토오컨 강화가 입원 아동의 약 복용행위에 미치는 효과 (A Study of the Effectiveness of Token Economy on Children′s Behaviors for Oral Medication)

  • Oral medication is one of the most frequent treatment in clinical care. and frequently refused by children. Children's refusal spends unuseful time and require nurses'Patience. So this study was attempted to develop token economy Program which Promote children's medication. and to evaluate its' effect to construct the experimental ground. This study was a quasi experimental study under the nonequivalent control group non-synchronized design. The subjects of this study were 48 children who were aged 3-12 years and admitted pediatric ward because of their respiratory disease. Token economy program was consisted of cartoon record paper with stickers which expresses the better medication is taken the more germs die, and gift was given for back up reinforcer. To evaluate this Program. time spent for oral medication was measured six time to both experimental group and control group. and measured the perceived effectiveness of this program by nurses. To determine the effect of the program, the data were analyzed by the SAS 6.12 Program with t-test and mean, standard deviation. The results of this study were as follows: 1 In experimental group, time spent for oral medication was significantly shorter than in control group(t=5.24. p=0.0001). Therefore, the hypothesis of this study was strongly accepted. 2. Nurses perceived this program as an effective one(mean=3.125~3.792). The effectiveness occurred before. during and after medication as well as in parental responses, especially in using verbal dissatisfaction after medication. In conclusion. it was found that the token economy program for admitted children was effective in inducing the children's behavior of oral medication. There was no effectiveness in very highly anxious children Whereas, it is recommended that this program should not be used for such children until they settle down. If the token economy program was made in various situation with creative thinking, it will be very useful nursing measurement, especially in caring for children. To improve the qualify of nursing care, the various programs, which can give joyfulness to stressed patients should be developed.

  • Purpose: The purpose of this study was to provide descriptive data about the characteristics of pediatric patients and nursing interventions in Regional Emergency Medical Centers (REMC). Methods: A retrospective design was used to examine the medical records of 4,310 children. The clinical data and nursing terminologies of REMC were analyzed using the Nursing Intervention Classification (NIC). Results: Male toddlers dominated the sample. The mean age of the children was 3.51 yr. In more than half of the visits, patients arrived between from 7 a.m. to 3 p.m., on a weekday. There were only 189 ambulance transports to REMC, (4.4% of visits). The most frequent injuries were due to falls (28.0%) and contusions (27.3%), but the most common reasons for visits were non-injury (73.4%): fever, cough/shortness of breath, seizures, and abdominal pain. Of the 4,310 visits, 27.8% spent 6-24 hours in the REMC, while 33% resulted in hospital admission and 2.1% in transfer to another hospital. Of the 17,929 nursing interventions, 17,909 elements (99.9%) were classified under NIC. All the listed NIC interventions, however, were not reflected in the level of practice demonstrated by REMC nurses. Conclusion: These results can enhance the understanding of pediatric emergency nursing interventions and can make NIC more applicable.

입원 환아 간호에 대한 부모의 참여 개념분석 (Parent Participation in care of Hospitalized Children: Concept Analysis)

  • 목적 본 연구는 입원 환아에게 최적의 간호를 제공하기 위한 전제조건인 입원 환아 간호에 대한 부모의 참여의 개념을 탐색함으로써 아동간호 실무에 대한 이해를 증진시키고자 시도되었다. 방법 Walker와 Avant (2005)의 개념분석 방법을 사용하였으며, 선행연구를 근거로 1994년부터 2012년 사이에 이루어진 부모의 참여 관련 논문 중 입원 환아 간호, 부모참여를 주요어로 하여 고찰한 논문 30편을 통해 속성, 선행요인, 결과를 확인하였다. 결과 입원 환아 간호에 대한 부모의 참여의 속성은 협상, 돌봄 활동 수행, 개별화된 간호 제공으로 확인되었다. 선행요인은 환아의 연령과 상태, 환아 부모 참여 필요성에 대한 환아 부모와 아동간호사의 태도 및 상호 이해이며, 결과는 환아 부모와 아동간호사의 효율적인 파트너십이 이루어지고, 환아 부모와 아동간호사를 임파워먼트시키게 되는 것으로 확인되었다. 결론 본 연구에서 확인된 입원 환아 간호에 대한 부모의 참여의 개념 정의와 속성은 아동간호실무 이론 개발의 기초자료로 활용될 것이다.

