The role of fecal calprotectin in pediatric disease

  • Fecal calprotectin (FC) is a calcium- and zinc-binding protein of the S100 family, mainly expressed by neutrophils and released during inflammation. FC became an increasingly useful tool both for gastroenterologists and for general practitioners for distinguishing inflammatory bowel disease (IBD) from irritable bowel syndrome. Increasing evidences support the use of this biomarker for diagnosis, follow-up and evaluation of response to therapy of several pediatric gastrointestinal diseases, ranging from IBD to nonspecific colitis and necrotizing enterocolitis. This article summarizes the current literature on the use of FC in clinical practice.

Clinical practice pattern on hematuria and proteinuria in children: the report of a survey for the Korean Society of Pediatric Nephrology

  • Purpose: Hematuria and proteinuria have various causes and consequential outcomes in children. Immunosuppressants are needed in some children with biopsy-proven glomerulonephropathy but have many adverse effects. Since the clinical practice patterns of Korean pediatric nephrologists are diverse, we surveyed their opinions. Methods: Using a clinical vignette, the survey was emailed to all Korean Society of Pediatric Nephrology members. The questionnaires included diagnosis, examination, medications, and dietary recommendations for patients with hematuria and proteinuria. Results: A total of 32 clinicians (5.48%, 22 pediatric certificated nephrologists) responded to the survey. Most responders (87.5%) suspected immunoglobulin A nephropathy, and 68.8% replied that kidney biopsies were a diagnostic tool. Renin-angiotensin system inhibition (62.5%) or steroids (18.8%) were selected as the treatment. Salt and protein intakes were usually encouraged as dietary reference intakes (34.4% and 65.6%, respectively). Conclusions: Children with abnormal urinalysis have various causes, treatments, and prognoses. As treatments such as immunosuppressants can have many adverse effects, it is necessary to confirm an accurate diagnosis and indications of treatments before starting the treatment. Recommendations for a diet should not hinder growth.

A Nationwide Survey on Gastrointestinal Endoscopy Practice Patterns among Pediatric Endoscopists in South Korea

  • Purpose: Gastrointestinal (GI) endoscopy is an important tool for diagnosing and treating GI diseases in children. This study aimed to analyze the current GI endoscopy practice patterns among South Korean pediatric endoscopists. Methods: Twelve members of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition developed a questionnaire. The questionnaire was emailed to pediatric gastroenterologists attending general and tertiary hospitals in South Korea. Results: The response rate was 86.7% (52/60), and 49 of the respondents (94.2%) were currently performing endoscopy. All respondents were performing esophagogastroduodenoscopy, and 43 (87.8%) were performing colonoscopy. Relatively rare procedures for children, such as double-balloon enteroscopy (DBE) (4.1%), endoscopic retrograde cholangiopancreatography (ERCP) (2.0%), and endoscopic ultrasound (EUS) (2.0%), were only performed by pediatric gastroenterologists at very few centers, but were performed by adult endoscopists in most of the centers; of all the respondents, 83.7% (41/49) performed emergency endoscopy. In most centers, the majority of the endoscopies were performed under sedation, with midazolam (100.0%) and ketamine (67.3%) as the most frequently used sedatives. Conclusion: While most pediatric GI endoscopists perform common GI endoscopic procedures, rare procedures, such as DBE, ERCP, and EUS, are only performed by pediatric gastroenterologists at very few centers, and by adult GI endoscopists at most of the centers. For such rare procedures, close communication and cooperation with adult GI endoscopists are required.

