  • Purpose: There are no detailed reports on the incidence of venous thromboembolism (VTE) in pelvic and acetabular fractures in the Asian population. The purpose of this study was to investigate the incidence of VTE in pelvic and acetabular fractures in the Korean population. Methods: The cases of 67 Korean patients with pelvic and acetabular fractures treated at our hospital from January 2009 to February 2012 were analyzed retrospectively. Until May 2010, VTE screening was performed by contrast-enhanced computed tomography (CT) or ultrasonography (US) when the D-dimer value did not decline predictably, still exceeded $20{\mu}g/ml$ at 5 days after trauma and surgery, or increased to greater than $20{\mu}g/ml$ after a period of decline. After May 2010, contrast-enhanced CT and US were performed routinely irrespective of the D-dimer value. Physical prophylaxis was performed on all patients. The effects of the presence of a pelvic and/or acetabular fracture, and the effects of fracture type, accompanying injuries, and screening strategies on the incidences of VTE were investigated. Results: Overall, 26 patients(38.8%) were diagnosed with VTE and PTE in 14(20.9%). All were asymptomatic. Significantly higher incidences of VTE and PTE were observed in trauma patients with pelvic and acetabular fractures than in trauma patients without pelvic and acetabula fractures treated during the same period. No significant differences were observed in the incidences of VTE and PTE between patients with pelvic fractures, and patients with acetabular fractures or between patients with and without accompanying injuries. Compared with the previous screening strategy, the detection rates for VTE and PTE were higher for the newer screening strategy; however, the difference did not reach statistical significance. Conclusion: In the Korean population, we should be vigilant for a high incidence of VTE, especially PTE, in patients with pelvic and acetabular fractures.

  • Objective: Using biofeedback in instructing pelvic floor muscle (PFM) activation is a great method to provide information on muscle contraction. This study aimed to determine the immediate effectiveness of a mobile game-based instruction with an extracorporeal biofeedback device (EBD) to improve PFM contraction in healthy subjects. Design: Cross-sectional study. Methods: Sixteen healthy subjects (4 men and 12 nulliparous women; age, 31±5 years) were enrolled. The subjects were randomly categorized into two groups, those who were to receive instructions on how to contract the PFM by using the EBD (n=7) and those who were to undergo a transabdominal ultrasonography (TAUS; n=9) with biofeedback. PFM function was measured as the displacement (mm) of PFM elevation by using the TAUS before and after the instructions in each group. Results: The EBD and TAUS groups showed a significant increase in the displacement of PFM elevation after the instructions from 5.93±4.03 mm to 7.62±3.77 mm and from 5.27±3.39 mm to 7.47±2.79 mm, respectively (p<0.05). No significant differences were found between the two groups. Conclusions: The results of this study indicated that instructions for PFM contraction using the EBD and TAUS showed an immediate effect; however, no significant difference in effectiveness was observed between the two instruction methods. Therefore, not only can the TAUS method be used but the EBD method can also be used as a PFM instruction method for noninvasive procedures. However, further studies are needed to demonstrate the effectiveness of training and exercise on larger sample sizes that includes patient populations with PFM dysfunction.

  • Percutaneous ultrasound-guided biopsy is useful for the pathologic confirmation of variable body lesions to establish diagnostic and therapeutic approaches. However, deep pelvic lesions are a challenge for pathologic diagnoses because of the presence of the bowel, bladder, major vessels, and pelvic bones which make a percutaneous approach difficult and dangerous. In female, the vagina is elastic and near the pelvic internal organs. Therefore, transvaginal ultrasound may serve as an effective and safe guide for the pathologic diagnosis of pelvis lesions. This review aimed to introduce the indications for, and the method of transvaginal ultrasoundguided biopsy, and to describe the reported diagnostic accuracy and safety.

