphatase (ALP) and multiple bone metastasis were demonstrated in bone scan. Palliative radiation therapy to pelvic bone was performed for the relief of bone pain. The prognosis of advanced pancreatic cancer is extremely poor. We report late occurrence of multiple bone metastasis in a patient with well controlled advanced pancreatic cancer with chemotherapy.

Outcomes of Malignant Ovarian Germ-Cell Tumors Treated in Chiang Mai University Hospital over a Nine Year Period

  • Malignant ovarian germ cell tumors (MOGCT) are rare neoplasms that most frequently occur in women at a young reproductive age. There have been limited data regarding this disease from Southeast Asian countries. We therefore conducted a retrospective study to analyze the clinical characteristics and the treatment outcomes of MOGCT treated at our institute between January, 2003 and December, 2012. Seventy-six patients were recruited from this period with the mean age of 21.6 years and 11.8% were pre-puberty. The two most common symptoms were pelvic mass and pelvic pain. Two-thirds of the studied patients presented at an early stage. The most common histology was immature teratoma (34.2%) followed by endodermal sinus tumor (28.9%), dysgerminoma (25%), mixed type (10.5%) and choriocarcinoma (1.3%). Over 80% of these patients received fertility sparing surgery and about 70% received adjuvant chemotherapy with the complete response rate at 73.3% and partial response at 11.1%. The most frequent chemotherapy was BEP regimen (bleomycin, etoposide, cisplatin). With the mean follow up time at 56.0 months, 12 patients (15.8%) developed recurrence and only an advanced stage was the independent prognostic factor. The ten year progression free survival (PFS) and overall survival rate of our study were 81.9% and 86.2%, respectively. In conclusion, MOGCT often occurs at a young age. Treatment with fertility sparing operations and adjuvant chemotherapy with a BEP regimen showed a good outcome. An advanced stage is a significant prognostic factor for recurrence.

Short-course palliative radiotherapy for uterine cervical cancer

  • Purpose: The purpose of this retrospective study was to evaluate the efficacy and feasibility of short-course hypofractionated radiotherapy (RT) for the palliation of uterine cervical cancer. Materials and Methods: Seventeen patients with cancer of the uterine cervix, who underwent palliative hypofractionated 3-dimensional conformal radiotherapy between January 2002 and June 2012, were retrospectively analyzed. RT was delivered to symptomatic lesions (both the primary mass and/or metastatic regional lymph nodes). The total dose was 20 to 25 Gy (median, 25 Gy) in 5 Gy daily fractions. Results: The median follow-up duration was 12.2 months (range, 4 to 24 months). The median survival time was 7.8 months (range, 4 to 24 months). Vaginal bleeding was the most common presenting symptom followed by pelvic pain (9 patients). The overall response rates were 93.8% and 66.7% for vaginal bleeding control and pelvic pain, respectively. Nine patients did not have any acute side effects and 7 patients showed minor gastrointestinal toxicity. Only 1 patient had grade 3 diarrhea 1 week after completion of treatment, which was successfully treated conservatively. Late complications occurred in 4 patients; however, none of these were of grade 3 or higher severity. Conclusion: Short-course hypofractionated RT was effective and well tolerated as palliative treatment for uterine cervical cancer.

발레 바와 탄성밴드의 복합운동이 중년여성의 신체조성, 체력, 자세교정에 미치는 영향 (Effects of Ballet Bar and Elastic Band Exercise on Body Composition, Physical Fitness and Postural Correction in Middle-Aged Women)

  • Purpose : The purpose of this study was to investigate the effects of a combined exercise program using a ballet bar and elastic bands on the body composition, physical strength, and postural correction of middle-aged women. Methods : The subjects were 28 middle-aged women with no ballet experience. They were divided into an experimental group (n=14; exercise group) and a control group (n=14; non-exercise group) through random sampling. The experimental group underwent an exercise program using a ballet bar and elastic bands for 50 minutes twice a week for 12 weeks, whereas the control group performed no exercise. The subjects in both group had their body composition, physical fitness, and postural angles measured before and after the intervention. A paired t-test was used to compare pre- and post-test values within each group, and a two-way repeated measures ANOVA was employed to compare pre- and post-test changes between the two groups. Results : Regarding body composition, statistically significant differences in BMI (p<.05), body fat percentage (p<.001), and muscle mass (p<.001) were observed between the two groups before and after the intervention. For physical fitness, the experimental group showed statistically significant increases in quick reflexes (p<.001), muscle endurance (p<.001), and muscle strength (p<.001). For postural correction, the experimental group exhibited statistically significant decreases in all variables: the head (p<.001), shoulder (p<.001), cervical tilt (p<.001), right-left pelvic tilt (p<.001), and anterior-posterior pelvic tilt (p<.001). Conclusion : A combined exercise program using a ballet bar and elastic bands had positive effects on the body composition, physical fitness, and postural correction of middle-aged women.