  • 최근 출생률 감소, 우식 유병율 감소, 소아치과에 대한 일반 대중의 인식변화 등의 사회경제적 변화 및 치의학 기술의 발전에 따른 진료기법의 변화등에 의하여 소아치과 진료의 양상이 변화하고 있다. 이러한 변화양상을 파악하고 앞으로 소아치과 의료의 방향성을 설정하고자 본 연구를 시행하였다. 2000년 9월 1일부터 2005년 8월 31일까지 서울대학교 치과병원 소아치과에 내원한 모든 초진 및 재진환자를 대상으로 하여 신환 분포 및 진료 현황 변화 추세를 조사 연구한 결과 다음과 같은 결론을 얻었다. 1. 2000년도에는 예방치료가 14.5%, 수복치료가 52.6% 외과치료가 11.4%, 교정치료가 21.5%를 차지하였으나, 2005년도에는 예방치료가 12.1%, 수복치료가 38.3%, 외과치료가 13.7%, 교정치료가 35.5%를 차지하여 예방과 수복치료의 비율의 감소와 외과와 교정치료의 증가를 보였다. 2. 수복치료의 비율에 있어서 아말감과 기성금관, 치수치료의 비율은 감소하였고 글래스 아이오노머와 레진의 비율은 증가하였다. 3. 내원환자의 수는 2002년까지 증가하다 이후로는 감소 추세를 보였다. 4. 연령별 분포에서 2000년도에 비하여 2005년도의 신환 평균 연령이 감소하였다.

International Cross-Sectional Survey among Healthcare Professionals on the Management of Cow's Milk Protein Allergy and Lactose Intolerance in Infants and Children

  • Purpose: The present international survey among healthcare providers aimed to collect data on theoretical knowledge and clinical practices in the diagnosis and management of cow's milk protein allergy (CMPA) and lactose intolerance (LI) in infants. Methods: A global survey was conducted in several countries with diverse health care settings. The survey consisted of multiple-choice questions in 3 main domains: (1) understanding and clinical practices around CMPA and LI; (2) case scenarios; and (3) disease-specific knowledge and potential educational needs. Results: Responses were available from 1,663 participants. About 62% of respondents were general practitioners or general pediatricians, and the remainder were pediatric allergists/gastroenterologists (18%) or other health practitioners (20%). The survey identified knowledge gaps regarding the types of CMPA (IgE-mediated vs. non-IgE-mediated) and the clinical overlap with LI. The survey suggested diverse clinical practices regarding the use of hypoallergenic formulas, as well as misconceptions about the prebiotic benefits of lactose in extensively hydrolyzed formulas in non-breastfed infants with CMPA. Responses to the two case scenarios highlighted varying levels of awareness of the relevant clinical practice guidelines. While respondents generally felt confident in managing infants with CMPA and LI, about 80% expressed an interest for further training in this area. Conclusion: The current survey identified some knowledge gaps and regional differences in the management of infants with CMPA or LI. Local educational activities among general and pediatric healthcare providers may increase the awareness of clinical practice guidelines for the diagnosis and treatment of both conditions and help improve clinical outcomes.

아동의 발열과 발열관리: 문헌고찰 (Fever and Fever Management in Children: A Literature Review)

  • Purpose: The purposes of this review were to identify whether available evidence supports the nursing interventions that are commonly used to reduce fever in children and to introduce research findings into practice. Methods: Journal databases and clinical guidelines from 1990 to 2009 were searched. The search terms were fever, febrile convulsion, fever management, fever phobia, child, antipyretics, temperature, external cooling, tepid sponge bath, and physical treatment. Results: Evidence suggests that uncomplicated fever is relatively harmless, but it is an important immunological defense. Antipyretics should not routinely be used with the sole aim of reducing body temperature in children with fever who are otherwise well. Currently a lack of evidence supports the practice of alternating acetaminophen and ibuprofen, and the routine use of tepid sponge bath. Conclusion: Currently, fever management in children does not reflect research evidence. Pediatric nurses can play an important role by encouraging clinical research in this area and also by enhancing research utilization in their practice. Moreover, pediatric nurses can educate parents about evidence-based fever management. Evidence-based educational interventions for pediatric nurses need to be developed and evaluated to improve the quality of nursing care in the management of childhood fever.