입원 아동 부모의 병원서비스 기대수준과 만족도 (Expectation and Satisfaction of Parents with Inpatient Hospital Service)

  • Purpose: The purpose of this study was to investigate parent expectation and satisfaction with respect to pediatric inpatient care and to identify the variables related to parent satisfaction. Methods: The study was conducted in pediatric wards of a tertiary children's hospital in Korea. The participants were 361 parents of children who were inpatients. Data were collected using a structured questionnaire (The Pediatric Family Satisfaction Questionnaire) at the time of discharge. Results: The highest parent expectation domain was medical service. The parents were most satisfied with nursing service and least satisfied with general hospital service and accommodation. The parents expressed lower satisfaction with hospital facilities, equipment, noise, cleanliness, and communication by health care professionals. Parents with younger children reported higher expectation from the complete hospital service and those who had a longer length of stay reported higher expectation from the nursing service. Conclusion: To improve the quality of hospital services, we need to understand parent expectation and improve and provide clear communication. In addition, the general hospital service and accommodation should not be overlooked for improvement.

소아 환자에서 욕창 위험도 사정 도구의 예측타당도 비교: Braden, Braden Q 및 Braden QD 도구 (Comparison of the Predictive Validity of the Pressure Injury Risk Assessment in Pediatric Patients: Braden, Braden Q and Braden QD Scale)

  • Purpose: The purpose of this study is to compare the predictive validity of pressure injury risk assessment, Braden, Braden Q and Braden QD for pediatric patients. Methods: Prospective observational study included patients under the age of 19 who were hospitalized to general wards, intensive care units of a children's hospital. Characteristics related to pressure injury were collected, and predicted validity was compared by calculating the areas under the curve (AUC) of the Braden, Braden Q, and Braden QD scales. Results: A total of 689 patients were included in the study. A total of 13 (1.9%) patients had pressure injuries, and the number of pressure injuries was 17. Factors related to the occurrence of pressure injuries were 9 (52.9%) immobility-related and 8 (47.1%) medical device-related. The AUC for each scale was .91 (95% CI .89~.94) for Braden, .92 (95% CI .90~.95) for Braden Q, and .94(95% CI .92~.96) for Braden QD. The optimal cut-off points were identified as 16 for Braden (sensitivity=88.8%, specificity=86.4%), 17 for Braden Q(sensitivity=63.6%, specificity=94.9%), and 12 for Braden QD (sensitivity=94.4%, specificity=88.7%). Conclusion: The Braden QD scale demonstrated the highest predictive validity for pressure injuries in pediatric patients and is expected to be valuable tool in preventing pediatrics pressure injuries.

장애인 입소시설과 특수학교의 구강건강관리 실태에 대한 조사 연구 (ORAL HEALTH CARE STATUS IN INSTITUTIONS AND SPECIAL SCHOOLS FOR THE DISABLED PEOPLE IN KOREA)

  • 전국의 123개 장애인 입소시설과 102개 특수학교를 대상으로 구강건강관리 실태를 파악하여 장애인의 구강건강관리 계획을 수립하는 데 사용할 수 있는 근거를 마련하고자 하였다. 설문지 조사법으로 연구하였다. 정기구강검진은 특수학교에서 연 2회 실시하고 있었으나, 장애인 입소시설에서는 56.8%에서 시행하였다. 우식 예방 프로그램은 장애인 입소시설의 69.1%, 특수학교의 86.3%에서 시행하였으며 프로그램의 종류는 입소시설에서 규칙적 칫솔질이 58.1%, 특수학교에서 예방교육이 65.7%로 가장 많았다. 불소를 이용한 우식예방은 입소시설과 특수학교에서 각각 8.6%, 11.8%에서 시행하고 있었으며, 열구전색은 각각 6.8%, 6.9%에서 시행하였다. 치과치료를 시행하고 있는 입소시설과 특수학교에서는 각각 84.2%, 39.2%이었고, 입소시설에서 치료를 받기 위해 이용하는 곳은 치과의원(60.2%), 보건소(16.8%), 자원봉사자(15.3%), 대학병원 (1.0%) 등이었다. 치료를 위해 치과의원에 방문하여 치료를 거부당한 경험이 있는 응답자는 17.7%이었다.

고유량 비강 캐뉼라 산소요법을 받은 소아중환자실 환아의 ROX Index와 ROX-HR Index 및 SpO2/FIO2 Ratio분석 (Analysis of ROX Index, ROX-HR Index, and SpO2/FIO2 Ratio in Patients Who Received High-Flow Nasal Cannula Oxygen Therapy in Pediatric Intensive Care Unit)