The Effects of Squat and Corrective Exercises on Body Composition and Body Shape Balance in Older Adults

  • 본 연구는 스쿼트 운동과 교정 운동을 통해 근감소증 예방을 위한 노인의 다양한 운동프로그램 적용 및 개선하고자 총 32명의 개별 근관절검사를 실시 후 스쿼트 운동과 교정운동을 12주간 주 2회 실시하였으며, 다음과 같은 결과를 도출하였다. 운동프로그램 참여 전보다 신체조성은 골격근량, 기초대사량에서 12주 후 유의하게 증가하였고, 정적 자세 균형은 전면 어깨, 골반의 균형 및 편향성, 좌측 다리 내반슬과 측면의 경추와 흉추, 골반 기울기가 12주 후 유의한 자세 균형을 나타냈다. 동적 자세 균형은 복합 불균형 지수, 과도한 상체 숙여짐, 허리 전만 또는 후만, 한쪽 무릎의 기울어짐과 하향 모두 12주 후 매우 유의한 자세 균형을 나타냈다. 본 연구에서 확인한 바와 같이, 맞춤형 노인 운동프로그램에 대한 다양성과 정밀한 분석을 통한 운동처방은 매우 중요하며, 현장에서 실제적 적용을 통해 노인 건강유지 및 증진 연구를 위한 관심과 노력이 필요하다.

양측성 원발성 난관암 1예 (Case of Bilateral Primary Carcinoma of the Fallopian Tube)

  • Primary carcinoma of the fallopian tube is exceedingly rare, accounting for 0.3 percent of female genital tract cancer. It is usually unilateral, but 20 percent of cases are bilateral. It occurs most commonly in women between 40 and 60 years old. Approximately 50 percent of the patients are nulliparous. Because of variable presenting symptoms and rarely incidence, preoperative diagnosis of fallopian tube carcinoma is seldom made. Evaluation and treatment are also essentially the same at that of ovarian carcinoma. We experienced a case of bilateral primary fallopian tube carcinoma in women presented with vaginal bleeding and pelvic mass, postoperatively pathologic examination of resected specimen after laparatomy and reported with brief review of literature.

Herlyn-Werner-Wunderlich Syndrome: A Mini-review

  • Herlyn-Werner-Wunderlich (HWW) syndrome is a rare congenital malformation syndrome that is characterized by a triad of uterine didelphys, blind hemivagina, and ipsilateral renal agenesis. There is a wide variety of phenotypic presentation which is recognized as a spectrum of disease rather than a separate entity. The exact incidence and pathogenesis of HWW syndrome are yet to be investigated. While this disease typically involves adolescent girls who present with abdominal pain or a pelvic mass that is secondary to hematocolpos, nowadays, a majority of potential patients with HWW are being prenatally screened for renal anomalies. Therefore, it is recommended to search for uterovaginal anomalies whenever a multicystic dysplastic kidney or the absence of a kidney is noted in a newborn female, and the role of pediatric nephrologists has become ever more important for early recognition of the disease.

Radiculopathy Caused by Internal Iliac Artery Pseudoaneurysm Managed with Endovascular Embolization

  • The authors describe a case of pseudoaneurysm arising from internal iliac artery presented with radiculopathy mimicking the symptoms of lumbar disc disease or spinal cord tumor. Among the several preoperative evaluation including CT, MRI, electrophysiologic study and ultrasonography, important diagnostic clue was obtained by ultrasonographic findings of turbulence flow at the core of partially enhanced mass in the pelvic cavity. The patient was managed with endovascular coil embolization successfully. The current case makes us remind that assessment of neurological symptoms on lower extremity should include consideration of extraspinal cause in pelvis.

Combination Therapy of Prednisolone and Toceranib Phosphate in a Dog with Malignant Metastatic Insulinoma

  • A 14-year-old intact female Yorkshire terrier was presented with a 2-month history of shivering, intermittent pelvic limb weakness and collapse. Biochemical abnormalities revealed inappropriately increased serum insulin concentration with persistent hypoglycemia. Abdominal ultrasound revealed multiple various sized nodules in liver and fine-needle aspirates of the nodule showed typical neuroendocrine cells with high cellularity. Computed tomography (CT) revealed well-defined hyperattenuating mass in the right pancreatic lobe with homogenous enhancement. CT findings were consistent with a pancreatic tumor with malignant metastasis. Treatment was initiated with low-dose prednisolone and toceranib phosphate. The dog was maintained stable with no more progression of clinical signs and it is worth to try toceranib phosphate in a dog with metastatic insulinoma for improving the quality of life.