  • PURPOSE: This study aimed to examine the effects of lumbar stabilization exercise during abdominal hollowing with conscious contraction of the pelvic floor muscles on trunk muscle in healthy twenties subjects. METHODS: The participants were randomly allocated to an experimental group (n=15) and a control group (n=15). The experimental group received lumbar stabilization exercise combined with conscious contraction of the pelvic floor muscles during abdominal hollowing. The control group received lumbar stabilization exercise with abdominal hollowing. Both groups trained with the respective exercise for 30 minutes a day, 3 times a week for 6 weeks. Muscle activation of the external oblique and erector spinae muscles, thickness of the transversus abdominis and multifidus, and contraction holding time of tranversus abdominis were measured before and after exercise. RESULTS: Activations of both the external oblique muscles were significantly decreased, and thickness of both the transversus abdominis and multifidus muscles, and contraction holding time of the transversus abdominis muscle were significantly increased in the experimental group (p<.05). The thickness of the left transversus abdominis and right multifidus muscles, and the contraction holding time of the transversus abdominis muscle significantly increased in the control group (p<.05). On comparing both groups, the activations of both external oblique muscles were significantly reduced and the contraction holding time of the transversus abdominis muscle was significantly increased in the experimental group (p<.05). CONCLUSION: These results suggest that lumbar stabilization exercise by abdominal hollowing and conscious contraction of the pelvic floor muscles is suitable and efficient for healthy twenties subjects.

  • The purpose of this study was to investigate effects of three pelvic floor muscle (PFM) exercises on the thickness of PFM and transverse abdominal muscle (TrA), the internal oblique muscle, and the external oblique muscle. The PFM and trunk muscles were measured using ultrasonography in 4 conditions.rest, conventional PFM contraction (Ex A), PFM contraction with hip adductor contraction (Ex B), and PFM contraction with real-time ultrasound imaging (Ex C). The thickness of PFM in Ex C showed a significantly more decrease compared to rest and others (all comparisons, p<.05). The thickness of TrA in Ex C showed a significantly more increase compared to rest and others (all comparisons, p<.05). This study would recommend the use of PFM contraction with real-time ultrasonographic imaging to improve women's incontinence.

  • The purpose of this study was to determine the effect of contralateral hip adduction (CHA) on thickness of lumbar stabilizers during hip abduction in side-lying. Twenty healthy subjects without back pain were recruited for this study. The thickness of transverse abdominis (TrA), internal oblique (IO) and quadratus lumborum (QL) were measured by ultrasonography. Pelvic lateral tilting motion was measured using a three-dimensional motion analysis system. Measurements were performed at rest position (RP), $35^{\circ}$preferred hip abduction (PHA) and $35^{\circ}$abduction with $10^{\circ}$contralateral hip adduction (CHA) in side-lying at the end of expiration. During the measurements, subjects were asked to maintain steady trunk alignment without hand support. Thickness of TrA and IO was significantly greater in CHA than in PHA and RP conditions. There was no significant difference in thickness of TrA and IO between PHA and RP conditions. Medio-lateral (M-L) thickness of QL was not significant between PHA and CHA conditions. Anterio-posterior (A-P) thickness of QL in PHA and CHA significantly decreased compared to RP condition. Angle of pelvic lateral tilting was significantly decreased in CHA compared to PHA condition. In conclusion, CHA can be recommended for increasing trunk stability without compensatory pelvic motion during hip abduction exercise in side-lying.

  • The urinary bladder retroflexion, flipped over backwards into the pelvic canal, by trauma has been rarely reported in dogs. This paper describes clinical and radiological features of urinary bladder retroflexion in two dogs with a history of traffic accident. The main clinical signs were hind limb lameness, severe pain and dysuria. Radiography and ultrasonography were performed to evaluate the patient's damages. No remarkable findings were observed except pelvic fracture on survey radiograph. On cystourethrography, caudal displacement and retroflexion of the bladders were identified. Cystopexy was performed and the bladders were repositioned. After surgery one dog with severe azotemia and bilateral hydroureteronephrosis was deteriorated noticeably and euthanized by request of the owner, while the other dog was recovered no recurrence of the problem by 12 months post-surgery. It should be considered that contrast study and serial assessment in urinary tract be important to demonstrate the evidence of bladder retroflexion and evaluate the prognosis in dogs with severe trauma.

  • The purpose of this study is to measure abdominal muscle thickness when Pelvic Floor contraction (PFC) and Abdominal Drawing-In Maneuver (ADIM) were separately applied and combined exercise was applied and to compare the effects of the exercise. After the pre-investigation, the subjects were given a explanation of the purpose and the method of the research and then an experiment was conducted targeting a total of 30 subjects, who voluntarily agreed with this. Thicknesses of internal oblique (IO), transverse abdominis (TrA) and external oblique (EO) were measured during a break and then three types of exercise. All the measured values of the experiment were processed using Repeated measure ANOVA, and Bonferroni method was applied. As a result, the three types of exercise showed significant differences in thicknesses of IO, TrA and EO. In conclusion, the subjects had the thickest muscles and muscular activity increased during PFC+ADIM, compared to PFC and ADIM.