  • Purpose: This study aimed to evaluate the use of the respiratory rate oxygenation (ROX) index, ROX-heart rate (ROX-HR) index, and saturation of percutaneous oxygen/fraction of inspired oxygen ratio (SF ratio) to predict weaning from high-flow nasal cannula (HFNC) in patients with respiratory distress in a pediatric intensive care unit. Methods: A total of 107 children admitted to the pediatric intensive care unit were enrolled in the study between January 1, 2017, and December 31, 2021. Data on clinical and personal information, ROX index, ROX-HR index, and SF ratio were collected from nursing records. The data were analyzed using an independent t-test, χ2 test, Mann-Whitney U test, and area under the curve (AUC). Results: Seventy-five (70.1%) patients were successfully weaned from HFNC, while 32 (29.9%) failed. Considering specificity and sensitivity, the optimal cut off points for predicting treatment success and failure of HFNC oxygen therapy were 6.88 and 10.16 (ROX index), 5.23 and 8.61 (ROX-HR index), and 198.75 and 353.15 (SF ratio), respectively. The measurement of time showed that the most significant AUC was 1 hour before HFNC interruption. Conclusion: The ROX index, ROX-HR index, and SF ratio appear to be promising tools for the early prediction of treatment success or failure in patients initiated on HFNC for acute hypoxemic respiratory failure. Nurses caring for critically ill pediatric patients should closely observe and periodically check their breathing patterns. It is important to continuously monitor three indexes to ensure that ventilation assistance therapy is started at the right time.

어린이병원학교 백혈병 환아어머니의 아동건강 학습 요구 (The child's health and learning needs of mother of children with leukemia in a Children School of Hospital)

  • Purpose: This study was to explore the child's health and learning needs of mother of children with leukemia in a children school of a hospital and to describe the basic material for intervention program development. Methods: Participants in this study was 7 mother of children with leukemia. Data collection consisted of in-depth focus group interviews done from April 21 to 30 in 2005. Results: 3 categories emerged from 9 theme clusters. The categories was 'tutorial service', 'emotional service' and 'disease control'. Conclusion: The results of this study show that pediatric nurses need to pay attention to the learning needs of mother and children in the children school of hospital and develop the program to support the children with leukemia and chronic disease.

학령기 아동의 입원생활 적응 (School aged Children's Adjustment to Hospital Life)

  • Purpose: The study was done to identify adjustment to hospital life of school aged children. This research was designed as a descriptive study. Method: A convenience sample of 186 patients who were hospitalized children, 6~12 years old. The instrument in this study were developed by researcher and constructed to include 5 sub categories about adjustment to hospital life. They ask children to rate each item on 5 Likert scale. The data were analyzed by SPSS Win Program. Result: The mean average score of adjustment to hospital life was 91.75(SD 11.22); the social support revealed the highest value, followed by self-esteem, stress on disease, stress on hospitalization, and defense strategies. There was differences on bibliographic data; hospital date, disease type. The relationship between adjustment and sub categories was significant, especially in the stress on hospitalization and disease, social support and self-esteem. The relationship between coping strategies and stress on diseases was reversed. Conclusion: These results suggest that pediatric nurses need to be aware of the adjustment to hospital life. A development of nursing intervention program may be useful and critical for hospitalized school aged children.

외래 및 퇴원환아 부모의 전화상담요구와 간호중재에 대한 조사연구 (An investigational study on telephone calls to the pediatric nursing unit)

  • The purpose of this study was to investigate the current status of the need of telephone call and to identify the status of nursing intervention through telephone. Head nurses of the pediatric nursing unit and a nurse of pediatric outpatient clinic wrote down the telephone record of calls by parents of children discharged from hospital from 7 am to 3 pm during the period of March to June, 1995. Content of 120 telephone calls but for 26 calls with incomplete record among 146 calls were analyzed into frequency of general characteristics, needs and nursing intervention. The needs of telephone call were identified and classified into 11 areas and analyzed into frequency of detailed content by 11 areas. Nursing intervention was identified and classified into 10 categories, and analyzed into frequency of detailed content by 10 categories. The findings of this study were as follows ; The need of telephone call was identified with nutritional state, medication, vital signs, language retardation, personal hygiene, vaccination, administration procedure, physical symptoms, follow up care management and others. The most frequent needs were physical symptoms and vaccination. A kind of food among nutrition dose of drugs among medication, fever among vital signs, cough among physical symptoms, and content of vaccination among vaccination was the most frequent needs. Nursing intervention through telephone was identified with instruction, knowledge offer, information offer, judgement, solicitation, referral and instruction, referral, connection, reassurance, reservation, and regulation. Instruction, knowledge offer and information offer was the most frequent nursing intervention by telephone call. Instruction was about a visit to hospital, a visit to nearby clinic, instruction about symptoms,, instruction about nursing care procedure, retelephoning and vaccination. Knowledge offer was about vaccination, knowledge related to medication, and dental care. Information offer and judgement was about vaccination and medication. Referral and instruction delivery was about instruction delivery following consultation to doctor, visit to emergency room and a visit to hospital following consultation to doctor. These results suggest that telephone call intervention program should be established as a field of extended pediatric nursing role in health care delivery system for the children.