Development and evaluation of a pediatric nursing competency-building program for nursing students in South Korea: a quasi-experimental study

  • Purpose: The present study aimed to develop and examine the effectiveness of a pediatric nursing competency-building program for nursing students. Methods: This was a quasi-experimental study with a nonequivalent control group pretest-posttest design conducted between October and December 2021. The participants included 40 nursing students (20 each in the experimental and control groups) at a university in a South Korean city. The pediatric nursing competency-building program integrated problem-based learning and simulation into clinical field practice. The experimental group participated in the program, while the control group did not. Data were analyzed using the 𝑥2 test, the independent t-test, and repeated-measures analysis of variance. Results: Pediatric nursing competency and clinical performance showed a greater increase in the experimental group than in the control group. However, the change in problem-solving ability in the experimental group was not significantly different from that in the control group. Conclusion: The pediatric nursing competency-building program effectively improved students' pediatric nursing competency and clinical performance.

  • 소아치과 전문 지역 의원의 증가, 소아치과에 대한 보호자의 인식 변화, 치의학의 발전, 소아치과 진료 범주의 확대 등에 의해 최근 소아치과의 진료 양상이 크게 변화하고 있다. 이러한 변화 양상을 파악하고 앞으로의 대학병원 소아치과 및 소아치과 전문 지역 의원의 진료분담에 대한 방향성을 제시하고자 본 연구를 시행하였다.2001년 1월 1일부터 2008년 12월 31일까지 서울대학교치과병원 소아치과에 내원한 모든 초진 및 재진 환자를 대상으로, 서울대학교치과병원 종합의료정보시스템(OCS)을 통하여 1년간의 외래환자집계 및 수가코드별 진료 행위 건수 및 수익 분포를 조사하여 다음과 같은 결과를 얻었다. 1. 내원환자의 수는 2001년부터 2005년까지 대체로 증가하다 2006년에 감소하였으며 그 이후에는 2006년도와 별다른 변화가 없었다. 2. 2001년에는 예방치료가 6.84%, 수복치료가 49.96%, 치수치료가 9.64%, 외과치료가 10.78%, 교정치료가 22.78%를 차지하였으나, 2008년에는 예방치료가 15.40%, 수복치료가 28.97%, 치수치료가 3.28%, 외과치료가 14.83%, 교정치료가 37.52%를 차지하여 수복치료와 치수치료의 비율은 감소하고 예방치료와 교정치료는 증가하였다. 3. 수복치료의 비율에 있어서 아말감과 글라스 아이오노머 수복 및 기성금관수복의 비율은 감소하였고, 레진의 비율은 증가하였다. 4. 진정요법하 치과치료는 2005년까지 증가하다 2006년, 2007년에는 감소하였고, 2008년에 다시 증가하였다. 5. 전신마취하 치과치료는 2001년부터 2008년까지 꾸준히 증가하였다.

Development of a virtual reality program in South Korea for the measurement of vital signs in children: a methodological study

  • Purpose: We developed a virtual reality (VR) program for use in pediatric nursing practicums to help nursing students learn to measure vital signs in children. Methods: The analysis, design, development, implementation, and evaluation model was employed between July 2021 and December 2021 at a university in South Korea. In the analysis phase, in-depth interviews were conducted with four nursing students, two nursing school graduates, and four experts. The topic and contents of the VR program were settled in the design phase. The VR program was developed and subsequently used and evaluated by 20 nursing students and four experts. Results: The contents of the VR program for pediatric nursing practicums included the measurement of vital signs in a newborn baby and a young child, as well as an evaluation system. The mean score for the nursing students' satisfaction with practice was 4.02 out of 5 points. The mean scores for overall satisfaction with the VR program were 4.15 and 4.79 out of 5 points for nursing students and experts, respectively. Conclusion: The VR program developed in this study allows nursing students to practice measuring vital signs in children, thus improving the students' clinical performance in pediatric nursing.
